Military metaphors are pervasive in biomedicine, including HIV research. Rooted in the mind set that regards pathogens as enemies to be defeated, terms such as “shock and kill” have become widely accepted idioms within HIV cure research. Such language and symbolism must be critically examined as they may be especially problematic when used to express scientific ideas within emerging health-related fields. In this article, philosophical analysis and an interdisciplinary literature review utilizing key texts from sociology, anthropology, history, and Chinese and (...) African studies were conducted to investigate the current proliferation of military metaphors. We found the use of these metaphors to be ironic, unfortunate, and unnecessary. To overcome military metaphors we propose to give them less aggressive meanings, and/or replace them with more peaceful metaphors. Fake news sharing in 2016 was rare but significantly more common among older Americans. So-called “fake news” has renewed concerns about the prevalence and effects of misinformation in political campaigns. Given the potential for widespread dissemination of this material, we examine the individual-level characteristics associated with sharing false articles during the 2016 U.S. presidential campaign. To do so, we uniquely link an original survey with respondents’ sharing activity as recorded in Facebook profile data. First and foremost, we find that sharing (...) this content was a relatively rare activity. Conservatives were more likely to share articles from fake news domains, which in 2016 were largely pro-Trump in orientation, than liberals or moderates. We also find a strong age effect, which persists after controlling for partisanship and ideology: On average, users over 65 shared nearly seven times as many articles from fake news domains as the youngest age group. In 2008, researchers reported that Timothy Brown , a man with HIV infection and leukemia, received a stem-cell transplant that removed HIV from his body as far as can be detected. In 2013, an infant born with HIV infection received anti-retroviral treatment shortly after birth, but was then lost to the health care system for the next six months. When tested for HIV upon return, the child had no detectable viral load despite cessation of treatment. Trust is indispensable not only for interpersonal relationships and social life, but for good quality healthcare. As manifested in the increasing violence and tension in patient-physician relationships, China has been experiencing a widespread and profound crisis of patient–physician trust. And globally, the crisis of trust is an issue that every society, either developing or developed, has to face in one way or another. Yet, in spite of some pioneering works, the subject of patient-physician trust and mistrust – a crucial matter (...) in healthcare especially because there are numerous ethical implications – has largely been marginalized in bioethics as a global discourse. Drawing lessons as well as inspirations from China, this paper demonstrates the necessity of a trust-oriented bioethics and presents some key theoretical, methodological and philosophical elements of such a bioethics. A trust-oriented bioethics moves beyond the current dominant bioethical paradigms through putting the subject of trust and mistrust in the central agenda of the field, learning from the social sciences, and reviving indigenous moral resources. In order for global bioethics to claim its relevance to the things that truly matter in social life and healthcare, trust should be as vital as such central norms like autonomy and justice and can serve as a potent theoretical framework. To investigate the phenomenon of patient–physician mistrust in China, a qualitative study involving 107 physicians, nurses and health officials in Guangdong Province, southern China, was conducted through semi-structured interviews and focus groups. In this paper we report the key findings of the empirical study and argue for the essential role of medical professionalism in rebuilding patient-physician trust. Health professionals are trapped in a vicious circle of mistrust. Mistrust leads to increased levels of fear and self-protection by doctors which exacerbate difficulties (...) in communication; in turn, this increases physician workloads, adding to a strong sense of injustice and victimization. These factors produce poorer healthcare outcomes and increasingly discontented and angry patients, escalate conflicts and disputes, and result in negative media coverage, all these ultimately contributing to even greater levels of mistrust. The vicious circle indicates not only the crisis of patient-physician relationship but the crisis of medicine as a profession and institution. Underlying the circle is the inherent conflict of interest in the healthcare system by which health professionals and hospitals have become profit-driven. This institutional conflict of interest seriously compromises the fundamental principle of medical professionalism—the primacy of patient welfare—as well as the traditional Chinese ideal of “medicine as the art of humanity”. Patient trust can be restored through rectifying this institutional conflict of interest and promoting medical professionalism via a series of recommended practical measures. (shrink)

As in other societies, medical professionalism in the Peoples’ Republic of China has been rapidly evolving. One of the major events in this process was the endorsement in 2005 of the document, “Medical Professionalism in the New Millennium: A Physician Charter,” by the Chinese Medical Doctor Association (hereafter, the Charter). More recently, a national survey, the first on such a large scale, was conducted on Chinese physicians’ attitudes toward the fundamental principles and core commitments put forward in the Charter. Based (...) on empirical findings from that study and comparing them to the published results of a similar American survey, the authors offer an in-depth interpretation of significant cross-cultural differences and important transcultural commonalities. The broader historical, socio-economic, and ethical issues relating to salient Chinese cultural practices such as family consent, familism (the custom of deferring decisions to family members), and the withholding of medical information, as well as controversial topics such as not respecting patients’ autonomy, are examined. The Chinese Survey found that Chinese physicians supported the principles of the Charter in general. Here we argue that Chinese culture and traditional medical ethics are broadly compatible with the moral commitments demanded by modern medical professionalism. Methodologically and theoretically—recognizing the problems inherent in the hoary but still popular habit of dichotomizing cultures and in relativism—a transcultural approach is adopted that gives greater (due) weight to the internal moral diversity present within every culture, the common ground shared by different cultures, and the primacy of morality. Genuine crosscultural dialogue, including a constructive Chinese-American dialogue in the area of medical professionalism, is not only possible, but necessary. (shrink)

