Susan Schneider argues that physicalism must be false if abstracta are part of the physicalist's dependence base. In opposition to her view, here I set out some reasons to think that abstracta in general, including abstracta that are woven into the dependence base, are something physicalists can countenance with consistency.

Objectives: To assess the knowledge and behaviour of researchers regarding criteria for authorship, and the practices of ghost and gift authorship. Design: Semidirective interviews of senior clinical researchers. Setting: University hospital. Participants: Thirty-nine main investigators of clinical research programmes. Main measurements: Awareness and use of International Committee of Medical Journal Editors (ICMJE) criteria for authorship, and perceptions about ghost and gift authorship. Results: A total of 48 protocols submitted by 42 principal investigators between 1994 and 1996 were identified. Thirty-nine investigators (...) were contacted; 37 (one of whom delegated a co-author) were interviewed between May 2002 and March 2003. Two co-authors of two principal investigators were also interviewed. In all, 42 studies were represented. The interviews lasted for 40–90 minutes and were conducted with openness and respect for confidentiality. The choice of names of co-authors did not follow the ICMJE recommendations. Half of the respondents stated they were aware of criteria for authorship and knew of ICMJE, but most of them did not cite any of the ICMJE criteria among those they applied in deciding authorship. Most of them disagreed with the obligation to meet the three criteria justifying co-authorship because they found these too rigid and inapplicable. Gift authorship was a common practice; 59% of the respondents had been a recipient of gift authorship. Twenty-five (64%) were aware of ghost authorship and the majority considered it questionable and blameworthy. Conclusions: The ICMJE criteria were ignored by clinicians at a university hospital. Ghost and gift authorship were frequent among them. There is a need for French guidelines for authorship to be prepared and implemented. (shrink)

Psychoactive drugs are being prescribed to millions of Americans at an increasing rate. In many cases these drugs are necessary in order to overcome debilitating emotional problems. Yet in other instances, these drugs are used to supplant, not supplement, interpersonal therapy. The process of overcoming emotional obstacles by introspection and the attainment of self knowledge is gradually being eroded via the gratuitous use of psychoactive medication in order to rapidly attain a release from the common problems that life inevitably presents (...) us with. In this paper, I argue that Kant’s formula of humanity, which maintains that persons ought never to treat others or themselves soley as a means to an end, proscribes this. Moreover, Kant argues that we have an imperfect duty of self development, and I argue that we fail to adhere to such a duty whenever we seek to evade the process of introspection and self knowledge in favour of the expedient results that drugs may provide us with as we attempt to overcome the emotional hurdles in our lives. Objective: To explore the ways in which health care practitioners working within the morally contested area of prenatal screening balance their professional and private moral values.Design: Qualitative study incorporating semistructured interviews with health practitioners followed by multidisciplinary discussion groups led by a health care ethicist.Setting: Inner city teaching hospital and district general hospital situated in South East England.Participants: Seventy practitioners whose work relates directly or indirectly to perinatal care.Results: Practitioners managed the interface between their professional and private moral values in (...) a variety of ways. Two key categories emerged: “tolerators”, and “facilitators”. The majority of practitioners fell into the “facilitator” category. Many “facilitators” felt comfortable with the prevailing ethos within their unit, and appeared unlikely to feel challenged unless the ethos was radically challenged. For others, the separation of personal and professional moral values was a daily struggle. In the “tolerator” group, some practitioners sought to influence the service offered directly, whereas others placed limits on how they themselves would contribute to practices they considered immoral.Conclusions: The “official” commitment to non-directiveness does not encourage open debate between professionals working in morally contested fields. It is important that practical means can be found to support practitioners and encourage debate. Otherwise, it is argued, these fields may come to be staffed by people with homogeneous moral views. This lack of diversity could lead to a lack of critical analysis and debate among staff about the ethos and standards of care within their unit. (shrink)

