Threat and power may often be present, but how they lead to the appearance of behaviors that meet certain diagnostic criteria, is, as Morgan states, unclear at best and only a partial explanation for psychiatric disorders (it explains some behaviors, not all). I suggest we view threat as one type of a broader category, which I propose to term 'Vulnerabilization' (Carel & Kidd, 2021).Vulnerabilization refers to the effects of interactions and actions that impact on people by harming them and making them more vulnerable to future adversity. For example, removing a struggling child from a classroom causes them to miss the lesson, and if done repeatedly, harms their capacity to continue studying. Leaving a weak patient unsupervised in the wrong kind of bed may cause them to fall and become more vulnerable to disease and infection. Failing to organize accommodation for an inmate about to be released from prison forces them onto the streets and makes them more vulnerable to reoffending. The point of the term is to draw attention to the fact that one is not born vulnerable, to paraphrase de Beauvoir, [End Page 73] but becomes vulnerabilized through acts and omissions of others. Such acts and omissions often take place under the auspices of an institution—health care, social care, education, criminal justice, and so on—and are therefore intimately tied to ethoi and conduct of institutions (for a full analysis see Carel & Kidd, 2021).Vulnerabilization is a common theme in the testimonies of people who have negative experiences of social institutions, including, of course, health care institutions. Ideally, health care systems, schools, courts, and other major institutions scaffold and protect our agency, efficiently support our endeavors, and fairly and reliably disperse social and epistemic resources. But while some people have reliably satisfying experiences of social institutions, others find that many, if not all, of the institutions they encounter abandon, ignore, silence, harm, or even kill them and people like them (this is not an exaggeration: inquiries such as the Francis Report (2013) on the Mid Staffordshire patient neglect case and the Sheldon Review (n.d.) provide plentiful examples of that).1 When vulnerable people encounter institutions, they are often at an epistemic and communicative disadvantage that makes their interactions with institutions a source of suffering and injustice and indeed makes such people even more vulnerabilized. They may be affected by poverty, disability (especially mental ill health and learning disabilities), trauma, domestic violence, ill health, racism, sexism, ageism, or other sources of disadvantage, prejudice and discrimination; and, of course, they can be disadvantaged by a combination of such factors. Seeing power as the source of much suffering, social injustice and poor mental health is, of course, entirely in line with this view and I agree with Morgan that Johnstone et al. In this regard, I assume two levels of analysis in Spinoza interpretation: the historic-critical and the philosophical one. My aim is to find out the relationships between these two levels, and also the differences of Spinoza’s interpretation with others, in particular, that one developed by Lodewijk Meyer in his Philosophia S. Scripturae Interpres.

This paper sketches how late seventeenth-century Dutch anatomists used printed publications to advertise their anatomical preparations, inventions and instructional technologies to an international clientele. It focuses on anatomists Frederik Ruysch and Lodewijk de Bils , inventors of two separate anatomical preparation methods for preserving cadavers and body parts in a lifelike state for decades or centuries. Ruysch's and de Bils's publications functioned as an ‘advertisement’ for their preparations. These printed volumes informed potential customers that anatomical preparations were aesthetically pleasing (...) and scientifically important but did not divulge the trade secrets of the method of production. Thanks to this strategy of non-disclosure and advertisement, de Bils and Ruysch could create a well-working monopoly market of anatomical preparations. The ‘advertising’ rhetorics of anatomical publications highlight the potential dangers of equating the growth of print culture with the development of an open system of knowledge exchange. (shrink)

