Robust stakeholder engagement in health research requires broad communicative integration, not only of patients but also other stakeholders such as health system leaders, clinicians, and researcher...

While many "benchtop-to-bedside" research pathways have been developed in "Type I" translational medicine, vehicles to facilitate "Type II" and "Type III" translation that convert scientific data into clinical and community interventions designed to improve the health of human populations remain elusive. Further, while a high percentage of physicians endorse the principle of citizen leadership, many have difficulty practicing it. This discrepancy has been attributed, in part, to lack of training and preparation for public advocacy, time limitation, and institutional resistance. As (...) translational medicine and physician-citizenship implicate social, political, economic and cultural factors, both enterprises require "integrative" research strategies that blend insights from multiple fields of study, as well as rhetorical acumen in adapting messages to reach multiple audiences. This article considers how argumentation theory's epistemological flexibility, audience attentiveness, and heuristic qualities, combined with concepts from classical rhetoric, such as rhetorical invention, the synecdoche, and ethos, yield tools to facilitate translational medicine and enable physician-citizenship. In 2000, US Secretary of Health and Human Services Secretary Tommy Thompson mobilized the US public health infrastructure to deal with escalating trends of excess body weight. A cornerstone of this effort was a report entitled The Surgeon General's Call to Action to Prevent and Decrease Overweight and Obesity. The report stimulated a great deal of public discussion by utilizing the distinctive public health terminology of an epidemic to describe the growing prevalence of obesity in the US population. We suggest (...) that the ensuing controversy was fueled in part by the report's ambiguous usage of the evocative term “epidemic.” In some passages, the report seems to use “epidemic” in a literal sense, suggesting that rising prevalence of excess body weight should be defined technically as a disease outbreak. Other passages of the report present the same key term metaphorically, leaving readers with the impression that the epidemic language is invoked primarily for rhetorical effect. Here, we explore dynamics and implications of both interpretations. This analysis sheds light on the ongoing public argument about the appropriate societal response to steadily increasing body sizes in the US population; likewise, it capitalizes on the accumulated knowledge that the field of public health has garnered from combating diverse historic epidemics. Our interdisciplinary approach deploys critical tools from the fields of rhetoric, sociology and epidemiology. In particular, we draw from metaphor theory and public address scholarship to elucidate how the Call to Action frames public deliberation on obesity. We turn to the applied public health literature to develop a reading of the report that suggests a novel approach to the problem—application of the Epidemic Investigation protocol to streamline the public health response and reframe the public argument about obesity. How can teacher development be characterised? In this paper we offer a conceptualisation of teacher development as the enhancement of knowledge and capabilities to function in the activity of a teacher and illustrate with a case study. Our analytic focus is on the development of a science teacher, David, as he engaged in an innovative, collaborative project on learning photonics at a metropolitan secondary school in Australia. Three dimensions of development emerged: technical confidence and competence, pedagogical development and personal agency. (...) We explore the transformative effects of intrapersonal tensions within the teacher’s constitution of his role in the emerging community of enquiry — positioning him in turn as learner, instructor and facilitator. We view the context for David’s actions as a complex and dynamic system and interpret David’s development as arising from his responses to the differences in his emerging roles in the project. Importance Informed consent is essential to ethical, rigorous research and is important to recruitment and retention in cancer trials.Objective To examine cancer clinical trial (CCT) participants’ perceptions of informed consent processes and variations in perceptions by cancer type.Design and Setting and Participants Cross-sectional survey from mixed-methods study at National Cancer Institute–designated Northeast comprehensive cancer center. Open-ended and forced-choice items addressed: (1) enrollment and informed consent experiences and (2) decision-making processes, including risk-benefit assessment. Eligibility: CCT participant with gastro-intestinal or genitourinary, hematologic-lymphatic (...) malignancies, lung cancer, and breast or gynecological cancer (N = 334).Main Outcome Measures Percentages satisfied with consent process and information provided; and assessing participation’s perceptions of risks/benefits. Multivariable logistic or ordinal regression examined differences by cancer type.Results Most patient-participants felt well informed by the consent process (more than 90% overall and by cancer type) and. most (87.4%) reported that the consent form provided all the information they wanted, although nearly half (44.8%) reported that they read the form somewhat carefully or less. More than half (57.9%) said that talking to research staff (i.e., the consent process) had a greater impact on participation decisions than reading the consent form (2.1%). A third (31.1%) were very sure of joining in research studies before the informed consent process (almost half of lung cancer patients did—47.1%). Most patients personally assessed the risks and benefits before consenting. However, trust in physicians played an important role in the decision to enroll in CCT.Conclusions and Relevance Cancer patients rely less on written features of the informed consent process than on information obtained from the research staff and their own physicians. Research should focus on information and communication strategies that support informed consent from referring physicians, researchers, and others to improve patient risk-benefit assessment and decision-making. Corruption can impede commerce and economic development, yet it seems to be tolerated in many countries. The purpose of this study was to develop and test a model that integrates socioeconomic factors related to corruption. The analysis revealed that a negative relationship between economic adversity and wealth was mediated by corruption. Economic adversity was positively related to corruption, and corruption was inversely related to wealth. Uncertainty avoidance moderated the relationship between economic adversity and corruption, whereas power distance and uncertainty avoidance (...) were positively associated with corruption. The implications of these results for enhancing the effectiveness of international agreements are discussed. (shrink)