Objectives: To compare attitudes of medical and law students toward informing a cancer patient about diagnosis and prognosis and to examine whether differences are related to different convictions about benefit or harm of information.Setting and design: Anonymous questionnaires were distributed to convenience samples of students at the University of Geneva containing four vignettes describing a cancer patient who wishes, or alternatively, who does not wish to be told the truth.Participants: One hundred and twenty seven medical students and 168 law students.Main (...) outcome measures: Five point Likert scale of responses to the vignettes ranging from “certainly inform” to “certainly not inform” the patient.Results: All medical students and 96% of law students favoured information about the diagnosis of cancer if the patient requests it. Seventy four per cent of medical students and 82% of law students favoured informing a cancer patient about his or her prognosis . Thirty five per cent of law students and 11.7% of medical students favoured telling about the diagnosis and 25.6% of law students and 7% of medical students favoured telling about the prognosis even if the patient had clearly expressed his wish not to be informed. Law students indicated significantly more often than medical students reasons to do with the patient’s good, legal obligations, and the physician’s obligation to tell the truth, and significantly less often than medical students that their attitude had been determined predominantly by respect for the autonomous choice of the patient.Conclusion: Differences in attitudes according to the type of case and the type of studies were related to convictions about the benefit or harm to the patient caused by being given information. The self reported reasons of future physicians and future lawyers are helpful when considering means to achieve a better acceptance of patients’ right to know and not to know. (shrink)

Screening of newborns for permanent congenital or early-onset hearing impairment has emerged as an essential component of neonatal care in developed countries, following favourable outcomes from early intervention in the critical period for optimal speech and language development. Progress towards a similar programme in developing countries, where most of the world’s children with hearing impairment reside, may be impeded by reservations about the available level of support services and the possible effect of the prevailing healthcare challenges. Ethical justification for the (...) systematic introduction of screening programmes for hearing in newborns based on the limitations in current primary prevention strategies, lack of credible alternative early-detection strategies and the incentives for capacity-building for the requisite support services is examined. The argument rests on earlier work questioning the Russellian separation of levels and arguing that Russellian levels should be taken to include the levels of particle and aggregate, and generality and detail. That earlier work argues from the non-separation of particle and aggregate that predictability is limited and that physics cannot come to an end. This leads to a view of the world as flux. Identifiable objects demanding explanation can only be temporary entities emerging from flux and explanation can only (...) be local and historical. This precludes explanation of totality and leads us to reject Leibniz’s question. Baldwin’s argument from possible worlds theory that a null world is possible is examined and questioned. Koon’s combination of the kalam argument with the grim reaper paradox is not queried but a way is found of circumventing it. It is noted in passing that the argument does not have the anti-theistic implications which may appear. (shrink)

Purpose: The purpose of the paper is to provide a constructivist account of the "self as subject" that avoids the need for any metaphysical assumptions. Findings: The thesis developed in this paper is that the human "psychological individual," "self" or "subject" is an emergent within the nexus of human social interaction. With respect to psychological and social wholes (composites) there is no distinction between the form of the elements and the form of the composites they constitute i.e., all elements have (...) the form of composites. Further, recursively, composites may serve as elements within higher order composites. Implications for a rational theory of ethics are discussed. Original Value: The thesis contributes in a fundamental way to the research programme of radical constructivism by demonstrating that metaphysical assumptions about the nature of the "subject" are not an a priori necessity. Although the thesis in itself is not original, the paper offers a useful synthesis of ideas from a number of key thinkers in the disciplines of cybernetics, biology, psychology and philosophy. On December 7, 1994, the U.S. District Court of the Northern District of Illinois ruled that ERISA preempts a participant in an ERISA plan from suing the plan's administrator under a state common law theory of respondeat superior ) : at 208). On September 12, 1995, the Seventh Circuit of the U.S. Court of Appeals reversed this decision and ordered that the case be tried in state court ). The court held that the case had been improperly removed to federal (...) court. The court of appeals stated that the federal court did not have jurisdiction because the plaintiff's claim did not fall within ERISA's provisions.In this case, plaintiff David Rice brought a medical malpractice suit against two doctors who provided treatment to him in accordance with his ERISA insurance plan. This short and highly compact book derives from Paul Ricoeur’s lectures at Texas Christian University in fall 1973. After the expansiveness of earlier works on evil, Freud, and metaphor, this book moves to consolidate and sum up this previous work, and to chart a certain shift in Ricoeur’s standpoint that has occurred within it.

