Scant consideration has been given to the ethical implications of the policy of closing down psychiatric hospitals in favour of community care. The recent adherents of this policy in government have been enthusiastic in encouraging its implementation. This paper has three sections: a brief resumé of the history and principles of community care for the mentally ill; a discussion on the merits and de-merits of psychiatric care in the hospital and in the community; and an outline of some preliminary categories (...) for ethical analysis. Background: Despite a gradual shift in the focus of medical care among terminally ill patients to a palliative model, studies suggest that many children with life-limiting chronic illnesses continue to die in hospital after prolonged periods of inpatient admission and mechanical ventilation.Objectives: To examine the characteristics and location of death among hospitalised children, investigate yearwise trends in these characteristics and test the hypothesis that professional ethical guidance from the UK Royal College of Paediatrics and Child Health would lead to significant (...) changes in the characteristics of death among hospitalised children.Methods: Routine administrative data from one large tertiary-level UK children’s hospital was examined over a 7-year period for children aged 0–18 years. Demographic details, location of deaths, source of admission , length of stay and final diagnoses were studied. Statistical significance was tested by the Kruskal–Wallis analysis of ranks and median test , χ2 test and Cochran–Armitage test .Results: Of the 1127 deaths occurring in hospital over the 7-year period, the majority were among infants. The main diagnoses at death included congenital malformations , perinatal diseases , cardiovascular disorders and neoplasms . Most deaths occurred in an intensive care unit environment , with a significant increase over the years . There was a clear increase in the proportion of admissions from in-hospital among the ICU cohort . Infants with congenital malformations and perinatal conditions were more likely to die in an ICU , and older children with malignancy outside the ICU . Children stayed for a median of 13 days on a hospital ward before being admitted to an ICU where they died.Conclusions: A greater proportion of hospitalised children are dying in an ICU environment. Our experience indicates that professional ethical guidance by itself may be inadequate in reversing the trends observed in this study. (shrink)

In lieu of an abstract, here is a brief excerpt of the content:Journal of the History of Ideas 61.4 (2000) 617-634 [Access article in PDF] Edmund Burke and Reason of State David Armitage Edmund Burke has been one of the few political thinkers to be treated seriously by international theorists. 1 According to Martin Wight, one of the founders of the so-called "English School" of international theory, Burke was "[t]he only political philosopher who has turned wholly from political theory (...) to international theory." 2 The resurgence of interest in Burke as an international theorist has not, however, generated any consensus about how he might be classified within the traditions of international theory. Wight variously divided thinkers into trichotomous schools of Realists, Rationalists, and Revolutionaries, Machiavellians, Grotians, and Kantians, or theorists of international anarchy, habitual intercourse, or moral solidarity; 3 more recent international theorists have refined or supplemented these categories to construct similar trinitarian traditions of Realism, Liberalism, and Socialism, and of Empirical Realism, Universal Moral Order, and Historical Reason. 4 Burke's place within any of these traditions remains uncertain. Debate over whether he was a realist or an idealist, a [End Page 617] Rationalist or a Revolutionist, has concluded variously that he was a "conservative crusader" or an "historical empiricist," a belated dualist or a Cold Warrior before the fact, or, most egregiously, "a proto-Marxist, or more precisely proto-Gramscian" theorist of hegemony. 5 The fact that Burke so obviously eludes definition may put in doubt the analytical utility of closely-defined "traditions" of international theory. 