We respond to a Letter to the Editor regarding "Principlism and contemporary ethical considerations for providers of transgender health care." We address criticisms by Castle & Klein (2024) of blatant fatphobia related to the ethical elements concerning BMI restrictions for gender-affirming surgery. Our response corrects several mischaracterizations of the article and clarifies our position. My co-authors and I remain focused on advocating for patient-centered, ethically sound, evidence-based, and equitable healthcare policies.

Background: Transgender health care is a subject of much debate among clinicians, political commentators, and policy-makers. While the World Professional Association of Transgender Health (WPATH) Standards of Care (SOC) establish clinical standards, these standards contain implied ethics but lack explicit focused discussion of ethical considerations in providing care. An ethics chapter in the SOC would enhance clinical guidelines. Aims: We aim to provide a valuable guide for healthcare professionals, and anyone interested in the ethical aspects of clinical support for gender (...) diverse and transgender people of all ages. Recognizing that the WPATH is a global association, we address broad challenges. We offer a reflection on general ethical principles, providing conceptual tools for healthcare providers, patients, and families to navigate the specific challenges they might encounter in transgender health care, in line with WPATH’s worldwide mission and scope. Method: This article employs a descriptive analysis, and our framework of reference is the four principles of biomedical ethics: respect for autonomy, beneficence, nonmaleficence, and justice. Results: The article presents a discussion on the four ethical principles as applied to transgender health care. We address issues such as respect for patient autonomy in decision-making, the role of beneficence and nonmaleficence in clinical interventions, and the importance of justice in equitable treatment and access to care. Some of the ethical concerns we address in this article pertain to the current sociopolitical climate, where there has been increasing legal interference, internationally, for transgender and nonbinary people, particularly youth, seeking medical care. Discussion: We highlight the interplay between ethical principles and clinical practice, underscoring the need for ethical guidance in addressing the diverse challenges faced by healthcare providers and patients in transgender health care. We advocate for continuous refinement of ethical thinking to ensure that transgender health care is not only medically effective but also ethically sound. (shrink)

La historia de la filosofía redescubierta a través del singular prisma de la fenomenología del cuerpo no se presenta ya como la historia de las grandes ideas, sino de algunas pequeñas preguntas: una historia de los restos, las sombras y los márgenes de la filosofía. Merleau-Ponty saca a la luz fragmentos de esa otra historia sirviéndose de un único procedimiento cuya práctica constante no parece expresar tanto un método como una obsesión, cuando insiste en formular a los filósofos del pasado (...) la misma pequeña pregunta con la que interpelaba a su amigo Althusser mientras discutían por la madrugada: "Pero usted también tiene un cuerpo, ¿no es así?". Cada filosofía, señala Merleau-Ponty, es una elección filosófica, pero "por más que los filósofos escojan, su elección siempre está acompañada, como al margen, de la sospecha de lo que han dejado de lado". Ese margen acompaña a cada filósofo como su propia sombra. Esta obra explora las distintas aproximaciones de Merleau-Ponty a ciertas obras canónicas de la historia de la filosofía. A própria França surge como a esperança de encarnar algum nicho de exceção na universalidade anglo-saxónica e germânica, entre as novas ciências da Natureza ou da cultura, de (...) qual ela não é mais o principal fornecedor. As obras espiritualistas francesas são frequentemente feitas de inversões de Kant ou Hegel, que encontram nas margens desses sistemas seu próprio centro. Portanto, eles se apegam à liberdade, à vontade da pessoa como a uma exceção de seus pensamentos, em torno do qual as filosofias francesas tentam reconstruir outra universalidade, nem a do conhecimento e da ciência, mas muitas vezes da ação. In this chapter, I draw on French phenomenologist Maurice Merleau-Ponty’s understanding of perception and corporeal sensibility to consider the significance of human situatedness as expressed via place and place experience. To illustrate how Merleau-Ponty’s conceptual understanding might be applied to real-world place experiences, I draw on two sources of experiential evidence, the first of which is a passage from Colombian writer Gabriel García Márquez’s magical-realist novel, One Hundred Years of Solitude. The second source is a set of first-person observations (...) describing events and situations that I happened to take note of during weekday walks between my home and university office over the course of several months. My aim is to use these two narrative accounts as a means to illustrate, via vignettes of everyday human experience, Merleau-Ponty’s central concepts of perception, body-subject, and lived embodiment. I contend that these accounts substantiate Merleau-Ponty’s phenomenological claims and point to additional significant elements of human situatedness and place experience. (shrink)

Embodied Care is the first work to argue for the body's centrality to care ethics, doing so by analyzing our corporeality at the phenomenological level.

