In recent years, hospitals, clinics, and professional organizations have with increasing frequency pledged their commitment to “patient-and family-centered care”. The movement toward PFCC is especially pronounced in pediatrics, where the American Academy of Pediatrics has a long-held, explicit commitment to PFCC. However, the unified movement toward PFCC obscures differing conceptions of its purpose. Hereditary structural completeness is established for a range of substructural logics, mainly without the weakening rule, including fragments of various relevant or many-valued logics. Also, structural completeness is disproved for a range of systems, settling some previously open questions.

Healthcare organisations are hierarchical; almost all are organised around the ranking of individuals by authority or status, whether this be based on profession, expertise, gender or ethnicity. Hierarchy is important for several reasons; it shapes the delivery of care, what is prioritised and who receives care. It also has an impact on healthcare workers and how they work and communicate together in organisations. The purpose of this scoping review is to explore the qualitative evidence related to hierarchy in healthcare organisations (...) defined broadly, to address gaps in macro‐level healthcare organisational research, specifically focusing on the (1) impact of hierarchy for healthcare workers and (2) how hierarchy is negotiated, sustained and challenged in healthcare organisations. After a search and screening, 32 papers were included in this review. The findings of this review detail the wide‐reaching impacts that hierarchy has on healthcare delivery and health workers. The majority of studies spoke to hierarchy's impact on speaking up, that is, how it shaped communication between staff with differential status: not only what was said, but how it had an impact on what was acceptable to say, by whom and at what time. Hierarchy was also noted to have substantial personal costs, impacting on the well‐being of those in less powerful positions. These findings also provide insight into the complex ways in which hierarchy was negotiated, challenged and reproduced. Studies not only detailed the way in which hierarchy was navigated day to day but also spoke to the reasons as to why hierarchy is often entrenched and difficult to shift. A number of studies spoke to the impact that hierarchy had in sustaining gender and ethnic inequalities, maintaining historically discriminatory practices. Importantly, hierarchy should not be reduced to differences between or within the professions in localised contexts but should be considered at a broad organisational level. (shrink)

In this paper, we discuss the development of robot use cases in an elderly care facility in the context of exploring the method of Integrative Social Robotics when used on top of a user-centered design approach. Integrative Social Robotics is a new proposal for how to generate responsible, i.e. culturally and ethically sustainable, social robotics applications. Starting point for the discussion are the five principles that characterize an ISR approach, which are discussed in application to the three use cases for (...) robot support in a Danish elderly care facility developed within the smooth project. The discussion by an interdisciplinary design team explores what attention to the five principles of ISR can offer for use case development. We report on the consequences of this short-time exposure to the basic ideas of ISR for use case development and discuss the value of approaching robot development from an ISR perspective. (shrink)

This paper offers a theoretical discussion on why the nursing profession has had a delayed response to the issue of climate change. We suggest this delay may have been influenced by the early days of nursing's professionalization. Specifically, we examine nursing's professional mandate, the generally accepted metaparadigm, and the grand theorists’ conceptualizations of both the environment and the nurse–environment relationship. We conclude that these works may have encouraged nurses to conceptualize the environment, as well as their relationship with it, mainly (...) in terms of the individual patient, and as such, nurses have not been encouraged to understand these concepts from a broader perspective. By not having the philosophical and theoretical foundations to understand the environment in relation to society, it is not surprising that nurses have had a delayed response to climate change and may not have viewed it as a professional concern. A planetary health perspective is suggested as a theoretical basis for nursing education, research and practice. Taking on a planetary health perspective could help nurses progress the profession and move healthcare systems towards supporting a climate‐resilient future. (shrink)

