_The Hearing Impaired Child_ introduces the background issues of hearing impairment then discusses specific aspects. These include causes of hearing loss, speech and language, personality and emotional development, and careers. Appendices provide checklists for language acquisition and reading and writing skills, lists of useful addresses, a helpful glossary and references for further reading.

In this article we explore and contrast the uses and gratifications of the internet for blind/visually impaired and deaf/hearing impaired individuals. The uses and gratifications approach integrates the different issues that surround disabled persons’ internet use into one rich and coherent framework which allows a better understanding of the relationship between benefits obtained from internet use, underlying needs and the barriers that create gaps between gratifications sought and obtained. Based on 21 in-depth interviews, our study shows that both visually and (...) hearing impaired individuals use the internet mostly for gathering information and for communicating with friends and family; meeting new people online was not a priority need. To a great extent these internet uses are driven by the underlying need for independence and active agency. Although our visually and hearing impaired informants share this need, the two groups differ in the gratifications obtained from the internet due to different barriers that hinder their use. ObjectivesMany older adults with visual impairment also have significant hearing loss. The aim was to investigate the effectiveness of a newly developed Dual Sensory Loss protocol on communication and wellbeing of older persons with DSL and their communication partners in the Netherlands and Belgium.MethodsParticipants and their communication partners were randomized in the “DSL-protocol” intervention group or a waiting-list control group. The intervention took 3 to 5 weeks. Occupational therapists focused on optimal use of hearing aids, home-environment modifications and effective communication (...) strategies. The primary outcome was the Communication Strategies domain of the Communication Profile for the Hearing Impaired. Secondary outcomes measured in participants were the Low Vision Quality Of Life Adjustment subscale, the Center for Epidemiological Studies - Depression Scale, De Jong Gierveld Loneliness Scale and the Fatigue Assessment Scale. The Hearing Handicap and Disability Inventory - Reaction of Others subscale and the Care-related Quality of Life - 7 Dimensions was measured in communication partners. Measurements were taken at baseline and 3-month follow-up. Linear mixed models were used to analyze effects between groups over time for every outcome measure.ResultsIntention-to-treat analyses showed a significant effect of the DSL-protocol on the use of verbal strategies in favor of the control group, however, this effect was non-significant after adjustment for confounding. Effect sizes of other outcomes varied between −0.23 [−0.57, 0.12] and 0.30 [−0.05, 0.64]. The LMM showed a significant effect on the HHDI-Reaction of others scale in favor of communication partners in the treatment group, however, the effect did not remain significant at a 0.01 significance level and the effect size was very small and non-significant 0.12, 95% CI [−0.27 to 0.51]. Adjusted analyses did not reveal treatment effects.ConclusionThe DSL-protocol did not clearly contribute to the enhancement of communication and wellbeing in DSL-patients. Possible reasons for the lack of effects are OTs not being comfortable giving advice on communication and psychosocial issues or the short-term treatment and follow-up period. IntroductionHearing loss has a great impact on the people affected, their close partner and the interaction between both, as oral communication is restricted. Nonverbal communication, which expresses emotions and includes implicit information on interpersonal relationship, has rarely been studied in people with hearing impairment. In psychological settings, non-verbal synchrony of body movements in dyads is a reliable method to study interpersonal relationship.Material and methodsA 10-min social interaction was videorecorded in 39 PHI and their significant others. Nonverbal synchrony, which means the (...) nonverbal behaviors of two interacting persons and verbal interaction were analyzed by computer algorithms and observer ratings. Hearing-related quality of life, coping mechanisms, general psychopathology, quality of relationship, and burden of hearing loss experienced by SOs were assessed using questionnaires.ResultsIn the 39 dyads, true nonverbal synchrony differed from pseudosynchrony [t = 2.41; p = 0.02] with a medium effect size. Gender of PHI had a significant effect on general synchrony and on leading by SOs. Age gap correlated with synchronic movements. Very short duration of hearing impairment was associated with lower nonverbal synchrony in the role of leading by SOs. Feeling of closeness by PHI correlated negatively with the role of leading by SOs and feeling of closeness by SOs was positively associated with leading by PHI. No correlation was detected between nonverbal synchrony and other questionnaires. Burden experienced by the SOs was higher in SOs who reported less closeness.DiscussionA longer hearing impairment leads to more nonverbal leading by SOs compared to PHI with very short duration of hearing loss, possibly because of the long-lasting imbalance in communication. If PHI felt more closeness, SOs led less and vice versa. Burden experienced by SOs negatively correlated with closeness reported by SOs. Use of nonverbal signals and communication might help to improve benefits of auditory rehabilitation for PHI and decrease burden experienced by SOs. (shrink)

