ABSTRACT The term “deep state” has enjoyed political prominence in recent years, especially in movements around former President Donald Trump. However, the term emerged in the activist milieu after the founding of Students for a Democratic Society, which sought to engender political realignment in the aftermath of the Kennedy assassination. Those on the far right who use the term to level accusations of conspiracy at supposed subversives in the administrative state are unwittingly drawing on a long-running but (...) little-analyzed intellectual tradition. In that tradition, conspiracy theories purport to unmask the real intentions of political actors in order to facilitate cooperation and recruitment across ideological and partisan divides. From the postwar period to the present, conspiracy theories, and the “deep state” vocabulary in particular, circulated freely across the left-right political divide. Authors and activists were attracted to these tropes and texts not merely because they appeared to reveal concrete truths about long-hidden elements of American political life, but also because their very ambiguity enabled them to be flexibly co-opted. Conspiracy theories allowed activists to mobilize around shared gaps in public knowledge about traumatic events like the assassination, activating public dissatisfaction with official explanations of those events. Their ambiguity, coupled with the seeming pervasiveness of conspiracy theorist tropes across political divides, appeared to activists seeking realignment of both the left and the right as a promising tactical opportunity to assert that they and their erstwhile enemies were actually engaged in a shared project. Activists thus came to see conspiracy theory not as the province of an exclusively left or right politics, but as an autonomous and contestable cultural space. This, in turn, led to cross-partisan encounters. (shrink)

International relations scholars and public administration experts associate deep states with authoritarian regimes, such as Egypt, Turkey, Pakistan and pre-civil-war Syria. Dystonia is a movement disorder that can have a debilitating impact on motor functions and quality of life. There are 250,000 cases in the United States, most with childhood onset. Due to the limited effectiveness and side effects of available treatments, pediatric deep brain stimulation has emerged as an intervention for refractory dystonia. However, there is limited clinical and neuroethics research in this area of clinical practice. This paper examines whether it is ethically justified to offer pDBS to children (...) with refractory dystonia. Given the favorable risk-benefit profile, it is concluded that offering pDBS is ethically justified for certain etiologies of dystonia, but it is less clear for others. In addition, various ethical and policy concerns are discussed, which need to be addressed to optimize the practice of offering pDBS for dystonia. Strategies are proposed to help address these concerns as pDBS continues to expand. (shrink)

Accurate solar irradiance prediction plays an important role in ensuring the security and stability of renewable energy systems. Solar irradiance modeling is usually a time-dependent dynamic model. As a new kind of recurrent neural network, echo state network shows excellent performance in the field of time series prediction. However, the memory length of classical ESN is fixed and finite, which makes it hard to map sufficient features of solar irradiance with long-range dependency. Therefore, a novel deep echo (...) class='Hi'>state network with variable memory pattern is proposed in this brief. VMP-DESN consists of multiple connected reservoirs in series, and there exist different types of memory modes in VMP-DESN. To remember more input history information in the states, the time delay links are added in each reservoir and between every two reservoirs. The VMP-DESN is more flexible to deal with different input signals due to its variable memory modes in the reservoir states. Additionally, the effect of different memory patterns on the VMP-DESN performance is discussed in detail, including the antidisturbance ability, memory capacity, and prediction accuracy. Finally, the effectiveness of VMP-DESN is evaluated by predicting the real solar irradiance task. (shrink)

The concept of futility is sometimes regarded as a cloak for medical paternalism in that it rolls together medical and value judgments. Often, despite attempts to disambiguate the concept, that is true and it can be applied in such a way as to marginalize the real interests of a patient. I suggest we replace it with a conceptual toolkit that includes physiological futility, substantial benefit (SB), and the risk of unacceptable badness (RUB) in that these concepts allow us to articulate (...) what is at stake in ethical judgments where outcomes are crucial in determining what should be done. A pre-test/post-test, intervention-versus-control experimental design was used to examine the effects, mechanisms and moderators of deep breathing on state anxiety and test performance in 122 Primary 5 students. Taking deep breaths before a timed math test significantly reduced self-reported feelings of anxiety and improved test performance. There was a statistical trend towards greater effectiveness in reducing state anxiety for boys compared to girls, and in enhancing test performance for students with higher autonomic reactivity in test-like situations. (...) The latter moderation was significant when comparing high-versus-low autonomic reactivity groups. Mediation analyses suggest that deep breathing reduces state anxiety in test-like situations, creating a better state-of-mind by enhancing the regulation of adaptive-maladaptive thoughts during the test, allowing for better performance. The quick and simple technique can be easily learnt and effectively applied by most children to immediately alleviate some of the adverse effects of test anxiety on psychological well-being and academic performance. In this study, the focus was on the development of green energy and future prediction for the consumption of current energy sources and green energy development using an improved deep learning algorithm. In addition to the analysis of the current energy consumption used for the natural gas and oil as fuel, deep neural network algorithm is used to train the system as well as to process the data