Background: Patients at the end-of-life and their families experience a strong emotional impact. The wellbeing of the patient at the end-of-life and their family caregiver are related. Aim: to explore the elements related with the emotional wellbeing of patients with and without cognitive impairment at the end-of-life and that of their primary family caregivers. Design: Cross- sectional study. Participants: data was collected from 202 patients at the end of life with different diagnosis (COPD, cancer and frail elderly) as well as (...) from their respective 202 primary family caregivers. Results: Structural equation models indicated that the emotional state of the patients was best predicted by their functional independence and the burden of their family caregivers. At the same time, the emotional state of the primary family caregiver was predicted by their burden and not by the cognitive state or the functional independence of the patient. Nevertheless, the burden of the family caregiver, which is the only variable predicting both the emotional state of the patient and that of the caregiver, was directly related with the functional independence of the patient and indirectly with the patient’s cognitive state. Conclusions: The burden of the family caregiver stands out as a key factor in the reduction of the emotional distress of patients at the end-of-life –whether or not they present significant cognitive impairment- and that of their family caregivers. In this research, a sliding mode regulator with sine mapping is suggested for the stabilization of electricity generators being affected by magnet interaction nonlinearities and generator nonlinearities. To reach this goal, our suggested regulator has the following contributions: it starts from the sliding mode regulator with the modifications that the saturation mapping is used to reach a smoother performance instead of the signum mapping, and the sine mapping is applied to reach an upper bound in the proportional gain error, it (...) is used to reach some chosen constant behaviors for the angle position, angle speed, and current in the electricity generators, and its stabilization is ensured based on the Lyapunov approach. In this work, we present a new approach to solve the location management problem by using the reporting cells strategy. Location management is a very important and complex problem in mobile computing which aims to minimize the costs involved. In the reporting cells location management scheme, some cells in the network are designated as reporting cells . The choice of these cells is not trivial because they affect directly to the cost of the mobile network. This article is focused on (...) the use of high performance computing to execute a parallel heuristic that places optimally the RCs in a mobile network, minimizing its total cost. The main goal of this work is to demonstrate that the collaborative work of different evolutionary algorithms can obtain very good results. For this reason, we have implemented a parallel heuristic and six evolutionary algorithms that works in a parallel way on a cluster to solve the RCs problem. (shrink)

The aim of the present study was to assess the impact of past traumatic war experiences on preadolescents in the Gaza Strip, which could be useful for psychological intervention with current and future child victims. Participants were 521 preadolescents from United Nations Relief and Works Agency for Palestine Refugees in the Near East (UNRWA) schools, aged 11 and 13 years old. Sections I to IV from Iraqi Version-Arabic of Harvard Trauma Questionnaire was used to assess trauma experiences and Post-Traumatic Stress (...) Disorder (PTSD). The results show that the preadolescents in the Gaza Strip witnessed the destruction of their homes and the murder of family members and friends. A quarter of the individuals assessed either suffered torture or witnessed others undergoing it, including sexual assaults. Almost half of them experienced a lack of food and clean water. The traumatic and torture experiences seriously affected preadolescents’ mental health as 26.29% met criteria for the diagnosis of PTSD. The data analysis revealed two PTSD modalities, with the severity of impact depending on whether social implications were involved. Further research is required to check whether these two modalities fit to PTSD and complex PTSD. Understanding the effects of past wars on preadolescents in Gaza and distinguishing between different PTSD types could enhance comprehension of the impacts of current attacks on child victims. It can also aid in determining the type of intervention needed to minimize the impact on the mental health of Palestinian youth, enhancing their resilience through psychological and social support. IntroductionRheumatologists are the primary healthcare professionals responsible for patients with rheumatic diseases and should acquire medical ethical competencies, such as the informed consent process (ICP). The objective clinical structured examination is a valuable tool for assessing clinical competencies. We report the performance of 90 rheumatologist trainees participating in a station designed to evaluate the ICP during the 2018 and 2019 national accreditations.MethodsThe station was validated and represented a medical encounter in which the rheumatologist informed a patient with systemic lupus erythematosus (...) with clinically active nephritis about renal biopsy. A trained patient–actor and an evaluator were instructed to assess ICP skills (with a focus on kidney biopsy benefits, how the biopsy is done and potential complications) in obtaining formal informed consent, delivering bad news and overall communication with patients. The evaluator used a tailored checklist and form.ResultsCandidate performance varied with ICP content and was superior for potential benefit information (achieved by 98.9% of the candidates) but significantly reduced for potential complications (37.8%) and biopsy description (42.2%). Only 17.8% of the candidates mentioned the legal perspective of ICP. Death (as a potential complication) was omitted by the majority of the candidates (93.3%); after the patient–actor challenged candidates, only 57.1% of them gave a clear and positive answer. Evaluators frequently rated candidate communications skills as superior (≥80%), but ≥1 negative aspect was identified in 69% of the candidates.ConclusionsEthical competencies are mandatory for professional rheumatologists. It seems necessary to include an ethics competency framework in the curriculum throughout the rheumatology residency. (shrink)

Objetivo: Realizar una revisión sistemática sobre la evolución de las publicaciones científicas del uso bioético de las tecnologías de salud en los últimos 20 años. Metodología: Se realizó una revisión bibliográfica usando la base de datos Scopus. Resultados: La aplicabilidad de las tecnologías de salud, desde un punto de vista bioético, ha aumentado en los últimos años. Conclusión. Gricean pragmatics has often been criticised for being implausible from a psychological point of view. This line of criticism is never backed up by empirical evidence, but more importantly, it ignores the fact that Grice never meant to advance a processing theory, in the first place. Taking our lead from Marr, we distinguish between two levels of explanation: at the W-level, we are concerned with what agents do and why; at the H-level, we ask how agents do whatever it is (...) they do. Whereas pragmatics is pitched at the W-level, processing theories are at the H-level. This is not to say that pragmatics has no implications for psychology at all, but it is to say that its implications are less direct than is often supposed. (shrink)