Hegel and the Spirit explores the meaning of Hegel's grand philosophical category, the category of Geist, by way of what Alan Olson terms a pneumatological thesis. Hegel's philosophy of spirit, according to Olson, is a speculative pneumatology that completes what Adolf von Harnack once called the "orphan doctrine" in Christian theology--the doctrine of the Holy Spirit. Olson argues that Hegel's development of philosophy as pneumatology originates out of a deep appreciation of Luther's dialectical understanding of Spirit and (...) that Hegel's doctrine of Spirit is thus deeply interfused with the values of Würtemberg Pietism. Olson further maintains that Hegel's Enzyklopdie is the post-Enlightenment philosophical equivalent of a Trinitätslehre and that his Rechtsphilosophie is an ecclesiology. Thus Hegel and the Spirit demonstrates the truth of Karl Barth's observation that Hegel is the potential Aquinas of Protestantism. Exploring Hegel's philosophy of spirit in historical, cultural, and personal religious context, the book identifies Hegel's relationship with Hölderlin and his response to Hölderlin's madness as key elements in the philosopher's religious and philosophical development, especially with respect to the meaning of transcendence and dialectic. Baker claims that the argument is invalid, and that both the premise and the conclusion are false. I attempt to defend argument, premise, and conclusion against her objections.

Meaning depends on context. This applies in obvious cases like deictics or sarcasm as well as more subtle situations like framing or persuasion. One key aspect of this is the identity of the participants in an interaction. Our interpretation of an utterance shifts based on a variety of factors, including personal history, background knowledge, and our relationship to the source. While obviously an incomplete model of individual differences, demographic factors provide a useful starting point and allow us to capture some (...) of this variance. However, the relevance of specific demographic factors varies between situations—where age might be the key factor in one context, ideology might dominate in another. To address this challenge, we introduce a method for combining demographics and context into situated demographic embeddings—mapping representations into a continuous geometric space appropriate for the given domain, showing the resulting representations to be functional and interpretable. We further demonstrate how to make use of related external data so as to apply this approach in low‐resource situations. Finally, we show how these representations can be incorporated into improve modeling of real‐world natural language understanding tasks, improving model performance and helping with issues of data sparsity. This article concerns the claim that it is possible to create living organisms, not merely models that represent organisms, simply by programming computers. I ask what sort of things these computer-generated organisms are supposed to be. I consider four possible answers to this question: The organisms are abstract complexes of pure information; they are material objects made of bits of computer hardware; they are physical processes going on inside the computer; and they are denizens of an entire artificial world, different (...) from our own, that the programmer creates. I argue that could not be right, that collapses into, and that would make strong alife either absurd or uninteresting. Thus, "virtual" strong alife amounts to the claim that, by programming a computer, one can literally bring bits of its hardware to life. (shrink)

Innovations in information technologies have facilitated the development of new styles of research networks and forms of governance. This is evident in genomics where increasingly, research is carried out by large, interdisciplinary consortia focussing on a specific research endeavour. The UK10K project is an example of a human genomics consortium funded to provide insights into the genomics of rare conditions, and establish a community resource from generated sequence data. To achieve its objectives according to the agreed timetable, the UK10K project (...) established an internal governance system to expedite the research and to deal with the complex issues that arose. The project’s governance structure exemplifies a new form of network governance called ‘pop-up’ governance. ‘Pop-up’ because: it was put together quickly, existed for a specific period, was designed for a specific purpose, and was dismantled easily on project completion. In this paper, we use UK10K to describe how ‘pop-up’ governance works on the ground and how relational, hierarchical and contractual governance mechanisms are used in this new form of network governance. We tend to imagine revolution as a vivid tableau of uprisings and armed conflicts. Much less notice is taken of the complex layers of meaning that shape our understanding of these events: why some are seen as just expressions of a popular will, while others are condemned as wild disruptions of political order. A critical understanding of politics must grapple with these subtle, often unnoticed normative issues. This essay tries to understand some of the processes that create political normativity, investigating (...) the way revolutionary movements use time to endow themselves with normative value. I sketch a genealogy of revolution across three centuries, noting changes in its meaning, observing the rise of discourse as an important mode of revolutionary activity, and tracing a shifting mosaic of temporalities that are politicized in important ways. Time is instrumentalized to justify political insurgency in this history. Time, in short, becomes a tool of revolution. (shrink)

