Multidrug resistant organisms are a public health threat that have reduced the effectiveness of many available antibiotics. Antimicrobial stewardship programs have been tasked with reducing antibiotic use and therefore the emergence of MDROs. While fecal microbiota transplant has been proposed as therapy to reduce patient colonization of MDROs, this will require additional evidence to support an expansion of the current clinical indication for FMT. This article discusses the evidence and ethics of the expanded utilization of FMT by ASPs for reasons (...) other than severe recurrent or refractory Clostridioides difficile infection. (shrink)

The similarities between Vico and Heidegger have been explored throughout the critical literature and in most detail by Ernesto Grassi. Both Vico and Heidegger were interested in elevating poetic thought, in emphasizing the ontological difference, and in criticizing the modern, technological age. Both thinkers began with the question of Being and were interested in reopening the thought that lies at philosophy's beginning in order to better understand modernity's present condition which they felt lies at philosophy's end. ;While these general themes (...) are indeed present in both Vico and Heidegger, thinkers like Grassi have failed to explore the vital differences between the two thinkers--particularly in the area of practical philosophy. For Vico, questions concerning Being always give us insight into questions concerning human action. Wisdom is always connected to prudence. Vico's work thus is concerned not simply with metaphysics and ontology, but also with ethics and morals. Heidegger, on the other hand, never provides his readers with an ethics. ;The differences between Vico and Heidegger on the question of practical philosophy can be traced directly to their respective ontologies. Both Vico and Heidegger are interested in the disclosure of Being instead of beings and thus are critical of purely logical attempts to categorize the attributes of individual objects. This interest prompts both to turn to poetry and poetic thought. For Heidegger, however, poetry is used to think Being as such. It allows us to preserve our essence--to let it be. Vico, on the other hand, turns to Being and poetic thought for the sake of the proper disclosure of beings. For Vico, we went to know Being as a whole so that we can understand how individual beings might fit in the context of that whole and so that we can act accordingly. ;Thinking Being, for Vico, thus always is for the sake of truth and proper action. Philosophical thought, for Heidegger, always will be concerned with that which is prior to such concerns--the poetic disclosure of Being itself. (shrink)

This book offers insights relevant to modern history and epistemology of physics, mathematics and, indeed, to all the sciences and engineering disciplines emerging of 19th century. This research volume is the first of a set of three Springer books on Lazare Nicolas Marguérite Carnot’s (1753–1823) remarkable work: Essay on Machines in General (Essai sur les machines en général [1783] 1786). The other two forthcoming volumes are: Principes fondamentaux de l’équilibre et du mouvement (1803) and Géométrie de position (1803). Lazare Carnot (...) – l'organisateur de la victoire – in Essai sur le machine en général (1786) assumed that the generalization of machines was a necessity for society and its economic development. Subsequently, his new coming science applied to machines attracted considerable interest for technician, as well, already in the 1780’s. With no lack in rigour, Carnot used geometric and trigonometric rather than algebraic arguments, and usually went on to explain in words what the formulae contained. His main physical– mathematical concepts were the Geometric motion and Moment of activity–concept of Work . In particular, he found the invariants of the transmission of motion (by stating the principle of the moment of the quantity of motion) and theorized the condition of the maximum efficiency of mechanical machines (i.e., principle of continuity in the transmission of power). While the core theme remains the theories and historical studies of the text, the book contains an extensive Introduction and an accurate critical English Translation – including the parallel text edition and substantive critical/explicative notes – of Essai sur les machines en général (1786). The authors offer much-needed insight into the relation between mechanics, mathematics and engineering from a conceptual, empirical and methodological, and universalis point of view. As a cutting–edge writing by leading authorities on the history of physics and mathematics, and epistemological aspects, it appeals to historians, epistemologist–philosophers and scientists (physicists, mathematicians and applied sciences and technology). Background: The factors influencing parents’ willingness to enroll their children in biobanks are poorly understood. This study sought to assess parents’ willingness to enroll their children, and their perceived benefits, concerns, and information needs under different consent and data-sharing scenarios, and to identify factors associated with willingness. Methods: This large, experimental survey of patients at the 11 eMERGE Network sites used a disproportionate stratified sampling scheme to enrich the sample with historically underrepresented groups. Participants were randomized to receive one of (...) three consent and data-sharing scenarios. Results: In total, 90,000 surveys were mailed and 13,000 individuals responded (15.8% response rate). 5737 respondents were parents of minor children. Overall, 55% (95% confidence interval 50–59%) of parents were willing to enroll their youngest minor child in a hypothetical biobank; willingness did not differ between consent and data-sharing scenarios. Lower educational attainment, higher religiosity, lower trust, worries about privacy, and attitudes about benefits, concerns, and information needs were independently associated with less willingness to allow their child to participate. Of parents who were willing to participate themselves, 25% were not willing to allow their child to participate. Being willing to participate but not willing to allow one’s child to participate was independently associated with multiple factors, including race, lower educational attainment, lower annual household income, public health care insurance, and higher religiosity. Conclusions: Fifty-five percent of parents were willing to allow their youngest minor child to participate in a hypothetical biobank. Building trust, protecting privacy, and addressing attitudes may increase enrollment and diversity in pediatric biobanks. (shrink)

