Background: Healthcare providers who are accountable for patient care safety and quality but who are not empowered to actualize them experience moral distress. Interventions to mitigate moral distress in the healthcare organization are needed. Objective: To evaluate the effect on moral distress and clinician empowerment of an established, health-system-wide intervention, Moral Distress Consultation. Methods: A quasi-experimental, mixed methods study using pre/post surveys, structured interviews, and evaluation of consult themes was used. Consults were requested by staff when moral distress was present. (...) The purpose of consultation is to identify the causes of moral distress, barriers to action, and strategies to improve the situation. Intervention participants were those who attended a moral distress consult. Control participants were staff surveyed prior to the consult. Interviews were conducted after the consult with willing participants and unit managers. Moral distress was measured using the Moral Distress Thermometer. Empowerment was measured using the Global Empowerment Scale. Results: Twenty-one consults were conducted. Analysis included 116 intervention and 30 control surveys, and 11 interviews. A small but significant decrease was found among intervention participants, especially intensive care staff. Empowerment was unchanged. Interview themes support the consult service as an effective mode for open discussion of difficult circumstances and an important aspect of a healthy work environment. Conclusions: Moral distress consultation is an organization-wide mechanism for addressing moral distress. Consultation does not resolve moral distress but helps staff identify strategies to improve the situation. Further studies including follow up may elucidate consultation effectiveness. (shrink)

For example, whether my killing is murder or manslaughter depends, in part, on whether I understand that what I am doing is killing you, and on whether I understand that my killing is unjustified. Similarly, if I know that the backpack I am taking is yours, then my taking it may be an act of theft; but (...) it is not theft if I simply mistook your backpack for my own. An investigative reporter documents the destructive impact of the environmental movement in North America and beyond. One is sometimes, but only sometimes, justified in pursuing a suboptimal course of action due to a concern that, in attempting the ideal course, one might fail to follow through and so make the situation even worse. This paper explains why such hedging is sometimes justified and sometimes not. -/- The explanation we offer relies on Elizabeth Anscombe’s distinction between reasons and logoi. Reasons are normative considerations that identify something good or bad that an act will secure or avoid, (...) while logoi constrain which actions one may consider at all. We argue that a careful unpacking of this distinction, in light of Anscombe’s associated notion of natural unintelligibility, explains why hedging is only sometimes justified. First, the intuitively inappropriate cases of hedging all involve treating logoi as though they had the normative bearing of reasons. Second, the action-theoretic differences between reasons and logoi ground a principled explanation of why one may hedge when considering reasons but not when considering logoi. -/- Having made this argument, we then show how our approach to hedging is superior to the two approaches recently developed by Berislav Marušić and Stephen White. First, we show that their accounts give counter-intuitive results in some cases. Second, we argue that their accounts violate a plausible principle of interpersonal parity (i.e., they sometimes allow us to consider the faults of others while precluding us from considering our own faults). Finally, we argue that our account better explains how intuitions shift as we vary the details of cases. (shrink)

Research conducted within the past decade contributes much to an understanding of the role and potential value of formative assessment in learning. As an Advisory Teacher within a local authority, the researcher was interested to find out how teachers actually perceive formative assessment. This study therefore set out to investigate the range and nature of such perceptions and whether any relationships exist between them and independent variables such as school phase and length of teacher experience. The sample was drawn from (...) teachers working in 24 lower and middle schools within one local authority and the research was conducted in two phases. This paper will focus on Phase 1, during which data was drawn from a questionnaire survey which elicited both quantitative and qualitative responses from 67 teachers. Analysis of this data revealed a range of perceptions about formative assessment among teachers. In particular, they acknowledged the value of formative assessment in promoting learning. However, these findings suggest that perhaps teachers were less confident than they claimed to be in putting actual strategies in place, supporting the findings of Black, Harrison, Lee, Marshall, and Wiliam . Statistical analysis revealed some significant relationships between some of these perceptions and both school phase and length of teacher experience. Both theoretical and practical implications of these findings are discussed and recommendations made for future policy and practice. (shrink)