In developing a theory of consciousness, one of the main problems has to do with determining what distinguishes conscious states from non-conscious ones—the delimitation problem. This paper explores the possibility of solving this problem in terms of self-awareness. That self-awareness is essential to understanding the nature of our conscious experience is perhaps the most widely discussed hypothesis in the study of consciousness throughout the history of philosophy. Its plausibility hinges on how the notion of self-awareness is unpacked. The idea that (...) consciousness involves self-awareness has been understood in two different ways: either as awareness of oneself—the subject of experience—or as awareness of the conscious episode itself. In this paper I argue (i) that every experience concerns the subject in a very specific way, involving what I will call ‘perspectival de se representation’, and (ii) that there is no need to appeal to the experience itself in order to characterize the awareness within that experience. The view I articulate explains the subjective nature of experience without over-intellectualizing it, accommodates the phenomenology of experience, and dispels any doubt about the need to find the self in introspection. Este artigo objetiva refletir sobre a relação linguagem, educação e hermenêutica. Realizamos uma pesquisa bibliográfica com base em Gadamer. Na educação, a dimensão dialógica da compreensão está situada como o locus primordial da ação educativa, lugar privilegiado onde o compreender se efetiva tendo por base a relação de intersubjetividade entre os sujeitos do diálogo, condição fundante da interpretação do que é proferido no ambiente escolar, mediante a abertura entre os sujeitos dessa postura. Através de uma educação dialógica podemos ter educandos (...) cada vez mais conscientes da sua individualidade e historicidade, que utilizam a linguagem para compreender a si mesmo e o mundo à sua volta. One problem that various thinkers have studied is the relationship between culture and social development. This article will try to explain the principal characteristics of South Korean culture and its Confucian heritage as positive factors in the development of South Korea. To do so, it addresses the existence of values of Confucian origin in government and society. Next, the article reviews historical factors that played an important part in South Korean development. Finally, the authors reach the conclusion that development is (...) possible in distinct cultural contexts, and that it is not possible to establish a deterministic connection between a specific social framework and the conditions of social development. In this article, I will try to explain the influence of Francesco Bausi’s works in the Machiavellian studies written in Spanish. First, I will show the particular characteristics of this influence. This has to do with the complex imagen of Bausi’s Machiavelli for Spanish historians of philosophy and political philosophers. Second, I will write about the description of Machiavelli contained in Bausi’s novel Scandalo Machiavelli, published in 2014. It is a paradox that this scholar has published a more defined portrait (...) of Machiavelli in a fictious tale rather than in his scholarly writtings. (shrink)

A processualistic approach is offered in the study of that thinking giving rise to modernism from the historical cultural context in which the phenomenon has arisen and evolves. Built upon two opposing forces, it has collapsed after various centuries of development. Modernism is thus called to heroism and must regenerate, availing itself of its best of qualities: critical thinking, reflection and rational dialogue.

El artículo analiza las ideas filosóficas de Lisandro de la Torre, dirigente político y destacado legislador argentino de vasta trayectoria pública entre 1890 y 1937, a partir de algunas conferencias y escritos de sus últimos años. En el enfoque de su cosmovisión se toman en cuenta, por un lado, la relación entre filosofía, ciencia y religión, y, por otro, la concepción antropológica, mostrando la influencia de perspectivas filosóficas como el panteísmo y el positivismo. El estudio somete a crítica la desvinculación (...) entre política y filosofía que hacía el propio de la Torre. Antes bien, concluye en que ambas esferas se encuentran conectadas y constituyen una raíz fundante del pensamiento latorrista.The article analyses Lisandro de la Torre's philosophical ideas. He was an important Argentinian politician, who had an active public life, mainly, as a legislator, between 1890 and 1937. The study is based upon some lectures and writings from his last years. His cosmological view is examined through the relation among philosophy, science and religion, on one hand, and through his anthropological account, on the other hand. The influence of pantheism and positivism is pointed out in both perspectives. It is criticised de la Torre's claim about politics as an anti-philosophical activity by arguing that philosophy and politics may be considered as foundations of his thought. (shrink)

En este artículo, quiero examinar el juicio que el filósofo colombiano realiza de España y del mundo hispánico en su obra. De su consideración completamente negativa sobre este legado cultural, se pueden extraer dos consecuencias. Por una parte, su hispanofobia lo alejaría de la tradición reaccionaria hispanoamericana. Sin embargo, por otra parte, su identificación entre cultura española e hispanoamericana lo acercaría de nuevo –si bien con renovada complejidad– a la identidad reaccionaria canónica.

In this essay we discuss recent attempts to analyse the notion of representation, as it is employed in cognitive science, in purely informational terms. In particular, we argue that recent informational theories cannot accommodate the existence of metarepresentations. Since metarepresentations play a central role in the explanation of many cognitive abilities, this is a serious shortcoming of these proposals.

La presente reflexión emana de la doctrina que llamo naturalismo universal: todo lo existente es natural sea o no descrito y explicado satisfactoriamente por las ciencias naturales. Ahora bien, a pesar de los progresos de las ciencias positivas y empíricas como la física, la química y la biología, no tenemos una explicación satisfactoria de lo que hace que un ser vivo lo sea. En consecuencia resulta indispensable reexaminar la tesis del principio vital y considero que la ciencia contemporánea se honraría (...) retomándolo con el objetivo de comprender mejor la constitución y el comportamiento del ser vivo.
