ObjectiveHead and neck cancer treatments are known to significantly affect functionality and appearance, leading to an increased risk for body image disturbances. Yet, few longitudinal studies exist to examine body image in these patients. Based on a conceptual model, the current study aimed to determine, in patients newly diagnosed with HNC: the prevalence, level, and course of body image concerns; correlates of upon cancer diagnosis body image concerns; predictors of immediate post-treatment body image concerns; and association between body image concerns (...) and levels of anxiety, depression, suicidal ideation, support, and alcohol and drug misuse.MethodsTwo hundred and twenty-three, newly diagnosed with a primary HNC were assessed using structured clinical interviews and psychometric measures at three, and 6 months after diagnosis. Primary outcome was 3-month, as it was most salient to body image disturbance. Multiple linear regression analyses were conducted on the potential body image predictors, based on the model.ResultsSixty-eight percent of patients with HNC presented some level of body image concerns. Body image concerns at baseline and post-treatment were significantly related and significantly increased from pre- to post-treatment. Immediately post-treatment, 89% presented some level of body image concerns. Correlates of body image concerns in patients with HNC at baseline included: physical symptom burden, difficulties with communication and eating, coping with the cancer diagnosis using denial, suicidal ideation, and having had a past anxiety diagnosis. When controlling for sociodemographic and medical variables, body image concerns in patients with HNC in the immediate post-treatment were predicted by: baseline body image, physical symptom burden, and neuroticism.ConclusionThis longitudinal study helps identify patients more susceptible to experience body image disturbance following head and neck cancer. Clinicians ought to pay special attention to body image concerns upon cancer diagnosis, physical symptom burden, and neuroticism, and may want to target these factors in future preventive interventions. Nearly a half-century later, this student of Lieberman’s met another church historian, Virginia Burrus, in Morningside Heights under happier circumstances, when both of us were participants at a conference on asceticism at Union Theological Seminary, and a similar intellectual interaction began. This paper represents some of the first fruits of that second encounter and aspires to modestly continue the enterprise begun by the first.[2] Intertextuality and InterdisciplinarityAveril Cameron has recently written:The myth of early Christianity as the resort of the poor and underprivileged is precisely that, and a very convenient one it has been. It is a myth that rests, moreover, on the fallacy of an original Christianity uncontaminated by external influences; but its holders then have to explain how this “new” faith could make [End Page 52] the leap to center stage. Thus, we have been told, “the naiveté of the early Christian speech came in the course of time to wed itself to the cultures of the world.” But while much of current New Testament scholarship is directed at the internal (that is, theological) articulation of the texts, there is also a perceptible trend towards a mode of interpretation that balances the external and internal factors operative in the literary texts. It is thus less a question of the degree of “influence” of Greco-Roman or Jewish literary or philosophical elements on early Christian writing than of their integral relationship.[37–38]Judaism also (and Jewish scholarship) has had a stake in inscribing itself as pure and uncontaminated, for reasons that Philip Alexander has articulated: “The attempt to [lay down a norm for Judaism in the first century] barely conceals apologetic motives—in the case of Christianity a desire to prove that Christianity transcended or transformed Judaism, in the case of Jews a desire to suggest that Christianity was an alien form of Judaism which deviated from the true path” [3]. Indeed, I will be hinting (and in future work explicitly arguing) that we have to begin seriously thinking about Judaeo-Christianity as a single cultural system: contentious, dialectical, polemical, and sometimes friendly, but—I hasten to add—not moralistic in the homogenized “family values” sense implied by the modern usage of this term. I put forth here, as a case in point, that the richest contexts for understanding the sets of cultural tensions that gave rise to a particular talmudic text are to be uncovered in contemporary patristic literature. A new research programme conceives of consciousness as a natural kind. One proposed virtue of this approach is that it can help resolve the methodological puzzle of consciousness, which involves distinguishing consciousness from cognitive access. The present article raises a novel problem for this approach. The problem is rooted in the fact that there may be episodes of conscious experience that have not been classified as such. I argue that conceiving of consciousness as a natural kind cannot distinguish consciousness from (...) cognitive access. (shrink)

