Genetic testing can be used to determine if unexplained fractures in children could have resulted from a predisposition to bone fractures, e.g., osteogenesis imperfecta. However, uncertainty is introduced if a variant of unknown significance is identified. Proper interpretation of VUS in these situations is critical because of its influence on clinical care and in court rulings. This study sought to understand how VUS are interpreted and used by practitioners when there is a differential diagnosis including both osteogenesis imperfecta and non-accidental (...) injury.A 15-question survey was emailed to physicians who requested analysis of two genes, COL1A1 and COL1A2, from the University of Washington from 2005-2013 for patient cases involving suspicion of child abuse.Among the 89 participants, responses differed about when genetic testing should be ordered for osteogenesis imperfecta, who should be consulted about utilization of VUS test results, follow-up procedures, and who should receive the VUS results.There are no clear guidelines for how to interpret and follow up on VUS. In the legal setting, misinterpreted VUS could lead to unintended consequences and deleterious ramifications for family members. The need for better practice guidelines to help promote more equitable handling of these sensitive legal cases is clear. (shrink)

In a series of recent papers, I have made three arguments about how to define “disease” and evaluate and apply possible definitions. First, I have argued that definitions should not be seen as traditional conceptual analyses, but instead as proposals about how to define and use the term “disease” in the future. Second, I have pointed out and attempted to address a challenge for dysfunction-requiring accounts of disease that I call the “line-drawing” problem: distinguishing between low-normal functioning and dysfunctioning. Finally, (...) I have used a dysfunction-requiring approach to argue that some extremely prevalent conditions, such as high blood pressure, high cholesterol, and ductal carcinoma in situ, are not diseases, but instead are risk factors. Four of the papers in this issue directly engage my previous work. In this commentary, I applaud the advances these authors make, address points of disagreement, and make suggestions about where the discussion should go next. (shrink)

Philosophical interest in the biological concept of function stems largely from concerns about its teleological associations. Assigning something a function seems akin to assigning it a purpose, and discussion of the purpose of items has long been off-limits to science. Analytic philosophers have attempted to defend ‘function’ by showing that claims about functions do not involve any reference to a problematic notion of purpose. To do this, philosophers offer short lists of necessary and sufficient conditions for the application of the (...) concept—where the conditions involve only acceptable physical or biological notions—and claim that the set of conditions captures the import of function statements. Nicholas Griffin has brought his customary expertise to the project, and in (...) snaring fourteen other contributors to this volume, has produced a welcome addition to an academic's library of Russell materials.The shape of the volume, its contents and its presentation, however, unfortunately reflect the lopsided nature of current Russell studies. One hopes for a companion volume that has the breadth of the earlier volume edited fifty years ago by Paul Schilpp, but in vain. This book is overwhelmingly about Russell before 1918, almost entirely about philosophy of mathematics, epistemology and logic, and several of the pieces easily could have been included in a festschrift for Frege instead.What is included is well done, but to call it "the most convenient and accessible guide to Russell available" is to speak in comparative terms. To be fair, it sets out to focus on a narrow definition of Russell's contributions to modern philosophy, and the contributors accomplish this task. Ian Grattan-Guinness writes on "Mathematics in and behind Russell's Logicism, and its Reception," a subject that continues into "Bertrand Russell's Logicism" by Martin Godwyn and Andrew D. Irvine. The roots of analytic philosophy are of primary concern in this volume, so Richard L. Cartwright explores "Russell and Moore, 1898-1905" and Michael Beaney relates "Russell and Frege." "The Theory of Descriptions" by Peter Hylton sets up two pieces on "Russell's Substitutional Theory" (Gregory Landini) and "The Theory of Types" (Alasdair Urquhart), while Paul Hager's "Russell's Method of Analysis" leads into "Russell's Neutral Monism" (R.E.Tully) and "The Metaphysics of Logical Atomism" (Bernard Linsky).The next portion of the book expands the intellectual focus somewhat, with "Russell's Structuralism and the Absolute Description of the World" by William Demopoulos—perhaps [End Page 349] the best in a series of good papers—and "From Knowledge by Acquaintance to Knowledge by Causation" by Thomas Baldwin. Approaching the end of the book, Anthony Grayling's essay on "Russell, Experience, and the Roots of Science" remains firmly planted in the pre-1918 milieu, though his tentative commentary on Human Knowledge at least glimmers the extension of Russell's intellectual life into the post-Great War period.Effectively the entire second half of Russell's life is left to the final contributor, Charles Pidgen, for commentary. Author of Russell on Ethics, much of Pigden's short essay here ("Bertrand Russell: Moral Philosopher or Unphilosophical Moralist?") somewhat plaintively directs the reader to his book for an elaboration of why Russell is worth reading after 1918, at least on the subject of ethics. Of the other authors, only Hager (317) observes the extent to which Russell's post-War work mistakenly has been ignored.The book concludes with a selective bibliography. The audience, for the most part, is a specialist one, with only some essays accessible to the general reader. On that score, Griffin's introduction and his own essay, "Russell's Philosophical Background," are easily the most readable. At the risk of quibbling, I was disturbed by the uneven production of the book, as each author set his own standard for footnotes, endnotes and bibliography (creating a jumble of styles). Even within individual contributions, there are a number of inconsistencies in notation as well as typographical errors.By the end, I was left hoping there might be a second companion volume for Bertrand Russell, one that reflects what he spent the last fifty years of his life thinking, writing, and doing. In the absence of such material (which is of far greater popular interest today just as it was in Russell's lifetime), this book by itself is more of a companion to current Russell studies than to Bertrand Russell himself.Peter H. Democracy in the 21st century is exhibiting some radical discontinuities in terms of its forms and institutions and needs to be rethought, if we wish to have a sustainable future. Democracy increasingly will be shaped by three realities: the demise of the nation state; the failure of representational liberal democracy; and the radical impacts of resource insufficiency and climate change. Yet if no government, however tyrannical, survives for long except by consent of the people, then that consent can serve as (...) the starting point for rethinking what is meant by “democracy.” Three terms are offered as functional categories that allow for an assessment of democratic forms and institutions: subsistence, operational and systemic. Each describes how and why the population acquiesces to governance and under what conditions. (shrink)