This study tests an integrative model of corporate political action, the filter model, based on the behavioral theory of the firm. The filter model posits that external political, economic, and industry environments are mediated by organizational structures and resources to affect a firm’s political actions. The authors rate the filter model’s predictive power against that of an economic-based direct-effects model by examining the efforts of about 1,100 U.S.-domiciled manufacturing firms to influence trade policy. This chapter defends a theory of objectification, conceiving of it as a species of what aestheticians have called ‘seeing‐as’, and more specifically, a kind of seeing‐as which to some degree is insensitive to the mind or mental aspects. An advantage of this view is that it covers both sexual and racial objectification, and can also explain how photographic images can objectify their subjects: namely, by encouraging the viewer to view in a way insensitive to the mind or mental aspects of (...) the subject. As the senses of sight and touch separated with the industrial mapping of the body in the nineteenth century, the visible and the visualized aligned themselves in medical, scientific, and sexological discourses; even history claimed to make the past “visible.” The criteria of the visible came to mark modernity. Cultural studies of visualization technologies help us to understand history itself as sign of the modern and to join its desires for the visible to those desires for spectacle produced among observers (...) of visualizing media in the late nineteenth and early twentieth centuries. In my dissertation I develop a series of guidelines for responsibly and respectfully navigating varying facets of aesthetic activity involving fat communities. I argue that fat people's engagement with the aesthetic can be used to foster community, resist anti-fat bias, and move towards fat justice. Moreover, I argue that considering representations and treatment of fat people in the production of art must be done carefully in order to avoid perpetuating negative stereotypes and anti-fat bias. My project aims to improve the (...) visibility of fat people, ensuring that fat communities can determine how and when they are seen in ways that affirm their identities and experiences. My project is broken down into four chapters, each of which tackles a different dimension of aesthetic engagement and activity. In the first, I develop a framework called "responsible aesthetic agency" for creators to use when depicting fat people in their work. I intend responsible aesthetic agency to be a way for creators to meet their bare minimum ethical obligations to fat people, and as such this framework is a precursor to Jeremy Fried's aesthetic allyship. I use this framework to argue that artists must attend to the narrative webs in which a work exists when designing their new work's narrative, and that fat people must be active participants in this design and in the realization of said narratives. My second chapter addresses how individuals can use aesthetic agency to advocate for themselves and their communities. I argue that beauty labor can be used to help dismantle anti-fat bias. Using Kathleen LeBesco's framework of agency and identity, coupled with Jeannine Gailey's assessment of the unique perception of fatness as simultaneously hypervisible and hyperinvisible, I argue that participants in the "Fuck Flattering!" movement shape their identities and advocate for fat communities by forcing society to acknowledge their visibility and worth. In my third chapter, I consider representations of fatness in popular media, arguing that we can best achieve fat visibility through centering imperfect representations of fatness. I use Anne Eaton's "Taste in Bodies and Fat Oppression" as a starting point. In this paper, Eaton argues that distaste for fat bodies has an affective element of disgust that we must address to mitigate anti-fatness. Eaton argues that we must habitually engage with vivid and engaging representations that encourage us to see fat bodies as likable and attractive. I argue that while this approach has worthwhile aims, it risks leaving behind those fat bodies which we cannot bring ourselves to see as likeable or attractive. To better combat our distaste for fat bodies, I argue that we must show fat people as complexly as we show thin characters onscreen, highlighting the pluralities of ways to exist as a fat person. Lastly, in my fourth chapter I discuss how to appropriately engage with marginalized communities as a member of a dominant group or identity. I examine the co-optation of the body positivity movement, outline the ethical harms that result from said co-optation, and discuss potential avenues for moving forward with the movement given its co-optation. I argue that in order to fully address the myriad levels of goals of the original body positivity movement, none of these three alternatives is sufficient, as each misses a crucial element of the original body positivity movement or asks participants to have too narrow of an attitude towards their own body. Given the shortcomings of these movements, I argue we should move forward from the body positivity movement’s co-optation by employing a modified framework of Aubrey Gordon’s concept of fat justice, one which also makes room to recognize the importance of fat people being able to experiment with beauty and see themselves as beautiful. A theory of implementation for multilateral regulation of business is introduced. Four types of implementation actors (international organizations, states, nongovernmental organizations, and business associations) and three implementation tasks (communicate, monitor, and sanction) are identified. The ease of implementation is affected by issues and events that occur early in the life cycle of a policy. The Oregon Health Division's report on assisted suicide under the state's new legislation does not provide enough data to support its reassuring conclusions. Especially glaring is the lack of data on the palliative care offered to patients who requested assisted suicide, on their financial situation, and on their emotional state.

