The informed consent process is the legal embodiment of the fundamental right of the individual to make decisions affecting his or her health., and the patient’s permission is a crucial form of respect of freedom and dignity, it becomes extremely important to enhance the patient’s understanding and recall of the information given by the physician. This statement acquires additional weight when the medical treatment proposed can potentially be detrimental or even fatal. This is the case of thalassemia patients pertaining to (...) class 3 of the Pesaro classification where Allogenic hematopoietic stem cell transplantation remains the only potentially curative treatment. Unfortunately, this kind of intervention is burdened by an elevated transplantation-related mortality risk , equal to 30% according to published reports. In thalassemia, the role of the patient in the informed consent process leading up to HSCT has not been fully investigated. This study investigated the hypothesis that information provided by physicians in the medical scenario of HSCT is not fully understood by patients and that misunderstanding and communication biases may affect the clinical decision-making process. BackgroundBeta thalassemia major is a severe inherited form of hemolytic anemia that results from ineffective erythropoiesis. Allogenic hematopoietic stem cell transplantation (HSCT) remains the only potentially curative therapy. Unfortunately, the subgroup of adult thalassemia patients with hepatomegaly, portal fibrosis and a history of irregular iron chelation have an elevated risk for transplantation-related mortality that is currently estimated to be about 29 percent.DiscussionThalassemia patients may be faced with a difficult choice: they can either continue conventional transfusion and iron chelation therapy or (...) accept the high mortality risk of HSCT in the hope of obtaining complete recovery.Throughout the decision making process, every effort should be made to sustain and enhance autonomous choice. The concept of conscious consent becomes particularly important. The patient must be made fully aware of the favourable and adverse outcomes of HSCT. Although it is the physician's duty to illustrate the possibility of completely restoring health, considerable emphasis should be put on the adverse effects of the procedure. The physician also needs to decide whether the patient is eligible for HSCT according to the "rule of descending order". The patient must be given full details on self-care and fundamental lifestyle changes and be fully aware that he/she will be partly responsible for the outcome.SummaryOnly if all the aforesaid conditions are satisfied can it be considered reasonable to propose unrelated HSCT as a potential cure for high risk thalassemia patients. The desire to rescue these vulnerable human beings is natural. However, the processes required to do so raise serious questions regarding the ethics of embryo adoptions. Underlying the growing epidemic of mental distress and suicidal ideation amongst certain marginalised groups are complex intersections of ecologies and interrelated structures of inequality such as class, culture, race and gender. Through the use of a multiracial feminist framework, the proposed intersectional elaboration technique examines how technology might be designed in ways that explicitly consider intersecting structures of inequality and eco-developmental contexts. The core of this technique involves co-constructing narratives using prompts that directly address specific layers of one's ecology and (...) the interactions across ecological layers – purposely addressing intersecting systems of inequality acting along these ecologies. This article describes the application of the intersectional elaboration technique in co-design research with adolescent Latinas, in a highly urbanised context, towards designing emotional health technology. Findings suggest that intersectional elaborations can serve as a useful generative co-design technique to inform designs that address complex arrangements of intra-ecological conflicts and cultural legitimacy. The COVID-19 pandemic is an unprecedented event entailing long-term consequences on population health and welfare. Those who contracted the coronavirus may have suffered from both physical and mental health issues that unfold the need for tailored intervention strategies. Hence, our study aims to investigate the psychological and social consequences of COVID-19 on a sample of 86 participants, encompassing 43 patients recruited from Bari University Hospital, 19 of whom were hospitalized due to the disease. The remaining 43 were individuals not fallen (...) ill with COVID-19 to date. The investigation yielded significant gender differences in post-traumatic stress symptoms, depression, and representation of interpersonal distance, evaluated through the IES-R, the BDI-II, and the IVAS task, respectively. This pattern of results was not replicated in the control group. In general, participants who reported having experienced the most intense post-traumatic symptoms also presented a