McCullough et al. propose an evolved cognitive revenge system which imposes retaliatory costs on aggressors. They distinguish between this and other forms of punishment (e.g., those administered by judges) which are not underpinned by a specifically designed evolutionary mechanism. Here we outline mechanisms and circumstances through which the revenge system might nonetheless infiltrate decision-making within the criminal justice system.

Although theories that examine direct links between behavior and brain remain incomplete, it is known that brain expansion significantly correlates with caloric and oxygen demands. Therefore, one of the principles governing evolutionary cognitive neuroscience is that cognitive abilities that require significant brain function (and/or structural support) must be accompanied by significant fitness benefit to offset the increased metabolic demands. One such capacity is self-awareness (SA), which (1) is found only in the greater apes and (2) remains unclear in terms of (...) both cortical underpinning and possible fitness benefit. In the current experiment, transcranial magnetic stimulation (TMS) was applied to the prefrontal cortex during a spatial perspective-taking task involving self and other viewpoints. It was found that delivery of TMS to the right prefrontal region disrupted self-, but not other-, perspective. These data suggest that self-awareness may have evolved in concert with other right hemisphere cognitive abilities. (shrink)

BackgroundThe COVID-19 pandemic has had a significant impact on the health system worldwide. The organ and tissue donation and transplantation (OTDT) system is no exception and has had to face ethical challenges related to the pandemic, such as risks of infection and resource allocation. In this setting, many Canadian transplant programs halted their activities during the first wave of the pandemic.MethodTo inform future ethical guidelines related to the COVID-19 pandemic or other public health emergencies of international concern, we conducted a (...) literature review to summarize the ethical issues.ResultsThis literature review identified three categories of ethical challenges. The first one describes the general ethical issues and challenges reported by OTDT organizations and transplantation programs, such as risks of COVID-19 transmission and infection to transplant recipients and healthcare professionals during the transplant process, risk of patient waitlist mortality or further resource strain where transplant procedures have been delayed or halted, and resource allocation. The second category describes ethical challenges related to informed consent in the context of uncertainty and virtual consent. Finally, the third category describes ethical issues related to organ allocation, such as social considerations in selecting transplant candidates.ConclusionThis literature review highlights the salient ethical issues related to OTDT during the current COVID-19 pandemic. As medical and scientific knowledge about COVID-19 increases, the uncertainties related to this disease will decrease and the associated ethical issues will continue to evolve. (shrink)