obtained previously, ranging from literature from the year 2003 until the (...) year 2019, for consumption of fuel. Also, using the proposed algorithm to predict the development of green energy consumption till 2030 is presented in terms of solar and wind generators. The resulting study also focuses on depletion of energy currently used or pollution caused because of it. The green energy controlling issue can take effect by using multiple layers of handling different features extracted from different sources and then learning the system to control it.This study aims to take advantage of carbon emissions to reduce their impact and dependence in the future on environmentally friendly renewable energies. Predicting the correct and precise amount of energy consumption and increasing the amount of environmentally friendly energy lead to a healthy ecosystem. The expected green energy consumption in the future is almost 78.25 EJ in 2030 and will be, in total energy average, 56% in 2045. The aim is to reduce dependency on costly and environmentally harmful fuels. (shrink)

Adopting deep approaches to learning can have a profound impact on learning outcomes. The extent of change in the learning approach could be attributed to the effect of contextual factors. After a substantive review, it was found that research interested in investigating the longitudinal effect of deep approaches to learning on learning outcomes were rarely directly concerned with the longitudinal state and trend of the approach itself. Moreover, the limitations of past analytical methods, has not been appropriately (...) acknowledged. This study examined deep approaches to learning in students from kindergarten to third grade using a multilevel structural equation modeling indicator-specific growth model. This was used to highlight the methodological issues to investigate the effect of context and the states, stability, and changes in deep approaches to learning over this developmental period. The current study comprised of a large-scale sample of 21,490 kindergarteners in kindergarten. Results showed: there was a contextual effect on the longitudinal changes of deep approaches to learning; deep approaches to learning was high but showed considerable individual differences; most indicators relating to deep approaches to learning declined, whereas the trend were relatively stable over time. Deep legal change occurs when, without legal justification, one legal rule is replaced by another. While often ignored in legal theory, these rule-breaking normative transformations are common and significant enough to warrant careful attention. In this thesis, I analyse the structure of deep legal change and discuss how a philosophically rigorous jurisprudence should approach a legal phenomenon that appears to be legally inexplicable. In particular, I focus on the implications of rule-breaking rule-changes for our conception of courts and (...) legal reasoning. To understand why deep legal change occurs, we need to provide a normative explanation that clarifies why courts, as courts, have reasons to break the law. My solution takes me to G. W. F. Hegel’s Philosophy of Right. Hegel invites us to think of the judicial process as being grounded in two institutional values: the establishment of legal security and the protection of the “universal interest” of the state. Sometimes, these two values come into conflict. In these moments, established legal rules give way to fundamental state-interests. With Hegel, we can say that it is inevitable that political reasons periodically override legal reasons. In deep legal change, the fundamentally political character of law becomes visible. While reminiscent of Carl Schmitt and the literature on emergency powers, this theory of law is much closer to the reason-of-state tradition. Against the background of several historical case studies and contemporary scholarship, I demonstrate the novelty, coherence, and explanatory power of a Hegelian account of deep legal change. Deep ecology is an ecological philosophy that promotes an ecocentric lifestyle to remedy the problems of depleting resources and planetary degradation. An integral part of this ecosophy is the process of forming a metaphysical connection to the earth, referred to as self-realisation; an unfolding of the self out into nature to attain a transcendental, non-egoic state. Findings from our research indicate that secondary school students in environment clubs align with the principles of deep ecology, and show a (...) capacity to become student eco-philosophers, and they report empathy for becoming ecocentric beings. Deep ecology's biocentric philosophy rejects the anthropocentrism of mainstream environmentalism. Biocentrism holds that all life has inherent value and, as such, is worthy of respect and protection. Deep ecology's action strategy emerges from disgust with the compromises made by mainstream environmentalism. Deep ecologists tend toward confrontational actions such as blockades, “tree sits,” and “ecotage” (“monkey wrenching” or covert direct action). Earth First! in the United States, and Rainforest Action Network at the international level, are two well-known (...) class='Hi'>deep ecology groups. Bound together in a complex relationship, deep ecology is both dependent on and antagonistic toward the life sciences. As yet, there is no explicit, deep ecological statement for scientific reform. But there have been scientific developments cheering to deep ecologists, including the development and growth of the new field of conservation biology. In his 2014 article “Motivations for Relativism as a Solution to Disagreements”, Steven Hales argues that relativism is a plausible disagreement resolution strategy for epistemically irresolvable disagreements. I argue that his relativistic strategy is not adequate for disagreements of this kind, because it demands an impossible doxastic state for disputants to resolve the disagreement. Contrarily, Fogelin’s :1–8, 1985) theory of deep disagreement does not run into the same problems. Deep disagreements, according to Fogelin, cannot be resolved through (...) argumentation because the conditions for argumentation are lacking in such contexts. I advance the view that deep disagreements arise due to differences in disputants’ mutually supporting interrelated beliefs. This view avoids the hurdles caused by the tiered structure of support found at