BackgroundSarcomas are a diverse group of neoplasms that vary greatly in clinical presentation and responsiveness to treatment. Given the differences in the sites of involvement, rarity, and treatment modality, a multidisciplinary approach is required. Previous literature suggests patients with sarcoma suffer from poorer quality of life especially physical and functional wellbeing. Adolescent and young adult patients are an underrepresented population in cancer research and have differing factors influencing QoL.MethodsRetrospective analysis of Young Adult patients enrolled in the Sarcoma Tissue Repository at (...) University of Iowa. QoL was assessed using the self-report FACT-G questionnaire at enrollment and 12 months post-diagnosis; overall scores and the 4 wellbeing subscales were calculated. Linear mixed effects models were used to measure the association between the rate of change in FACT-G subscale scores and baseline clinical, comorbidity, and treatment characteristics.Results49 patients were identified. 57.1% of patients had a malignancy involving an extremity. Mean FACT-G scores of overall wellbeing improved from baseline to 12 months. Social and emotional wellbeing did not differ significantly between baseline and 12 months. Physical wellbeing and functional wellbeing scores improved from baseline to 12 months. No difference was seen for FACT-G overall scores for age, sex, laterality, marital status, performance status, having children, clinical stage, limb surgery, chemotherapy, or tumor size. A difference was demonstrated in physical wellbeing scores for patients with baseline limitation compared to those with no baseline limitation. A difference was demonstrated in social wellbeing based on anatomical site.ConclusionYoung adults with sarcoma treated at a tertiary center had improvements in overall reported QoL at 12 months from diagnosis. Overall baseline QoL scores on FACT-G were lower than the general adult population for YA patients with sarcoma but at 12 months became in line with general population norms. The improvements seen merit further investigation to evaluate how these change over the continuum of care. Quality of life changes may be useful outcomes of interest in sarcoma trials. (shrink)

IntroductionYounger age at diagnosis is a risk factor for poor health-related quality of life in long-term breast cancer survivors. However, few studies have specifically addressed HRQOL in young adults with breast cancer, nor have early changes in HRQOL been fully characterized.MethodsEligible female patients with breast cancer were identified through our local cancer center. To establish HRQOL, patients completed the Functional Assessment of Cancer Therapy-Breast around diagnosis and 12 months later. Sociodemographic factors, genetic susceptibility to cancer, tumor- and treatment-related factors, and (...) comorbidities were abstracted from medical records and the local oncology registry. Mixed-effects models were used to identify changes in FACT-B scores during the first year of treatment and to determine whether any demographic/treatment-related factors modulated changes in scores.ResultsHealth-related quality of life in young patients with breast cancer was within normal limits at baseline, with a FACT-B overall well-being score of 108.5. Participants reported slight improvements over a 12-month period: FACT-B overall well-being scores increased 6.6 points, functional well-being improved 3.0 points, emotional well-being improved 1.9 points, and physical well-being improved 1.5 points, on average. Participants with anxiety/depression at baseline reported greater improvements in FACT-B overall well-being and functional well-being than participants who did not have anxiety/depression at baseline. Marital status, reconstructive surgery, and baseline clinical staging were also significantly associated with changes in aspects of HRQOL, although their impact on change was relatively minimal.ConclusionYoung women with breast cancer do not report HRQOL concerns during the first year of treatment. Improvements in HRQOL during the first year of treatment may be attributable to a sense of relief that the cancer is being treated, which, in the short run, may outweigh the negative late effects of treatment. The lesson to be learned from the paradoxical St. Petersburg game and Pascal’s Mugging is that there are situations where expected utility maximizers will needlessly end up poor and on death’s door, and hence we should not be expected utility maximizers. Instead, when it comes to decision-making, for possibilities that have very small probabilities of occurring, we should discount those probabilities down to zero, regardless of the utilities associated with those possibilities.

