In the Netherlands, in 2002, euthanasia became a legitimate medical act, only allowed when the due care criteria and procedural requirements are met. Legally, an Advanced Euthanasia Directive can replace direct communication if a patient can no longer express his own wishes. In the past decade, an exponential number of persons with dementia share a euthanasia request with their physician. The impact this on physicians, and the consequent support needs, remained unknown. Our objective was to gain more insight into the (...) experiences and needs of Dutch general practitioners and elderly care physicians when handling a euthanasia request from a person with dementia. We performed a qualitative interview study. Participants were recruited via purposive sampling. The interviews were transcribed verbatim, and analyzed using the conventional thematic content analysis. Eleven general practitioners and elderly care physicians with a variety of experience and different attitudes towards euthanasia for PWD were included. Euthanasia requests appeared to have a major impact on physicians. Difficulties they experienced were related to timing, workload, pressure from and expectations of relatives, society’s negative view of dementia in combination with the ‘right to die’ view, the interpretation of the law and AEDs, ethical considerations, and communication with PWD and relatives. To deal with these difficulties, participants need support from colleagues and other professionals. Although elderly care physicians appreciated moral deliberation and support by chaplains, this was hardly mentioned by GPs. Euthanasia requests in dementia seem to place an ethically and emotionally heavy burden on Dutch GPs and elderly care physicians. The awareness of, and access to, existing and new support mechanisms needs further exploration. (shrink)

BackgroundGeneral practitioners often act as gatekeeper, authorizing patients’ access to hospital care. This gatekeeping role became even more important during the current COVID-19 crisis as uncertainties regarding COVID-19 made estimating the desirability of hospital referrals (for outpatient or inpatient hospitalization) complex, both for COVID and non-COVID suspected patients. This study explored Dutch general practitioners’ experiences and ethical dilemmas faced in decision making about hospital referrals in times of the COVID-19 pandemic.MethodsSemi-structured interviews with Dutch general practitioners working in the Netherlands were (...) conducted. Thematic analysis was conducted using content coding.ResultsFifteen interviews were conducted, identifying four themes: one overarching regarding (1) COVID-19 uncertainties, and three themes about experienced ethical dilemmas: (2) the patients’ self-determination vs. the general practitioners’ paternalism, (3) the general practitioners’ duty of care vs. the general practitioners’ autonomy rights, (4) the general practitioners’ duty of care vs. adequate care provision.ConclusionsLack of knowledge about COVID-19, risks to infect loved ones, scarcity of hospital beds and loneliness of patients during hospital admission were central in dilemmas experienced. John Cottingham In the anglophone philosophical world, there has, for some time, been a curious relationship between the history of philosophy and contemporary philosophical - quiry. Many philosophers working today virtually ignore the history of their s- ject, apparently regarding it as an antiquarian pursuit with little relevance to their “cutting-edge” research. Conversely, there are historians of philosophy who seldom if ever concern themselves with the intricate technical debates that ll the journals devoted to modern analytic philosophy. Both sides are (...) surely the poorer for this strange bifurcation. For philosophy, like all parts of our intellectual culture, did not come into existence out of nowhere, but was shaped and nurtured by a long tradition; in uncovering the roots of that tradition we begin see current philoso- ical problems in a broader context and thereby enrich our understanding of their signi cance. This is surely part of the justi cation for the practice, in almost every university, of including elements from the history of philosophy as a basic part of the undergraduate curriculum. But understanding is enriched by looking forwards as well as backwards, which is why a good historian of philosophy will not just be c- cerned with uncovering ancient ideas, but will be constantly alert to how those ideas pre gure and anticipate later developments. Technical thinking predominates in industrial society. It also predominates ethics. Virtually everything is viewed in terms of the technical model or—more broadly—the reductionistic machine model. Neither of these models has any room for life as a fundamental and decisive factor. Huge problems have been the result. Our appreciation of technology will change completely if the will to power and mastery will be exchanged for respect for all that lives, in all its multi-coloured variety and multiplicity. The aim of technology should (...) become, not to break down and to reduce reality in order to master and control, but to unfold and cause to flourish. We should nurture the perspective of the living and vibrant garden-city, of a culture that takes care of nature and the environment. An ethics of responsibility involves a large agenda. It calls for its own, distinct development, and a spiritual and philosophical struggle with the dominant traditions of ethics in our culture. (shrink)