the heart of Hales’s view on disagreement: the assumption that belief and perspective can be separated, and that disagreement is located. Introduction As deep brain stimulation (DBS) has shifted to being used earlier during Parkinson’s disease (PD), data is lacking regarding patient specific attitudes, preferences, and factors which may influence the timing of and decision to proceed with DBS in the United States. This study aims to identify and compare attitudes and preferences regarding the earlier use of DBS in Parkinson’s patients who have and have not undergone DBS. Methods We developed an online survey concerning attitudes about DBS and its (...) timing in PD. The survey was distributed nationally in the United States via the Michael J. Fox Foundation Trial Finder, the American Parkinson Disease Association flyers, and as a link on the Parkinson Alliance website. Differences in responses between PD DBS and non-DBS patients were assessed. Results A total of 445 patients with PD met eligibility criteria for the survey of which 160 self-identified as having undergone DBS. Fifty-five percent (n = 124) of non-DBS patients believed that DBS for PD should only be considered after all medication options have been tried. Patients favoring early DBS had fewer concerns regarding the surgery than those favoring later DBS. Conclusion Our findings highlight a variety of important considerations and concerns patients have regarding DBS and its timing. These viewpoints are important aspects of shared decision-making, as they help to identify patients’ preferences, values, and goals, which should enable providers to better navigate, with their patients, the decision path for therapeutic options to consider. Long and David Sedley in their The Hellenistic Philosophers (1987). Conditions are right for the latest assimilation of this philosophical culture, whose conventional dates are those of the Hellenistic age—from the death of Alexander of Macedon in 323 B.C. to the suicide of the last of his successors, Cleopatra VII of Egypt in 31 B.C., but which in fact extends through the second century A.D. In the fracturing of the autonomous Greek city state and the new empires and cosmopolitanism that grew up in its place, in the turning inward of ethical thought and the simultaneous turning outward of science, the Hellenistic age holds up a mirror to our own. Especially congenial to philosophers in this country and in England is the radical skepticism that reaches from Socrates in the fifth century to Sextus Empiricus in the second century A.D.Martha Nussbaum’s The Therapy of Desire is very much a book for this age. Her topic is the “compassionate medical philosophy” of three of [End Page 501] the Hellenistic schools, the Epicurean, the Skeptic, and the Stoic. In focusing sharply on therapy, she leaves to the side—and on the outside—the connections that made Epicurean and Stoic philosophy systematic. But her awareness of the psychiatric possibilities of modern philosophy and what Michel Foucault called a technique de soi provide the contemporary context for the project of this book, at a stage when Freudian psychology is preparing to make its exit at the end of its century. Medicine is the only science that enters this book’s field of vision. Its thirteen studies (some already familiar) concentrate on the practical and therapeutic aims of Hellenistic philosophy and what the author terms the therapy of the passions, particularly the passions of erotic love, anger, and aggression, and fear of the gods. In The Fragility of Goodness: Luck and Ethics in Greek Tragedy and Philosophy (1986), Nussbaum took the side of Aristotle against Plato. Here she takes the part of the Epicurean, Skeptic, and Stoic against the ethical theory of Aristotle, who is impressed to illustrate the practical turn in ethics which Nussbaum explores in Therapy. Plato is glimpsed on the horizon of this book “standing on the rim of the heavens” (p. 23). What links the intellectual program of both Fragility and Therapy is Nussbaum’s keen and sympathetic attention to poetry as a vehicle for philosophy or as a provocation to philosophical engagement. Four of the chapters of this study are devoted to poetry: three to Lucretius’ De Rerum Natura; one to Seneca’s Medea.The ethical project illustrated throughout these studies is formulated by Democritus, who first enunciated the claim: “medicine heals the sickness of bodies; but wisdom rids the soul of its sufferings” (DK 68B31). Each of the Hellenistic schools Nussbaum studies repeated this claim. This contribution aims at describing the experiences of Deep Brain Stimulation (DBS) as discussed on Internet forums. Since the 2000s, increasing attention has been paid to health practices associated with the use of the Internet, whether by medical professionals, public authorities or researchers in the social sciences. We know that Internet is used by patients with Parkinson’s disease, in order to discuss about their lived experiences. This contribution will present how these Internet users address the specific theme of DBS. (...) We will examine how their use and the information sharing associated to it participate in the “shaping” of the lived experience of deep cerebral stimulation. This contribution will be introduced by an analysis of the state-of-the-art in sociological and anthropological international research on the topic of “Internet health”. On the basis of this analysis, it will propose a framework to describe and understand the various uses of the web that are made to give an account of the experience of stimulation. The existence of collective mobilizations in Parkinson’s disease, an old Internet experience and media oppositions concerning certain antiparkinsonian drugs, are conditions which push us to question these themes concerning DBS. This paper is a description, for the first time, of roles played by the users of these Internet forums and the different online activities produced about DBS subject. In an ethnographic perspective, we had to imagine that we were an Internet user seeking patient experiences with this surgical procedure to understand these interactions about DBS. The main goal of this contribution is to describe the attention paid by users for medical information about DBS in the identity and self-help process. The interest of such an approach is to open an anthropological reflection on the place and the role of the medical