greater mood deflection and, more specifically, within the clinical group women obtained the highest scores on both scales. Women reported higher IES-R and BDI-II scores compared to men, that could indicate that women who have contracted COVID-19 are more exposed to post-traumatic and depressive symptoms. Our results also showed a significant effect of COVID-19 on IPD with a tendency of disease-experienced individuals to increase their preferred IPD from adults, children, and elderly people. Regarding gender differences in mood and proxemic behavior, a correlation between depressive symptoms and probable PTSD and a further correlation between probable PTSD and greater IPD were found in women from both clinical and control group. Overall, these findings might contribute to a better understanding of gender-based implications of the current pandemic on mental health, also leading to the development of integrated yet personalized intervention strategies. However, the original meaning of Confucius's teachings have been filtered and interpreted by the commentaries of Confucianists of later ages, particularly the Neo-Confucianists of the Song dynasty, not altogether without distortion.In this monumental translation by Professor D. C. Lau, an attempt has been made to interpret the sayings as they stand. The corpus of the sayings is taken as an organic (...) whole and the final test of the interpretation rests on the internal consistency it exhibits. In other words, _The Analects_ is read in the light of _The Analects._This results in a truer understanding of Confucius' thought than the traditional interpretation and paves the way for a re-assessment of its importance in the history of Chinese thought and its relevance to the present day world.This volume also contains an introduction to the life and teachings of Confucius, and three appendices on the events in the life of Confucius, on his disciples, and on the composition of _The Analects._. An anonymous biography of Lucan known as the Life of Vacca attributes to the poet the composition of a work called Siluae. This information has been accepted by scholars with regard to both Lucan and Statius, thus transforming Lucan into a predecessor of Statius’ Siluae. This article seeks to demonstrate that neither the manuscript tradition of Lucan's biography nor alleged references to Lucan's Siluae in Statius’ collection substantiate the affirmation that Lucan composed a work called Siluae. It is argued (...) that the Life of Vacca is most probably late and its very long catalogue of works (mostly problematic or otherwise unknown) aggrandizes Lucan in response to the need for a commendatory biography of the poet when he became a school author. Pomponio Leto is responsible for perpetuating the mention of Lucan's Siluae by including it in his influential biography of Lucan (the first to reach the printed press). It is further argued that the circumstances of the tradition of Statius’ Siluae explain Leto's acceptance of Lucan's work of the same title. Leto was working on a commentary on the recently rediscovered Siluae, which explains his inclusion of a work called Siluae in his biography of Lucan, a great poet who had preceded Statius. Finally, this article deconstructs the theories that find references to Lucan's Siluae in Statius’ Siluae or that justify Statius’ silence about Lucan's Siluae with its political motifs. (shrink)

This is Part 1 of a paper on fibred semantics and combination of logics. It aims to present a methodology for combining arbitrary logical systems L i , i ∈ I, to form a new system L I . The methodology `fibres' the semantics K i of L i into a semantics for L I , and `weaves' the proof theory (axiomatics) of L i into a proof system of L I . There are various ways of doing this, we (...) distinguish by different names such as `fibring', `dovetailing' etc, yielding different systems, denoted by L F I , L D I etc. Once the logics are `weaved', further `interaction' axioms can be geometrically motivated and added, and then systematically studied. The methodology is general and is applied to modal and intuitionistic logics as well as to general algebraic logics. We obtain general results on bulk, in the sense that we develop standard combining techniques and refinements which can be applied to any family of initial logics to obtain further combined logics. The main results of this paper is a construction for combining arbitrary, (possibly not normal) modal or intermediate logics, each complete for a class of (not necessarily frame) Kripke models. We show transfer of recursive axiomatisability, decidability and finite model property. Some results on combining logics (normal modal extensions of K) have recently been introduced by Kracht and Wolter, Goranko and Passy and by Fine and Schurz as well as a multitude of special combined systems existing in the literature of the past 20-30 years. One of Thomas Kuhn's profoundest arguments is introduced in the 1970 “Postscript” to The Structure of Scientific Revolutions . Kuhn is discussing the idea of a “disciplinary matrix” as a more adequate articulation