This is the first comprehensive study of the early modern logic of ideas. It is also a profound contribution to our understanding of the interaction between Aristotelianism and new philosophy and between rationalism and empiricism.

Measurement error is omnipresent in psychological data. However, the vast majority of applications of autoregressive time series analyses in psychology do not take measurement error into account. Disregarding measurement error when it is present in the data results in a bias of the autoregressive parameters. We discuss two models that take measurement error into account: An autoregressive model with a white noise term (AR+WN), and an autoregressive moving average (ARMA) model. In a simulation study we compare the parameter recovery performance (...) of these models, and compare this performance for both a Bayesian and frequentist approach. We find that overall, the AR+WN model performs better. Furthermore, we find that for realistic (i.e., small) sample sizes, psychological research would benefit from a Bayesian approach in fitting these models. Finally, we illustrate the effect of disregarding measurement error in an AR(1) model by means of an empirical application on mood data in women. We find that, depending on the person, approximately 30–50% of the total variance was due to measurement error, and that disregarding this measurement error results in a substantial underestimation of the autoregressive parameters. This book discuss the key concepts that philosophers and generals of this era developed to grasp and influence the dramatic and horrific phenomenon of war.

The first sophisticated wargames were developed between 1770 and 1830 and are models of military conflict. Designers of these early games experimented fruitfully with different concepts that were formulated in interaction with the external dynamics of the military systems that they tried to represent and the internal dynamics of the design process itself. The designers of early wargames were confronted with a problem that affects all models: the trade-off between realism and simplicity, which in the case of wargames amounts to (...) the trade-off between realism and playability. I try to show how different game concepts were developed as an answer to this problem, and how these seemingly arcane concepts form a relevant topic of investigation in the history of ideas. Moreover, a direct offshoot of this conceptual experimentation between 1770 and 1830 was the ‘free’ German wargame, which became an integral part of German operational planning in the nineteenth and twentieth centuries, thus adding another chapter to the story of the influence of ideas on human history. Locke has often been hailed as the father of an empiricism that provided a philosophical basis to natural science in the Age of Enlightenment. In this article his empiricism is compared with that of the little known Dutch Aristotelian professor Gerardus de Vries. There are striking parallels between Locke's brand of mechanist empiricism and the pragmatic and flexible Aristotelianism of De Vries. These parallels put strictures on both the archaic character of the Aristotelianism embraced by De Vries and on the (...) modern and forward-looking character of Locke's philosophy of science. Montesquieu's Considérations sur les causes de la grandeur des Romains et de leur décadence (1733/1734) is a methodological exercise in causal explanation on the meso-level applied to the subject of the military rise and fall of Rome. Rome is described as a system with contingent initial conditions that have a strong path-determining effect. Contingent and plastic initial configurations become highly determining in their subsequent operation, thanks to self-reinforcing feedback loops. Montesquieu's method seems influenced by the ruthless commitment to efficient causality (...) and the reductionism of seventeenth-century mechanicist philosophy; but in contrast to these predecessors, he is more interested in dynamic processes than in unchangeable substances, and his use of efficient causality in the context of a system approach implies a form of holism that is lacking in his predecessors. The formal and conceptual analysis in this article is in many ways complementary with Paul Rahe's recent predominantly political analysis of the Considérations. At the same time, this article points to a problem in the works on the Enlightenment by Jonathan Israel: his account stresses a one-dimensional continuum consisting of Radical, Moderate and Counter-Enlightenment. This invites Israel to place the combined religious, political and philosophical views of each thinker on one of these three points. His scheme runs into trouble when a thinker with moderate religious and political views produces radical philosophical concepts. Montesquieu's Considérations is a case in point. (shrink)