This article reveals a 91-year-old cognitively intact man’s lived experiences of being cared for in a geriatric context in which the majority of the patients were cognitively impaired. A narrative patient story was analysed phenomenologically. The findings indicate that this patient’s basic needs for ethical care were not met. The staff did not see him as a unique individual with his own preferences, resources and abilities to master his life. In order to survive this lack of ethical care, he played (...) the role of an ‘old cognitively impaired man’, which provided him with at least the understanding and attention the cognitively impaired patients received from the staff. The findings also indicate that ethical care is independent of whether or not older cognitively intact and impaired patients stay or live in the same unit, but it is more dependent on a caregiver’s ability to respect and confirm each and every patient for who he or she is and would like to be. La diversificación de prácticas y estrategias laborales y organizativas suscitadas en Argentina en las últimas dos décadas ha puesto en evidencia la dificultad de asir la complejidad que las caracteriza con las categorías clásicas de observación y análisis utilizadas por la sociología del trabajo y las relaciones laborales. Esta dificultad tiene a su vez un correlato en el ámbito de las políticas públicas, lo cual conlleva diversos obstáculos al momento de regular y garantizar derechos y protecciones para los/as trabajadores/as no (...) asalariados/as, así como el marco de organización y negociación de sus condiciones laborales. Ante ello, este artículo tiene por objetivo analizar las características que adquieren las relaciones laborales en las organizaciones de trabajadores/as asociados/as y autogestionados/as (no asalariados/as) a partir de la experiencia del caso argentino en el período 2003-2018. El recorte propuesto retoma diversos aspectos que hacen al campo de las relaciones laborales, como la seguridad social y el acceso a protecciones, las formas de gestión y organización del trabajo y las modalidades de representación y negociación colectiva. Se propone que el abordaje de las relaciones laborales en este sector requiere tener en cuenta dos dimensiones imbricadas. La primera —una dimensión normativa— refiere al análisis de las normas y regulaciones formales que delimitan el accionar de estas organizaciones. La segunda —una dimensión sociocultural— se refiere a las modalidades organizativas en términos políticos, sociales y productivos adoptadas por las organizaciones. Las reflexiones aquí presentadas se basan en observaciones y análisis realizados a partir de trabajos propios de investigación, vinculación y transferencia llevados a cabo con organizaciones asociativas. As early as Bringing It All Back Home (1965), peaking with Blood on the Tracks (1975), and persisting into his most recent work, Dylan has consistently merged aspects of blues, country, and literary traditions, to invoke images of mobility that examine individual freedom in a world where technology, fragmentation, and homogeneity seem to result in a destructive dehumanizing of individuals. In so doing, Dylan uses an intensely innovative aesthetic as he ambivalently romanticizes uprooted wandering in a manner that both mirrors and expands upon what I term an ontology of the hobo-hero that permeates American letters from Walt Whitman to John Dos Passos (and beyond) as it glorifies those who remain in constant motion in order to maintain a sense of integrated self in the face of crushing modernity.Gilles Deleuze and Félix Guattari similarly identify such an ontology within American culture when they, in A Thousand Plateaus, declare America a “special case” defined by its ceaseless search for a “rhizome,” or a “space in which two things that have nothing to do with one another form a parallel constructed by the mind. It is not imitation or resemblance, but becoming” (10). For Deleuze, Guattari, and, as I argue, Bob Dylan, the rhizome occupies a singular place within American letters. In other words, Deleuze and Guattari admit that America “is not immune from [...] the search for roots” in its “quest for a national identity or even for a European ancestry or genealogy (Kerouac going off in search for his ancestors),” yet they adamantly assert,every thing important that has happened or is happening takes the route of the American rhizome [...]. American books are different from European books, even when Americans set off in search of trees. The conception of the book is different. Leaves of Grass. And directions in America are different: the search for arborescence and the return to the Old World occur in the East. In fact, the lone commonality that Americans share is descent from immigrants who came to the United States from a “home” or “homeland” where cultural identity was not far removed from national identity. It would stand to reason, therefore, that a group of uprooted individuals thrust into a self-declared melting pot would begin to define themselves by their dislocation. For instance, eighteenth-century French immigrant J. Hector St. John de Crèvecœur claimed that, within the American continent, “individuals of all nations are melted into a new race of men” (44) as America represents, for Crèvecœur, an “asylum” in which “the poor of Europe have by some means met together” (42). Crèvecœur proceeded to privilege those people, such as farmers, who planted “roots” as they helped to build and improve their society, but Crèvecœur’s assertion that we are united by our mutual uprootedness proves indicative of what critic Robert James Butler has observed as a recurrent American “quest for pure motion” (80, emphasis in original). Or, one encounters what the novelist John Steinbeck claimed he perceived in “nearly every American”: a “burning desire to go, to move, to get under way, anyplace, away from any Here” (10). A structural characterization of the subdi-rectly irreducible members of CRS(k) is proved, and is then used to provide a constructive approach to the axiomatization of varieties generated by positive universal subclasses of CRS(k).