This essay explores small “ordinary” experiences of moral harm as problems of social injustice. Starting with two stories, we first argue against a dominant framework of personal responsibility that assigns responsibility to particular blameworthy agents. Instead we sketch an account of why structural responsibility for social harm must be considered, drawing on the work of Iris Marion Young and Pierre Bourdieu. Finally, drawing on Margaret Walker’s notion of moral repair and Christopher Marshall’s interpretation of the parable of the Good (...) Samaritan, we sketch an account of Christian moral responsibility grounded in restorative justice that seeks to address daily experiences of moral harm through the moral repair work of neighbor-love. Most of the chapters are based on presentations at the conference of the European Network of Buddhist-Christian Studies that met at the Benedictine Archabbey of St. Ottilien in 2019. Some of the authors, including Wesley Ariarajah, Aloysius Pieris, and Shantha de Silva (youngest son of Lynn de Silva), knew de Silva personally and offer warm tributes to his memory. Other authors work for institutions that carry on the types of projects that de Silva pioneered: Marshal Fernando is the Director of the Ecumenical Institute for Study and Dialogue, which de Silva founded and directed from 1962–1982; Indunil J. Kodithuwakku Kankanamalage, who is from Sri Lanka, serves as the Secretary of the Pontifical Council for Interreligious Dialogue; and Peniel Rajkumar leads the Interreligious Dialogue and Cooperation Programme of the World Council of Churches.Some authors examine how de Silva developed and modified his understanding of the relationship between Buddhist and Christian perspectives. In his early work in the 1960s, de Silva responded to Buddhist misunderstandings of Christian faith and practice. In the 1970s, he explored how Buddhist teachings could be related to biblical and Christian perspectives in ways that could enhance Christian faith. Toward the end of his life de Silva pondered the degree to which Buddhist and Christian perspectives could be relativized and harmonized.Benedictine Abbot Petrus Hohensteiger traces the development of de Silva's responses to Buddhist belief in reincarnation. De Silva came to reject traditional Protestant Christian eschatology that teaches that a single lifetime will be followed by eternal heaven or hell; but he also rejected Theravada belief in rebirth. Instead, while remaining a Protestant Christian, he developed a belief in purgatory that allowed for self-purification and growth in sanctity after death. De Silva interpreted Buddhist belief in rebirth and Christian belief in resurrection as symbolic pointers toward a single, shared truth of self-negating, self-emptying love.Andreas Nehring focuses on de Silva's study of Buddhism as a popular religion combining Buddhist perspectives with folk religious practices. While noting many points of convergence, Yong finds an important difference from Theravada anthropology in de Silva's eschatological interpretation of the corporate and resurrected Spiritual Body of the Apostle Paul.Kurt Gakuro Krammer reflects on how Buddhist-Christian dialogue can change one's understanding of the human being. Observing that most European Buddhists have thus far avoided dialogue, he proposes that de Silva can offer a helpful model of dialogical understanding but also poses the pointed question of whether dialogue has any soteriological impact, such as upon efforts toward social action. Is there something more to our relationship with them than simply companionship? What is it we look for in our pets and what does this say about us as human beings? Living with pets provides thought-provoking perspectives on our notions of possession and mastery, mutuality and cohabitation, love and dominance. We might think of pets as simply happy, loved additions to human homes but as this captivating book reveals perhaps it is the pets that make the home and without pets perhaps we might not be the humans we think we are. I argue that our traditional conception of the duties imposed by human rights is unable to acknowledge the nature of many contemporary human rights violations. The traditional conception is based on a broadly deontological view according to which human rights impose primarily negative and perfect duties, and these duties are held to be specific prohibitions on certain kinds of actions . I argue that given this conception of the nature of the duties imposed by human rights, not only claims to (...) aid, but in addition, claims against many of the most serious and prevalent contemporary active harms will not count as genuine human rights claims. These harms increasingly result from extremely complex causal chains involving the behaviour of a huge number of agents, few or none of whom can be singled out as responsible for a serious harm to any specific victim. Institutional structures can have just as central a role to play in specifying and allocating responsibility for fulfilling many of the negative duties imposed by human rights as the positive duties. These structures can therefore be equally important in the realisation of both kinds of rights. Before the necessary institutional reforms have taken place, both kinds of rights are equally genuine, and ground the imperative of justice to reform existing social institutions. (shrink)