of the “paradigm” notion he'd introduced in the first, 1962, edition of his famous work . He notes that one “element” of disciplinary matrices is likely to be common to most or even all such matrices, unlike the other elements which serve to distinguish specific disciplines and sub-disciplines (...) from one another. This is the element which he calls “values”, which, as he notes , being common to a number of otherwise distinct disciplinary matrices, “do much to provide a sense of community to natural scientists as a whole”. On the other hand, they also do much, and crucially in Kuhn's view, to promote and sustain a healthy diversity among the practitioners who share any specific disciplinary matrix. In particular, Kuhn claims that “individual variability in the application of shared values may serve functions essential to science.”. (shrink)

Since the first decade of the 14th Century, Thomas Aquinas’s disciples have struggled to explain and defend his doctrine of analogy. Analogy after Aquinas: Logical Problems, Thomistic Answers relates a history of prominent Medieval and Renaissance Thomists’ efforts to solve three distinct but interrelated problems arising from their reading both of Aquinas’s own texts on analogy, and from John Duns Scotus’s arguments against analogy and in favor of univocity in Metaphysics and Natural Theology. The first of these three problems concerns (...) Aquinas’s at least apparently disparate statements on whether a name is said by analogy through a single concept or through diverse concepts. The second problem concerns the model of analogy suited for predicating names analogously across the categories of being or about God and creatures. Is “being” said analogously about God and creatures, or substance and accidents, on the model of how “healthy” is said of medicine and an animal, or on the model of how “principle” is said of a point and a line? The third problem comes from outside challenges to Aquinas’s thought, in particular Scotus’ claims that univocal names alone can mediate valid demonstrations, and any demonstration that failed to use its mediating terms univocally would fail by the fallacy of equivocation. Analogy after Aquinas makes a unique contribution to the study of philosophical theology in the tradition of Thomas Aquinas by showing the historical and philosophical connection between these three problems, as well as the variety of solutions proposed by leading representatives of this tradition. “Do Anthropocene narratives confuse an important distinction between the natural and the historical past?” asks Giuseppina D’Oro. D’Oro defends the view that the concept of the historical past is sui generis and distinct from that of the geological past against a new, Anthropocene-inspired challenge to the possibility of a humanistically oriented historiography. She argues that the historical past is not a short segment of geological time, the time of the human species on Earth, but the past investigated from the perspective (...) of a distinctive kind of interest, that of uncovering the norms which governed historical agents in different periods of time. The past for the Egyptologist, or for the Roman historian, is not the same past studied by the palaeontologist or the geologist, not because it is infinitesimal short in comparison to geological time, but because the questions asked by historians concerned with the Egyptian or Roman civilization are not the same kind of questions asked by empirically minded scientists. She argues that the accusation that the distinction between the historical and the natural/geological past, rests on unacceptable form of human exceptionalism is based on the conflation of the concept of the historical past with that of the human past and that keeping alive the nature/culture distinction has important implications for praxis. If the distinction between nature and culture is collapsed, and the corollary that historical agents are not distinct in kind from natural agents (such as yeast and microbes) is accepted, then “the anticipation of the future would become a mere spectator’s sport analogous to the activity of predicting the weather”: collapsing the nature culture distinction, D’Oro argues, undermines the possibility of political action against the very threat (climate change) that motivates Anthropocene narratives in the first instance. Though there exist a few classic articles comparing Aquinas's thought in this letter and elsewhere, and substantial introductions to the De Regno in editions by Eschmann (1949) and Dondaine (1979), The Christian Structure of Politics is the first book-length study of Aquinas's letter to the king of Cyprus. In this respect, the volume makes a very welcome contribution to our understanding of Aquinas's understanding of political order, bringing out a dimension of his thought—its awareness of history and its pedagogical sensitivity—not so apparent in Aquinas's other writings.The book is very much what it presents itself as being, namely, a "close reading" of the text, following the letter's two parts (the second of which [End Page 150] Aquinas