PurposeThis study aims to develop and validate a stigma scale for Chinese patients with breast cancer.MethodsPatients admitted to the Affiliated Cancer Hospital of Xiangya School of Medicine, Central South University, for breast cancer treatment participated in this study. Development of the Breast Cancer Stigma Scale involved the following procedures: literature review, interview, and applying a theoretical model to generate items; the Breast Cancer Stigma Scale’s content validity was assessed by a Delphi study and feedback from patients with breast cancer ; (...) exploratory factor analysis was used to assess the construct validity; convergent validity was assessed with the Social Impact Scale ; internal consistency Cronbach’s α, split-half reliability, and test–retest reliability were used to identify the reliability of the scale.ResultsThe final version of the Breast Cancer Stigma Scale consisted of 15 items and showed positive correlations with the Social Impact Scale. Exploratory factor analysis revealed four components of the Breast Cancer Stigma Scale: self-image impairment, social isolation, discrimination, and internalized stigma, which were strongly related to our perceived breast cancer stigma model and accounted for 69.443% of the total variance. Cronbach’s α for the total scale was 0.86, and each subscale was 0.75–0.882. The test–retest reliability with intra-class correlation coefficients of the total scale was 0.947, and split-half reliability with intra-class correlation coefficients of the total scale was 0.911. The content validity index was 0.73–1.0.ConclusionThe newly developed Breast Cancer Stigma Scale offers a valid and reliable instrument for assessing the perceived stigma of patients with breast cancer in clinical and research settings. We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. The use of forward models is well established in cognitive and computational neuroscience. We compare and contrast two recent, but interestingly divergent, accounts of the place of forward models in the human cognitive architecture. On the Auxiliary Forward Model account, forward models are special-purpose prediction mechanisms implemented by additional circuitry distinct from core mechanisms of perception and action. On the Integral Forward Model account, forward models lie at the heart of all forms of perception and action. We compare these neighbouring (...) but importantly different visions and consider their implications for the cognitive sciences. We end by asking what kinds of empirical research might offer evidence favouring one or the other of these approaches. (shrink)

Criticism and interpretation of Murtaz̤á Muṭahharī; Iranian religious scholar.

In this essay I attempt to unpack Andy Kaufman in his many manifestations, ultimately arguing that traditional notions of comedy cannot help us get at the root of what is going on here. Through a discussion and criticism of the theories of comedy presented by Christopher Fry, Susanne Langer, Walter Kerr, and Maurice Charney, I suggest how Andy's comedy employs a rejection of the modernist conceits of a fixed identity, a denotative language, a progressive history, and a separation (...) of temporality from Being. Ultimately focusing on the way in which Freud's theory of comedy gets deconstructed when we read it through the lens of Andy’s work, I take apart specific jokes and comedic “moments” in Andy's work and show how the Freudian framework cannot support such an aesthetic. What is, in fact, most challenging in Andy's work as a comedian is not merely the way in which his own identity is up for grabs but the way in which he forces the audience to have its identity as an audience called into question, the way in which a Baudrillardean simulacrum stands in for a missing punchline while we, his fans, wait for something that is never coming. And yet we, his fans, laugh. Sometimes uncomfortably, but always more authentically. (shrink)

