This article draws on the sociology of expectations to examine the construction of expectations of ‘ethical AI’ and considers the implications of these expectations for communication governance. We first analyse a range of public documents to identify the key actors, mechanisms and issues which structure societal expectations around artificial intelligence and an emerging discourse on ethics. We then explore expectations of AI and ethics through a survey of members of the public. Finally, we discuss the implications of our findings for (...) the role of AI in communication governance. We find that, despite societal expectations that we can design ethical AI, and public expectations that developers and governments should share responsibility for the outcomes of AI use, there is a significant divergence between these expectations and the ways in which AI technologies are currently used and governed in large scale communication systems. We conclude that discourses of ‘ethical AI’ are generically performative, but to become more effective we need to acknowledge the limitations of contemporary AI and the requirement for extensive human labour to meet the challenges of communication governance. An effective ethics of AI requires domain appropriate AI tools, updated professional practices, dignified places of work and robust regulatory and accountability frameworks. (shrink)

Given the wide-reaching and detrimental impact of COVID-19, its strain on healthcare resources, and the urgent need for—sometimes forced—public health interventions, thorough examination of the ethical issues brought to light by the pandemic is especially warranted. This paper aims to identify some of the complex moral dilemmas faced by senior physicians at a major medical centre in Saudi Arabia, in an effort to gain a better understanding of how they navigated ethical uncertainty during a time of crisis. This qualitative study (...) uses a semistructured interview approach and reports the findings of 16 interviews. The study finds that participants were motivated by a profession-based moral obligation to provide care during the toughest and most uncertain times of the pandemic. Although participants described significant moral dilemmas during their practice, very few identified challenges as ethical in nature, and in turn, none sought formal ethics support. Rather, participants took on the burden of resolving ethical challenges themselves—whenever possible—rationalising oft fraught decisions by likening their experiences to wartime action or by minimising attention to the moral. In capturing these accounts, this paper ultimately contemplates what moral lessons can, and must be, learnt from this experience. (shrink)

Realists about practical reasons agree that judgments regarding reasons are beliefs. They disagree, however, over the question of how such beliefs motivate rational action. Some adopt a Humean conception of motivation, according to which beliefs about reasons must combine with independently existing desires in order to motivate rational action; others adopt an anti-Humean view, according to which beliefs can motivate rational action in their own right, either directly or by giving rise to a new desire that in turn motivates the (...) action. I argue that the realist who adopts a Humean model for explaining rational action will have a difficult time giving a plausible account of the role that desire plays in this explanation. I explore four interpretations of this role and argue that none allows a Humean theory to explain rational action as convincingly as an anti-Humean theory does. The first two models, in different ways, make acting on a reason impossible. The third allows this possibility, but only by positing a reason-sensitive desire that itself demands an explanation. The fourth avoids this explanatory challenge only by retreating to an empty form of the Humean view. In contrast, an anti-Humean theory can provide an intuitively plausible explanation of rational action. I conclude that the realist about reasons should adopt an anti-Humean theory to explain rational action. (shrink)

In Finite and Infinite Goods, Adams develops a sophisticated and richly detailed Platonic-theistic framework for ethics. The view is Platonic in virtue of being Good-centered; it is theistic both in identifying God with the Good and, more distinctively, in including a divine command theory of moral obligation. Readers familiar with Adams’s earlier divine command theory will recall that in response to the worry that God might command something evil, Adams introduced an independent value constraint, claiming that only the commands of (...) a loving God were fit to constitute moral obligations. In Finite and Infinite Goods he develops this notion of a loving and good God in what is now a fundamentally Good-centered ethical framework with a subordinate divine command theory of moral obligation. There is much of worth here, especially for theists wishing to think through the merits of a good-based theistic ethics, but also for those nontheists like myself who have a general interest in the nature of value and obligation. Malaria is a disease of developing countries whose local health services do not have the time, resources or personnel to mount studies of drugs or vaccines without the collaboration and technology of western investigators. This investigative collaboration requires a unique bridging of cultural differences with respect to human investigation. The following debate, sponsored by The Institute of Medicine and The American Society of Tropical Medicine and Hygiene, raises questions concerning the conduct of trans-cultural clinical malaria research. Specific questions are raised (...) about the difficulties of informed consent in different cultural settings and whether there is any role for community involvement. Discussants debate whether drug and vaccine trials not approved in an industrialised country are ever defensible if performed in a third-world setting. The objective of this study is to evaluate if use of a patient decision aid, when choosing between a repeat cesarean or a trial of labor after a cesarean, reduces medical liability exposure. The authors conclude that use of a PDA conferred liability protection when potential jurors were presented with a hypothetical malpractice claim against an obstetrician following a TOLAC.

Screening for prostate cancer with the prostate-specific antigen blood test is controversial, as evidence to date has not demonstrated such screening does more good than harm. While the potential benefit of PSA screening on reducing prostate cancer mortality has not been documented in randomized trials, many risks of PSA screening have been well documented. These risks include a substantially higher risk of a prostate cancer diagnosis over a screenee’s lifetime, false-positive and false-negative test results, possible complications from biopsies done in (...) response to suspicious test results, and substantial risks of side effects from subsequent prostate cancer treatments. Nevertheless, PSA screening is widespread in the United States, and many physicians in a national survey reported supporting routine PSA screening as well as getting PSA tests themselves, despite the lack of evidence. To determine the effectiveness of current ethical teaching and to suggest ways to reform the current ethical curriculum in light of students’ perspectives and experiences. Students of Dow Medical College were selected for this cross-sectional study conducted between the year 2020 till 2023. The sample size was 387, calculated by OpenEpi. A questionnaire consisting of 17 close-ended questions was used to collect data from participants selected via stratified random sampling. The questionnaire consisted of two parts. The first part included the (...) demographics. While the second contained 15 questions designed to assess the participants’ current teaching of ethics and effective ways to further improve it. The data obtained were analyzed using IBM SPSS statistics 26. Out of the 376 students who gave consent, the majority of the respondents (64.6%) encountered situations where they felt that their current teaching of ethics was insufficient and (54%) believed that the current teaching of ethics could be improved and made further effective. Practical sessions, PBLs (problem-based learning), case analysis, and ward visits were some of the ways the participants believed could help improve the teaching of medical ethics. Most students (92.8%) agreed that external factors like burnout and excessive workload have an impact on medical professionals’ ethical practices. In light of our study, a refined curriculum with a focus on ethical teaching must be established, with input from students to ensure that the medical students have the necessary expertise to manage an ethical dilemma. Prostate cancer has become a major health concern of male Americans. It is now the most common nondermatologic cancer and the second leading cause of cancer death among men. The incidence of detected prostate cancer rose rapidly in recent years, partly because of prostate-specific antigen testing; it is only now tapering off. Screening for prostate cancer with PSA is widespread in the United States, yet controversial: the American Urological Association recommends PSA screening and the American Cancer Society recommends offering screening; (...) however, the United States Preventive Services Task Force and the American College of Physicians recommend against routine screening; and the American Academy of Family Physicians believes that the decision to screen should be left to the patient. In Slaves of the Passions, Mark Schroeder provides a systematic, rigorously argued defense of a Humean theory of reasons for action, taking pains to respond to influential objections to the view. While inspired by Hume, Schroeder makes it clear that he aims to develop a Humean theory, not necessarily one that Hume himself embraced, and for this reason little is said about Hume in the book. One respect in which Schroeder takes himself to be departing from Hume is in developing (...) a normative account. On his reading, Hume held that only beliefs could stand in the reason relation (187, n11), whereas Schroeder, like many contemporary Humeans, holds that actions can as well.