6Burke's relationship to conceptions of reason of state provides a more precise example of the confusion within such taxonomies. According to one recent historian of international theory, Burke "laid the foundations" of the "conservative approach to International Relations informed by the two modern notions of state interest and necessity, by raison d'état"; however, in the words of another, "Burke... was vehemently opposed to the idea of Reason of State and did not subscribe to the view that national interests override moral laws." 7 The assumptions on which each of these judgments rests are clearly incompatible: on the one hand that a "conservative approach" in the realm of foreign affairs implies an espousal of reason of state defined as the primacy of "state interest and necessity," that Burke did indeed acknowledge; on the other hand that reason of state is defined more exactly as "the view that national interests override moral laws" and that Burke did not hold such a view, so could not be defined as a reason-of-state theorist. It might of course be possible that Burke held various views on such matters at various points in his long literary and political career or that he argued for differing conceptions of reason of state in differing contexts. To test such a hypothesis demands a historical account of Burke's relationship to the theories of reason of state held by his contemporaries and predecessors. After all, he famously scorned "dashing Machiavellian politicians," deplored "the odious maxims of a Machiavellian policy," condemned "the dreadful maxim of Machiavel that in great affairs men are not to be wicked by halves," and identified the Discorsi as the inflammatory textbook of French republicanism. 8 His strictures on Machiavelli and Machiavellianism affirmed avant la lettre the classic modern account of reason of state offered by Friedrich Meinecke, which counterposed "raison d'état on the one hand, and ethics and law on the other" and traced the emergence of this separation to the heathen Florentine who had given the tradition its familiar nickname. 9 Such accounts of reason of state and of Machiavelli reinforced the long-standing interpretation of Burke as the last of the medieval theorists of natural law, for whom no merely human calculations of advantage or interest could override the dictates of divine reason. The question of what exactly is irrational about recalcitrant emotions—those that occur in tension or conflict with our beliefs—has been widely debated. Sabine Döring claims that such irrationality only emerges if we act on our recalcitrant emotion or engage in emotion-relevant reasoning in light of it. I here provide an account that acts as an extension to the latter part of this claim by considering in more depth the question of what is irrational about that reasoning, i.e., emotionally recalcitrant reasoning. (...) In doing so, I offer a somewhat alternative explanation to Döring's as to the form of irrationality involved. Where Döring argues that emotionally recalcitrant reasoning involves epistemic irrationality by way of conflicting judgments that are involved, I argue that our engagement in this reasoning is, in fact, practically irrational. This is because, I claim, it involves us “accepting”—treating as true—propositions that we do not believe to be true, in the absence of the kind of reasons that would usually rationalize doing so. Doing so constitutes a form of practical, rather than epistemic, irrationality. This is a conclusion I arrive at through a consideration of the nature of the reasoning our emotions in general tend to motivate. (shrink)

This paper explores the philosophical perspectives of Nietzsche and Heidegger, tracing their analyses of the death of God and its aftermath. My aim is to clarify the diagnosis of this nihilism and its underlying causes, as well as evaluate the proposed remedies put forth by Nietzsche and Heidegger. Ultimately, I argue that the seemingly ambiguous consequences of the death of God are not only hopeful, but necessary, if human beings are to rise above and transmute a meaningless, resentment-laced existence, however, (...) not by jettisoning Judeo-Christianity and its values, but rather by reinterpreting them. Jonathan Wolff and Timothy Hinton have criticized a version of liberal egalitarianism, often associated with Ronald Dworkin, for promoting an account of social justice that fails to treat everyone with respect. This paper analyses Wolff’s and Hinton’s critiques, particularly with regard to how notions of self-respect and respect-standing are deployed. The paper argues that the analyses of both Wolff and Hinton display affinities with a dualist approach to social justice. A dualist approach theorizes respect as an aspect of both distributive, (...) socioeconomic injustice and cultural injustice, rather than of the former only, which is typical of liberal egalitarianism. Nancy Fraser is widely associated with such a dualist framework, so her version is used to assess Wolff’s and Hinton’s work. The paper argues that both make use of ideals and commitments from the dualist approach to justice in their respect objection. However, despite their evident sympathy for the notion of cultural injustice, both continue to theorize respect primarily as an aspect of distributive justice. Thus, for cultural justice theorists, Wolff’s and Hinton’s critiques of Dworkinian justice may leave something to be desired. (shrink)