left unfinished) paragraph by paragraph, considering various interpretative possibilities and suspending judgment—the words "might" and "perhaps" are no doubt two of the most frequent in the book—until more evidence comes into view, and working up to general principles only at the end. McCormick thus eschews substantial engagement with theoretical issues that would form the backdrop to the discussion (for example, the relation between nature and grace, and between practical and theoretical reason), opting instead to focus on a more immediate engagement with what is presented in the text itself. He also does not make more than passing comments on Aquinas's better-known formulations on political matters from the Summa or Sentences Commentary. On the other hand, the "close reading" does not keep McCormick from discerning a general order in Aquinas's letter, which he divides up thematically according to a proposed schema. His approach shows a sensitivity to the historical context in which Aquinas was writing, even if particular details about Aquinas's addressee and the occasion of composition remain uncertain, but his aim is not simply to produce a historical study. Instead, as the book's title itself already indicates, he investigates the De Regno in order to show that the text offers fundamental principles for a distinctively Christian approach to politics, and thus can shed light even on our contemporary situation.The book is divided into five basic chapters, in addition to a substantial introduction and brief conclusion. The first four chapters follow the order of the De Regno, though the basic lines of division are McCormick's own interpretative proposal. Chapter 1 treats the letter's prooemium and the first two chapters of part one (I.1-2) under the title "The Aristotelian State of Nature," showing that, contrary to ideas found in a certain current of Christian thought, political order is fully natural to man and not simply a practical response to violence. Aristotle began his great study on causes, which he called Metaphysics, with a simple connection to physics: "All men by nature desire to know. An indication of this is the delight we take in our senses." Catholic high school physics teacher Matt D'Antuono makes a similar connection in his own teaching. While discussing the nature of science with his physics students, Matt pointed out that their topic of conversation was technically not science any more. Instead, when they were talking about (...) the nature of science, they were doing philosophy. The students then wanted Matt to teach them some philosophy, and Philosophy Fridays became a classroom tradition. As a consequence, we obtain the existence of the model-companion of the following theories, for $p>0$ a prime number: • $\mathrm {ACF}_p$, $\mathrm {SCF}_{e,p}$, $\mathrm {Psf}_p$, $\mathrm {ACFA}_p$, $\mathrm {ACVF}_{p,p}$ in appropriate languages expanded by arbitrarily many predicates for additive subgroups;• $\mathrm {ACF}_p$, $\mathrm {ACF}_0$ in the language of rings expanded by a single predicate for a multiplicative subgroup;• $\mathrm {PAC}_p$ -fields, in an appropriate language expanded by arbitrarily many predicates for additive subgroups.From an independence relation in T, we define independence relations in $TS$ and identify which properties of are transferred to those new independence relations in $TS$, and under which conditions. This allows us to exhibit hypotheses under which the expansion from T to $TS$ preserves $\mathrm {NSOP}_{1}$, simplicity, or stability. We give a full description of imaginaries, forking, and Kim-forking in this example.The second part studies expansions of the group of integers by p-adic valuations. We prove quantifier elimination in a natural language and compute the dp-rank of these expansions: it equals the number of independent p-adic valuations considered. Thus, the expansion of the integers by one p-adic valuation is a new dp-minimal expansion of the group of integers. RÉSUMÉ: Cet article discute l'idée que la philosophie tardive de Collingwood soit d'orientation historiciste et relativiste. Je soutiens que cette accusation de relativisme historique est basée sur deux erreurs, l'une exégétique et l'autre philosophique. L'erreur exégétique est le résultat de l'hypothèse d'une prétendue «conversion radicale». L'erreur philosophique repose sur la conception selon laquelle il n'y a pas de différences substantielles entre le projet d'une métaphysique descriptive et le projet de la sociologie de la connaissance. Die indische Philosophin und Aktivistin Radha d’Souza begegnet dem Begriff Weltzivilgesellschaft mit größter Vorsicht und Skepsis. Selbst jahrelang in existenzielle Auseinandersetzungen um Grundrechte auf natürliche Ressourcen wie Wasser und Ackerland und höchstgerichtliche Entscheidungen verwickelt, sieht d’Souza im Begriff der Weltzivilgesellschaft in der Hauptsache den Versuch, einer neuerlichen Bemäntelung imperialistischer, westlicher Interessen, die sich nun nicht nur durch Entscheidungen von Regierungen legitimieren, sondern gar durch den Willen ihrer zivilen Bevölkerung.