The question of how to interpret spontaneous collapse theories of quantum mechanics is an open one. One issue involves what link one should use to go from wave function talk to talk of ordinary macroscopic objects. Another issue involves whether that link should be taken ontologically seriously. In this paper, I ague that the link should be taken ontologically seriously; I argue against an ontology consisting solely of the wave function. I then consider three possible links: the fuzzy link, the (...) accessible mass density link, and the mass density simpliciter link. I show that the first two links have serious anomalies which render them unacceptable. I show that the mass density simpliciter link, in contrast, is viable. (shrink)

This paper addresses the question: how should the traditional doxastic attitude of agnosticism be represented in a Bayesian framework? Bas van Fraassen has one proposal: a Bayesian is agnostic about a proposition if her opinion about the proposition is represented by a probability interval with zero as the lower limit. I argue that van Fraassen's proposal is not adequate. Mark Kaplan claims that this leads to a problem with constructive empiricism; I show that Kaplan's claim is incorrect.

On Hume’s account of motivation, beliefs and desires are very different kinds of propositional attitudes. Beliefs are cognitive attitudes, desires emotive ones. An agent’s belief in a proposition captures the weight he or she assigns to this proposition in his or her cognitive representation of the world. An agent’s desire for a proposition captures the degree to which he or she prefers its truth, motivating him or her to act accordingly. Although beliefs and desires are sometimes entangled, they play very (...) different roles in rational agency. In two classic papers (Lewis 1988, 1996), David Lewis discusses several challenges to this Humean picture, but ultimately rejects them. We think that his discussion of a central anti-Humean alternative – the desire-as-belief thesis – is in need of refinnement. On this thesis, the desire for proposition p is given by the belief that p is desirable. Lewis claims that ‘[e]xcept in trivial cases, [this thesis] collapses into contradiction’(Lewis 1996, p. 308). The problem, he argues, is that the thesis is inconsistent with the purportedly plausible requirement that one’s desire for a proposition should not change upon learning that the proposition is true; call this the invariance requirement. In this paper, we revisit Lewis’s argument. We show that, if one carefully distinguishes between non-evaluative and evaluative propositions, the desire-asbelief thesis can be rendered consistent with the invariance requirement. Lewis’s conclusion holds only under certain conditions: the desire-as-belief thesis conflicts with the invariance requirement if and only if there are certain correlations between non-evaluative and evaluative propositions. But when there are such correlations, we suggest, the invariance requirement loses its plausibility. Thus Lewis’s argument against the desire-as-belief thesis appears to be valid only in cases in which it is unsound. (shrink)

It is often thought that presentism is incompatible with time travel. I will argue that this common view is incorrect. Specifically, I will argue that presentism is compatible with some stories that involve closed timelike curves, and that some of these stories are time-travel stories.

This paper explores some aspects of the relation between different ways of achieving a consensus on the judgemental values of a group of indviduals; in particular, aggregation and deliberation. We argue firstly that the framing of an aggregation problem itself generates information that individuals are rationally obliged to take into account. And secondly that outputs of the deliberative process that this initiates is in tension with constraints on consensual values typically imposed by aggregation theory, at least when deliberation is modelled (...) as process of learning from others compatible with Bayesian updating principles. In this article I argue that understanding the role that the virtues of love play in Kant’s ethical theory requires understanding not only the nature of the virtues themselves, but also the unique nature of wide Kantian duties. I begin by making the case that while the Doctrine of Virtue supports attributing an affective component to the virtues of love, we are right to resist attributing anaffective success conditionto these virtues. I then distinguish wide duties from negative and narrow (positive) (...) duties in order to make the case that prudential considerations often unavoidably and unproblematically play a role in deliberation about how we fulfil our wide duties. In the final section I combine these findings, arguing that the virtues of love play an important moral role by shaping these prudential considerations. (shrink)

