This article explores the case of informal car parkers in Mexico City, to whom drivers regularly entrust the keys to their vehicles. In contrast to literature on social trust that expects institutional trust and interpersonal trust to support one another, the article shows that interpersonal trust improbably arises in the context of corrupt and inefficient institutions. Coercive and market dynamics undergird the interactions among car parkers, police officers, and drivers, making possible the emergence of an informal market and noncontractual agreements. (...) The exchange between car parkers and drivers depends on the performance of class—a performance that becomes legible and formulaic enough to appear safe, precisely because it coalesces classist tropes that are clearly distinct and recognizable. Interpersonal trust becomes possible not only despite class cleavages and institutional shortcomings but, paradoxically, because of them. (shrink)

Bien que les minorités sexuelles et de genre courent un risque accru de problèmes de santé, on dispose de peu de données sur les interactions patient-prestataire de soins. Dans cette étude, nous avons exploré les perspectives des patients LGBTQ et leurs rencontres avec les médecins afin d’améliorer notre compréhension des expériences patient-médecin. À l’aide d’une sélection ciblée de patients LGBTQ auto-identifiés, nous avons réalisé quatorze entrevues semi-structurées en profondeur sur des sujets comme l’orientation sexuelle et l’identité de genre, ainsi que (...) leur rôle perçu dans la relation patient-fournisseur. Un codage utilisant une approche modifiée de la théorie ancrée a été effectuée pour générer des thèmes. Nous avons identifié trois grands thèmes qui démontrent la complexité des expériences vécues par les patients LGBTQ. La première, le manque de confiance, identifie la méfiance et la perte de la relation médecin-patient résultant d’une mauvaise communication des médecins ou d’un jugement porté sur eux, ou du fait que les médecins font des suppositions sur le genre, utilisent des pronoms incorrects et ne reconnaissent pas l’hétérogénéité dans la communauté transgenre. Un deuxième thème, être vulnérable, décrit les défis et les craintes liés au réconfort des patients face à la divulgation de leur orientation sexuelle ou de leur identité de genre. Un dernier thème, la discrimination sélective, met en relief la discrimination raciale ou ethnique qui crée un fardeau supplémentaire en plus de la maladie et de l’identité stigmatisée. Nos résultats révèlent les besoins complexes des personnes aux multiples identités stigmatisées lorsqu’il s’agit d’établir des relations avec les prestataires. En utilisant une perspective intersectionnelle qui tient compte de la pluralité des identités des patients, les prestataires peuvent aider à améliorer leurs relations avec les patients LGBTQ. L’intégration d’une formation intersectionnelle pour les étudiants en médecine et les résidents pourrait grandement profiter aux patients LGBTQ et à leurs médecins. (shrink)