representations in the testimonies of experience of the disease or the treatments. Emotion-based sentimental analysis has recently received a lot of interest, with an emphasis on automated identification of user behavior, such as emotional expressions, based on online social media texts. However, the majority of the prior attempts are based on traditional procedures that are insufficient to provide promising outcomes. In this study, we categorize emotional sentiments by recognizing them in the text. For that purpose, we present a deep learning model, bidirectional long-term short-term memory, for emotion recognition that takes into (...) account five main emotions. We use our experimental assessments on the emotion dataset to accomplish the emotion categorization job. The datasets were evaluated and the findings revealed that, when compared to state-of-the-art methodologies, the proposed model can successfully categorize user emotions into several classifications. Finally, we assess the efficacy of our strategy using statistical analysis. This research’s findings help firms to apply best practices in the selection, management, and optimization of policies, services, and product information. (shrink)

Repetitive practice is one of the most important factors in improving the performance of motor skills. This paper focuses on the analysis and classification of forearm gestures in the context of violin playing. We recorded five experts and three students performing eight traditional classical violin bow-strokes: martelé, staccato, detaché, ricochet, legato, trémolo, collé, and col legno. To record inertial motion information, we utilized the Myo sensor, which reports a multidimensional time-series signal. We synchronized inertial motion recordings with audio data to (...) extract the spatiotemporal dynamics of each gesture. Applying state-of-the-art deep neural networks, we implemented and compared different architectures where convolutional neural networks models demonstrated recognition rates of 97.147%, 3DMultiHeaded_CNN models showed rates of 98.553%, and rates of 99.234% were demonstrated by CNN_LSTM models. The collected data contained sufficient information to distinguish the bowing techniques studied, and deep learning methods were capable of learning the movement patterns that distinguish these techniques. Each of the learning algorithms investigated produced high classification accuracies which supported the feasibility of training classifiers. The resulting classifiers may provide the foundation of a digital assistant to enhance musicians' time spent practicing alone, providing real-time feedback on the accuracy and consistency of their musical gestures in performance. Deep Self views of moral responsibility have been criticized for positing mysterious concepts, making nearly paradoxical claims about the ownership of one’s mental states, and promoting self-deceptive moral evasion. I defend Deep Self views from these pervasive forms of skepticism by arguing that some criticism is hasty and stems from epistemic injustice regarding testimonies of experiences of alienation, while other criticism targets contingent features of Deep Self views that ought to be abandoned. To aid in this project, (...) I provide original naturalistic analyses of “Self” and “internality” that replace the view’s metaphorical language with common-sensical concepts that make clear their usefulness. (shrink)

Current methods of chaos-based action recognition in videos are limited to the artificial feature causing the low recognition accuracy. In this paper, we improve ChaosNet to the deep neural network and apply it to action recognition. First, we extend ChaosNet to deep ChaosNet for extracting action features. Then, we send the features to the low-level LSTM encoder and high-level LSTM encoder for obtaining low-level coding output and high-level coding results, respectively. The agent is a behavior recognizer for producing (...) recognition results. The manager is a hidden layer, responsible for giving behavioral segmentation targets at the high level. Our experiments are executed on two standard action datasets: UCF101 and HMDB51. The experimental results show that the proposed algorithm outperforms the state of the art. (shrink)

This study suggests a new approach to EEG data classification by exploring the idea of using evolutionary computation to both select useful discriminative EEG features and optimise the topology of Artificial Neural Networks. An evolutionary algorithm is applied to select the most informative features from an initial set of 2550 EEG statistical features. Optimisation of a Multilayer Perceptron is performed with an evolutionary approach before classification to estimate the best hyperparameters of the network. Deep learning and tuning with Long (...) Short-Term Memory are also explored, and Adaptive Boosting of the two types of models is tested for each problem. Three experiments are provided for comparison using different classifiers: one for attention state classification, one for emotional sentiment classification, and a third experiment in which the goal is to guess the number a subject is thinking of. The obtained results show that an Adaptive Boosted LSTM can achieve an accuracy of 84.44%, 97.06%, and 9.94% on the attentional, emotional, and number datasets, respectively. An evolutionary-optimised MLP achieves results close to the Adaptive Boosted LSTM for the two first experiments and significantly higher for the number-guessing experiment with an Adaptive Boosted DEvo MLP reaching 31.35%, while being significantly quicker to train and classify. In particular, the accuracy of the nonboosted DEvo MLP was of 79.81%, 96.11%, and 27.07% in the same benchmarks. Two datasets for the experiments were gathered using a Muse EEG headband with four electrodes corresponding to TP9, AF7, AF8, and TP10 locations of the international EEG placement standard. The EEG MindBigData digits dataset was gathered from the TP9, FP1, FP2, and TP10 locations. (shrink)