This article offers an account of the philosophy of language expounded in the Cogitata de interpretatione (1692) of the Dutch philosopher Johannes De Raey (1620-1702). In this work, De Raey provided a theory of the formation and meaning language based on the metaphysics of René Descartes. De Raey distinguished between words signifying passions and sensations, ideas of the intellect, or external things. The aim of this article is to shift away the discussion of De Raey’s critique on the application of (...) the language of practical matters by Lodewijk Meijer and Spinoza, and to redirect modern interpretations to the originality of De Raey’s own reflections on the uses of language. In his linguistic thought, De Raey criticized philosophers such as Hobbes, who supposedly deprived Aristotelian terminology of any reference and meaningful use. The analysis of De Raey testifies to the emergence of the philosophy of language out of the double traditions of logic and metaphysics. It is to be interpreted as an effect of the emergence of an alternative worldview to Aristotelianism. This called for an update of the semantic catalogue of philosophy and practical disciplines. ‘As in a view from afar’. Vondel’s epistemological objections to Spinoza’s circle From a philosophical and theological stance, Spinoza and Vondel are traditionally portrayed as downright antipodes. Whereas Spinoza is seen as a pre-eminent representative of ‘radical Enlightenment’, Vondel is generally considered as a rather reactionary author who does not fit at all in this radical version of Enlightenment. The central aim of this article is to modify this generally agreed perception. On the basis of recent historical research in the (...) relation between the circle of Spinoza and Vondel, the affinities between both authors can be brought to the fore, in particular when looking at their writings on the relation between politics and religion. The gap between their views becomes only apparent when their respective views of God are at stake: there is an undeniable contrast between Spinoza’s naturalist pantheism and Vondel’s classic theism. In order to understand that very difference, the focus will be laid on two writings, conceived in more or less the same period: Vondel’s philosophical-theological poem Bespieghelingen van Godt en Godsdienst and Spinoza’s Verhandeling over de verbetering van het verstand (Treatise on the Emendation of Understanding). The central difference between the two authors in these writings relates to their methodological approach of existential questions about God and salvation, in particular to their dissent concerning the epistemological role of the geometrical method and the specific role of language. One of the hypotheses I propound in this respect is that Vondel’s relation to Lodewijk Meyer has been of utmost importance in order to explain their different methodological positions. What is illness? Is it a physiological dysfunction, a social label, or a way of experiencing the world? How do the physical, social and emotional worlds of a person change when they become ill? And can there be well-being within illness? Chronic fatigue syndrome or myalgic encephalomyelitis remains a controversial illness category. This paper surveys the state of knowledge and attitudes about this illness and proposes that epistemic concerns about the testimonial credibility of patients can be articulated using Miranda Fricker’s concept of epistemic injustice. While there is consensus within mainstream medical guidelines that there is no known cause of CFS/ME, there is continued debate about how best to conceive of CFS/ME, including disagreement about how to interpret clinical studies of treatments. Against this background, robust qualitative and quantitative research from a range of countries has found that many doctors display uncertainty about whether CFS/ME is real, which may result in delays in diagnosis and treatment for patients. Strikingly, qualitative research evinces that patients with CFS/ME often experience suspicion by health professionals, and many patients vocally oppose the effectiveness, and the conceptualization, of their illness as psychologically treatable. We address the intersection of these issues and healthcare ethics, and claim that this state of affairs can be explained as a case of epistemic injustice. We find evidence that healthcare consultations are fora where patients with CFS/ME may be particularly vulnerable to epistemic injustice. We argue that the marginalization of many patients is a professional failure that may lead to further ethical and practical consequences both for progressive research into CFS/ME, and for ethical care and delivery of current treatments among individuals suffering from this debilitating illness. We develop a broader, more fine-grained taxonomy of forms of ‘transformative experience’ inspired by the work of L.A. Paul. Our vulnerability to such experiences arises, we argue, due to the vulnerability, dependence, and affliction intrinsic to the human condition. We use this trio to distinguish a variety of positively, negatively, and ambivalently valenced forms of epistemically and/or personally transformative experiences. Moreover, we argue that many transformative experiences can arise gradually and cumulatively, unfolding over the course of longer periods of time.

But unlike death, illness, and in particular the experience of being ill, has received little philosophical attention. Patient support tools have drawn on a variety of disciplines, including psychotherapy, social psychology, and social care. One discipline that has not so far been used to support patients is philosophy. This paper proposes that a particular philosophical approach, phenomenology, could prove useful for patients, giving them tools to reflect on and expand their understanding of their illness. I present a framework for a resource that could help patients to philosophically examine their illness, its impact on their life, and its (...) meaning. I explain the need for such a resource, provide philosophical grounding for it, and outline the epistemic and existential gains philosophy offers. Illness often begins as an intrusion on one’s life but with time becomes a way of being. I argue that this transition impacts on core human features such as the experience of space and time, human abilities, and adaptability. It therefore requires philosophical analysis and response. The paper uses ideas from Husserl and Merleau-Ponty to present such a response in the form of a phenomenological toolkit for patients. The toolkit includes viewing illness as a form of phenomenological reduction, thematizing illness, and examining illness as altering the ill person’s being in the world. I suggest that this toolkit could be offered to patients as a workshop, using phenomenological concepts, texts, and film clips to reflect on illness. I conclude by arguing that examining illness as a limit case of embodied existence deepens our understanding of phenomenology. (shrink)