Aunque el modelo de atención en salud ha evolucionado, aún suele desplazar la importancia de incluir la narrativa del paciente e intenta uniformar las necesidades de las personas enfermas. La medicina narrativa nos da la capacidad de interpretar y comunicar los relatos de nuestros pacientes, a través de la escucha activa, así como de valorar la complejidad del ser humano, a diferencia de la salud basada en la evidencia, donde las experiencias de un individuo son extrapolables a otros pacientes sin (...) importar su historia, con lo que se reduce gran parte de la narración a una clasificación objetiva en la ficha clínica, cuando lo correcto sería integrar el paradigma narrativo en la atención de personas enfermas. Creemos que enseñar competencias narrativas y saber científico a profesionales de odontología permitirá estrechar la relación clínica con los pacientes a partir de valores como la empatía y la compasión, mermando el sufrimiento ligado a lo objetivo de sus enfermedades. Laurent Henninger intervenant sur les « révolutions militaires » (notion située au carrefour du débat historique lancé dans les années 1980 par Geoffrey Parker – en polémique avec Jeremy Black – et du débat stratégique américain dans les années 1990) souligne que les avancées dans l’art de la guerre ont été depuis cinq siècles une des composantes majeures de la barbarisation et du totalitarisme. La notion de « révolution militaire » est aujourd’hui contestée par ceux qui y voient un (...) outil interprétatif de trop longue durée et lui préfèrent l’idée de « mutations militaires » (selon laquelle l’accumulation de changements millimétriques sur la longue durée débouche sur des ruptures). Après avoir passé en revue la première révolution militaire – XVe-XVIIe siècles – fondée sur la mise à distance de la cavalerie grâce à l’infanterie nouvelle, à l’artillerie du champ de bataille, Henninger s’interroge sur le critère discriminant pour qualifier les nouvelles guerres de la Renaissance (l’hyperviolence n’en est pas un à ses yeux) et voit dans les armes à feu la vraie rupture car elles influent de façon radicale sur la peur et le courage des combattants et sont source d’une véritable « inhumanité » dans la mesure où leur feu est « imparable ». Il insiste sur l’émergence d’un nouveau type de courage guerrier non archaïque et plus « stoïcien » marqué par l’acceptation de la mort. Dans cette perspective, l’« inhumanisation » technique porterait en elle la possibilité de la barbarisation. L. Henninger développe ensuite cette thèse à partir de l’examen des guerres mécanisées de la période 1860-1950 en s’attardant sur la guerre de Sécession puis sur la Grande Guerre. Il relie aussi la question du totalitarisme à la réflexion sur les « bombardements stratégiques massifs » et les débuts de la dissuasion nucléaire (terme qui ne s’impose que vers 1960). Il conclut en insistant sur le fait que, selon lui, la brutalisation des conflits naît toujours d’un retard de la pensée de la guerre et sur le paradoxe d’une culture occidentale qui produit en même temps barbarie de la guerre et codes qui tentent de l’enrayer. Este capítulo se propone explorar cómo el replanteo de las causas finales en la ciencia natural suscitado por la Revolución Científica impactó en el desarrollo del materialismo y el ateísmo del Siglo de las luces. Con el surgimiento de la ciencia moderna en Europa, en el siglo XVII ocurrieron cambios sustantivos en la concepción de la causalidad. Ciertos estudios pioneros sobre el tema, sostenían que la nueva ciencia anulaba las causas finales y establecía que todos los fenómenos naturales debían explicarse (...) en términos de materia y movimiento (Burtt, 1952:98‒99; Schmitt, 1983:5). Más tarde, otros especialistas —con argumentos más sólidos y pruebas más convincentes— mostraron que la causalidad final no fue anulada por completo; siguió presente en muchos autores, aunque ya no como parte indispensable de la explicación científica, sino como un componente ontológico de la naturaleza (Osler, 1996 y 2001). Ahora bien, ¿qué sucedió con las causas finales en el siglo XVIII europeo, época de profundas turbulencias políticas y controversias filosóficas, en la cual el materialismo, el ateísmo y el deísmo fueron ganando terreno? Para hacer un recorte dentro de la vasta variedad de posiciones existentes en este escenario, comenzaremos por repasar la posición sobre las causas finales de Francis Bacon (1561‒1626), quien fue una figura muy influyente en la Ilustración. 2 Luego, analizaremos la recepción de esa posición por parte de algunos autores británicos (D. This article focuses on portrayals of fictional characters with neurological synesthesia in seven selected 20th and 21st century English-language novels in the detective-spy genre. Characters are discussed in terms of the five categories of literacy depiction of synesthete characters as outlined in the chapter, “Synesthesia and Literature” in the Oxford Handbook of Synesthesia. I will suggest that depictions of synesthete characters in the detective genre link synesthetic perceptions with glimpses of ultimate truth, and trace these tendencies back to descriptions of (...) synesthesia in 19th century seminal European works, including Arthur Rimbaud’s “Letter of a Seer” and Max Nordau’s Degeneration. Novels included in this study: Invitation to a Beheading by Vladimir Nabokov, The Synesthete by Brent Kiernan, Top Ten by Gene Ha and Alan Moore, The Fallen by T. Jefferson Parker, Miracle Myx by Dave Diotalevi, Still Waters by Nigel McCrery, Red Sparrow by Jason Matthews. Cet article se concentre sur les représentations de personnages fictifs ayant une synesthésie neurologique, dans sept romans anglais des xxe et xxie siècles, sélectionnés dans le genre policier-espionnage. Les personnages sont examinés à partir des cinq catégories de représentation littéraire des personnages synesthésiques, exposées dans le chapitre « Synesthesia and Literature » de l’Oxford Handbook of Synesthesia. Je suggérerai que les représentations de personnages synesthésiques dans le genre policier relient les perceptions synesthésiques à des intuitions sur une vérité supérieure et rapporte ces tendances aux descriptions de la synesthésie dans les œuvres européennes essentielles du xixe siècle, y compris la « Lettre d’un Voyant » d’Arthur Rimbaud et le Dégénérescence de Max Nordau. Unlike some other reproductions of classic texts (1) We have not used OCR(Optical Character Recognition), as this leads to bad quality books with introduced typos. (2) In books where there are images such as portraits, maps, sketches etc We have endeavoured to keep the quality of these images, so they represent accurately the original artefact. Although occasionally there may be certain imperfections with these old texts, we feel they deserve to be made available for future generations to enjoy.