BackgroundEmergency separation of conjoined twins is performed when one twin is already dead or dying and threatens the survival of the other. The particular decision to perform an emergency separation of conjoined twins provides an ethical dilemma that needs special attention. Adding to the complexity of surgical and postsurgical management in emergency separation, ethical and sociocultural aspects further complicate decision-making.Case presentationFrom 1987 to 2022, 18 conjoined twin separations were performed in our centre. This paper describes three conjoined twin emergency separations. (...) In the first case of thoracoomphalopagus babies at nine days of age, one baby was diagnosed with necrotizing enterocolitis with frequent desaturation and seizures, and the other baby was healthy. Emergency separation was performed on the twelfth day of age; unfortunately, neither baby survived the surgery. In the second case, emergency separation was performed on the 110th day of life due to sepsis in one baby. The nonseptic twin passed away six hours after surgery, while the septic twin died 12 days after surgery due to wound dehiscence and abdominal sepsis. The third case was of an omphalopagus conjoined twin with a parasitic twin. The healthy baby was deemed nonviable but found to be healthy upon birth. Immediate emergency separation was performed at 2 h of age. The living baby survived the surgery but passed away two months later.ConclusionsWhen separation is deemed necessary to save one twin, it becomes difficult to apply standard ethical medical reasoning. The decision to separate results in most cases in very high-risk surgeries with poor outcomes during surgery and postsurgery. Compounded by the complexity of the case, sociocultural and religious aspects further add to the dynamics of decision-making. A multidisciplinary team must work together with a health ethics committee and navigate through this ethical conundrum with the patient and family at its decision-making centre to decide on the best plan of care. Processes of rapid and truncated agrarian change—driven through expanding urbanisation, infrastructure development, extractive industries, and commodity crops—are shaping the livelihood opportunities and aspirations of Indonesia’s rural youth. This study describes the everyday experiences of youth as they navigate the changing character of agriculture, aquaculture, and fishing livelihoods across gender, class, and generation. Drawing on qualitative field research conducted in the Maros District of South Sulawesi, we examine young people’s experiences of agrarian change in a landscape of entangled rural, coastal and (...) increasingly urbanised spaces. We find that young people aspire to secure, modern, and salary-based work, while continuing to seek and sustain intergenerational farming or aquaculture-based livelihoods. Youth take advantage of increased connectivity to diversify their incomes, yet their dependence on mobility also introduces new forms of gendered and class based precarity such as insecure working arrangements, disruption to education and violence (especially for young unskilled women and youth from financially insecure households). Our study highlights the persistent conditions of precarity that many young people encounter in both rural and urban settings, while challenging assumptions that youth are uninterested in rural futures. Some highly saturated models of Peano Arithmetic are constructed in this paper, which consists of two independent sections. In § 1 we answer a question raised in [10] by constructing some highly saturated, rather classless models of PA. A question raised in [7], [3], ]4] is answered in §2, where highly saturated, nonstandard universes having no bad cuts are constructed.Highly saturated, rather classless models of Peano Arithmetic were constructed in [10]. The main result proved there is the following theorem. If (...) λ is a regular cardinal andis aλ-saturated model of PA such that ∣M∣ >λ, thenhas an elementary extensionof the same cardinality which is alsoλ-saturated and which, in addition, is rather classless. The construction in [10] produced a modelfor which cf() =λ+. We asked in Question 5.1 of [10] what other cofinalities could such a modelhave. This question is answered here in Theorem 1.1 of §1 by showing that any cofinality not immediately excluded is possible. Its proof does not depend on the theorem from [10]; in fact, the proof presented here gives a proof of that theorem which is much simpler and shorter than the one in [10].Recursively saturated, rather classless κ-like models of PA were constructed in [9]. In the case of singular κ such models were constructed whenever cf(κ) > ℵ0; no additional set-theoretic hypothesis was needed. (shrink)

Some accept the demarcations of skin and skull, and say that what is outside the body is outside the mind. Others are impressed by arguments suggesting that the meaning of our words "just ain't in the head", and hold that this externalism about meaning carries over into an externalism about mind. It explains and explores the biological basis of PDP, its psychological importance, and its philosophical relevance.

The last ten years have seen an increasing interest, within cognitive science, in issues concerning the physical body, the local environment, and the complex interplay between neural systems and the wider world in which they function. --œPhysically embodied, environmentally embedded--� approaches thus loom large on the contemporary cognitive scientific scene. Yet many unanswered questions remain, and the shape of a genuinely embodied, embedded science of the mind is still unclear. I begin by sketching a few examples of the approach, and (...) then raise a variety of critical questions concerning its nature and scope. A distinction is drawn between two kinds of appeal to embodiment: 'simple' cases, in which bodily and environmental properties merely constrain accounts that retain the focus on inner organization and processing, and more radical appeals, in which attention to bodily and environmental features is meant to transform both the subject matter and the theoretical framework of cognitive science. Of course, words aren’t magic. Neither are sextants, compasses, maps, slide rules and all the other paraphenelia which have accreted around the basic biological brains of homo sapiens. In the case of these other tools and props, however, it is transparently clear that they function so as to either carry out or to facilitate computational operations important to various human projects. The slide rule transforms complex mathematical problems (ones that would baffle or tax the unaided subject) into simple tasks of (...) perceptual recognition. The map provides geographical information in a format well-suited to aid complex planning and strategic military operations. The compass gathers and displays a kind of information that (most) unaided human subjects do not seem to command. These various tools and props thus act to generate information, or to store it, or to transform it, or some combination of the three. In so doing, they impact our individual and collective problem- solving capacities in much the same dramatic ways as various software packages impact the performance of a simple pc. (shrink)

There is a definite challenge in the air regarding the pivotal notion of internal representation. This challenge is explicit in, e.g., van Gelder, 1995; Beer, 1995; Thelen & Smith, 1994; Wheeler, 1994; and elsewhere. We think it is a challenge that can be met and that (importantly) can be met by arguing from within a general framework that accepts many of the basic premises of the work (in new robotics and in dynamical systems theory) that motivates such scepticism in the (...) first place. Our strategy will be as follows. We begin (Section 1) by offering an account (an example and something close to a definition) of what we shall term Minimal Robust Representationalism (MRR). Sections 2 & 3 address some likely worries and questions about this notion. We end (Section 4) by making explicit the conditions under which, on our account, a science (e.g., robot- ics) may claim to be addressing cognitive phenomena. (shrink)