Phenomenology is a useful methodology for describing and ordering experience. As such, phenomenology can be specifically applied to the first person experience of illness in order to illuminate this experience and enable health care providers to enhance their understanding of it. However, this approach has been underutilized in the philosophy of medicine as well as in medical training and practice. This paper demonstrates the usefulness of phenomenology to clinical medicine. In order to describe the experience of illness, we need a (...) phenomenological approach that gives the body a central role and acknowledges the primacy of perception. I present such a phenomenological method and show how it could usefully illuminate the experience of illness through a set of concepts taken from Merleau-Ponty. His distinction between the biological body and the body as lived, analysis of the habitual body, and the notions of motor intentionality and intentional arc are used to capture the experience of illness. I then discuss the applications this approach could have in medicine. These include narrowing the gap between objective assessments of well-being in illness and subjective experiences which are varied and diverse; developing a more attuned dialogue between physicians and patients based on a thick understanding of illness; developing research methods that are informed by phenomenology and thus go beyond existing qualitative methods; and providing medical staff with a concrete understanding of the impact of illness on the life-world of patients. ABSTRACT This paper offers an account of institutional testimonial justice and describes one way that it breaks down, which we call institutional opacity. An institution is opaque when it becomes resistant to epistemic evaluation and understanding by its agents and users. When one cannot understand the inner workings of an institution, it becomes difficult to know how to comport oneself testimonially. We offer an account of an institutional ethos to explain what it means for an institution to be testimonially just; (...) we then describe how an ethos of institutional testimonial justice can break down when the institution becomes opaque. An opaque institution is especially problematic for individuals and groups already rendered epistemically vulnerable during their interactions with that institution, which we call epistemically vulnerabilised individuals. We articulate the features of an encounter between an epistemically vulnerabilised individual and an opaque institution. We end by tracing ameliorative strategies that could help repair a deteriorated institutional ethos of testimonial justice. (shrink)

We propose that certain forms of chronic illness can be transformative experiences, in the sense described by L.A. Paul.

In this paper I explore the tacit underlying sense of bodily certainty that characterizes normal everyday embodied experience. I then propose illness as one instance in which this certainty breaks down and is replaced by bodily doubt. I characterize bodily doubt as radically modifying our experience in three ways: loss of continuity, loss of transparency, and loss of faith in one's body. I then discuss the philosophical insights that arise from the experience of bodily doubt. The paper uses a Humean (...) framework with regards to bodily certainty, treating it as a taken for granted tacit aspect of normal experience. I argue that although bodily certainty is not rationally justifiable, we are nonetheless unable to reject it. Bodily certainty is thus revealed to be part of our brute animal nature. I conclude by suggesting that the study of pathology is a philosophical method useful for illuminating tacit aspects of experience. (shrink)

Can one be ill and happy? I use a phenomenological approach to provide an answer to this question, using Merleau-Ponty’s distinction between the biological and the lived body. I begin by discussing the rift between the biological body and the ill person’s lived experience, which occurs in illness. The transparent and taken for granted biological body is problematised by illness, which exposes it as different from the lived experience of this body. I argue that because of this rift, the experience (...) of illness cannot be captured within a naturalistic view and propose to supplant this view with a phenomenological approach. The latter approach accounts for changes in the ill person’s relationship to her social and physical world. These changes, I argue, cannot be captured by a naturalistic perspective. I then propose the notion of health within illness as a useful concept for capturing the experience of well-being reported by some ill people. I present empirical evidence for this phenomenon and assess its philosophical significance. Finally, I suggest that adaptability and creativity are two common positive responses to illness, demonstrating that health within illness is possible. The three elements combined – the transformed body, health within illness and adaptability and creativity – serve as the basis for a positive answer to the question posed above. (shrink)

The phenomenological method has been fruitfully used to study the experience of illness in recent years. However, the role of illness is not merely that of a passive object for phenomenological scrutiny. I propose that illness, and pathology more generally, can be developed into a phenomenological method in their own right. I claim that studying cases of pathology, breakdown, and illness offer illumination not only of these experiences, but also of normal function and the tacit background that underpins it. In (...) particular, I claim that the study of embodiment can be greatly enhanced, and indeed would be incomplete, without attending to bodily breakdown and what I term bodily doubt. I offer an analogy between illness and Husserl’s epoché, suggesting that both are a source of distancing, and therefore motivate a reflective stance. (shrink)