Background Deep brain stimulation is approved for treating refractory obsessive-compulsive disorder in adults under the US Food and Drug Administration Humanitarian Device Exemption, and studies have shown its efficacy in reducing symptom severity and improving quality of life. While similar deep brain stimulation treatment is available for pediatric patients with dystonia, it is not yet available for pediatric patients with obsessive-compulsive disorder, although soon could be. The prospect of growing indications for pediatric deep brain stimulation raises several (...) ethical concerns relating to bodily integrity, the ability to offer informed assent, and the role pediatric patients play in the decision-making process. Objective The aim of this study is to solicit and assess the views of stakeholders (children, parents, clinicians) on pediatric assent, autonomy, and bodily integrity in the context of potential pediatric deep brain stimulation for obsessive-compulsive disorder. Methods Semi-structured interviews were conducted with pediatric obsessive-compulsive disorder patients ( n = 21), caregivers of pediatric obsessive-compulsive disorder patients aged 14–18 ( n = 19), and clinicians with experience treating refractory obsessive-compulsive disorder ( n = 25). Interviews were transcribed and coded in MAXQDA 2018 and 2020 software and processed for thematic content analysis to isolate and compare specific themes. Results A majority of respondents (74%, 48/65) across all three stakeholder groups voiced that the decision-making process should be collaborative and involve everyone (clinicians: 84% or 21/25, caregivers 71% or 15/21, and patients 63% or 12/19). We identified a split between respondents’ views on who should have the final say in the event of disagreement (38% or 25/65 favored the patient versus 35% or 23/65 favoring caregivers). A split between respondents also emerged concerning the maturity relevant for deep brain stimulation decision-making, with 45% (29/65) favoring developmental maturity (age/physiological development) and 45% (29/65) favoring decisional maturity (capacity to understand and weigh information). A majority of clinicians indicated that they would not move forward with deep brain stimulation without securing patient assent (80% or 20/25), with some stating the only exception is if patient quality of life was very poor and/or they lacked insight. Both caregivers and patients expressed a significant respect for the patient's right to bodily integrity, with 67% of caregivers (14/21) and 68% of patients (13/19) justifying patient involvement in decision-making specifically with reference to infringements of bodily integrity. Conclusion Our findings demonstrate that despite broad agreement across stakeholders that the decision-making process for pediatric deep brain stimulation for obsessive-compulsive disorder should be collaborative and somehow involve pediatric patients, there is disagreement about what this process entails and what factors determine patient involvement in the process. However, there is agreement that children have a right to bodily and brain integrity, which should only be infringed upon in rare circumstances. (shrink)

There is more than one way to think. Most people are familiar with the systematic, rule-based thinking that one finds in a mathematical proof or a computer program. But such thinking does not produce breakthroughs in mathematics and science nor is it the kind of thinking that results in significant learning. Deep thinking is a different and more basic way of using the mind. It results in the discontinuous "aha!" experience, which is the essence of creativity. It is at (...) the heart of every paradigm shift or reframing of a problematic situation. The identification of deep thinking as the default state of the mind has the potential to reframe our current approach to technological change, education, and the nature of mathematics and science. For example, there is an unbridgeable gap between deep thinking and computer simulations of thinking. Many people suspect that such a gap exists, but find it difficult to make this intuition precise. This book identifies the way in which the authentic intelligence of deep thinking differs from the artificial intelligence of "big data" and "analytics." Deep thinking is the essential ingredient in every significant learning experience, which leads to a new way to think about education. It is also essential to the construction of conceptual systems that are at the heart of mathematics and science, and of the technologies that shape the modern world. Deep thinking can be found whenever one conceptual system morphs into another. The sources of this study include the cognitive development of numbers in children, neuropsychology, the study of creativity, and the historical development of mathematics and science. The approach is unusual and original. It comes out of the author's lengthy experience as a mathematician, teacher, and writer of books about mathematics and science, such as How Mathematicians Think: Using Ambiguity, Contradiction, and Paradox to Create Mathematics and The Blind Spot: Science and the Crisis of Uncertainty. This study concerned the role of gestures that accompany discourse in deep learning processes. We assumed that co‐speech gestures favor the construction of a complete mental representation of the discourse content, and we tested the predictions that a discourse accompanied by gestures, as compared with a discourse not accompanied by gestures, should result in better recollection of conceptual information, a greater number of discourse‐based inferences drawn from the information explicitly stated in the discourse, and poorer recognition of verbatim of (...) the discourse. Building upon recent work on epistemic varieties of liberalism, avant-garde political agency and the theory and practice of activism, I claim that a liberal defence of more open borders does not presuppose either indifference to the problem of the deep structural sources of poverty in poorer countries, or the absence of an account of those structures’ transformation. Rather, it is claimed that in addition to the remittance of money and other economic goods to alleviate the symptoms of poverty, more (...) open borders facilitate the remittance of norms and values in terms of which individuals and activist communities in poorer countries may transform the deep structures and institutions that are poverty’s sources. This liberal account of more open borders and deep structural transformation also makes possible the non-coercive discharge of obligations of global justice on the part of wealthier states to the world’s poor, when poorer nations are either unable or unwilling to discharge them themselves. Focusing, in part, on the