While negatives are fundamental to the functioning of human languages and while they are acquired extremely early by children, there is some evidence that an aware-ness of the logical and representational functions of negation is late to develop and may depend in part on the invention of notational means for representing it. This hypothesis is explored by reference to the presence or absence of notations for negation in the world's writing systems, the acquisition of notational devices for representing negation by (...) young children, and the invention of numerical notation for representing nothing, namely, zero. In each case it is argued that while negation is part of oral language, conceptualizing absence may be related to the invention of notations for negation. (shrink)

The aim of this book is to understand and critically appraise science-based transgression dynamics in their whole complexity. It includes contributions from experts with different disciplinary backgrounds, such as philosophy, history and sociology. Thus, it is in itself an example of boundary transgression. Scientific disciplines and their objects have tended to be seen as permanent and distinct. However, science is better conceived as an activity that constantly surpasses, erases and rebuilds all kinds of boundaries, either disciplinary, socio-ethical or ecological. We propose that in pictures both the extended limbs of actors and the ground are involved in gestalts for movement. Limbs extend to suggest more motion literally when a dancer is in air and refer to a canonical pose when on the ground. A running pony’s curled limbs off the ground depict fast action literally. A horse’s flying-gallop off the ground suggests speedy motion metaphorically. Cast shadows indicate the actor’s location with respect to the ground. This article discusses connections between grounded action, leverage points, and paradigm transformation and transcendence. Part one provides background on Meadows' (1999) approach to leverage points as well as the concept of paradigms. Part two argues that grounded action is inherently suited for leverage points analysis and paradigm-based interventions. Part three offers an example of individual paradigm transformation in the educational context vis-à-vis Olson's (2006) grounded theory of driven succeeding. The article concludes with considerations of how the application of grounded (...) action in one classroom might leverage system-wide paradigm transformation. (shrink)

Mythos und Neue Musik' erforscht erstmalig die spannende Beziehung zwischen den antiken Mythen und der Musik. Birgit Johanna Wertenson widmet sich vor allem zeitgenössischen Kompositionen und weist über die beiden Figuren Orpheus und Kassandra anschaulich nach, dass die Beschäftigung mit dem Mythos auch heute eine komplexe Auseinandersetzung mit elementaren Lebensfragen des Menschen bedeutet. Da die archaischen Figuren stets neue Rezeptionen herausfordern, bleibt der Mythos als Ort kulturellen Wissens aktuell. Das Buch widmet sich dem Thema über einen Dialog aus Philosophie, (...) Kultur- und Musikwissenschaft: Damit wird nicht zuletzt das mittlerweile unübersichtliche Dickicht aus Mythosbegriffen durchdrungen und eine klare Terminologie gewonnen, die auch für andere Kulturwissenschaften eine Basis zum Umgang mit dem Mythos liefern kann. This paper is a critical notice of a recent significant contribution to the debate about fitting attitudes and value, namely Toni Rønnow-Rasmussen’s Personal Value. In this book, Rønnow-Rasmussen seeks to analyse the notion of personal value—an instance of the notion of good for a person—in terms of fitting attitudes. The paper has three main themes: Rønnow-Rasmussen’s discussion of general problems for fitting attitude analyses; his formulation of the fitting attitude analysis of personal value and the notion of ‘for someone’s sake (...) attitudes’; and his critique of the dichotomy between agent-neutral and agent-relative reasons.
