At least since the seminal work of the (US) National Commission for the Protection of Human Subjects of Biomedical and Behavioral Research in the 1970s, a fundamental distinction between research and practice has underwritten both conceptual work in research ethics and regulations governing research involving human subjects. Notwithstanding its undoubted historical importance, I believe the distinction is problematic because it misrepresents clinical inquiry. In this essay, I aim to clarify the character of clinical inquiry by identifying crucial contextual constraints on (...) justification constitutive of clinical science. This analysis shows that, from an epistemological point of view, clinical research and clinical practice are not sharply distinct but intimately intertwined. This result is important in its own right—an enriched understanding of the epistemology of clinical research is valuable in and of itself. But this result is also important because it has profound implications for the ethics of human subjects research. (shrink)

A diagnosis of dementia often comes with difficulties in understanding a conversational context and expressing how one feels. So far, research on how to facilitate advance care planning for people with dementia focused on defining relevant themes and topics for conversations, or on how to formalize decisions made by surrogate decision makers, e.g., family members. The aim of this review is to provide a better scope of the existing research on practical communication aspects related to dementia in ACP conversations. In (...) November 2020, seven databases were searched to select papers for inclusion. This search was updated in December 2021. The search strategy consisted of three tiers, intersected by using the Boolean term “AND,” and resulted in 787 studies. Two researchers followed explicit criteria for two sequential levels of screening, based on titles and abstracts and full papers. A total of 22 studies were included for data analysis. Seven topics emerged clustered around two themes. This scoping review provides practical suggestions for healthcare professionals to improve ACP communication and uncovered gaps in research on communication aspects related to dementia in ACP conversations, such as non-verbal behavior, timing and implementation, and personal preferences. Any theory of information needs to comply with what we call the implementation, formalization, and representation constraints. These constraints are justified by basic considerations concerning scientific modelling and methodology. In the first part of this paper, we argue that the implementation and formalization constraints cannot be satisfied because the relation between Shannon information and IIT must be clarified. In the second part of the paper, we focus on the representation constraint. We argue that IIT cannot succeed in satisfying this constraint (...) for semantic contents without offering models for concepts, conceptual roles, and context sensitivity. In the final part of the paper, further complications are raised with respect to the distinction between consciousness and attention, which apply more specifically to the representation constraint. We conclude with some recommendations as to how IIT may succeed in solving these problems, highlighting the advantages and enormous potential of IIT as a scientific theory. (shrink)

In schema therapy, the identification of schema modes is central to case conceptualization and the planning of interventions. Differences in the naming and description of specific modes in the literature suggest the need for systematic phenomenological investigation. This paper presents the second part of an interpretative phenomenological analysis of schema modes within the single case of Linda, a young woman with anorexia nervosa. In this paper, the focus is on Linda’s Coping modes and on several important superordinate themes: mode dyads, (...) mode conflicts and balance of power, mode differentiation, and mode sequences. The findings support the value of the mode framework that is standard in schema therapy, based on Child modes, Parent modes, Coping modes, and the Healthy Adult. They furthermore highlight the idiosyncratic nature of schema modes within an individual case. Research and clinical implications of the findings are discussed, and links are made to the phenomenological perspective of Merleau-Ponty. (shrink)

Dr Gray leaves us with a question at the conclusion of his article--how should we choose priorities? He says that the debate so far has been mainly on what we should choose, but perhaps we should consider how to choose even more. Under the various subheadings of Criteria, Principles and Persons Dr Gray sets out the pros and cons of the arguments in the priority debates and tries to offer some more specific guidelines to offset the criticism that the government's (...) priority discussions have been too generalised. An alternative approach to analyze the nonrelativistic quantum dynamics of a rigid and extended charged particle taking into account the radiation reaction is discussed with detail. Interpretation of the field operators as annihilation and creation ones, theory of perturbations and renormalization are not used. The analysis is carried out in the Heisenberg picture with the electromagnetic field expanded in a complete orthogonal basis set of functions which allows the electromagnetic field to satisfy arbitrary boundary conditions. The corresponding coefficients are the (...) field operators which satisfy the usual commutation relations. A nonlinear equation of motion for the charged particle is obtained. A careful consideration of the quantum effects allows the derivation of a linear equation of motion which is free of both runaway solutions and preacceleration, even for a point charge. Also, the electromagnetic mass, which is defined as the coefficient of the acceleration operator, vanishes for a point particle. However, this does not mean that the results are free of ambiguities which are exhibited and discussed. (shrink)