influence of Jurassic-era seas on the philosophical beliefs of the Hopi and Zuni cultures of the American Southwest, the article investigates the fluid cognitive processes of the human mind and methodically investigates the presence of historical content in works from oracy-based peoples. Formal analyses of the embedded data encoded in historical oral narratives are provided and are based on Barber and Barber’s mytho-linguistic framework. The application of Roger C. Echo-Hawk’s methodology for the assessment of narrative (...) works that can contribute to cultural affiliation in consultations relating to the Native American Graves Protection Repatriation Act is considered as a possible model for similar change in the areas of assessing the relevance of material culture. The necessity of creating a similar critical framework for evaluations of the visual arts of ancient pre-historic era in the United States in keeping with the trend in narrative studies is discussed in support of the development of ‘period eye’ perspectives on which future objective studies may be based. The valid contexts an approximated period eye perspective can provide for more authentic replications of complex metaphorical relationships and cosmographies on the visual plane is demonstrated in the article’s final drawings. Our understanding of the cognitive functions of the human brain has tremendously benefited from the population functional Magnetic Resonance Imaging (fMRI) studies in the last three decades. The reliability and replicability of the fMRI results, however, have been recently questioned, which has been named the replication crisis. Sufficient statistical power is fundamental to alleviate the crisis, by either “going big,” leveraging big datasets, or by “going small,” densely scanning several participants. Here we reported a “going small” project implemented in our (...) department, the Bergen breakfast scanning club (BBSC) project, in which three participants were intensively scanned across a year. It is expected this kind of new data collection method can provide novel insights into the variability of brain networks, facilitate research designs and inference, and ultimately lead to the improvement of the reliability of the fMRI results. I think Ongaro is right to argue that the biopsychosocial model requires an account of these three also being one—integrated—and that systems theoretic concepts such as dynamic, nonlinear causation (...) are helpful for capturing this. In a recent paper (Bolton, 2023) I have described Engel’s original (1997) paper as visionary but programmatic: visionary in seeing radical theory change afoot, citing von Bertalanffy’s new general system theory, but programmatic because the changes had not happened. The systems theoretic approach applies across the life, behavioral and social sciences, and typically envisages multiple interacting systems and causal pathways, including feedback and feedforward mechanisms, within and between systems. However, linear mechanisms are less complicated to research, and early social epidemiology used standard linear regression models (e.g., see the Whitehall II study [Marmot et al., 1991]). Such models do not capture non-linear connections in which the dependent variable also functions over time as an independent variable. However, the background theory typically would not exclude nonlinear dynamic causation, but on the contrary envisages it. Lower social status raises risk of ill health in a linear model, but it is also a plausible hypothesis that ill health in turn raises risk of lowering social status, which is testable in another or in a composite linear model. As well as social epidemiology, another large-scale theory change was underway at the time Engel wrote, the so-called cognitive revolution in psychology. Axiomatic in this shift was the idea that behavior is a response to the environment as perceived, thus merging the psychological/behavioral with the environmental, including the social. This latter point bears on Ongaro’s main proposal that the biopsychosocial model should envisage social causes as externalist, distinguished from the bio-psychological orientation of current psychiatry—and will be picked up below.Paper 2 makes some bold claims about the positive therapeutic value of construing causes [End Page 321] of mental health problems in external terms that have shared social meaning. These claims are made for functional neurological problems, but with extension to other kinds, in relation to a culture in which the shared social meanings refer to spirit possession, which, as a general type, is found in many non-modern Western cultures, as well as now, in our globalized world, in subcultures worldwide. One of the author’s claims is that within the spirit possession framework the shared framework of external causes enables society to adapt to the problem of living, to make it manageable, by shifting the social environment around the patient. As appropriate for ethnomethodology, there is no reference to controlled studies of outcome or mechanisms and I follow this immersed observational methodology here, with an experiential story that illustrates absence of personal and social acceptance and manageability of problematic conditions. While wandering recently with a friend in the mountains in the northern part of Africa, the local guide, in conversation in the local language and in English, having found out that two of us worked in “psychiatry/psychology,” asked us what we knew about; our brief reply included “obsessive-compulsive disorder” (OCD) and “anxiety”; the guide responded by asking whether OCD was washing hands a lot, and then wanted to discuss with us two people he knew well. One was a relation engaged, since many years, for many hours a day washing and cleaning, avoiding many daily tasks, sometimes childcare; friends and wider kinship structures had tried everything, except modern Western psychiatry (which was unavailable anyway), and, to put it briefly and colloquially, they were not at all happy about the person’s state and wished the person would get better. This person lived at a distance and we could not see them, so we do not know their view, but to judge by people with definite or probable OCD that we are familiar with, they may or may not have been happy in their reclusive state. Deep brain stimulation (DBS) of the globus pallidus interna and subthalamic nucleus