In this article, I propose that illness is philosophically revealing and can be used to explore human experience. I suggest that illness is a limit case of embodied experience. By pushing embodied experience to its limit, illness sheds light on normal experience, revealing its ordinary and thus overlooked structure. Illness produces a distancing effect, which allows us to observe normal human behavior and cognition via their pathological counterpart. I suggest that these characteristics warrant illness a philosophical role that has not (...) been articulated. Illness can be used as a philosophical tool for the study of normally tacit aspects of human existence. I argue that illness itself can be integral to philosophical method, insofar as it facilitates a distancing from everyday practices. This method relies on pathological or limit cases to illuminate normally overlooked aspects of human perception and action. In this paper we propose that our understanding of pathocentric epistemic injustices can be enriched if they are theorised in terms of predicaments. These are the wider socially scaffolded structures of epistemic challenges, dangers, needs, and threats experienced by ill persons due to their particular emplacement within material, social, and epistemic structures. In previous work we have described certain aspects of these predicaments - pathocentric epistemic injustices, pathophobia, and so on. In fact, I can't immediately think of anyone I've worked with for whom it did not, in the end, turn out to be true.1The two commentaries on my paper (...) point to two levels where this sense-making might operate; a structural and an individual level.Havi Carel (2023) draws attention to the important concept of power in the Power Threat Meaning Framework and suggests a widening of the concept of threat to a concept of vulnerabilization that refers to the multiple factors that expose people to institutional neglect and violence that is interlaced with a range of forms of discrimination, neglect and disadvantage. Whilst the main strength of the PTMF approach is to draw attention to the structural importance of power in contributing to forms of psychic distress, the emphasis on a concept of threat tends towards a reactive and reductive concept of power. Carel (2023) helpfully points to a wider concept of power that can be utilised for further work in understanding the impact of societal violence on forms of mental distress.Such a widening of the concept of power does not only rest on a critique of institutions but also an understanding of the ways in which people can be abandoned and neglected by state support of one kind or another. Power operates through a dual logic of exposure and abandonment. Some people are particularly exposed to forms of institutional power from an early age and vulnerabilized by such exposure, but there is also a logic of abandonment within such exposure; the ever present possibility of neglect. Such an operation of power is not univocal (or embedded in one profession) but dispersed and differentially applied. As Didier Fassin (2018, p. 43) writes: "All lives are precarious but certain lives much more so than others—and above all, quite differently."Psychiatric power is one important focus when trying to understand this process of the exposure and abandonment of power, however, as Carel (2023, p. 75) notes, "the medical realm is not its own kingdom." Psychiatry is inserted into a network of dispersed power that has multiple and differential effects, both positive and negative. The focus on the question of psychiatric diagnosis as the apotheosis of societal power neglects the multiple effects of diagnosis or the lack of diagnosis on people who are particularly vulnerable to the operations of power. In situations where institutional care is crumbling and under-resourced the focus on psychiatric care alone as a problem of power risks just compounding such an institutional neglect. [End Page 77] Veena Das (2020) asks the question of what happens before the arrival of the psychiatrist on the scene of care and after her disappearance. Das (2020, p. 174) is interested in the multiple ways that power flows across and between both formal and informal institutions to produce a "milieu of madness." Such flows of power will also apply to increasingly hegemonic psychological formulations of signs of distress as problems in living, as problems in adaptive reactions to adversity. Such formulations are themselves inserted into a nexus of power relations and processes of vulnerabilization and are not somehow free of the taint of other forms of understanding. Indeed, as Carel notes, the process of demedicalization and de-professionalisation can very easily be inserted into the dual logic of exposure and abandonment that I discussed earlier. People can be exposed to power in a wider range of settings as psychic distress is demedicalized but also subjected to abandonment and neglect as mental distress is no longer thought of as an illness but just one of many wider and non-medical problems in living that does not require state support.The structural account of the determinants of mental distress offers an important corrective to individualised and medicalised approaches to meaning, but doesn't completely erase the problem of understanding and interpreting mental distress. La autora del artículo vuelve sobre el debate abierto en tomo a las implicaciones de los conocidos trabajos de Carel Gilligan para la presunta universalidad y pertinencia del llamado «paradigma de KohIberg » del desarrollo moral. Considera que, desde el punto de vista de la teoría feminista, no deben interpretarse los trabajos empíricos de Gilligan en el sentido de la propuesta de una «ética femenina del cuidado» como alternativa a la orientación universalista de la ética normativa de la justicia (...) Una teoría moral «adecuada para la forma de vida de las modernas sociedades complejas» requiere «alguna caracterización universalista de la idea de la imparcialidad y del punto de vista ético». Pero de esta caracterización no se deriva una delimitación del ámbito de lo ético como el ámbito de las cuestiones de justicia por oposición a las «cuestiones evaluativas de la vida buena» que serian consideradas meramente «personales». En opinión de S. Benhabib, Habermas y Kohlberg «confunden el punto de vista de una ética universalita con una definición estricta del ámbito moral», centrado en la justicia. This article takes a linguistic perspective of argumentation, as proposed by Marion Carel and Oswald Ducrot with the “Théorie des blocs sémantiques” (SBT: Semantic Block Theory). This theory argues that the meaning of a linguistic entity is determined by a collection of discourses that this entity calls to mind. Describing the meaning of a word, a syntagm or an utterance amounts to specifying the argumentative linkages (“enchaînements argumentatifs”) allowed by these entities. We propose a semantic and argumentative analysis of (...) syntagms mujer fácil , femme facile [easy woman] and hombre fácil , homme facile [easy man] that, in Romance languages in particular, hold different meanings: both hombre fácil/homme facile describe a man’s character or nature, whereas mujer fácil/femme facile, in their most common usage, imply a certain sexual behavior. We will compare the argumentative linkages that make up the meaning of mujer fácil/femme facile with those of other expressions that are part of the same semantic block. Also, this analysis will connect the proposed description to certain proverbial discourse about women, and it will call attention to the role that these expressions can play in a persuasive strategy. In this paper, I address the effects of social distancing for embodied lived experience in relation to dementia care and experiences of dementia. From a critical phenomenological perspective, I focus specifically on the safety measures of physical distancing and face-masking in pandemic times, asking whether they might risk marginalizing and disembodying people with dementia, especially in isolated healthcare settings. As much as these measures offer physical protection against spreading the virus, I consider how they might disrupt intersubjective processes (e.g., calming (...) social or therapeutic touch) and contribute to “an alienated bodily communication” (Dolezal 2020). From Husserl and Merleau-Ponty’s philosophies of embodiment and enactive theories of intersubjectivity, to more recent phenomenological work on illness, many authors argue that our existential being-in-the-world is embodied in multimodal and intersubjective ways (Fuchs and De Jaeger 2009; Carel 2016; Ratcliffe 2008; Kontos 2011). For this reason, our lived bodies need to stay in touch and positively interact with other lived bodies. Based on empirical studies of multisensory and interactive therapy in nursing homes before and during the COVID-19 pandemic, I emphasize the effects of distant socializing for patients, care, and carers. My aim is to address the broader structures and discursive practices, which have undergone dramatic changes due to the spread of COVID-19. (shrink)