This study drew on three theoretical perspectives – attribution theory, power, and role identity theory – to compare the job-related outcomes of sexual harassment from organizational insiders and organizational outsiders in a sample of UK police officers and police support staff. Results showed that sexual harassment from insiders was related to higher intentions to quit, over-performance demands, and lower job satisfaction, whereas sexual harassment from outsiders was not significantly related to any of the outcome variables investigated. We also examined two (...) moderator variables: equal opportunity support and confidence in grievance procedures. Consistent with our hypotheses, equal oppor- tunity support mitigated the effects of sexual harassment from supervisors on intent to quit and over-performance demands. Confidence in grievance procedures moderated the relationship between sexual harassment from supervisors and all outcome variables. Implications for theory and practice are discussed. (shrink)

In order to involve children in the decision-making process about participation in medical research it is widely recommended that the child’s assent be sought in addition to parental consent. However, the concept of assent is fraught with difficulties, resulting in confusion among researchers and ethics committees alike.

Employing the National Institute of Mental Health-funded Prevention of Suicide in Primary Care Elderly Collaborative Trial as a case study, we discuss 2 sets of ethical issues: obtaining informed consent for a clinic-based intervention study and using treatment as usual (TAU) as the control condition. We then address these ethical issues in the context of the debate about the quality improvement efforts of health care organizations. Our analysis reveals the tension between ethics and scientific integrity involved with using TAU as (...) a control condition and the difficulty in designing high-quality research in a community-based setting. IntroductionDeep brain stimulation (DBS) is increasingly used to treat the symptoms of various neurologic and psychiatric conditions. People can undergo the procedure during reproductive years but the safety of DBS in pregnancy remains relatively unknown given the paucity of published cases. We thus conducted a review of the literature to determine the state of current knowledge about DBS in pregnancy and to determine how eligibility criteria are approached in clinical trials with respect to pregnancy and the potential for pregnancy.MethodsA literature (...) review was conducted in EMBASE to identify articles involving DBS and pregnancy. Two reviewers independently analyzed the articles to confirm inclusion. Data extracted for analysis included conditions treated, complications at all stages of pregnancy, neonatal/pediatric outcomes, and DBS target. A second search was then conducted using www.clinicaltrials.gov. The same two reviewers then assessed whether each trial excluded pregnant individuals, lactating individuals, or persons of childbearing age planning to conceive. Also assessed was whether contraception had to be deemed adequate prior to enrollment.ResultsThe literature search returned 681 articles. Following independent analysis and agreement of two reviewers, 8 pregnancy related DBS articles were included for analysis. These articles described 27 subjects, 29 pregnancies (2 with subsequent pregnancies), and 31 infants (2 twin pregnancies). There was 1 preterm birth at 35 weeks, and 3 patients who experienced discomfort from the DBS battery (i.e., impulse generator) placement site. All 27 patients had a DBS device implanted before they became pregnant, which remained in use throughout their pregnancy. There was exclusion of pregnant individuals from 68% of 135 interventional trials involving DBS. Approximately 44% of these trials excluded persons of childbearing age not on “adequate contraception” or wishing to conceive in the coming years. Finally, 22% excluded breastfeeding persons.ConclusionThe data from 29 pregnancies receiving DBS treatment during pregnancy was not associated with unexpected pregnancy or post-partum complication patterns. Many clinical trials have excluded pregnant individuals. Documentation of outcomes in larger numbers of pregnancies will help clarify the safety profile and will help guide study designs that will safely include pregnant patients. (shrink)