We analyse the connection between the computability and continuity of functions in the case of homomorphisms between topological algebraic structures. Inspired by the Pour-El and Richards equivalence theorem between computability and boundedness for closed linear operators on Banach spaces, we study the rather general situation of partial homomorphisms between metric partial universal algebras. First, we develop a set of basic notions and results that reveal some of the delicate algebraic, topological and effective properties of partial algebras. Our main computability concepts (...) are based on numerations and include those of effective metric partial algebras and effective partial homomorphisms. We prove a general equivalence theorem that includes a version of the Pour-El and Richards Theorem, and has other applications. Finally, the Pour-El and Richards axioms for computable sequence structures on Banach spaces are generalised to computable partial sequence structures on metric algebras, and we prove their equivalence with our computability model based on numerations. The status of photographs as keystones of historical explanation has become a topic of urgent intellectual and cultural interest around the world, at the same time as methods of shaping historical narratives are also changing in ways that compel attention to the employment of photographs in historiography. By exposing the questions we ought to raise about all historical evidence, photographs reveal not simply the potential and limits of photography as a historical source, but the potential and limits of all historical (...) sources and historical inquiry as an intellectual project. As the papers in this issue make apparent, this is precisely the promise and ultimate potential of the historical study of photographs—that it pushes their interpreters to the limits of historical analysis. This essay, which serves as an introduction to the Theme Issue, contextualizes issues raised by the articles and offers a critical synthesis of their impact on future scholarship about photography in historical analysis. Background Community advisory boards have expanded beyond high-income countries and play an increasing role in low- and middle-income country research. Much research has examined CABs in HICs, but less is known about CABs in LMICs. The purposes of this scoping review are to examine the creation and implementation of CABs in LMICs, including identifying frequently reported challenges, and to discuss implications for research ethics. Methods We searched five databases for publications describing or evaluating CABs in LMICs. Two researchers independently reviewed (...) articles for inclusion. Data related to the following aspects of CABs were extracted from included publications: time, country, financial support, research focus, responsibilities, and challenges. Thematic analyses were used to summarize textual data describing challenges. Results Our search yielded 2005 citations, 83 of which were deemed eligible for inclusion. Most studies were published between 2010 and 2017. Upper-middle-income countries were more likely to have studies describing CABs, with South Africa, China, and Thailand having the greatest numbers. The United States National Institutes of Health was the main source of financial support for CABs. Many CABs focused on HIV research. Thirty-four studies reported how CABs influenced the informed consent process for clinical trials or other aspects of research ethics. CAB responsibilities were related to clinical trials, including reviewing study protocols, educating local communities about research activities, and promoting the ethical conduct of research. Challenges faced by CABs included the following: incomplete ethical regulations and guidance; limited knowledge of science among members of communities and CABs; unstable and unbalanced power relationships between researchers and local communities; poor CAB management, including lack of formal participation structures and absence of CAB leadership; competing demands for time that limited participation in CAB activities; and language barriers between research staff and community members. Several challenges reflected shortcomings within the research team. Conclusions Our findings examine the formation and implementation of CABs in LMICs and identify several ethical challenges. These findings suggest the need for further ethics training among CAB members and researchers in LMICs. (shrink)

One of the persistent features of historical writing about the sciences in the last twenty years has been the concern of a number of historians who insist on the need for a new awareness of the role of visual images and image making. The author believes that, rather than reducing the analysis of visual culture to a single set of principles, the point of the academic study of scientific images is the recognition of their heterogeneity, the different circumstances of their (...) production, and the variety of cultural and social functions they serve. This essay challenges historians to discover new ways of framing the historical meanings of scientific images within the larger contexts of signifying symbols, images, and mediations that make up culture. The study of nineteenth‐century practices of building collections of scientific photographs provides the background for a discussion of the significance of picture archives in the history of science, of the historical mechanisms that frame some pictures as “scientific” and others as “unscientific,” and of the need for further research on how scientific images generate meaning. The term ‘artificial intelligence’ has arguably come to function in political discourse as, what Laclau called, an ‘empty signifier’. This article traces the shifting political discourse on AI within three key institutions of global governance–OHCHR, WHO, and UNESCO–and, in so doing, highlights the role of ‘crisis’ moments in justifying a series of pivotal re-articulations. Most important has been the attachment of AI to the narrative around digital automation in human healthcare. Greatly enabled by the societal context of the pandemic, all (...) three institutions have moved from being critical of the unequal power relations in the economy of AI to, today, reframing themselves primarily as facilitators tasked with helping to ensure the application of AI technologies. The analysis identifies a shift in which human health and healthcare is framed as in a ‘crisis’ to which AI technology is presented as the remedy. The article argues the need to trace these discursive shifts as a means by which to understand, monitor, and where necessary also hold to account these changes in the governance of AI in society. Arithmetical texts involving division are governed by conventions that avoid the risk of problems to do with division by zero (DbZ). A model for elementary arithmetic texts is given, and with the help of many examples and counter examples a partial description of what may be called traditional conventions on DbZ is explored. We introduce the informal notions of legal and illegal texts to analyse these conventions. First, we show that the legality of a text is algorithmically undecidable. As a (...) consequence, we know that there is no simple sound and complete set of guidelines to determine unambiguously how DbZ is to be avoided. We argue that these observations call for further explorations of mathematical conventions. We propose a method using logics to progress the analysis of legality versus illegality: arithmetical texts in a model can be transformed into logical formulae over special total algebras that are able to approximate partiality but in a total world. The algebras we use are called common meadows. Our dive into informal mathematical practice using formal methods opens up questions about DbZ which we address in conclusion. (shrink)