And how might it be rendered methodologically robust as well as historically compelling? This article proposes a model of transtemporal history, proceeding via serial contextualism to create a history in ideas spanning centuries, even millennia: key examples come (...) from work in progress on ideas of civil war from ancient Rome to the present. The article concludes with brief reflections on the potential impact of the digital humanities on the practice of long-range intellectual history. We all find ourselves worrying at one point or another, and we have an intuitive sense of what is communicated by phrases such as ‘I’m worried about this’ or ‘I can’t stop worrying about that’. Despite worry’s ubiquity, however, it is not altogether clear what exactly worrying is, or why it is we worry. And, surprisingly, there has been no dedicated philosophical account given of the nature of worry specifically, although there is a body of psychological literature concerned with it (...) as well as a recent insurgence of philosophical literature concerned with the nature of anxiety. My aim in this paper, therefore, is to provide such an account. I here provide an account of the nature of worry.On the view I develop, worry is to be understood as a form of affectively motivated cognition. More specifically, I argue that worrying is a cognitive activity constituted by our engagement with forms of practical or epistemic reasoning, supplemented by imaginative engagement and motivated by anxiety. I develop this view primarily by marrying together considerations from the psychological literature on worry and the philosophical literature on anxiety. With this characterization in place, I then go on to make some novel claims about why exactly it is that we worry. The upshot should be an account of worry that addresses the questions of what it is and why we do it. (shrink)

This essay examines the peritrope argument within the history of philosophy and discusses its various permutations, beginning with Plato and eventually mathematized with Gödel, each of which presents a philosophical system that either stands or collapses with this “peritropic” insight. I argue that the peritrope or self-reference argument itself presupposes a certain anti-reductionism, in terms of both anthropology and metaphysics, and is ultimately grounded in Aristotle’s anthropological insight that the human being is the “rational animal”. Thus the root of the (...) anti-reductionist, peritropic argument belongs to the self-transcendent nature of rationality itself. After discussing Aristotle’s anthropology in terms of this rational transcendence, I trace the history of the self-reference argument from Plato to Gödel and discuss its various implications as applied to any and every form of reductionism. My hope is that engagement with this most basic and often overlooked philosophic insight can counter certain reductionist trends in modernity. (shrink)

In this work of philosophy, theology, and intellectual history, Duane Armitage offers a clear interpretation of Heidegger’s enigmatic theology as uniquely Pauline and Lutheran. He argues that the real impetus, aim, and structure of Heidegger’s philosophy of religion as well as his philosophy as a whole, are rooted in Pauline (and Lutheran) ontology. He thus demonstrates that continental philosophy of religion, and, to an extent, continental philosophy as a whole, is indebted to St. Paul and Martin Luther. This examination (...) also shows how much of continental thinking itself is traceable to Heidegger’s onto-theological critique and hence to Luther and St. Paul. In terminally ill individuals who would otherwise end their own lives, active voluntary euthanasia (AVE) can be seen as life-extending rather than life-shortening. Accordingly, AVE supports key pro-euthanasia arguments (appeals to autonomy and beneficence) and meets certain sanctity of life objections. This paper examines the extent to which a terminally ill individual’s wish to donate organs after death contributes to those life-extension arguments. It finds that, in a terminally ill individual who wishes to avoid experiencing life he considers to be (...) not worth living, and who also wishes to donate organs after death, AVE maximizes the likelihood that such donations will occur. The paper finds that the wish to donate organs strengthens the appeals to autonomy and beneficence, and fortifies the meeting of certain sanctity of life objections, achieved by life-extension arguments, and also generates appeals to justice that form novel life-extension arguments in favour of AVE in this context. Jeremy Bentham's career as a writer spanned almost seventy years, from the Seven Years' War to the early 1830s, a period contemporaries called an age of revolutions and more recent historians have seen as a world crisis. This article traces Bentham's developing universalism in the context of international conflict across his lifetime and in relation to his attempts to create a 'Universal Jurisprudence'. That ambition went unachieved and his successors turned his conception of international law in a more particularist direction. (...) Going back