Postnatal parent-infant physiological regulatory effects described in the previous paper (Part I) are viewed here as being biologically contiguous with events that occur prenatally, preparing and sensitizing the fetus to the average microenvironment into which the infant is expected, based on its evolutionary past, to be born. Following McKenna (1986), evidence (some of which is circumstantial) is presented concerning fetal hearing and fetal amniotic liquid breathing as they are affected both by maternal cardiovascular blood flow sounds in the uterus and (...) by fluctuating maternal blood sugar levels. These data are linked in turn to the infant’s postulated postnatal responsivity to parental sensory cues, including auditory and vestibular respiratory cues that may assist infants as they “learn” to breathe and, for some, to resist a SIDS event. Data on the respiratory and vocalizing behavior of normal and hearing-impaired persons are used to show that not all forms of human breathing are innate; some forms develop with experience. These data reveal how human infants learn, for example, to coordinate higher and lower brain respiratory nuclei in the context of learning initially to cry with intent and purpose and later to speak. Voluntary, cortex-based breathing emerges at the same time that infants are most likely to die from SIDS, between 2 and 4 months of age. This switch between voluntary and involuntary breathing during both sleep (while dreaming) and wake cycles, which depends on the integration of higher cortical and lower brain stem nuclei, is complex and is possibly the basis of the human species’ unique susceptibility to SIDS—a syndrome as yet unrecognized in other species. These human infant vulnerabilities, including delayed maturity, can explain in part why natural selection ought to favor increased infant sensitivity to parental sensory cues provided by a caregiver—stimuli available in the evolving parental care environment that included parent-infant co-sleeping for more than 4–5 million years of human evolution. BackgroundFried physical frailty, with mobility frailty and non-motor frailty phenotypes, is a heterogeneous syndrome. The coexistence of the two phenotypes and cognitive impairment is referred to as cognitive frailty. It remains unknown whether frailty phenotype has a different association with hearing loss and tinnitus.MethodsOf the 5,328 community-dwelling older adults, 429 participants aged ≥58 years were enrolled in the study. The participants were divided into robust, mobility, and non-mobility frailty, mobility and non-mobility CF, and cognitive decline [subdivided into mild cognitive impairment (...) and pre-MCI] groups. The severity and presentations of HL and/or tinnitus were used as dependent variables in the multivariate logistic or nominal regression analyses with forward elimination adjusted for frailty phenotype stratifications and other covariates.ResultsPatients with physical frailty or who are robust were found to have lower probability of developing severe HL and tinnitus, and presented HL and/or tinnitus than those with only cognitive decline, or CF. Patients with RCF and non-mobility RCF had higher probability with less HL and tinnitus, and the presentation of HL and/or tinnitus than those with PRCF and mobility RCF. Other confounders, age, cognitive and social function, cardiovascular disease, depression, and body mass index, independently mediated the severity of HL and tinnitus, and presented HL and/or tinnitus.ConclusionFrailty phenotypes have divergent association with HL and tinnitus. Children with developmental language disorder (DLD) have significant deficits in language ability that cannot be attributed to neurological damage, hearing impairment, or intellectual disability. The symptoms displayed by children with DLD differ across languages. In English, DLD is often marked by severe difficulties acquiring verb inflection. Such difficulties are less apparent in languages with rich verb morphology like Spanish and Italian. Here we show how these differential profiles can be understood in terms of an interaction between properties of the input (...) language, and the child's ability to learn predictive relations between linguistic elements that are separated within a sentence. We apply a simple associative learning model to sequential English and Spanish stimuli and show how the model's ability to associate cues occurring earlier in time with later outcomes affects the acquisition of verb inflection in English more