Narcolepsy type 1 deeply impacts on quality of life, especially during adolescence, with NT1 children and adolescents that frequently report difficulties in integration with peers and decreased participation in after-school activities. Here we describe the case of NT1 teenager girl presenting with severe physical and social withdrawal, fulfilling the proposed diagnostic criteria for hikikomori, together with the classic NT1 symptoms. Social withdrawal is an overlooked phenomenon among NT1 children and adolescents that, if present, require a multidisciplinary approach and personalized interventions, (...) but patients can benefit from NT1 pharmacological treatment. Bernays has pointed out that, in order to prove the consistency of classical number theory, it is necessary to extend Hilbert's finitary stand‐point by admitting certain abstract concepts in addition to the combinatorial concepts referring to symbols. The abstract concepts that so far have been used for this purpose are those of the constructive theory of ordinals and those of intuitionistic logic. It is shown that the concept of a computable function of finite simple type over the integers can be used instead, where no other procedures of constructing such functions are necessary except simple recursion by an integral variable and substitution of functions in each other. The essay departs from a detailed discussion of Jakobson’s description of the »poetic function.« It comes to the conclusion that the distinctive feature of poetic language, defined by Jakobson as the production of paradigmatic similarity, cannot be understood as a result of the projection of the paradigmatic axis of language onto its syntagmatic axis. Furthermore, the semantic effect of these similarities does not consist exclusively in a production of ambivalence; frequently, they produce coherence, as can be shown with reference to (...) examples of metaphors, metonymies and allegories. In a second step, the essay turns to a detailed interpretation of the 8th sonnet of Petrarch’s Canzoniere, which examines its intertextual relations to other son- nets of this cycle as well as those to scholastic theology and Platonic philosophy. This interpretation further develops the idea that poetic language produces coherence; for, precisely those lines that at first seem incomprehensible can be understood as implicit propositions and coherencecreating statements. In its concluding section, the paper characterizes the implicit production of coherence as the specific feature of poetic language and discusses the necessity of a hermeneutics of literary texts. In 2012, a number of news headlines captured a growing trend in fertility medicine—the increased presence of lenders specializing in fertility loans, often promoted or featured by fertility clinics and physicians. Streamlining the financing of assisted reproduction connects patients with loan company representatives either on‐site or through a clinic's webpage. While medical professionals and clinics do not receive a commission for product promotion, this recent spate of news reports identified investment opportunities for physicians in private lending firms and some of (...) the physicians involved in such investments. In facilitating the process to obtain credit and, in some instances, benefiting financially through equity ownership in private lending firms, participating physicians declare this a “win‐win situation” with no clear conflict of interest. The emergence of these specialty lenders seems a natural development in the current fertility market, but the growing use of private fertility loans suggests the need for further inquiry. (shrink)

The awareness of educators in the health sciences with respect to promoting ethical conduct when instructing dental students is reflected in the undergraduate curriculum. This study is intended to discover the different ways these subjects are taught. A manual review of the literature published in databases indexed in SciELO during the years from 1999 to 2014 was done to that end, under the heading health sciences, humanities, applied social sciences, specifically literature that can be accessed in full, free of charge (...) and anonymously. Also reviewed was the literature in two specialized journals in the area of health education; namely, the Journal of Dental Education and the Journal of Medical Education, with the same titles, during the same period, and through the PubMed search engine, via Medline. The results show 43 articles of thematic relevance. The conclusion is that education in bioethics, as part of the undergraduate curriculum at dental schools, is increasing and differs according to the educational and cultural environment where it is imparted. Case-based learning and problem-based learning are mentioned as being the most efficient methods of instruction. Training dentists to be well-rounded professionals requires these subjects to be taught by interdisciplinary teams that are sufficiently prepared to do so. (shrink)