This article examines the philosophical role of illness. It briefly surveys the philosophical role accorded to illness in the history of philosophy and explains why illness merits such a role. It suggests that illness modifies, and thus sheds light on, normal experience, revealing its ordinary and therefore overlooked structure. Illness also provides an opportunity for reflection by performing a kind of suspension (epoché) of previously held beliefs, including tacit beliefs. The article argues that these characteristics warrant a philosophical role for (...) illness. While the performance of most philosophical procedures is volitional and theoretical, however, illness is uninvited and threatening, throwing the ill person into anxiety and uncertainty. As such it can be viewed as a radical philosophical motivation that can profoundly alter our outlook. The article suggests that illness can change the ways in which we philosophise: it may shape philosophical methods and concerns and change one's sense of salience and conception of philosophy. What counts as health or ill health? How do we deal with the fallibility of our own bodies? Should illness and disease be considered simply in biological terms, or should considerations of its emotional impact dictate our treatment of it? Our understanding of health and illness had become increasingly more complex in the modern world, as we are able to use medicine not only to fight disease but to control other aspects of our bodies, whether mood, blood pressure, or cholesterol. This collection of essays foregrounds the concepts of health and illness and patient experience within the philosophy of medicine, reflecting on the relationship between the ill person and society. Mental illness is considered alongside physical disease, and the important ramifications of society's differentiation between the two are brought to light. Health, Illness and Disease is a significant contribution to shaping the parameters of the evolving field of philosophy of medicine and will be of interest to medical practitioners and policy-makers as well as philosophers of science and ethicists. In this paper I respond to Edward Harcourt’s suggestion that human excellences are structured in a way that allows us to see the multiplicity of life forms that can be instantiated by different groups of excellences. I accept this layered model, but suggest that Harcourt’s proposal is not pluralistic enough, and offer three critical points. First, true pluralism would need to take a life-cycle view, thus taking into account plurality within, as well as between, lives. Second, Harcourt’s pluralism still posits (...) physical health as a requirement for excellence, whereas I claim that the challenges of illness give more, not less, opportunity for excellence. Third, I make a more general claim that in certain salient cases it is precisely the absence of excellence that can facilitate virtue. (shrink)