We intuitively think and talk about health care as a human right. Moreover, we tend to talk about health in the language of basic rights or human rights without a clear sense of what such rights mean, let alone whose duty it is to fulfill them. Additionally, in the care ethics literature, we tend to think of a dividing line between care and justice. In this dissertation I aim to draw care and justice together in what I call care justice. (...) To attend to care justice requires the reconceptualization of the value and practice of health care, and of the moral communities in which we enact care and justice. First, I argue for a strong right to health care, though not to health. Second, I argue that health care includes much more than mere medical care. On this account health care is special for the practices of care it values and communicates. Third, I establish a theoretical framework for why, and importantly how, health care entails other forms of care: social, economic and political. My approach engages a complicity framework to rethink moral community formation and participation, and the rights and responsibilities of individuals as participants in a collective moral community. I argue that it is within communities of membership, which I define as ethical homes, that we have duties to and expectations from others. The moral community as complicit ethical home provides a way to reimagine individual shared responsibility within moral communities, and particularly regarding responsibility for practices of care. Four-month-old infants can perceive bimodally speciiied events. They respond to relationships between the optic and acoustic stimulation that carries information about an object. Infants can do this by detecting the temporal synchrony of an object’s sounds and its optically specified impacts. They are sensitive both to the common tempo and to the simultaneity of such sounds and visible impacts. These findings support the view that intermodal perception depends at least in part on the detection of invariant relationships in patterns of (...) light and sound. (shrink)

Through readings of works by Marivaux, Diderot, Rousseau, and Mary Shelley, David Marshall provides a new interpretation of the eighteenth-century preoccupation with theatricality and sympathy. Sympathy is seen not as an instance of sensibility or natural benevolence but rather as an aesthetic and epistemological problem that must be understood in relation to the problem of theatricality. Placing novels in the context of eighteenth-century writing about theater, fiction, and painting, Marshall argues that an unusual variety of authors and texts (...) were concerned with the possibility of entering into someone else's thoughts and feelings. He shows how key eighteenth-century works reflect on the problem of how to move, touch, and secure the sympathy of readers and beholders in the realm of both "art" and "life." This article examines the responses of Parisian noble women to campaigns for women's rights in France of the early Third Republic. The methodology of the article is based on the works of Pierre Bourdieu. His concept of the habitus is used to analyse the effects of class and gender in noble women's attitudes to French feminisms before the First World War. The conditioning of Parisian noble women explains their resistance, indeed often outspoken opposition, to feminists' demands. Patient and family emotional harm after medical errors may be profound. At an Agency for Healthcare Research and Quality conference to establish a research agenda on this topic, the authors used visual images as a gateway to personal reflections among diverse stakeholders. Themes identified included chaos and turmoil, profound isolation, organizational denial, moral injury and betrayal, negative effects on families and communities, importance of relational skills, and healing effects of human connection. This paper examines trust as a fundamental aspect of fiduciary relationships in education. The specific relationship under examination is that of academic employee and university employer. Both have the value of trust assigned to them as an implicit part of their social and professional contract. The setting is Australia, but the principles apply to any democratic jurisdiction and educational level or location, where fiduciary principles are a pre-condition for healthy and trustworthy working relationships. The paper firstly discusses the meaning and (...) application of ‘trust’ and ‘fiduciary’, and considers the codes that regulate fiduciary relationships in education. It then asks what happens if and when the fiduciary duties and obligations are not upheld, advancing that question through an employment law case against an Australian university, for alleged breach of the Fair Work Act. The analysis shows what may be the outcome when fiduciary trust between employee and employer is breached, how it may happen, and the consequences of this. The aim of the paper is to throw light on the value of trust for robust working relationships in academic life. Ultimately, the paper shows that trust is axiomatic to a working system of justice in a healthy educational workplace. (shrink)

Walker Percy was both a medical doctor and a serious Catholic—a scientist and a religious believer. He thought, however, that science had become hegemonic in the twentieth century and that it was incapable of answering the most fundamental needs of human beings. He thus leveled a critique of the scientific method and its shortcomings in failing to address the individual person over against the group. The Christian faith holds out the promise of abundant life and yet many writers have exposed the ambiguities experienced from the words used when that faith is expressed or discussed or described. While there are many aspects to this exploration, this article investigates a set of words that are used to and for the divine because the one spoken of as the author of this abundant life is described in terms that limit the possibilities for too many people. Suicide is a major problem in the United States, with the number of suicides annually exceeding the number of homicides by 10,000. Many studies have examined the relationship between media coverage of suicides and the suicide rate. This article reviews literature on imitative suicide and discusses implications of suicide stories on people in crisis. In addition, it explores the options for suicide coverage and gives suggestions for more ethical coverage that could save people's lives, rather than reinforcing suicide as an (...) option. (shrink)