has restored some degree of motor control in many patients in advanced stages of Parkinson’s disease. DBS has also been used to treat dystonia, essential tremor (progressive neurological condition causing trembling), chronic pain, obsessive-compulsive disorder, Tourette’s syndrome, major depressive disorder, obesity, cerebral palsy, and the minimally conscious state. Although the underlying mechanisms of the technique are still not clear, DBS can modulate underactive or overactive neural circuits (...) and restore disrupted communication between and among groups of neurons in interacting regions of the brain. This can control and relieve many symptoms associated with a range of neurological and psychiatric disorders.But the procedures of implanting and stimulating the electrodes are brain-invasive and entail significant risks. Some patients receiving DBS have experienced intracerebral hemorrhage, infection, cognitive disturbances such as impulsive behavior, and affective disturbances such as mania. It is not known whether continuous electrical stimulation of the brain would reshape synaptic connectivity and permanently alter neural circuits in ways that may not be salutary. The risk of these effects indicates that DBS should be used only when a patient’s condition is refractory to all other interventions and when there is a high probability that the technique will benefit the patient and improve his or her quality of life. If a patient’s quality of life is poor and all other treatment options have been exhausted, then the likelihood of benefit can justify physicians’ exposing patients to some risk. The clinical and ethical significance of the risk in DBS underscores the obligation of physicians to obtain fully informed consent from patients undergoing the procedure. (shrink)

IntroductionDeep brain stimulation (DBS) is increasingly used to treat the symptoms of various neurologic and psychiatric conditions. People can undergo the procedure during reproductive years but the safety of DBS in pregnancy remains relatively unknown given the paucity of published cases. We thus conducted a review of the literature to determine the state of current knowledge about DBS in pregnancy and to determine how eligibility criteria are approached in clinical trials with respect to pregnancy and the potential for pregnancy.MethodsA (...) literature review was conducted in EMBASE to identify articles involving DBS and pregnancy. Two reviewers independently analyzed the articles to confirm inclusion. Data extracted for analysis included conditions treated, complications at all stages of pregnancy, neonatal/pediatric outcomes, and DBS target. A second search was then conducted using www.clinicaltrials.gov. The same two reviewers then assessed whether each trial excluded pregnant individuals, lactating individuals, or persons of childbearing age planning to conceive. Also assessed was whether contraception had to be deemed adequate prior to enrollment.ResultsThe literature search returned 681 articles. Following independent analysis and agreement of two reviewers, 8 pregnancy related DBS articles were included for analysis. These articles described 27 subjects, 29 pregnancies (2 with subsequent pregnancies), and 31 infants (2 twin pregnancies). There was 1 preterm birth at 35 weeks, and 3 patients who experienced discomfort from the DBS battery (i.e., impulse generator) placement site. All 27 patients had a DBS device implanted before they became pregnant, which remained in use throughout their pregnancy. There was exclusion of pregnant individuals from 68% of 135 interventional trials involving DBS. Approximately 44% of these trials excluded persons of childbearing age not on “adequate contraception” or wishing to conceive in the coming years. Finally, 22% excluded breastfeeding persons.ConclusionThe data from 29 pregnancies receiving DBS treatment during pregnancy was not associated with unexpected pregnancy or post-partum complication patterns. Many clinical trials have excluded pregnant individuals. Documentation of outcomes in larger numbers of pregnancies will help clarify the safety profile and will help guide study designs that will safely include pregnant patients. (shrink)

The report of the President's Council on Bioethics, Human Cloning and Human Dignity, addresses the central ethical, political, and policy issue in human embryonic stem cell research: the moral status of extracorporeal human embryos. The Council members were in sharp disagreement on this issue and essentially failed to adequately engage and respectfully acknowledge each others' deepest moral concerns, despite their stated commitment to do so. This essay provides a detailed critique of the two extreme views on the Council (i.e., embryos (...) have full moral status or they have none at all) and then gives theoretical grounding for our judgment about the intermediate moral status of embryos. It also supplies an account of how to address profound moral disagreements in the public arena, especially by way of constructing a middle ground that deliberately pays sincere respect to the views of those with whom it has deep disagreements. Disorders of Consciousness (DoC) result in profound functional impairment, adversely affecting the lives of a predominantly younger patient population. Currently, effective treatment options for those who have reached chronicity (prolonged symptom duration over 4 weeks) are extremely limited, with the majority of such cases facing life-long dependence on carers and a poor quality of life. Here we briefly review the current evidence on caseload, diagnostic and management options in the United Kingdom (UK), United States of America (USA) and the European (...) Union (EU). We identify key differences as well as similarities in these approaches across respective healthcare systems, highlighting unmet needs in this population. We subsequently present past efforts and the most recent advances in the field of surgical modulation of consciousness through implantable neurostimulation systems. We examine the ethical dilemmas that such a treatment approach may pose, proposing mediating solutions and methodological adjustments to address these concerns. Overall, we argue that there is a strong case for the utilisation of deep brain stimulation (DBS) in the DoC patient cohort. This is based on both promising results of recent clinical trials as well as technological