This casebook offers a window into important aspects of the ethical landscapes that researchers, communities, health professionals, policy makers – and ethicists – had to navigate during the first 15 months of the COVID-19 pandemic. The cases presented in this casebook are inevitably a selection informed by and constrained by the processes through which they were sought, and by the pandemic itself. Additional cases could valuably complement all the thematic chapters in this casebook. In addition, this casebook calls for a (...) broader approach to research ethics, both in terms of the issues to be considered, and the range of stakeholders having ethical responsibilities relating to the conduct of research. However a broad range of stakeholders have differing values, remits, authorities and capacities to exercise power in pandemic contexts, and in many situations, exercises of power, and their impact on research, are not direct and explicit. As such they are less amenable to clear representation in real-world cases, highlighting the importance of complementing discussions of the cases in this casebook with conceptual literature. Reflection on the research that has not been conducted is also critical. The COVID-19 pandemic has reemphasized that global health emergencies are never only about health. The wide-ranging impacts of the pandemic on economies, employment, education and a range of socially and culturally important activities, accentuates the importance of an equally comprehensive research agenda, which goes beyond a narrow conception of ‘health’, and addresses a broad range of pandemic impacts on populations. A further way in which we believe debate on pandemic research ethics both could and should be broadened is in relation to aspects of pandemic science beyond those relating to ‘response’. Inevitably, in the context of an emerging and continuing pandemic, scientific research attention has tended to focus on interventions that can enable more effective responses. However pandemic science can be thought of as divisible into four interdependent and overlapping domains: prevention, preparedness, response, and recovery. Research is essential to the development, evaluation, and deployment of interventions in each of these domains and effective, valuable, trustworthy and trusted research will require ethical questions to be identified and addressed. This chapter concludes by inviting the connection of additional cases and conceptual resources to this casebook, to enhance and expand the themes and topics covered. (shrink)

Ethical discussions about Artificial Intelligence (AI) often overlook its potentially large impact on nonhuman animals. In a recent commentary on our paper about AI’s possible harms, Leonie Bossert argues for a focus not just on the possible negative impacts but also the possible beneficial outcomes of AI for animals. We welcome this call to increase awareness of AI that helps animals: developing and using AI to improve animal wellbeing and promote positive dimensions in animal lives should be a vital ethical (...) goal. Nonetheless, we argue that there is some value in focusing on technology-based harms in the context of AI ethics and policy discourses. A harms framework for AI can inform some of our strongest duties to animals and inform regulation and risk assessment impacts designed to prevent serious harms to humans, the environment, and animals. Despite having paved the way for face, womb and penis transplants, hand transplantation today remains a small hybrid of reconstructive microsurgery and transplant immunology. An exceptionally limited patient population internationally complicates medical researchers’ efforts to parse outcomes “objectively.” Presumed functional and psychosocial benefits of gaining a transplant hand must be weighed in both patient decisions and bioethical discussions against the difficulty of adhering to post-transplant medications, the physical demands of hand transplant recovery on the patient, and the serious long-term health (...) risks of immunosuppressant drugs. This paper relates five narratives of hand transplantation drawn from an oral history project to show how narrative methods can and should inform ethical evaluations and the clinical process of hand transplantation. The interviews with patients and their partners analyzed here lead us to suggest that qualitative accounts of patient experiences should be used to complement clinical case studies reported in medical journals and to help develop instruments to assess outcomes more systematically. En primer lugar, se estudian los elementos de retribución, reciprocidad y altruismo implicados. En segundo lugar, se desarrollan algunas de las habituales críticas sobre esta Regla y dos argumentos de defensa. Finalmente, se aplica el análisis sobre esta Regla a la resolución de cuatro casos controvertidos. Finally, it applies the analysis of this Rule to the resolution of four controversial cases. Ainsi, les impératifs communs des magistrats et avocats permettent de comprendre le sens des règles déontologiques que sont l’indépendance, la notion de conflit d’intérêts, l’impartialité, l’intégrité, le secret, la probité, le respect d’autrui, la (...) dignité, la conscience… En dépit de la spécificité de chaque profession, l’Éthique demeure la boussole des comportements professionnels dans l’intérêt du Droit et de la Personne afin de garantir les libertés et la confiance que celle-ci doit avoir dans les institutions. Propone develar cómo influye el narcotráfico, las formas de resistencia y reexistencia en la escuela del Norte del Valle del Cauca (Colombia). Desde un enfoque cualitativo se forman textos de las historias de vida y relatos de experiencias construidos en diálogo sobre los temas generadores Escuela y narcotráfico, analizados por medio de matrices de interpretación y su triangulación; autores, actores y maestro participante en una perspectiva hermenéutica pluritópica. En ella participan estudiantes, padres y madres de familia, docentes y otros miembros (...) de la comunidad escolar. En sus conclusiones se destaca: la marca colonial en la economía nacional (glocal); el imperativo impuesto por las precarias condiciones económicas; los altos niveles de corrupción que crean un caldo de cultivo para el surgimiento de grupos delincuenciales; la escuela como posibilidad de formación humana que resiste y reexiste al narcotráfico; las contradicciones a las que son sometidos los jóvenes en el enfoque de atención al problema del consumo y la capacidad de algunos miembros de la comunidad para construir un proyecto de vida que les permite reexistir por fuera de las lógicas del narcotráfico como sujetos históricos.