: Most discussions of Yamaga Soko's philosophical development as a Confucian scholar in Tokugawa Japan suggest that in his later years he moved away from Confucianism and toward a religio-philosophical celebration of Japan's supposed uniqueness. It is shown here, however, that Soko's nativism, set forth in his Chucho jijitsu, was later eclipsed by his final philosophical work, the Gengen hakki, wherein he articulated a kind of naturalistic numerology, based vaguely on the Yijing. This shift in Soko's thought can be viewed (...) as a return to Neo-Confucianism, the earliest philosophical paradigm that he had embraced. Moreover, the successive shifts in his thinking can be understood in terms of the vicissitudes of his life, first in his exile to the Kansai area, near the ancient imperial capital of Kyoto, and then later in his pardon and return to Edo, the shogun's capital. Perhaps most importantly, this final shift in Soko's thought reveals that this prominent early modern thinker did reach his philosophical climax not in defiant opposition to Neo-Confucianism, nor in a sustained celebration of Japan's political traditions and their superlative nature, but instead in a return to modes of metaphysics akin to those typically deployed by Neo- Confucians themselves in their attempts to understand the changing nature of the cosmos and their political place within its flux. Earlier, we have studied computations possible by physical systems and by algorithms combined with physical systems. In particular, we have analysed the idea of using an experiment as an oracle to an abstract computational device, such as the Turing machine. The theory of composite machines of this kind can be used to understand (a) a Turing machine receiving extra computational power from a physical process, or (b) an experimenter modelled as a Turing machine performing a test of a known physical (...) theory T. Our earlier work was based upon experiments in Newtonian mechanics. Here we extend the scope of the theory of experimental oracles beyond Newtonian mechanics to electrical theory. First, we specify an experiment that measures resistance using a Wheatstone bridge and start to classify the computational power of this experimental oracle using non-uniform complexity classes. Secondly, we show that modelling an experimenter and experimental procedure algorithmically imposes a limit on our ability to measure resistance by the Wheatstone bridge. The connection between the algorithm and physical test is mediated by a protocol controlling each query, especially the physical time taken by the experimenter. In our studies we find that physical experiments have an exponential time protocol, this we formulate as a general conjecture. Our theory proposes that measurability in Physics is subject to laws which are co-lateral effects of the limits of computability and computational complexity. (shrink)

We have begun a theory of measurement in which an experimenter and his or her experimental procedure are modeled by algorithms that interact with physical equipment through a simple abstract interface. The theory is based upon using models of physical equipment as oracles to Turing machines. This allows us to investigate the computability and computational complexity of measurement processes. We examine eight different experiments that make measurements and, by introducing the idea of an observable indicator, we identify three distinct forms (...) of measurement process and three types of measurement algorithm. We give axiomatic specifications of three forms of interfaces that enable the three types of experiment to be used as oracles to Turing machines, and lemmas that help certify an experiment satisfies the axiomatic specifications. For experiments that satisfy our axiomatic specifications, we give lower bounds on the computational power of Turing machines in polynomial time using nonuniform complexity classes. These lower bounds break the barrier defined by the Church-Turing Thesis. (shrink)

We define and study the class of all stack algebras as the class of all minimal algebras in a variety defined by an infinite recursively enumerable set of equations. Among a number of results, we show that the initial model of the variety is computable, that its equational theory is decidable, but that its equational deduction problem is undecidable. We show that it cannot be finitely axiomatised by equations, but it can be finitely axiomatised by equations with a hidden sort (...) and functions. This class of all stack algebras, together with its specifications, can be used to survey the many models in the literature on stacks in a systematic way, and hence give the study of the stack some mathematical coherence. An algebra is effective if its operations are computable under some numbering. When are two numberings of an effective partial algebra equivalent? For example, the computable real numbers form an effective field and two effective numberings of the field of computable reals are equivalent if the limit operator is assumed to be computable in the numberings . To answer the question for effective algebras in general, we give a general method based on an algebraic analysis of approximations by elements of (...) a finitely generated subalgebra. Commonly, the computable elements of a topological partial algebra are derived from such a finitely generated algebra and form a countable effective partial algebra.
