Around the world, unhealthy diets are a leading cause of disease. Shifting population diets in a healthier direction will require downstream policy interventions. This means changing the composition of the processed food supply, particularly reducing salt, sugar and fat. Mandatory nutrient limits imposed by government are one way of achieving this. However, they have been criticized as a particularly intrusive regulatory option, interfering with both free markets and free choices. At the same time, voluntary industry reformulation has become an intervention (...) favoured by national governments, the World Health Organization and the food industry. This article uses a comparison of the two interventions—which share a common public health goal, albeit achieved through different regulatory means—as a basis to evaluate the ethical charges against mandatory nutrient limits. It makes three main findings: that both affect free dietary choice in very similar ways; that dominant public health ethics frameworks are not well equipped to compare mandatory and voluntary forms of regulation; and that food governance is inherently multisectoral, involving markets, governments and the public. Taking these findings into account, the article calls for a more nuanced ethical evaluation of food reformulation policies. (shrink)

Living together peacefully in a world of differences asks for a practice of respecting the irreducible difference of the other. Acknowledging this `not-me' of the other subject generates an in-between: a space/time between subjects that cannot be transgressed other than by violence. Following Irigaray, I argue that this `in-between' comes about through the passion of wonder, a being touched in the flesh in the encounter with the other, which opens the subject to him/herself and to the other. To perceive this (...) touch asks for a life-style directed at keeping open and sensitizing the sensibilities of the senses, so that the subject's aesthesis or awareness for the perceptions of the sensible flesh and of its reactions to the other is heightened. Key-element in this aesthetics is the practice of breathing. It enables the subject to remain rooted in him/or herself and open to the other: generating and respecting the in-between between them. We describe research carried out as part of a text summarisation project for the legal domain for which we use a new XML corpus of judgments of the UK House of Lords. These judgments represent a particularly important part of public discourse due to the role that precedents play in English law. We present experimental results using a range of features and machine learning techniques for the task of predicting the rhetorical status of sentences and for the task of selecting (...) the most summary-worthy sentences from a document. Results for these components are encouraging as they achieve state-of-the-art accuracy using robust, automatically generated cue phrase information. Sample output from the system illustrates the potential of summarisation technology for legal information management systems and highlights the utility of our rhetorical annotation scheme as a model of legal discourse, which provides a clear means for structuring summaries and tailoring them to different types of users. BackgroundImplementation science research, especially when using participatory and co-design approaches, raises unique challenges for research ethics committees. Such challenges may be poorly addressed by approval and governance mechanisms that were developed for more traditional research approaches such as randomised controlled trials.DiscussionImplementation science commonly involves the partnership of researchers and stakeholders, attempting to understand and encourage uptake of completed or piloted research. A co-creation approach involves collaboration between researchers and end users from the onset, in question framing, research design and delivery, (...) and influencing strategy, with implementation and broader dissemination strategies part of its design from gestation. A defining feature of co-creation is its emergent and adaptive nature, making detailed pre-specification of interventions and outcome measures impossible. This methodology sits oddly with ethics committee protocols that require precise pre-definition of interventions, mode of delivery, outcome measurements, and the role of study participants. But the strict requirements of ethics committees were developed for a purpose – to protect participants from harm and help ensure the rigour and transparency of studies. We propose some guiding principles to help square this circle. First, ethics committees should acknowledge and celebrate the diversity of research approaches, both formally and informally ; without active support, their members may not understand or value participatory designs. Second, ground rules should be established for co-design applications and communicated to committee members and stakeholders. Third, the benefits of power-sharing should be recognised and credit given to measures likely to support this important goal, especially in research with vulnerable communities. Co-design is considered best practice, for example, in research involving indigenous peoples in New Zealand, Australia and Canada. (shrink)

In this paper back-and-forth structures are applied to the semantics of natural language. Back-and-forth structures consist of an event structure and an interval structure communicating via a relational link; transitions in the one structure correspond to transitions in the other. Such entities enable us to view temporal constructions (such as tense, aspect, and temporal connectives) as methods of moving systematically between information sources. We illustrate this with a treatment of the English present perfect, and progressive aspect, that draws on ideas (...) developed in Moens & Steedman (1988), and discuss the role of rich ontologies in formal semantics. Location Based Services (LBS) are information services that can be\naccessed with mobile phones. They rely on the system's knowledge of the\ngeographical position and the identifier of the phone being used, and\ntherefore on collecting data on an identified or identifiable user. In\nthis respect, the French data protection act stipulates that users agree\nto be supplied with an LBS service on a fully transparent basis and that\ntheir location data must be erased or made anonymous once the service\nhas been provided. In some cases, location-based (...) services are provided\non the user's own request, but in other cases, the individual concerned\nis unaware of the fact that third parties know where s/he is. This\ncreates potential for surveillance and misuse on the part of both police\nand businesses. What is at stake here is not only the right of\nindividuals to freedom of movement and to privacy but also, and more\nperniciously, citizens' autonomy with regard to information and\ndecision-making. This paper explores an approach to the implementation and evaluation of integrated health service delivery. It identifies the key issues involved in integration evaluation, provides a framework for assessment and identifies areas for the development of new tools and measures. A proactive role for evaluators in responding to health service reform is advocated.

The proposed national PrEP program would serve people who are uninsured as well as those enrolled in Medicaid. In this article, the authors propose a set of recommendations for the proposed program’s implementers as well as state Medicaid agencies and Medicaid managed care organizations to ensure PrEP access for people enrolled in Medicaid, addressing gaps without undermining the important role of the Medicaid program in covering and promoting PrEP.