than in Spanish. We relate this to the high frequency of the English bare form (which acts as a default) and the English process of question formation, which means that (unlike in Spanish) bare forms frequently occur in third‐person singular contexts. Finally, we hypothesize that the pro‐drop nature of Spanish makes it easier to associate person and number cues with the verb inflection than in English. Since the factors that conspire to make English verb inflection particularly challenging for learners with weak sequential learning abilities are much reduced or absent in Spanish, this provides an explanation for why learning Spanish verb inflection is relatively unaffected in children with DLD. Children with hearing impairment (HI) show disorders in morphology and syntax. The question is whether and how these disorders are connected to problems in the auditory domain. The aim of this paper is to examine whether moderate to severe hearing loss at a young age affects the ability of German-speaking orally trained children to understand and produce sentences. We focused on sentence structures that are derived by syntactic movement, which have been identified as a sensitive marker for syntactic impairment in (...) other languages and in other populations with syntactic impairment. Therefore, our study tested the repetition and/or comprehension of subject and object relatives, subject and object Wh-questions, passive sentences, and topicalized sentences, as well as sentences with verb movement to second sentential position. We tested 19 HI children aged 9;5-13;6 and compared their performance with hearing children using sentence-picture matching and sentence repetition tasks. For the comprehension tasks, we included HI children who passed an auditory discrimination task; for the sentence repetition tasks, we selected children who passed a screening task of simple sentence repetition without lip-reading; this made sure that they could perceive the words in the tests, so that we could test their grammatical abilities. The results clearly showed that most HI children had considerable difficulties in the comprehension and repetition of sentences with syntactic movement: they had significant difficulties understanding object relatives, Wh-questions, and topicalized sentences, and in the repetition of object who and which questions and subject relatives, as well as in sentences with verb movement to second sentential position. Repetition of passives was only problematic for some children. Object relatives were still difficult at this age for both HI and hearing children. An additional important outcome of the study is that not all sentence structures are impaired—passive structures did not pose a problem for the HI children in comprehension, and caused fewer problems in repetition than other structures. Cochlear implants (CI) support the development of oral language in hearing-impaired children. However, even with CI, speech recognition in noise (SRiN) is limited. This raised the question, whether these restrictions are related to the quality of life (QoL) of children and adolescents with CI and how SRiN and QoL are related to each other. As a result of a systematic literature research only three studies were found, indicating positive moderating effects between SRiN and QoL of young CI users. Thirty studies (...) addressed the quality of life of children and adolescents with CI. Following the criteria of the World Health Organization (WHO) for pediatric HRQoL (1994) only a minority used validated child centered and age appropriate QoL instruments. Moreover, despite the consensus that usually children and adolescents are the most prominent informants of their own QoL (parent- reports complement the information of the children) only a minority of investigators used self-reports. Restricted SRiN may be a burden for the QoL of children and adolescents with CI. Up to now the CI community does not seem to be aware of a possible impairment of QoL in young CI users. Further studies addressing this topic are urgently needed, which is also relevant for parents, clinicians, therapists, teachers, and politicians. Additionally investigators should use valid pediatric QoL instruments. They should be aware, that most of the young CI users are able to inform about their quality of life also themselves (additionally to parents). (shrink)

The birth of a child with hearing impairment imposes more parentaldemands than having a child without a disability. Events such as industrialization, population growth and old age have made the disability more visible. We think that the disabled people's attitude towards being disabled and religion is an important issue to be investigated in terms of formation of the social sensitivity about the learning of the thoughts of disabled people. In this context, it is aimed to investigate the function of the religion in terms of how the disabled identify, understand and overcome the problems related to being disabled. The (...) sample of the study consisted of 58 people, 20 (34.5%) female and 38 (65.5%) male. Semi-structured interview technique was used as the research qualitative