The global dispersal of prehistoric ancient humans from Africa to North America, and the existence of artistic innovation evidenced in the Late Pleistocene are, by now, parts of a familiar and fascinating story. But the explanation of how our human career was possible cries out for clarification. In this chapter, I argue that dual process theory can provide the needed explanation. My claim will be that the advent of System-2 reasoning running offline, aided by executive cognitive control and language, and (...) facilitated by neural plasticity, made possible the remarkable human dispersal from Africa to North America by way of the Middle East and Asia. System-1 modular adaptations, together with System-2 reasoning, gave rise to the flexible, culturally-informed, mental operations that were essential to shape the psychologically modern mind. The result was, inter alia, the surprising Late Pleistocene dispersal of ancient humans to North America. (shrink)

Modern, science-centered naturalisms can be charged with a certain moral laxity, according to S. T. Coleridge. This fault reflectsnbsp; a devitalizing, materialistic metaphysics informed by a narrow and self-serving conception of reason. Thus seeking a remedy that can bring justice to the spiritual as well as the physical aspects of experience, Coleridge envisages a lsquo;true naturalismrsquo; that will not only address the question lsquo;What is Life?rsquo; but also frame a lsquo;true realismrsquo; that includes what might be called a lsquo;true moralismrsquo;. This calls, however, for a Heraclitean metaphysics capable of linking lsquo;goodnessrsquo; in both thinking and acting to a *Logos*mdash;that is, an essentially nonmodern theory of actuality that can do justice at once to the quicknesses and the uniformities of both Life and Thought. Coleridgersquo;s thus presents an outline of how one might respond to a challenge that can be best met, I argue, with the help of certain insights of Maurice Merleau-Ponty, A. N. Whitehead, Gilles Deleuze, and C. S. Peirce. By enlisting Hannah Arendtrsquo;s individual-centered conception of morality, which ties ethics to public concerns, it is also possible to sketch a metaphysically grounded response to Friedrich Nietzschersquo;s call for a lsquo;healthy moralityrsquo; capable of overturning the nihilistic values entrenched in modern thought. br /. This paper treats the political and ethical issues associated with the new caretaking technologies. Given the number of feminists who have raised serious concerns about the future of care work in the United States, and who have been critical of the degree to which society “free rides” on women's caretaking labor, I consider whether technology may provide a solution to this problem. Certainly, if we can create machines and robots to take on particular tasks, we may lighten the care burden (...) that women currently face, much of which is heavy and repetitious, and which results in injury and care “burnout” for many female caretakers. Yet, in some contexts, I argue that high-tech robotic care may undermine social relationships, cutting individuals off from the possibility of social connectedness with others. (shrink)

The idea of integrating evolutionary biology and psychology has great promise, but one that will be compromised if psychological functions are conceived too abstractly and neuroscience is not allowed to play a contructive role. We argue that the proper integration of neuroscience, psychology, and evolutionary biology requires a telelogical as opposed to a merely componential analysis of function. A teleological analysis is required in neuroscience itself; we point to traditional and curent research methods in neuroscience, which make critical use of (...) distinctly teleological functional considerations in brain cartography. Only by invoking teleological criteria can researchers distinguish the fruitful ways of identifying brain components from the myriad of possible ways. One likely reason for reluctance to turn to neuroscience is fear of reduction, but we argue that, in the context of a teleological perspective on function, this concern is misplaced. Adducing such theoretical considerations as top-down and bottom-up constraints on neuroscientific and psychological models, as well as existing cases of productive, multidisciplinary cooperation, we argue that integration of neuroscience into psychology and evolutionary biology is likely to be mutually beneficial. We also show how it can be accommodated methodologically within the framework of an interfield theory. (shrink)