behind Bentham's legacies to his own writings, both published and unpublished, reveals a thinker responsive to specific events but also committed to a universalist vision that helped to make him a precociously global figure in the history of political thought. Doctors hold coexisting ethical duties to avoid causing deliberate harm to their patients (non-maleficence), to act in patients’ best interests (beneficence), to respect patients’ right to self-determination (autonomy) and to ensure that costs and benefits are fairly distributed among patients (justice). In the context of non-directed altruistic kidney donations (NDAKD), doctors’ duties of autonomy and justice are in tension with those of non-maleficence and beneficence. This article examines these competing duties across three scenarios in which general practitioners (GPs) could promote (...) NDAKD to healthy adults. In the first—when a healthy adult patient prompts the GP to discuss NDAKD—the GP is ethically obligated to counsel the patient about NDAKD to respect their autonomy, yet this does not constitute any form of promotion of NDAKD. In the remaining scenarios, healthy adult patients are unaware of the possibility of NDAKD. In the second, it is ethically permissible for GPs to indirectly raise awareness of NDAKD among healthy adults by displaying recruitment campaign material to non-specified groups of patients in their waiting rooms. In the third, it is ethically impermissible for GPs to directly promote NDAKD to individual healthy adults by raising the possibility of NDAKD with such individuals. The major counterarguments raised against this position are problems with kinds of counselling that fail to reach expected professional standards, rather than problems with the ethical claims made in this article. A distinguished panel of speakers from various disciplines -- psychology, philosophy, neuroscience and artificial intelligence -- were challenged to respond to Dr. Miller's query: "What has happened to cognition? In other words, what has the past 30 years contributed to our understanding of the mind? Do we really know anything that wasn't already clear to William James?" Recent scholarship on John Locke’s Two Treatises of Government has drawn particular attention to the colonial antecedents and applications of the theory of appropriation in chapter V of theSecond Treatise. This attention has coincided with a more general interest among political theorists in the historical and theoretical relationship between liberalism and colonialism. This essay reviews the surviving evidence for Locke’s knowledge of the Carolina colony and argues that it was both more extensive and more enduring than previous commentators have suggested. (...) In particular, the essay provides evidence that Locke was engaged in revising the Fundamental Constitutions of Carolina at just the moment in the summer of 1682 when he was most likely to have composed chapter V of theSecond Treatise and hence that there was an immediate and identifiable colonial context that contributed to his distinctive theory of property. However, the focus of the interview is on the core concepts of ‘discourse networks’, ‘the military-industrial complex’, and ‘technology’, arguably the three central themes of Kittler’s work to date. Extensive training with a musical instrument results in the automatisation of the bodily operations needed to manipulate the instrument: the performer no longer has to consciously think about the instrument while playing. The ability of the performer to automate operations on the instrument is due to sensorimotor mechanisms that can predict changes in the state of the body and the instrument in response to motor commands. But how strong are these mechanisms? To what extent can we alter the structure of (...) the instrument before they disappear? We performed an exploratory study to understand whether and how sensorimotor predictions survive instrument modification. We asked seven professional violinists to perform repertoire pieces and sight-reading exercises on four different violins: their own, a cheap violin, a small violin, and a violin whose strings had been put on in reverse order. We performed a series of quantitative investigations on performance intonation and duration, and on bowing gestures and errors. The analysis revealed that participants struggled adapting to the altered instruments, suggesting that prediction mechanisms are a function of instrument configuration. In particular, the analysis of bowing errors, intonation, and of performance duration suggested that the performance with the reverse violin was much less fluent and precise than the performer's own instrument; the performance with the small violin was also sub-standard though to a lesser extent. We also observed that violinists were differently affected by instrument modifications, suggesting that the capability to adapt to a new instrument is highly personal. (shrink)