research method and descriptive analysis technique was used in the analysis of the data. In the article, the relation between disability and religion has been tried to be understood in terms of sociology of religion in the context of functionalist approach. In general, disabled people evaluate disability (81.1%) as "examination", (10.3%) as "problem" and (8.6%) as "difference”. It is concluded that religion affects the thoughts of disabled person in a positive way to understand and accept the disability. It has been seen that the disabled people have applied to the state, social support systems and spiritual values ​​to overcome their problems. However, it is the duty of all social institutions to increase the social sensitivity to the disabled people.Summary: Events such as industrialization, population growth and old age have further increased disability. It is important that the social sensitivity towards disabled people and disabled people’s thoughts about disability should be learned. We think that the disabled people's attitude towards being disabled and towards religion is an important issue to be investigated. In this context, with the thought and modernization of enlightenment, people's view of human beings and environment has changed. The body which is thought as the trust of God has turned into a body in which one is his proprietor. Therefore, this situation brought the contest, the debate, the comparison and the alienation together.Today, people with disabilities face many financial, spiritual, social and cultural problems. Poverty arising from unemployment, livelihood, accommodation constitute financial problems that are the first to come to mind; access to places of worship, instruction of worship, fulfillment of worship, lack of spiritual care and support constitute spiritual problems; education, health, access and social exclusion constitute social and cultural problems.Disabled people ask themselves these questions related to disability: Why did it happen to me? If it is a test, why do not I start the test on equal conditions with healthy people? Here the disabled person consults the religion to understand and find an answer to the situation that he is in. Because religion is an important factor that responds to the search for meaning, it reduces the feeling of deprivation, and fulfills the task of compensation mechanism. In addition to this, religion gives life a goal and power to the person to overcome the problems he encounters with.In our research on the relationship between disability and religion, religion as in many subjects has contributed positively to the individual’s view towards the fact of disability and to overcome some problems. For example, as a result of the research, the participants evaluated "disability" (81.1 %) as ‘test’ (10.3%) as ‘problem’ and (8.6 %) as ‘difference’.They answered the question ‘How has the disability affected your religious thoughts?’, 71% of the participants expressed that it has affected in a positive way (as a test, made me closer to God, increased my loyalty to God, a motivation source). Besides, 23 % of the participants expressed that it hasn't caused any changes (already believed, fate, disability isn't an obstruct to worship) and 6 % of the participants expressed that it has affected in a negative way (my life is upside down, I am questioning this situation).In addition to those who answered the question ‘Has the disability affected your life?’ in a ‘positive’ way, there is a considerable amount of people who said that the entrances of the mosques are not suitable for the disabled people. In addition, the disabled people stated that they have problems with the way of worshipping (orthopedically disabled), listening to the Qur’ân, hearing the azan (hearing impaired), and cannot go to the mosques and cannot fast because of the health problems, all of which have affected their religious life in a ‘negative’ way. We can gather the answers given to the question in three categories about what kind of behaviors the disabled people do to overcome/eliminate problems. The first is that they receive a variety of services and assistance by applying to the state for material support in order to overcome the problems related to disability and the second is that the social support services are mainly given within the family and friends, and the third is they worship, pray and fast, which are must of Islam, to eliminate the spiritual distress.The rate of people, whose answers are ‘positive’ to this question ‘Does your religious belief have an effect to deal with the disability?’ because they believe the afterlife and who say they feel relieved by praying and worshipping, is very high (95%). Moreover, it is remarkable that the ones say they are ‘not positive’ still explain this with religious concepts such as test and examination which require patience. In short, religion relieves disabled people by fulfilling the task of compensation mechanism with understanding disability and overcoming problems.Almost half of the participants (45%) said no to the following question: Is there any kind of material/spiritual support given to you by the religious officials or religious people in your community? Some factors affecting this situation are that religious officials have no knowledge and education about how to treat disabled people and how to help them.As a result, disabled people face various financial and spiritual problems. They consult religion to solve some of these problems. The vast majority of the disabled consider disability as a test and a fate. Therefore, religion plays an important role for disabled people to understand disability and cope with it. Besides, disabled people have stated that they face various problems in fulfilling their worship and in accessing the places of worship. In this context, physically disabled people also need the supportive power of religion just like other disabled people. (shrink)

The developmental trajectory of speechreading skills is poorly understood, and existing research has revealed rather inconsistent results. In this study, 209 Chinese students with hearing impairment between 7 and 20 years old were asked to complete the Chinese Speechreading Test targeting three linguistics levels. Both response time and accuracy data were collected and analyzed. Results revealed no developmental change in speechreading accuracy between ages 7 and 14 after which the accuracy rate either stagnates or drops; no significant developmental pattern in (...) speed of speechreading across all ages. Results also showed that across all age groups, speechreading accuracy was higher for phrases than words and sentences, and overall levels of speechreading speed fell for phrases, words, and sentences. These findings suggest that the development of speechreading in Chinese is not a continuous, linear process. (shrink)

This article discusses the importance of timely overcoming of language barrier between a deaf person and the society for formation and functionality of such component of the psyche of a deaf person as image of the world. A question is raised on the need for elapse of socialization process of the people with severe hearing impairments in the environment of verbal-gesture bilinguality. The subject of this research is interrelation between the process of popularization of Russian sign language and (...) the process of inclusion of deaf people in the modern Russian society. The scientific novelty consists in revealing mutual determinacy of the process of popularization of Russian sign language and the process of inclusion of deaf people in the modern Russian society from the socio-philosophical perspective. It is proven that full inclusion of deaf people in the society is impossible without a significant increase in the number of the hearing socialization agents who know the Russian sign language. The author underlines that there are certain prerequisites in the modern Russian society for popularization of sign language, as well as indicates the main methods for its popularization. The presented materials can be applied in further research of the various aspects of socialization processes and inclusion of deaf people in socio-philosophical aspect; linguistics (first and foremost psycholinguistics and sociolinguistics), and linguistic philosophy for better understanding of psychosocial component of sign languages; as well as serve as the theoretical framework for solution of practical questions of interaction between deaf people and the society. The COVID-19 pandemic has precipitated profound global transformations, and in the Philippines, Emergency Remote Learning (ERT) emerged as a vital response to address the educational needs of students during this crisis. While existing research has extensively examined the challenges faced by parents during ERT, limited attention was devoted to understanding the unique experiences of parents of children with intellectual disabilities (ID) and hearing impairments (HI). Using a qualitative descriptive case study within interpretative paradigm, this study aims to fill this gap (...) by exploring the challenges encountered and coping strategies employed by 15 parents in the Philippines who have children with ID and HI. Findings illuminate the multifaceted challenges faced by parents, encompassing their roles as caregivers and facilitators of their children’s education. Moreover, it reveals the support mechanisms available to these parents, while also highlighting the pressing need for additional assistance to better meet the complex needs of these children. The research also identifies the most effective strategies employed by parents to navigate the challenges of remote learning. The results serve as a critical resource for parents, educators, and policymakers, providing a roadmap for enhancing the support system for families with children who have ID and HI, particularly during times of crisis. Screening of newborns for permanent congenital or early-onset hearing impairment has emerged as an essential component of neonatal care in developed countries, following favourable outcomes from early intervention in the critical period for optimal speech and language development. Progress towards a similar programme in developing countries, where most of the world’s