Inspired by Ina Praetorius’ expression that seven billion dignitaries walk the earth, the author connects the different aspects of the concept of human (...) dignity with the image of the dignitary, arguing that this image evokes the idea that the subject is free to move and is meant to flourish according to their capabilities. The author also discusses the difficulties of the idea of human flourishing as an ideal for humanity, especially in the context of disability and illness. This book provides an up-to-date introduction to the study of generics and pursues the enterprise of the influential Generic Book edited by Gregory Carlson and Jeffry Pelletier, which was published in 1995. Genericity is a key notion in the study of human cognition as it reveals our capacity to organize our perceived reality into classes and to describe regularities. The generic can be expressed at the level of a word or phrase (ie the potato in The Irish economy became dependent (...) upon the potato) or an entire sentence (eg in John smokes a cigar after dinner, the generic aspect is a property of the expression, rather than any single word or phrase within it). This book gathers new work from senior and young researchers to reconsider the notion of genericity, examining the distinct contributions made by the determiner phrase (eg the notions of kind/individual) and the verbal predicate (eg the notions of permanency, disposition, ability, habituality, and plurality). Finally, in connection with the whole sentence, the analytic/synthetic distinction is discussed as well as the notion of normality. Based on a sociological survey carried out in a camp for asylum seekers in Belgium, the article questions the modes of existence in this “out of place” and “out of time” that is the camp. Behind the apparent emptiness of waiting in a decelerated present, the investigation highlights three temporalities that together shape the breathing of the camp and the living conditions of asylum seekers: the rhythm of the framework that holds together daily life, the cycle and the passage that (...) gives a sense of the ordeal and finally the singular time of the articulations between present, past and future which characterize the “being in time” of asylum seekers. After exploring these three temporalities, the author proposes a concept that allows them to hold together in order to understand the texture of time that unfolds in these “out of place”, “out of time” that are the camps. This commentary questions the applicability of the Newell Test for evaluating the utility of connectionism. Rather than being a specific theory of cognition (because connectionism can be used to model nativist, behaviorist, or constructivist theories), connectionism, we argue, offers researchers a collection of computational and conceptual tools that are particularly useful for investigating and rendering specific fundamental issues of human development. These benefits of connectionism are not well captured by evaluating it against Newell's criteria for a unified theory of cognition.

Nursing Inquiry 2012; 19: 334–344 Trappings of technology: casting palliative care nursing as legal relationsCommunity palliative care nurses in Perth have joined the throng of healthcare workers relying on personal digital assistants (PDAs) to store, access and send client information in ‘real time’. This paper is guided by Heidegger’s approach to technologies and Habermas’ insights into the role of law in administering social welfare programs to reveal how new ethical and legal understandings regarding patient information add to nursing’s (...) professional responsibilities. This qualitative research interprets data from interviews with twenty community palliative care nurses about clients’ legal rights to informational privacy and confidentiality. It explores nurses’ views of their nursing responsibilities regarding clients’ legal rights, liability issues, bureaucratic monitoring and enforcement procedures. It concludes that nurses and clients are construed as legal subjects entrenched in legal relations that have magnified since these nurses began using PDAs in 2005/2006. (shrink)

In ‘The End of Law’, Bill Scheuerman illustrates the ways normativity, context and decision interlace, putting the lie to Carl Schmitt’s claim that decision is pure will. In doing so, Scheuerman gestures toward a truth about the alchemical nature of constitutions. Like decisions, I argue, constitutions are alchemical mechanisms for actualizing norms and normativizing facts. They accomplish this in part through mediating between dynamic (individual and political) selves before and after the moment of decision or coming-into-force. Schmitt’s error – or (...) perhaps his strategy – is to make static this dynamic process of political self-formation. Viewed as static, it is more difficult to discern the process of normativizing facts and concretizing norms. I show how contemporary populist authoritarians are particularly skilled at harnessing this strategy. Populist authoritarians often use constitutional change to consolidate not just power but constructed identity. They are able to do so because constitutions provide this strategy of dynamic identity formation, which, by generating new normative imperatives, in turn shores up legitimacy. (shrink)

A simplified model using kinetic logic is proposed to approach the problem after Hepatitis B viral (HBV) infection. It accounts for several stable regimes or attractors corresponding to the essential dynamic behaviour of the replication of the Hepatitis B virus. Infection with the virus can result in viral clearance, fulminant hepatic failure and death, or chronic transmissible infection, that is multistationarity corresponding to the existence of the positive feedback circuit in our modelling. Another implication of this model is the existence (...) of oscillations or homeostatic mechanisms, sometimes observed in the viral cycle, consistent with the existence of the negative feedback circuit. Thus, this report shows how a simple model of kinetic logic may be used to account for the variety of manifestations of HBV infection. This model implies the presence of the Hepatitis B e antigen, whose conservation suggests that it plays an important role in the life cycle of hepadnaviruses. Its function in the viral cycle is still unknown, but our model suggests that this antigen could explain the passage from one state of the viral infection (acute or latent) to another, as well as the oscillatory behavior which may account for the intermittent symptoms of hepatitis observed in some patients. Furthermore, this model shows a virgin state. This state is also reached after recovery. The model proposed demonstrates that starting from a viral acute infection, the host's immune response, depending on the immunological status of the patient, can lead to viral clearance, or to periodic spontaneous reactivation.
