There is no agreed minimum standard with regard to what is considered safe, competent nursing care. Limited resources and organizational constraints make it challenging to develop a minimum standard. As part of their everyday practice, nurses have to ration nursing care and prioritize what care to postpone, leave out, and/or omit. In developed countries where public healthcare is tax-funded, a minimum level of healthcare is a patient right; however, what this entails in a given patient’s actual situation is unclear. Thus, (...) both patients and nurses would benefit from the development of a minimum standard of nursing care. Clarity on this matter is also of ethical and legal concern. In this article, we explore the case for developing a minimum standard to ensure safe and competent nursing care services. Any such standard must encompass knowledge of basic principles of clinical nursing and preservation of moral values, as well as managerial issues, such as manpower planning, skill-mix, and time to care. In order for such standards to aid in providing safe and competent nursing care, they should be in compliance with accepted evidence-based nursing knowledge, based on patients’ needs and legal rights to healthcare and on nurses’ codes of ethics. That is, a minimum standard must uphold a satisfactory level of quality in terms of both professionalism and ethics. Rather than being fixed, the minimum standard should be adjusted according to patients’ needs in different settings and may thus be different in different contexts and countries. (shrink)

The authors of the preceding articles1 raise legitimate questions about patient and staff rights and the unintended consequences of allowing ABC News to film inside teaching hospitals. We explain why we regard their fears as baseless and not supported by what we heard from individuals portrayed in the filming, our decade-long experience making medical documentaries, and the full un-aired context of the scenes shown in the broadcast. The authors don’t and can’t know what conversations we had, what documents we reviewed, (...) and what protections we put in place in each televised scene. Finally, we hope to correct several misleading examples cited by the authors as well as their offhand mischaracterization of our program as a “reality” show. (shrink)

This paper demonstrates the undecidability of a number of logics with quantification over public announcements: arbitrary public announcement logic, group announcement logic, and coalition announcement logic. In APAL we consider the informative consequences of any announcement, in GAL we consider the informative consequences of a group of agents all of which are simultaneously making known announcements. So this is more restrictive than APAL. Finally, CAL is as GAL except that we now quantify over anything the agents not in that group (...) may announce simultaneously as well. The logic CAL therefore has some features of game logic and of ATL. We show that when there are multiple agents in the language, the satisfiability problem is undecidable for APAL, GAL, and CAL. In the single agent case, the satisfiability problem is decidable for all three logics. (shrink)

We describe a broad class of problems, called “uncompromising problems,” which are characterized by the requirement that solutions must perform optimally on each of many test cases. Many of the problems that have long motivated genetic programming research, including the automation of many traditional programming tasks, are uncompromising. We describe and analyze the recently proposed “lexicase” parent selection algorithm and show that it can facilitate the solution of uncompromising problems by genetic programming. Unlike most traditional parent selection techniques, lexicase selection (...) does not base selection on a fitness value that is aggregated over all test cases; rather, it considers test cases one at a time in random order. We present results comparing lexicase selection to more traditional parent selection methods, including standard tournament selection and implicit fitness sharing, on four uncompromising problems: 1) finding terms in finite algebras; 2) designing digital multipliers; 3) counting words in files; and 4) performing symbolic regression of the factorial function. We provide evidence that lexicase selection maintains higher levels of population diversity than other selection methods, which may partially explain its utility as a parent selection algorithm in the context of uncompromising problems. The author continues the preceding exchange of views on the nature of metaphor with the American philosopher Nelson Goodman that has been published in 'Poetics Today'. He offers his own theory of metaphor, which assumes a middle ground between Goodman's semantic theory and Davidson's pragmatic conception. The article also presents another argument in support of the thesis that there are only true metaphors, and that the logical space assumed by Goodman for the metaphorical falsehood is already reserved for the category (...) of statements that are nonsensical. There exists a derived negative right to procreative freedom, including a right to in vitro fertilisation (IVF) and to the exercise of selective techniques such as preimplantation genetic diagnosis. This is an extensive freedom, including not only the right to the exercise of a responsible parenthood, but also, in rare cases, to wrong decisions. It includes also a right for less than perfect parents to the use of IVF, and for IVF doctors to assist them, if they want and can (...) agree about the terms. (shrink)

As ethical attitudinists say, ethical statements cannot be strictly true or false, since they express wishes or attitudes, not beliefs. However, the wishes expressed by basic moral judgments about human rights are such that it is a necessary truth that those who know what human beings are have them, and those who do not acknowledge these rights show their lack of a living sense of human reality. The same goes for basic judgments about the rights of animals, and it is (...) blindness about the ontology of animal reality which lies behind the cruelty to animals inherent in vivisection and factory farming. One current source of this blindness may be the physicalist metaphysics which is typical of our day, and which should be sharply distinguished from metaphysical naturalism. (shrink)