One of the ethical issues that has been raised recently regarding emerging neurotherapies is that people will be coerced explicitly or implicitly in the workplace or in schools to take cognitive enhancing drugs. This article builds on this discussion by showing how the law may pressure people to adopt emerging neurotherapies. It focuses on a range of private law doctrines that, unlike the criminal law, do not come up very often in neuroethical discussions. Three doctrines—the doctrine of mitigation, the standard (...) of care in negligence, and child custody determinations in family law—are addressed to show how the law may pressure people to consent to treatment by offering a choice between accepting medical treatment and suffering a legal disadvantage. The doctrines considered in this article apply indirect pressure to submit to treatment, unlike court-ordered medical treatment, which applies direct pressure and is not addressed here. The outcome of this discussion is to show that there is a greater range of social pressures that may encourage the uptake of novel neurotherapies than one might initially think. Once treatments that were developed and offered with therapeutic benefits in mind become available, their existence gives rise to unintended legal consequences. This certainly does not mean we should cease developing new therapies that may be of tremendous benefit to patients, but it does raise some questions for physicians and for legal policy-makers. How should physicians, who are required by medical ethical principles to obtain valid consent to treatment, react to a patient’s reluctant consent that is driven by legal pressure? From the legal policy perspective, are our legal doctrines satisfactory or should they be changed because, for example, they unduly promote the collective interest over individual freedom to reject medical treatment or because they channel us toward economically efficient treatments to the detriment of more costly but potentially superior approaches of dealing with behavioural problems? The author argues that Michael Polanyi’s philosophy of science can be understood as an (unconscious) attempt to recapture elements of experience largely forgotten or repressed in modernity. Specifically, the author argues that Polanyi’s epistemology appears to draw on elements of oral—aural experience that have been relatively ignored by the heavily visual sensory orientation typical of modern Western societies. The author does this by first deriving the primary features of the modern objectivist conception of knowledge from Polanyi’s critique of objectivism and (...) then using the anthropological literature on the differences between oral and literate cultures to demonstrate that these same aspects of the objectivist paradigm correspond closely to typical features of literate, visual consciousness while Polanyi’s alternative formulations correspond to a more oral—aural orientation. In this paper, I will argue that there is a deep connection between home-based care, technology, and the self. Providing the means for persons to receive care at home is not merely a kindness that respects their preference to be at home: it is an important means of extending their selfhood and respecting the unique selves that they are. Home-based technologies like telemedicine and robotic care may certainly be useful tools in providing care for persons at home, but they also (...) have important implications for sustaining selfhood in ways that are of value to individuals and those who care for them. I will argue, by appealing to Hilde Lindemann’s notion of “holding” persons’ identities in place, that technological interventions are not only useful tools for improving and sustaining health and good care at home, but that they may also help to extend our personal identities and relational capacities in ways that are practically and ethically good. Because of these important goods, I will claim that there is a prima facie moral duty to do this “holding” work and that it is best done by family members and loved ones who are well suited to the job because of their history and relationship with the individual that needs to be “held” in place. This article suggests that looking at the ways in which subjects relate to and internalise gender norms is a fruitful way to explore socially constructed differences between masculinity and femininity in the U.S. Throughout this article, I am in dialogue with Judith Butler’s theory of gender performativity as I focus on practices of subject formation that I denote as ‘logics’ of subject formation. I propose several key ways to distinguish a feminine logic of subject formation from a masculine logic of (...) subjectivity. I suggest that femininity is a multiplicitous, self-proliferating strategy that forecloses truth as a means for achieving selfhood. Alternatively, I postulate that masculinity tends to masquerade as an (oftentimes failed) achievement that is singular and self-consolidating and that generates truth as a means for realising selfhood. In order to contextualise these distinctions between masculinity and femininity, I look to the recent influx of American women into masculine-marked domains such as the workforce, politics and sports without a corresponding entrance of men into feminine-marked domains. Known as the stalled gender revolution, these shifts, I argue, have created the context for the gendered divergences in subjectivity I suggest here. Focusing on examples of women in sports to illustrate these distinctions, I provide a detailed characterisation of femininity as a logic of subject formation. As such, I argue that this approach to gender may provide relevant insights to understanding gender practice, agency and resistance, particularly with respect to Butler’s formulation of gender performativity and subversion. I conclude by providing some preliminary thoughts on how to re-envision agency and subversion in light of the proposed framework. (shrink)