children with hearing impairment reside, may be impeded by reservations about the available level of support services and the possible effect of the prevailing healthcare challenges. Ethical justification for the (...) systematic introduction of screening programmes for hearing in newborns based on the limitations in current primary prevention strategies, lack of credible alternative early-detection strategies and the incentives for capacity-building for the requisite support services is examined. In this controversial new book O'Hear takes a stand against the fashion for explaining human behavior in terms of evolution. He contends that while the theory of evolution is successful in explaining the development of the natural world in general, it is of limited value when applied to the human world. Because of our reflectiveness and our rationality we take on goals and ideals which cannot be justified in terms of survival-promotion or reproductive advantage. O'Hear examines the nature of human (...) self-consciousness, and argues that evolutionary theory cannot give a satisfactory account of such distinctive facets of human life as the quest for knowledge, moral sense, and the appreciation of beauty; in these we transcend our biological origins. It is our rationality that allows each of us to go beyond not only our biological but also our cultural inheritance: as the author says in the Preface, "we are prisoners neither of our genes nor of the ideas we encounter as we each make our personal and individual way through life.". The study was carried out in order to reveal the problems of hearing-impaired students, who have an important place in our society and the reli-gious coping methods they use to cope with their problems. In the first part of the study, the problems of the hearing impaired students and their religious coping methods were examined. In the second part, the findings obtained as a result of the interviews were interpreted under two headings as the participants' problems and religious coping methods. (...) In accordance with the purpose of the study, semi-structured interview technique, which is one of the qualitative research methods, was used. The study group of the research, which was designed according to the phenomenology method, one of the qualitative research methods, consisted of 11 students, 9 boys and 2 girls, studying at the 9th, 10th, 11th and 12th grades of Çorum Special Education Vocational High School. The data obtained in the study were analyzed by content analysis method. As a result of the study, it was seen that hearing-impaired students frequently resort to positive religious coping methods to overcome their problems. It is also concluded that the students are willing to share their problems; religious thoughts, feelings and behaviors have an important effect on coping with their problems; they use religious coping methods such as trust in Allah, prayer and worship while coping with their problems but they are open to support from the environment; they try to overcome the problems not with complaints but with patience. It has been observed that hearing-impaired students sometimes question why they are disabled rather than others, and they feel uneasy in this situation. For hearing-impaired individuals to protect their mental health, necessary training should be provided without neglecting the religious aspect of their problems. (shrink)

His approach is firmly within the classical tradition of natural theology, but an underlying theme is the differences between the personal Creator of the Bible or the Koran and a God conceived of as the indeterminate ground of everything determinate. Drawing on several religious traditions and on the resources of contemporary philosophy, specific chapters analyse the nature of religious faith and of religious experience. They examine connections (...) between religion and morality, and religion and human knowledge – the cosmological, teleological and ontological arguments, process thought, and the problem that evil presents for religion. This book expounds and analyses notions of transcendence, creation and incarnation reflectively and personally, combining both philosophical and religious insights. Preferring tender-minded approaches to reductively materialistic ones, it shows some ways in which reductive approaches to human affairs can distort the appreication of our lives and activities. In the book's first half it examines a number of aspects of human life and experience in the thought of Darwin, Ruskin, and Scruton with a view to exploring the extent to which there (...) could be intimations of transcendence. The second half is then devoted to outlining an account of divine creation and incarnation, deriving initially, though not uncritically, from the thought of Simone Weil. The text concludes by examining the extent to which grace is needed to engage in religious practice and belief. Taking in art, literature, music and classical Greek writings, this is a multifaceted thesis on transcendence. The ethics of involving persons with cognitive impairments and/or mental illness in research continues to gain academic