Background: Informed consent is a legal as well as ethical prerequisite in clinical research. For dementia research, informed consent can be a problem if subjects with dementia, whose capacity for understanding and thus also decision making might be limited, are to be exam- ined. This might result in exclusion of dementia patients from research, as capacity for understanding and decision making are often equated with the ability for rational decision making. However, this valuation has been criticized at times for attaching (...) too much impor- tance to the cognitive aspect of decision making. -/- Methods: This qualitative study investigates the actual consent procedure of a clini- cal research study in Germany with regard to dementia patients’ subjective and objective understanding of informed consent information. Research participants were ten dementia patients, who volunteered in two clinical research studies, as well as their caregivers. Data were collected by use of semi-structured interviews. -/- Results: It was determined that the patients’ comprehension of informed consent infor- mation was rather limited. However, a number of patients were quite aware of this. In con- trast, all caregivers claimed to have fully understood the provided information, while their objective comprehension was also incomplete. Several participants indicated that they did not attach much importance to the information given in the consent procedure and that their consent did not primarily depend on this information. Rather, participation in the research study for them seemed to be more of a problem-focused coping strategy for dealing with their diagnosis of dementia. -/- Conclusion: For research ethics these results raise the question whether the currently prevailing emphasis on the cognitive aspect of autonomous decision making, i.e., compre- hension, may be too one-sided, and to what extent the “volitional” aspect in giving consent should be given greater consideration. Curry’s paradox is well known. The original version employed a conditional connective, and is not forthcoming if the conditional does not satisfy contraction. A newer version uses a validity predicate, instead of a conditional, and is not forthcoming if validity does not satisfy structural contraction. But there is a variation of the paradox which uses “external validity”. And since external validity contracts, one might expect the appropriate version of the Curry paradox to be inescapable. In this paper we show that (...) this is not the case. We consider two ways of formalising the notion of external validity, and show that in both of these the paradox is not forthcoming without the appropriate forms of contraction. (shrink)

English summary: 'Metamorphoses of the holy' denotes the changing aesthetic presentation of the sacred which is connected to the creation and institutionalization of religion as art. The focus of the essays in this volume is the structure and dynamic of this process, something which affects all art genres. The sacralization of aesthetic subjectivity is prerequisite to religion as art as evidenced by the belief in the partaking of the sacred through aesthetic experience. On the basis of aesthetic subjectivity's sacralization, it (...) becomes possible to interpret its various forms of expression as witnesses to the holy and award them a fitting reception. These contributions dedicate themselves to an interdisciplinary scrutiny of religion as art and its cross-points of reference with political and civil religion. German description: 'Metamorphosen des Heiligen' bezeichnen hier den Gestaltwandel asthetischer Vergegenwartigung des Heiligen, der sich mit der Entstehung und Institutionalisierung von Kunstreligion verbindet. The dispositional properties encountered in everyday experience seem to have causal bases in other properties, e.g., the microstructure of a vase is the causal basis of its fragility. In contrast, the Pure Dispositions Thesis maintains that some dispositions require no causal basis. This thesis faces the Problem of Being: without a causal basis, there appears to be no grounds for the existence of pure dispositions. This paper establishes criteria for evaluating the problem, critically examines four theories of the being of (...) pure dispositions, and develops an explanation of how a pure disposition grounds itself via its own power. BackgroundWith the emergence of Brain Computer Interfaces, clinicians have been facing a new group of patients with severe acquired brain injury who are unable to show any behavioral sign of consciousness but respond to active neuroimaging or electrophysiological paradigms. However, even though well documented, there is still no consensus regarding the nomenclature for this clinical entity.ObjectivesThis systematic review aims to 1) identify the terms used to indicate the presence of this entity through the years, and 2) promote an informed discussion (...) regarding the rationale for these names and the best candidates to name this fascinating disorder.MethodsThe Disorders of Consciousness Special Interest Group of the International Brain Injury Association launched a search on Pubmed and Google scholar following PRISMA guidelines to collect peer-reviewed articles and reviews on human adults published in English between 2006 and 2021.ResultsThe search launched in January 2021 identified 4,089 potentially relevant titles. After screening, 1,126 abstracts were found relevant. Finally, 161 manuscripts were included in our analyses. Only 58% of the manuscripts used a specific name to discuss this clinical entity, among which 32% used several names interchangeably throughout the text. We found 25 different names given to this entity. The five following names were the ones the most frequently used: covert awareness, cognitive motor dissociation, functional locked-in, non-behavioral MCS and higher-order cortex motor dissociation.ConclusionSince 2006, there has been no agreement regarding the taxonomy to use for unresponsive patients who are able to respond to active neuroimaging or electrophysiological paradigms. Developing a standard taxonomy is an important goal for future research studies and clinical translation. We recommend a Delphi study in order to build such a consensus. (shrink)