I prefer to put this in a letter to you instead of writing an article that would lead one to believe that I have any authority to speak on the subject of what has, in a roundabout way, become the H. and H. affair . In other words, a cause of extreme seriousness, already discussed many times although certainly endless in nature, has been taken up by a storm of media attention, which has brought us to the lowest of passions, (...) intense emotions, and even violence. I understand why people are talking about Victor Farias, who has contributed some unpublished information—with a polemical intent, it is true, that does not help one to appreciate its true value. But how has it happened that Philippe Lacoue-Labarthe’s book, published in 1987, was greeted by a silence that I am perhaps the first to break?1 It is because he avoids anecdotal accounts, all the while citing and situating most of the facts mentioned by Farias. This volume contains sOOeen essays written by his students and colleagues in honor of Maurice Natanson. The essays explore some of the diverse themes Professor Natanson has pursued through forty years of teaching and philosophizing in the tradition of existential phenomenology. Because it also includes a lengthy biographical and philosophical interview where one can find an absorbing account of Natanson's Lebens/au/in his own words, there is no need to detail that polypragmatic career here. Suffice to say that even passing (...) acquaintance with the man and the work will reveal that Edmund Husserl's self-description holds equally of his distinguished interpreter: "I seek not to instruct but only to lead, to point out and describe what I see. I claim no other right than that of speaking according to my best lights, principally before myself but in the same manner also before others, as one who has lived in all its seriousness the fate of a philosophical existence. "l For Natanson, as the diversity of the contributions to this volume attest, such seriousness involves something other than that narrow technical vision for which a topic is the more philosophical the less it has to do with anything else. How do we use our mental images of the present to reconstruct our past? Maurice Halbwachs addressed this question for the first time in his work on collective memory, which established him as a major figure in the history of sociology. This volume, the first comprehensive English-language translation of Halbwach's writings on the social construction of memory, fills a major gap in the literature on the sociology of knowledge. Halbwachs' primary thesis is that human memory can only function within (...) a collective context. Collective memory, Halbwachs asserts, is always selective; various groups of people have different collective memories, which in turn give rise to different modes of behavior. Halbwachs shows, for example, how pilgrims to the Holy Land over the centuries evoked very different images of the events of Jesus' life; how wealthy old families in France have a memory of the past that diverges sharply from that of the nouveaux riches; and how working class construction of reality differ from those of their middle-class counterparts. Ce livre ne vise pas à donner un aperçu de ce que devrait être une religion. Il ne vise pas davantage à mettre en valeur l'argumentaire de l'athéisme. Il entend plutôt montrer qu'une personne qui se dit parfaitement agnostique et adepte d'une philosophie qui valorise la rationalité avant tout peut parfaitement être croyante, voire même pratiquante, sans qu'il y ait là la moindre contradiction. Dans une telle démarche, l'armature cognitive à laquelle de nombreux croyants ont jugé approprié d'arrimer leur foi (...) risque de se voir quelque peu déboulonnée. Et nombre d'arguments qui ont fortement contribué à miner les croyances religieuses pourraient s'en trouver passablement affaiblis. Dans les deux cas, tout découlerait de la simple reconnaissance du fait que la foi n'est en rien un savoir. Croire, expose Maurice Lagueux, en un Dieu transcendant ne veut pas dire connaître quoi que ce soit à propos d'un monde transcendant qui, par définition, échappe à nos capacités cognitives et est conçu comme le lieu de mystères insondables. Croire veut dire avoir sincèrement confiance en ce qui pourrait nous réserver l'existence de ce Dieu qui, toutefois, demeure inconnaissable. In its traditional form epistemology has always rested on the exclusive reality of the subject-object relationship. If one asks how the subject knows the object, one has in brief form the essence of theory of knowledge from Plato to Bergson; the differences between the many schools of philosophy can all be understood as variations on this theme. There are, first of all, differences in emphasis as to whether the subject or the object is the more real--as in rationalism and empiricism, (...) idealism and materialism, personalism and logical positivism. There are differences, secondly, as to the nature of the subject, which is variously regarded as pure consciousness, will to life, will to power, the scientific observer, or the intuitive knower. There are differences, thirdly, as to the nature of the object--whether it is material reality, thought in the mind of God or man, pantheistic spiritual substance, absolute and eternal mystical Being, or simply something which we cannot know in itself but upon which we project our ordered thought-categories of space, time, and causation. There are differences, finally, as to the relation between subject and object: whether the object is known through dialectical or analytical reasoning, scientific method, phenomenological insight into essence, or some form of direct intuition. Les Œuvres respectives de Piaget et de Kuhn sont des études sur l'évolution d'un certain type de connaissance: la connaissance enfantine chez Piaget, la physique et la chimie chez Kuhn. Dans les deux cas, on essaie d'analyser les facteurs qui déterminent les changements des contenus de la connaissance, et les relations qui existent entre ces divers contenus successifs. Il nous semble qu'une comparaison de ces deux études révèle des convergences intéressantes pour la théorie de la connaissance.