Fiona Nicoll and Melissa Gregg met on the job at a new university having both moved from Sydney to Brisbane to take up their appointments. Here they share reflections on teaching a cultural theory course that they inherited from a prominent Australian Professor of Cultural Studies, offering the perspectives of two consecutive generations of cultural studies theorists now teaching in the field since the early 1990s. This situation gives rise to new interpretations regarding the value and uses of theory (...) in the classroom. Noting the subtle differences involved in teaching the same theoretical material in different cities, the ironies of teaching radical cultural theory in a conservative institutional environment, and the specific opportunities and challenges of teaching cultural studies theory as opposed to others, the article considers some of the silences teachers must also contend with in their classroom practice, drawing on and expanding the terrain established by Thorkelson's thesis. In 1996, L. S Cousins published a groundbreaking piece on paths of monastic practice titled ‘Scholar Monks and Meditator Monks Revisited’. As the title suggests, this work reconsiders the role of two types of monks, doing so by closely analyzing a famous sutta that depicts a strong dispute between jh?yins or ‘meditators’ and dhammayogas, whom scholarship has almost universally defined as ‘scholars’. Because of this, almost all have interpreted this debate as the first sign in early Indian Buddhism of a (...) great bifurcation in the sa?gha between those concentrating on book learning and those concentrating on practice — a split that became more and more marked over the centuries until the division became more or less official in medieval Sri Lanka. Cousins convincingly contests this history, with one of his main points being that the dhammayogas were not at all just scholars. Like the meditators, theirs was a practical path that resulted in profound realization of the Dhamma, albeit a different path from that of the meditators. Cousins then goes even further, arguing that the split between scholars and meditators is not very evident in South Asian Buddhist history until the time of Buddhaghosa and thereafter. My intention here is to respond as fully as possible to Cousins’ methods and conclusions, by offering evidence and arguments that sometimes support his work further and sometimes critique his work. This is done in the spirit of spurring on more discussions on this important, complex, and contested issue. (shrink)

It is an appealing idea that deontic modality is a modality of the open future, and that the indeterminacy of the open future is the key, within natural language, to understanding the deontic modal puzzles that form the traditional subject-matter of deontic logic. In this paper, I pull together three well-studied strands of indeterminism—Thomason (1980)’s settledness operator, the modal base of Kratzer (1981, 1991)’s analysis of modals, and Stalnaker (1978)’s notion of diagonal acceptance—to argue for two theses governing a deontic (...) logic for natural language. -/- The first thesis makes a claim about postsemantic truth, and is couched in terms of Stalnaker’s dagger operator. The second concerns what it is for an act to be permissible, when agents exercise partial control over their obligations as well as their bodily movements. (shrink)

En 2017, des spécialistes de différentes disciplines (santé, éducation et travail social) issus de cinq pays européens (France, Géorgie, Italie, Norvège et Suisse) ont créé un réseau afin de poursuivre leurs recherches sur le handicap et la migration. Le présent article expose les principaux résultats d’un premier workshop qu’ils ont réalisé et les pistes qui en découlent pour de futures recherches. D’une part, les enfants migrants en situation de handicap restent invisibles sur le plan statistique dans plusieurs pays. Leurs besoins (...) en tant que migrants et personnes en situation de handicap sont pris en compte par des politiques et des dispositifs distincts. D’autre part, dans les cinq pays, un écart important entre les normes légales et la situation des familles migrantes ayant un enfant en situation de handicap est constaté. Il en est de même en ce qui concerne la collaboration entre les différents services publics, ou entre ces services et les associations soutenant les personnes en situation de handicap, les personnes migrantes et/ou les minorités. Les futures recherches seront centrées sur les moyens d’augmenter la participation sociale des enfants migrants en situation de handicap et de leurs familles dans des contextes d’interventions interculturelles et interdisciplinaires. Recalling past transformations, this book examines what makes the current challenge different, and especially urgent. It analyses how green transformations must take place in the context of the particular moments of capitalist development, and in relation to particular alliances. The book emphasises the role of the state and the role of citizens, as innovators, entrepreneurs, green consumers and members of social movements. Green transformations must be both 'top-down', involving elite alliances between states and business, but also 'bottom up', pushed by (...) grassroots innovators and entrepreneurs, and part of wider mobilisations among civil society. Mindfulness based interventions (MBIs) are rapidly emerging in health care settings for their role in reducing stress and improving physical and mental health. In such settings, the religious roots and affiliations of MBIs are downplayed, and the possibilities for developing spiritual, even mystical, states of consciousness are minimized. This article helps rebalance this trend by using the tools of medical humanities and narrative medicine to explore MBI as a bridge between medical and spiritual approaches to health related suffering. My narrative (...) medicine method draws insights from the arts and humanities that are rarely used in standard clinical research but are increasingly common among medical humanities and narrative medicine scholars. The specific path I take will be to work through historical, linguistic, and philosophic dimensions of mindfulness and mysticism as relevant to illness, suffering, death, and dying. I close with two case examples in which mindfulness is used as an intentionally spiritual practice for health concerns. (shrink)