Drawing on fieldwork and in-depth interviews with homeschooling mothers in the Pacific Northwest, the author reveals several ways the temporal experience of motherhood was emotionally problematic. The intensive demands of homeschooling left them stressed and dissatisfied with the amount of time they had to pursue their own interests. Mothers tried to allocate their time differently to manage these feelings, yet their efforts were unsuccessful, which led them to become frustrated and resentful. To resolve these troublesome feelings, mothers resorted to manipulating (...) their subjective experiences of time through a process the author calls “temporal emotion work.” In the conclusion, the author examines the theoretical confluence of emotions and temporality, suggesting that the dominant form of motherhood is culturally defined as a “time-sensitive identity” and that “temporal emotions” are unique tools in managing the emotional difficulties inherent in the trajectories of some identities. The passing of the Patient Protection and Affordable Care Act is a triumph for the field of public health. Its inclusion of many provisions intended to prevent illness and promote health endorses the core belief of public health as expressed by Dr. Georges Benjamin, the long-time executive director of the American Public Health Association, in a Washington Post opinion piece praising ACA for “provid[ing] care as far upstream as possible… [in order to] reduce costs by identifying problems early and then (...) managing them to reduce or eliminate the need for more costly care in the future.” In this article, I consider the conflict between ACA’s adoption of public health goals seeing population health and societal interests in protecting individuals from discrimination based on their health. The article focuses on one aspect of ACA which seeks to lower the costs to employers who provide health insurance for their employees by making it easier for them to offer their employees substantial incentives for participating in and meeting the goals of employer-sponsored Wellness Programs. The analytic method by which Descartes discovered the first principle of his philosophy—cogito, ergo sum—is a unique cognitive process of direct insight and nonlogical inference. It differs markedly from inductive as well as deductive procedures, but also from older models of the direct noetic apprehension of first principles, notably those of Plato and Aristotle. However, a critical examination of Descartes’s argument for the innateness of the idea of God shows that there are serious obstacles in the way of his employment (...) of the analytic method of discovery to reach this or any other conclusion about ideas that do not fall within the scope of ordinary human experience. Children and pregnant women are often excluded from clinical research. This has resulted in a paucity of evidence on how medicines work for fetuses, neonates, infants, and adolescents. It also raises bioethics, scientific, and public health concerns. For over half a century, doctors have prescribed medicines to children largely on the basis of how they work in adults, despite children’s varied physiologies and differences in how their bodies absorb and metabolize drugs. Regulations and legislation have led to an increase in (...) the number of pediatric studies and to better drug labeling. However, children at all stages of their lives often remain “therapeutic orphans” owing to insufficient evidence about how medicines work for them. (shrink)

With the systematic aim of clarifying the phenomenon sometimes described as “the intellectual apprehension of first principles,” Descartes’ first principle par excellence is interpreted before the historical backcloth of Aristotle’s Posterior Analytics. To begin with, three “faces” of the cogito are distinguished: (1) the proto-cogito (“I think”), (2) the cogito proper (“I think, therefore I am”), and (3) the cogito principle (“Whatever thinks, is”). There follows a detailed (though inevitably somewhat conjectural) reconstruction of the transition of the mind from (1) (...) via (3) to (2) and back again to (3). What emerges is, surprisingly, a non-circular, non-logical, and ultimately non-mysterious process by which first principles implicitly contained in a complex intuition are gradually rendered explicit (and, if abstract, grasped in their abstract universality). This process bears a striking family resemblance to that intuitive induction (“grasping the universal in the particular”) which Aristotle scholars have distinguished from empirical forms of induction.