We investigate under what conditions contrary-to-duty (CTD) structures lacking temporal and action elements can be given a coherent reading. We argue, contrary to some recent proposals, that CTD is not an instance of defeasible reasoning, and that methods of nonmonotonic logics are inadequate since they are unable to distinguish between defeasibility and violation of primary obligations. We propose a semantic framework based on the idea that primary and CTD obligations are obligations of different kinds: a CTD obligation pertains to, or (...) pre-supposes, a certain context in which a primary obligation is already violated. This framework is presented initially as an extension of Standard Deontic Logic (SDL), a normal modal logic of type KD, and is illustrated by application to a series of examples. The concluding section is concerned with some resemblances between CTD and defeasible reasoning. We show first that the SDL-based framework contains a flaw and must be adjusted. A discussion of possible adjustments, including an alternative treatment in terms of a preference-based semantics, reveals difficulties that are reminiscent of problems in defeasible reasoning and intensional accounts of defeasible conditionals. One of the most influential of the Victorian philosophers, Henry Sidgwick also made important contributions to fields such as economics, political theory and classics. A proponent of the utilitarianism of Jeremy Bentham and John Stuart Mill, which he analysed in his classic work The Methods of Ethics , he later turned to the practical side of politics in this work, published in 1891. His aim was to have a 'rational discussion of political questions in modern states', and he offers (...) a thorough examination of Victorian politics, beginning with a discussion of political theory before moving on to more concrete matters such as property and law. The later chapters discuss the function of government through themes such as procedural matters, international affairs, war, political parties, and public participation, giving a comprehensive account of both ideas about and the functioning of late-nineteenth-century government. Most people will grant that we bear special moral obligations toward at least some nonhuman animals that we do not bear toward inanimate objects like stones, mountains, or works of art. These moral obligations are plausibly grounded in the fact that many if not all nonhuman animals share important psychological states and capacities with us, such as consciousness, suffering, and goal-directed behavior. But which of these states and capacities are really critical for a creature’s possessing moral status, and how can (...) we determine which animals do in fact have them? In this paper, I examine three main approaches to answering these questions. First are what I term consciousness-based approaches that tackles these questions by first asking which animals are conscious. Second are affective-state approaches that focus on identifying behavioural and physiological signatures of states like pain, fear, and stress. Finally, I consider what I call preference-based approaches whose focus is on the question of which organisms have robust motivational states. I examine the prospects and challenges—both theoretical and empirical—faced by these seemingly contrasting methodologies. I go on to suggest that there are reasons why, despite challenges, we should be robustly committed to the project of identifying psychological grounds of moral status. I conclude by suggesting we should also take seriously the idea of pluralism about moral status, according to which each of these approaches might be capable of providing independent grounds for moral consideration. Raz's method is as unusual, and as admirable, as the substance of his sometimes rather unfortunately labeled "perfectionist liberalism"—unfortunate because "it is not perfectionist in the more ordinary sense of the term" in that it recognizes that "imperfect ways of life may be the best which is possible for people" and "is strongly pluralistic", while understanding its fundamental value of well-being as the active and autonomous making of a life of one's own. Raz's approach is simultaneously alert to the complexity (...) of argument, to a degree that practitioners of practical ethics sometimes are not, and aware of the limits of theorizing, as devotees of theoretical ethics rarely are. Thus, while scorning "supermarket liberalism" and rejecting, in company with Rawls, "hand-to-mouth piecemeal intuitionism", Raz also doubts whether ideal theory in the Rawlsian mode is "a meaningful enterprise", emphasizing wisely, in my judgment, that "not everything we know can be exhaustively stated in the abstract". The "dense webs of complex actions and interactions" that give meaning to our lives "defy explicit learning or comprehensive articulation. They are available only to those who have or can acquire practical knowledge of them, that is, knowledge embodied in social practices and transmitted by habituation". (shrink)