and public attention. Concerns about the ability of such persons to provide ethically and legally valid consent and about the appropriateness of their research involvement in certain categories of studies have resulted in publication of guidelines, position papers, standards, and court decisions. These analyses address not only when and from whom informed consent may be obtained but also under what conditions it is ethically (...) permissible to involve persons in research who are too decisionally impaired to provide their own consent.It is an advance in research ethics that there is heightened appreciation of the need for greater protections for, and possibly research participation limitations on, persons unable to give their own consent. In our zeal to protect potentially vulnerable subjects, however, we must craft new protections carefully. Prudence is required in shaping and implementing any new protections. (shrink)

Medical students’ mask-making can provide valuable insights into personal and professional identity formation and wellness. A subset of first- and second-year medical students attending a medical school wellness retreat participated in a mask-making workshop. Faculty-student teams examined student masks and explanatory narratives using visual and textual analysis techniques. A quantitative survey assessed student perceptions of the experience. We identified an overarching theme: “Reconciliation/reclamation of authentic identity.” The combination of nonverbal mask-making and narrative offers rich insights into medical students’ experience and (...) thinking. Objective: Individually, vision and hearing impairments have been linked to higher levels of anxiety and depression. We investigated the effect of dual sensory impairment in a large representative sample of Spanish adults. Methods: Data from a total of 23,089 adults from the Spanish National Health Survey 2017 were analyzed. Self-reported difficulty of seeing and hearing, and depression and chronic anxiety were analyzed. Multivariable logistic regression was assessed for difficulty with vision alone, hearing alone and with difficulty with both, adjusting for (...) gender, age, marital status, living as a couple, education, smoking, alcohol consumption, BMI, physical activity, use of glasses/contact lenses, and hearing aid. Results: Visual difficulty, hearing difficulty, and dual difficulties were all associated with significantly higher odds for depression and for chronic anxiety. Dual sensory difficulty was associated with higher odds ratios for depression and anxiety when compared to either impairment alone. Conclusion: Dual sensory difficulty is associated with significantly higher odds of anxiety and depression when compared to either vision or hearing difficulty alone. Appropriate interventions are needed to address any reversible causes of vision and hearing as well as anxiety and depression in people in these specific groups. (shrink)

Healthcare depends increasingly on advanced medical technology. In addition, other forms of technology contribute to determine how our lives are influenced by disease and disability. The extent to which persons with impaired bodily functions are forced to live their lives differently than other people depends to a large part on a variety of technologies, from wheelchairs to computer interfaces, from hearing aids to garage doors. This wide-ranging influence of technology has important ethical aspects, but has seldom been discussed in bioethics, (...) the ethics of technology, or other branches of applied ethics. (shrink)

One central and unfortunately unavoidable characteristic of the aging process is its association with chronic physiological deterioration. Frailty, cognitive impairment, and physical conditions such as cardiovascular disease and vision and hearing loss are more frequent in this phase of life, and these conditions translate into an increasing need for care and support of multiple kinds. In traditional bioethical scholarship, these distinctive features of aging have been examined predominantly through a health‐focused lens. My main contention in this essay, however, is that (...) viewing aging within bioethics as primarily a health problem, to be addressed through frameworks for decision‐making or for resource allocation, is inadequate. My aim is to consider how the health conditions associated with aging affect older people’s lives in a much more expansive way than has typically been acknowledged. Just as importantly, I intend to show how shifting our bioethical imagination in this way raises different and challenging questions about what a good life in late life consists in and about what is owed to older people, in their personal and social lives, as a matter of justice. In lieu of an abstract, here is a brief excerpt of the content:Deepening and Expanding Both–And ApproachesTasia Scrutton, PhDExcitingly for the topic of religion and mental health, both Gipps’ and Finley’s commentaries point to the emergence of a both-and consensus. Finley does this in a number of ways, for example by pointing to the ways in which her own brilliant research has provided