developments. We propose a revitalization of surgical neuromodulation for DoC with a multicenter, multidisciplinary approach and strict monitoring guidelines, in order to not only advance treatment options but also ensure the safeguarding of patients’ welfare and dignity. A brain tumor is an abnormal mass or growth of a cell that leads to certain death, and this is still a challenging task in clinical practice. Early and correct diagnosis of this type of cancer is very important for the treatment process. For this reason, this study aimed to develop computer-aided systems for the diagnosis of brain tumors. In this research, we proposed three different end-to-end deep learning approaches for analyzing effects of local and deep features for (...) brain MRI images anomaly detection. The first proposed system is Directional Bit-Planes Deep Autoencoder which extracts and learns local and direction features. The DBP-DAE by decomposition of a local binary pattern into eight bit-planes extracts are directional and inherent local-structure features from the input image and learns robust feature for classification purposes. The second one is a Dilated Separable Residual Convolutional Network which extracts high and low-level features. The main advantage of this approach is that it is robust and shows stable results regardless to size of image database and to solve overfitting problems. To explore the effects of mixture of local and deep extracted feature on accuracy of classification of brain anomaly, a multibranch convolutional neural network approach is proposed. This approach is designed according to combination of DBP-DAE and DSRCN in an end-to-end manner. Extensive experiments conducted based on brain tumor in MRI image public access databases and achieves significant results compared to state-of-the-art algorithms. In addition, we discussed the effectiveness and applicability of CNNs with a variety of different features and architectures for brain abnormalities such as Alzheimer’s. (shrink)

Research ethics education has taken many forms since federal funding agencies issued regulatory guidance directing those supported by these agencies to complete required training. In the absence of a standard training approach among institutions such as universities, the design and content of courses, workshops, and seminars varies widely. Here we describe a southwestern United States research university program that employed six teaching strategies to assist students in deep learning of ethical principles and behavior. Our purpose was to determine how (...) these strategies impact understanding and empowerment among an interdisciplinary group of graduate students. Results suggested that deep learning design can result in learning that requires students not only to remember and understand key ethics concepts but also to analyze and apply them in a broader ethical domain. Key findings inform design and delivery of future research ethics courses for instructional designers and instructors in higher education. (shrink)

Legal text retrieval serves as a key component in a wide range of legal text processing tasks such as legal question answering, legal case entailment, and statute law retrieval. The performance of legal text retrieval depends, to a large extent, on the representation of text, both query and legal documents. Based on good representations, a legal text retrieval model can effectively match the query to its relevant documents. Because legal documents often contain long articles and only some parts are relevant (...) to queries, it is quite a challenge for existing models to represent such documents. In this paper, we study the use of attentive neural network-based text representation for statute law document retrieval. We propose a general approach using deep neural networks with attention mechanisms. Based on it, we develop two hierarchical architectures with sparse attention to represent long sentences and articles, and we name them Attentive CNN and Paraformer. The methods are evaluated on datasets of different sizes and characteristics in English, Japanese, and Vietnamese. Experimental results show that: (i) Attentive neural methods substantially outperform non-neural methods in terms of retrieval performance across datasets and languages; (ii) Pretrained transformer-based models achieve better accuracy on small datasets at the cost of high computational complexity while lighter weight Attentive CNN achieves better accuracy on large datasets; and (iii) Our proposed Paraformer outperforms state-of-the-art methods on COLIEE dataset, achieving the highest recall and F2 scores in the top-N retrieval task. In this article we will draw on experiences from our own research on deep brain stimulation of the central thalamus in the minimally conscious state. We describe ethical challenges faced in clinical research involving medical devices and offer several cautionary notes about its funding and the interplay of market forces and scientific inquiry and suggest some reforms.

Sentiment analysis aims to predict sentiment polarities (positive, negative or neutral) of a given piece of text. It lies at the intersection of many fields such as Natural Language Processing (NLP), Computational Linguistics, and Data Mining. Sentiments can be expressed explicitly or implicitly. Arabic Sentiment Analysis presents a challenge undertaking due to its complexity, ambiguity, various dialects, the scarcity of resources, the morphological richness of the language, the absence of contextual information, and the absence of explicit sentiment words in an (...) implicit piece of text. Recently, deep learning has obviously shown a great success in the field of sentiment analysis and is considered as the state-of-the-art model in Arabic Sentiment Analysis. However, the state-of-the-art accuracy for Arabic sentiment analysis still needs improvements regarding contextual information and implicit sentiment expressed in different real cases. In this paper, an efficient Bidirectional LSTM Network (BiLSTM) is investigated to enhance Arabic Sentiment Analysis, by applying Forward-Backward encapsulate contextual information from Arabic feature sequences. The experimental results on six benchmark sentiment analysis datasets demonstrate that our model achieves significant improvements over the state-of-art deep learning models and the baseline traditional machine learning methods.