Summary In his novel The Adventures of Telemachus, François de la Mothe-Fénelon (1651–1715) presents a utopian society, Boetica, in which the role of luxury, war and trade is extremely limited. In unreformed Salentum, on the other hand, Fénelon shows the opposite image, one in which the three elements reinforce each other in a fatal feedback-loop. I analyse the relationship between luxury, war and trade in the Telemachus and I sketch the background to Fénelon's views, with special attention to the military (...) expansion and the mercantilism of Louis XIV, Fénelon's quietist spirituality, and the development of the concept of self-interest in seventeenth-century philosophy by mechanicist philosophers and economic thinkers. The central theme of John Locke's Of the Conduct of the Understanding is human error. The Conduct was conceived as an additional chapter to An Essay concerning Understanding, but it was never finished and published posthumously in 1706 as a separate work. Modern authors have regarded the Conduct as an educational treatise. Indeed, the analysis in this work of the nature and causes of error and the ways to prevent and remedy error gives rise to numerous educational reflections. However, the (...) aim of the present article is to show that these views should be understood within the specific epistemological context of a two-stage analysis of ideas, the first stage consisting of individual ideas that should all be clear and distinct, and the second stage consisting of reasonings based on combinations of these ideas. © 2001 Elsevier Science Ltd. The editor’s General Introduction is divided into two parts. The first part, ‘Context’, discusses Locke’s analysis of the nature of error, the causes of error and the prevention and cure of error in the Conduct. His enquiry is placed in the context of his way of ideas as given in his Essay concerning Human Understanding. Locke’s two-stage way of ideas, his occupation with our mental faculties and with method form the interrelated main ingredients of his logic of ideas. There is (...) a complicated relation of continuity and change between the content and the structure of this new logic on the one hand and the content and structure of works by both scholastic predecessors and novel philosophers on the other hand. Once this context is taken into account, the Conduct can be understood as work that has a function within the structure of Locke’s logic of ideas that runs parallel to the function of the De sophisticis elenchis in the Aristotelian Organon. The second part of the General Introduction, ‘Text’, gives a description of the relevant MSS, an overview of references to the Conduct in Locke’s correspondence, a history of the genesis of the Conduct until its first publication in 1706 in the Posthumous Works, an analysis of the evidence provided by the MSS on how the Conduct grew out of the Essay, and a statement of the principles that underlie the present editon. The interpretation of Descartes’ physics as given in the Principia Philosophiae should take into account the fundamental differences between its two levels. First, in Principia Part II there is the level formed by his general physical principles, i.e. his three Laws of Nature. Second, in Principia Part III and Part IV there is the level of Descartes’ explanations for the different phenomena. While pretending to give physical principles that are certain and not based on empirical evidence, he admits that his (...) explanatory models for the physical phenomena have a hypothetical character. In these models empirical evidence plays a vital role — if not in practice than at least in theory. The article ends with a discussion of the problems, posed by these differences between two levels, for Descartes' view of one unified science. (shrink)

SummaryIn the present article it is argued that Pieter de la Court's Political Maxims of the State of Holland presented a remarkably consistent grand strategy for Holland in relation to its Dutch allies and the European powers. I present an outline of this strategy, which was built around the accomplishment and defence of commercial goals; I sketch a historical context that takes into account the general historical shift from tribute-taking agrarian societies towards commercial wealth-generating polities, and also the violent contemporary (...) military and ideological background against which De la Court's strategy stands out; I argue that his strategy can be understood by his use of three basic game theoretic concepts ; and I stress the distinctive character of De la Court's work, by comparing the practical and strategic use of these concepts in the Maxims with the function of the same concepts in the philosophical contract theories of Thomas Hobbes and Benedict de Spinoza.