This chapter explores the term “landscape” and its utility for indigenous people. If indigenous people do not have an understanding of the term, the question is posed whether “landscape” is merely a form of “restricted” speech that is meant to signify power and authority over them and the land they call home. In Australia, certain literary works describe the rich relationship indigenous people have with their land, providing a foundation for the study of “cultural landscapes.” These works share the common (...) trait of presenting a way in which landscape becomes evidence of the cosmological processes that define kinship, group alignments, and cultural practices. There is a growing movement towards construing some classic debates in ontology as meaningless, either because the answers seem obvious or the debates seem intractable. In this paper, I respond to this movement. The response has three components: First, the members of the two sides of the ontological debates that dismissivists have targeted are using different quantifiers. Second, the austere ontologist is using a more fundamental quantifier than her opponent. Third, the austere ontologist’s more fundamental quantifier is a restriction of (...) her opponent’s quantifier. This response takes seriously the intuition that there is something wrong with the ontological disputes, but does not entail dismissivism. (shrink)

In philosophy, much attention has rightly been paid to the need to diversify teaching with regard to who teaches, who is taught, and which authors and questions are the focus of study. Less attention, however, has been paid to inclusive pedagogy—the teaching methods that are used, and how they can make or fail to make classes as accessible as possible to the diverse students who enter them. By drawing on experiences from our own teaching as well as research on student-centered, (...) inclusive best practices, we advocate for five principles of inclusive pedagogy: fostering a growth mindset, examining inclusive conceptions of authority, promoting transparency, encouraging flexibility, and, finally, continually promoting self-reflection for both students and teachers. Dmitri Nikulin is one of the few contemporary philosophers to have devoted books to the topic of dialogue and the dialogical self, especially in the last fifteen years. Yet his work on dialogue and the dialogical has received scant attention by philosophers, and this neglect has hurt the ongoing development of contemporary philosophical work on dialogicality. I want to address this lacuna in contemporary philosophical scholarship on dialogicality and suggest that, although Nikulin’s account is no doubt insightful and thought-provoking, it (...) is problematic for two main reasons: first, his account fails to recognize the proper relationship between dialogue and agency; and second, his enumeration of the necessary and sufficient conditions for dialogue contains conceptual inconsistencies. The Second Vatican Council called for moral theology to be more firmly rooted in scripture, yet bioethical reflection on alcohol use and addiction often fails to provide a rigorous scriptural analysis of these problems. Themes related to alcohol use, abuse, and addiction recur throughout scripture. This article begins by examining these themes in the Old and New Testaments. Some synthetic comments related to alcohol use and eschatological themes are then offered. The article concludes that the Bible does not condemn moderate (...) alcohol use, but it frequently condemns drunkenness as a source of harm, part of the dominion of sin, and unsuitable for God’s people. Practical applications related to catechesis, priestly formation, and recovery are offered. (shrink)
