Objective: To assess the opinions and practice patterns of obstetrician-gynaecologists on acceptance and use of free drug samples and other incentive items from pharmaceutical representatives.Methods: A questionnaire was mailed in March 2003 to 397 members of the American College of Obstetricians and Gynecologists who participate in the Collaborative Ambulatory Research Network.Results: The response rate was 55%. Most respondents thought it proper to accept drug samples , an informational lunch , an anatomical model or a well-paid consultantship from pharmaceutical representatives. A (...) third of the respondents thought that their own decision to prescribe a drug would probably be influenced by accepting drug samples. Respondents were more likely to think the average doctor’s prescribing would be influenced by acceptance of the items than theirs would be . Respondents who distributed drug samples to patients indicated doing so because of patients’ financial need and for their convenience and less so as a result of knowledge of the efficacy of the sample product . A third of respondents agreed that interactions with industry should be more strictly regulated.Conclusion: Obstetrician-gynaecologists largely indicated that they would act in accordance with what they think is proper regarding accepting incentive items from pharmaceutical representatives. Although accepting free drug samples was considered to be appropriate more often than any other item, samples were most commonly judged to be influential on prescribing practices. The widely accepted practice of receiving and distributing free drug samples needs to be examined more carefully. (shrink)

A method of developing a relational semantics and relational proof systems for many-valued logics based on finite algebras of truth values is presented. The method is applied to Rosser-Turquette logic, logics based on symmetric Heyting algebras with operators and a Post-style logic.

In lieu of an abstract, here is a brief excerpt of the content:Human Genome Research in an Interdependent WorldAlexander Morgan Capron (bio)This has been the year of agenda-setting conferences for the ambitious ELSI (ethical, legal and social issues) program of the Human Genome Project (HGP). But of the dozen or more major meetings of this sort held across the country, the one held at the National Institutes of Heakh (NIH) in Bethesda, MD, June 2-4, 1991, was distinctive in several (...) respects.1As its name implies, "Human Genome Research in an Interdependent World" was a global look at the issues raised by gene mapping and sequencing. Unlike previous conferences, however, this meeting looked beyond a comparative analysis of clinical and domestic legal issues, and concentrated on topics that may require responses on a truly international level if they are to be successfully resolved.The meeting's organizers gathered an impressive group of more than seventy participants from fifteen countries, including the first significant contingent of Soviet and Japanese scientists, physicians, and philosophers at an ELSI meeting. In addition to talks by the heads of the genome projects in the Soviet Union, Japan, and the United States, the meeting was keynoted by Dr. James Wyngaarden, director general of the Human Genome Organization (HUGO), and I had the honor of serving as its general chairman.The goal of the meeting was to consider an array of issues that scientists, philosophers, and lawyers from around the world suggested might have international significance, with an eye to deciding three things: (1) Does the issue deserve further attention? (2) Is the issue best addressed in an international or domestic context? and (3) Do structures exist to which the issue can be referred for resolution? In the final sessions, the entire group debated proposed resolutions on the various topics and narrowed the list needing further, intensive exploration.To provide an international framework for this process, the conference [End Page 247] established a Global Steering Committee on Ethical and Social Issues in Genome Research, which will coordinate the efforts of several task forces charged with refining the tentative resolutions adopted by the conference participants on June 4. The task forces are expected to be able to complete work on some topics by the time of the steering committee's first meeting, which was described by one participant as a "check point," probably within the coming year; on other topics, the task forces will provide interim reports and arrange to continue their analysis. Several topics—less complex or more embryonic—were referred directly to the steering committee. On all issues, a major objective will be to ascertain what existing or newly created structure or structures are capable of implementing the group's recommendations and then to monitor the issue to ensure that appropriate implementation is occurring.Issues to be Explored by Task ForcesInsurance and EmploymentWhile individuals may derive medically useful information from the expanded range of genetic tests that will flow from the HGP, they could also be harmed if test results become a basis for discrimination. An area of particular concern at the conference in Bethesda was whether genetic information should be used to decide eligibility for employment or the scope and cost of health and life insurance. The complexity of this issue is increased as barriers to worker migration fall (especially in Europe) and as more firms carry on business internationally. Furthermore, questions arise about the limits of required testing and whether individuals may decline to be informed of the results of tests.The task force will not only gather information on practices and legal standards throughout the world—especially in light of the differences among nations with regard to the linkage of employment and various forms of insurance—but will also examine international anti-discrimination laws as a possible means of supplementing domestic statutes and regulations. As a starting point, the Bethesda meeting agreed on a set of principles for the task force to refine.2ForensicsAlthough their legal status is not fully resolved, DNA tests are being used increasingly in judicial proceedings and civil and criminal investigations as a highly probative means to identify people. THAT DESCARTES WAS INTERESTED from the very start of his philosophic career in developing a method for problem-solving that could be applied generally to the solution of "unknowns" is well known. Also well known is the further development of the method by the introduction of the technique of hyperbolic doubt in his mature, metaphysical works, especially in the Meditations. Perhaps less widely appreciated is the important role that accounts of systems of signs played in the development of his early accounts (...) of a method; it is crucial to the method that the elements of problems be identified and signified by simple and convenient signs that can then be arranged to display the relations among the known elements and the unknown solution. Commentators such as Michel Foucault have brought this interest in signification to our attention and shown the widespread importance of such projects in the seventeenth century. Few, however, have examined the relation between the early projects of signification and the later, metaphysical works. I suggest that one way to understand Descartes' metaphysical turn in his mature work is as an attempt to ground the accounts he has offered of signification in something further: representation, particularly mental representation. From the start, Descartes explains signification by reference to mental representation: a word, figure, or symbol signifies its objects by prompting the mind to think of those objects. Although Descartes offers detailed and comprehensive accounts in works such as the Rules for the Direction of the Mind of what constitutes effective and convenient systems of signification by way of the relations between signifying element, system, and the objects signified thereby, the mental operations that allow signification to operate, especially that of the "intuition of simple natures," is never fully explained. It remains to the mature phase of Descartes' career to crack open the basic operations of our thought by developing the full-fledged doctrine of ideas--a doctrine that makes explicit how an object is represented to a subject. There is a problem about belief and knowledge in Plato's epistemology that has exercised serious students of Plato only to settle into a recent orthodoxy. Guthrie characterizes the problem and its current resolution this way: ‘In the Meno doxa appeared to be a dim apprehension of the same objects of which knowledge is a clear and complete understanding … in the Republic each is directed to different objects, knowledge to the Forms and doxa to the sensible world alone … at (...) least the opinion seems now to prevail that on the relationship between doxa and knowledge Meno and Republic are irreconcilable, and exhibit a complete change of mind on Plato's part.'. In this short paper I reply to Wright's response to my original essay on his Christianity and Critical Realism. Wright makes a number of important points, and these are interesting in so far as they indicate how the same issues can be read in quite different ways. However, this difference ultimately highlights the central issue I explore in the original essay. That is, the capacity for basic dividing lines to occur and persist because of the fundamental nature of the issues (...) at stake. (shrink)

For a homicidal tyrant Domitian was disconcertingly droll. A number of examples of is ‘sardonic wit’ survive. One of them was so good that Marcus Aurelius supposedly repeated it, and attributed it to Hadrian rather than Domitian on the grounds that good sayings had no moral force if they came from tyrants.3 Domitian also possessed a talent for writing.