In this concise yet powerful book, one of the twentieth century's most respected political philosophers presents a controversial reassessment of the political ideas and intellectual legacy of Edmund Burke. A practicing politician and powerful writer, full of ideas, Burke was intent on getting those ideas translated into government policies. But he was too much the impatient practitioner to set out his principles in a single book in the manner of Locke or Hume, leaving both admirers and opponents ample scope to (...) reinterpret his work in different ways. Macpherson, however, finds Burke's views on political economy to be the one consistent factor in his thinking. Today Burke is often viewed as one of modern conservatism's founding lights, and in an era of global capitalism unfettered by national borders, Macpherson's reassessment of Burke's ideas is perhaps more timely than ever. The theory that AIDS originated from contaminated polio vaccines raises a number of challenging issues with ethical dimensions. The Journal of Medical Ethics dealt with a submission about the theory a decade ago; subsequent developments have raised further issues. Four areas of contention are addressed: whether the theory should be investigated; whether anyone should be blamed; whether defamation actions are appropriate, and whether the scientific community has a responsibility to examine unorthodox theories.

There is among many moral philosophers today a renewed emphasis on the connection between reason and morality, and an attempt to exhibit moral behaviour as characteristically rational. What is original in Mr. Kneale's extremely interesting paper is the following-up of a suggestion that certain words like “right,” “wrong,” “ought,” are used in the same way both by lawyers and by moralists; this leads to a logical rehabilitation of the somewhat unpopular concept of the moral law, which in turn argues objectivity (...) and rationality in morals, and makes it possible “to distinguish between good and bad moralists in much the same way as we can distinguish between good and bad lawyers.” The legal pattern is essentially the same as the moral pattern. The label `semiotic grammar' captures a fundamental property of the grammars of human languages: not only is language a semiotic system in the familiar Saussurean sense, but its organizing system, its grammar, is also a semiotic system. This proposition, explicated in detail by William McGregor in this book, constitutes a new theory of grammar. Semiotic Grammar is `functional' rather than `formal' in its intellectual origins, approaches, and methods. It demonstrates, however, that neither a purely functional nor a purely formal account (...) of language is adequate, given the centrality of the sign as the fundamental unit of grammatical analysis. The author distinguishes four types of grammatical signs: experiential, logical, interpersonal, and textural. The signifiers of these signs are syntagmatic relationships of the following types, respectively: constituency, dependency, conjugational and linking. McGregor illustrates and exemplifies the theory with data from a variety of languages including English, Acehnese, Polish, Finnish, Japanese, Chinese, and Mohawk; and from his pioneering research on Gooniyandi and Nyulnyul, two languages of the Kimberleys region of Western Australia. According to tranditional philosophical terminology and to most interpretations of Cartesianism, Descartes is a dualist. This dualism is expressed in his fundamental distinction between two substances—mind and matter—and, though admitted to be full of difficulties and by many to be untenable, it has very generally been regarded as at least a clearly intelligible doctrine, consistently held by Descartes. That this is not so has been shown by Professor Boyce Gibson in his able and careful analysis of Cartesianism. The aim of (...) the present essay is to draw attention to another difficulty that has not been sufficiently noticed; whether it is an actual one or only an apparent one due to a misunderstanding of Descartes is a question that may be left to those who read him differently and who may be able to remove one more charge of inconsistency. The general contention is that Descartes—and this point is conceded in expositions of Descartes—seeks to prove that there are material bodies, that material bodies are necessary for his own account of the physical universe, and that yet his own theory of matter as extension, even combined with motion, makes the existence of bodies impossible and hence any proof of such existence impossible. In fact, the employment of the idea of motion only serves to emphasize the difficulty and inconsistency of the theory. (shrink)