Advances in medicine depend not only on the generation of information but also on its dissemination. Clinically relevant data must be transmitted to the practitioners who will use it. Health care professionals in North America are aware of their ethical and legal obligations to inform patients adequately concerning interventions and treatments so that they may make informed choices about medical care. This obligation has been well described and defined by the courts and in the literature of medicine, ethics, and law. (...) But do investigators and others who are responsible for disseminating information have any obligation to divulge adequately the results of clinical trials to practitioners so that the latter might offer the choice of appropriate care to their patients? Can incomplete disclosure of trial outcomes result in harm to patients? If this is the case, what possible avenues are available to avoid such harm? (shrink)

It discusses what one's whole life entails within the context of decision making. It looks at decisions between competing values. In this essay, the author “scrutiniz[es] the ‘signs of the times’ and seek[s] to detect the meaning of emerging history” to explore the call to conversion issued by the 1971 Synod of Bishops in Justice in the World (JW). In that document, they condemn oppressive systems of domination that hinder authentic human development and urge people toward conversion of the Spirit, which “frees [them] from personal sin and from its consequences in social life.” To determine what it is that people (...) are to convert from, this essay builds on an evolutionary framework—developed through Augustine’s ecology and contemporary scientific theory—and explores how this framework can help limit the pursuit of domination in favor of promoting a more integrated and just world. Doing so contributes to the dialogue concerning how Christians can “work out their salvation by deeds of justice,” in light of the prescient framework delivered in JW. (shrink)

Perhaps consciousness is nothing less and nothing more than story, and all we need do to continue to feel conscious is maintain identity through computer‐based narrative. Applied nanotechnology has generated uncountable (...) applications in electronics, pharmacology, and materials engineering. The best approach to life extension and consciousness expansion might lie in our own marvelously complex and entire bodies, meshed with and augmented by tiny bionan machines that become a part of us, rather than the opposite vision of humans migrating into a machine substrate. In 2008, renowned Australian composer Peter Sculthorpe remarked that almost everything he has written since the early 1960s has been influenced by Indigenous music “because that was a music … shaped by the landscape over 50,000 years.” 3 His preference for accumulating “an effect of relentless prolongation” through the use of long drones has seen his music fail, until recently, to appeal to an Australian ear attuned to Bach and Mozart. 4 His aim, however, has not been to satisfy the (...) European-trained ear, but to “mirror the Australian outback”; to capture a sense of time and space that many Australians have repeatedly failed to access. 5 Sculthorpe‟s efforts to form connections with the landscape are by no means singular; representations of the bush have dominated the arts from early settlement, albeit in a largely negative fashion. The contradiction between an accepted Australian vision of the landscape as empty, unwelcoming bush or desert, and the social reality of a nation living mainly in coastal communities has not gone unnoticed. 6 Nevertheless, contemplating this “accepted vision” alongside the lukewarm reception of Sculthorpe‟s work gives rise to the following question: what conclusions, if any, may be drawn from insistently negative representations of the landscape? In consideration of this question, I propose an analysis of two texts in which the representations of landscape have been overlooked in favour of feminist or classical readings: Barbara Baynton‟s Bush Studies and Joan Lindsay‟s Picnic at Hanging Rock. If “landscape is personal and tribal history made visible,” what is one to make of Bush Studies? 7 The parallels between Baynton‟s narratives of isolation and her own experiences cannot fully account for her portrayals of the bush as “a lonely, hostile place, antagonistic to its inhabitants,” where time recedes into the vast distances of the plains, and the past and future are uncertain. 8 Baynton‟s settlers, cut adrift from cultural and religious traditions, fail to form communities and conform to the notion that “where si gnificant tradition counts for little, places may be virtually without time,” leading to “no awareness of history.” 9 In short, Baynton‟s settlers are plagued with a sense of timelessness that appears to originate from perception, rather than the environment. Much of neuroscience is currently dominated by an information processing metaphor which is largely conceptualized in discrete terms. An alternative metaphor conceptualizes information flow as continuous. A qualitative set of hypotheses based on this metaphor, the energy model, is described here. This model considers information transfer in terms of the flow of an abstract variable, energy, between points in a field comprising the extent of the nervous system. Although extremely simple, it generates some intriguing consequences. In particular, it provides a (...) useful way in which to look at consciousness. Traditional problems of consciousness, such as qualia and the unity of consciousness, are briefly addressed, and outlines are sketched of the answers given by the energy model. (shrink)