further and more specific support for a “honeysuckle on a broken fence” model, and also inviting a renewed (...) examination of Model 3 in response to studies on absorption, psychosis and religious experience. Gipps does this by putting forward an additional both-and model, according to which a potential or early-stage instance of mental illness, responded to by a grace-ful engagement with psychotherapy, becomes instead an instance of metanoia through which the person’s drives and emotions are integrated into a more rounded adult psyche.As well as pointing to the emergence of a both-and consensus, Finley and Gipps’ commentaries importantly reflect the fact that both-and accounts are likely to be diverse, complex and multi-faceted. As Gipps notes, my account focuses on spiritual guidance and medication, especially psychiatric medication; the therapeutically informed psychological perspective that Gipps provides is a very welcome addition to the discussion. In addition to both-and accounts being diverse, different psychiatric conditions and symptoms may invite different models, and multiple models may be in play in one person’s life at the same time. Finley gives an example of this possibility when she posits that a version of model 3 might fruitfully by understood as part of, or perhaps combined with, Model 4.Hopefully this paper and the two commentaries on it open the door to a deeper account of the “honeysuckle on a broken fence” model and potentially to other both-and models too. I want to see that door open—but perhaps not too open, and I think less open certainly than Finley and perhaps also Gipps would like. This is because of my view of the relationship between suffering and psychopathology, and suffering and evil (or badness), which I want to discuss in more detail below.Suffering, Badness, and PathologyThe first thing I want to say is that I think that suffering is bad, by which I mean that it is undesirable. This is true to the way we talk about suffering (if someone tells us that they are suffering, we express regret, not joy), and it is important [End Page 265] morally to remember that suffering is an evil if we are to work to prevent and overcome it (including in ways relevant to mental illness, such as through social justice, peace-building and medical care). It can be tempting to romanticize suffering and to try to justify its romanticization on the basis that sometimes good things come out of situations of suffering. However, to do this is to conflate what something is with other things it may become intertwined with. It is also to encourage masochistic and socially and politically conservative forms of spirituality that are antithetical to the Christian Gospel, since it baptizes suffering rather than seeing suffering as something people are called to be liberated from.The second thing I want to say is that I think that pathology, including psychopathology, involves suffering, and that if it does not involve suffering (either in a specific moment or over time) then it is not pathology. The centrality of suffering to pathology is indicated by the fact that the term ‘distress,’ which denotes suffering, appears in the DSM-5 618 times. The presence of either distress or impairment in social, occupational or important other areas of functioning (which, over time, is likely to lead to suffering) is a prerequisite of many psychiatric conditions, according to DSM-5. Disorders of the self figure prominently in psychotic experiences. Subjects de­scribe that “alien” thoughts are inserted in their mind by foreign powers, can sometimes hear their thoughts aloud or describe complex voices interacting with each other. Such experiences can be conceptualized in the framework of a Philosophical Anthropology, which suggests that human experience is characterized by centric and excentric positionality: subjects experience their environment centered around their enlived body and at the same time can reflect upon their place in a (...) shared lifeworld from an excentric point of view. Pre-reflective self awareness has been suggested to ensure that subjects can identify their own thoughts or actions as belonging to themselves, even when they reflect upon them from an excentric point of view. This pre-reflective self awareness appears to be impaired during psychotic experiences, when subjects no longer identify thoughts in their own stream of consciousness as belonging to themselves and instead attribute them to an outside agent. Among several potential causes, it is suggested that such impairments can be due to discrimi­natory or traumatic experiences, which affect the enlived (centric) position of a person and make her feel encircled and deeply threatened by aversive powers. As a consequence, the afflicted individual may fundamentally distance herself from her current centric position in a hostile environment, at the price of experiencing her own thoughts or actions as alien. Philosophical Anthropology may thus help to explain how social exclusion, discrimination and traumatization can promote psychotic experiences and why social support is of primary importance for any treatment of psychosis.