One of the main economic debates taking place in late-nineteenth and early-twentieth-century America was between supporters of protectionism and advocates of free-trade policies. Protectionists won this debate, as the 1897 Dingley Tariff raised tariff rates to record highs. An analysis of this outcome highlights the overlapping interests of Republican politicians and business groups. Both of these groups endorsed particular economic arguments in favour of protectionism. Contemporary studies by academic economists informed the debates surrounding protectionist policies at this time, and also (...) analysed the impacts of these policies. Evidence from politicians, business owners, and economists provides a broad view of who favoured protectionist policies in turn-of-the-century America. This analysis also focuses on how the impacts of these policies were studied and presented in contemporary academic and public discourse. (shrink)

Studies of patient decision‐making use many different measures to evaluate the quality of decisions and the decision‐making process, partly to determine whether the ethical goals of informed consent, patient autonomy, and shared decision‐making have been achieved. We describe these measures, grouped under three main approaches, and review their limitations, leading to three conclusions. First, no measure or combination of measures can provide a complete assessment of decision quality. Second, the quality of a decision is best characterized vaguely, for instance as (...) “good,” “satisfactory,” or “poor,” and these categorizations depend on qualitative judgments that go beyond quantitative measures. Third, bioethicists should focus on identifying and addressing poor or problematic decisions, rather than trying to incrementally increase decision quality, quantified by a measure. Decision‐quality measures can be useful in research and in advancing important goals of bioethics, as long as the challenges of defining and measuring decision quality are recognized. (shrink)

The biochemical mechanism by which the phytochrome family of plant sensory photoreceptors transmit perceived informational light signals downstream to transduction pathway components is undetermined. The recent sequencing of the entire genome of the cyanobacterium Synechocystis, however, has revealed a protein that has an NH2‐terminal domain with striking sequence similarity to the photosensory NH2‐terminal domain of the phytochromes, and a COOH‐terminal domain strongly related to the transmitter histidine kinase module of bacterial two‐component sensors. The Synechocystis protein is capable of autocatalytic chromophore (...) ligation and exhibits photoreversible light‐absorption changes analogous to the phytochromes, indicating its capacity to function as an informational photoreceptor. Together with earlier observations that the COOH‐terminal domains of the plant phytochromes also have sequence similarity to the histidine kinases, these data suggest that the cyanobacteria utilize photoregulated histidine kinases as a sensory system and that the plant phytochromes may be evolutionary descendants of these photoreceptors. There is nothing so difficult to understand as the obvious, because the assumptions underlying it are rarely examined or challenged. Technology has become just such an obvious facet of our existence at the start of the twenty-first century. Our daily lives are shaped and directed by technology; it dominates our work and our leisure, constructs our home and work and our means of traveling between them, in ways so obvious it remains, for the most part, unexamined.Technology has the Janus-like character (...) of being both benefactor and threat. Unfortunately, neither aspect of technology is well understood by most of the participants in our current global culture. Until we understand the nature of technology, however, we will not recognize the assumptions and choices inherent in the technology that, at the moment, holds as much peril as promise for our future. Without that critical understanding, we will accept as inescapable what is in fact open to choice. We may make poor choices without recognizing that we are even choosing, and we may miss the opportunities to choose wisely for ourselves and for future generations.To better understand technology, therefore, we need to unpack the obvious and examine technology more closely, both to identify some prevalent assumptions about it, and to provide the conceptual tools we need to make wise choices about the technology we develop and use.These twelve statements about the nature of technology are a starting point for such an examination, a framework for conversation among the choosers and users of technology. Our inability to feel what others feel makes it difficult to understand how they think. Because moral intuitions organize political attitudes, moral empathy gaps can exacerbate political conflict (and other kinds of conflict as well) by contributing to the perception that people who do not share our moral opinions are unintelligent and/or have malevolent intentions.

'Modern History' views claim that in order for a trait X to have the proper function F, X must have been recently favored by natural selection for doing F (Griffiths 1992, 1993; Godfrey-Smith 1994). For many traits with prototypical proper functions, however, such recent selection may not have occurred, since traits may have been maintained owing to lack of variation or selection for other effects. I explore this flaw in Modern History accounts and offer an alternative etiological theory, which I (...) call the 'Continuing Usefulness' account. According to my view, a trait has the proper function F if and only if, first, the trait was favored by selection for doing F at some point (perhaps far in the past), and, secondly, the trait has recently contributed to survival and reproduction by doing F. This separates the requirement involving natural selection from the one involving the recent past, two issues that the Modern History account conflates. The clear separation allows a detailed analysis of the causal judgments and form of adaptationism that ground the etiological approach to function. (shrink)

In this paper, we move toward offering psychological benchmarks to measure success in building increasingly humanlike robots. By psychological benchmarks we mean categories of interaction that capture conceptually fundamental aspects of human life, specified abstractly enough to resist their identity as a mere psychological instrument, but capable of being translated into testable empirical propositions. Nine possible benchmarks are considered: autonomy, imitation, intrinsic moral value, moral accountability, privacy, reciprocity, conventionality, creativity, and authenticity of relation. Finally, we discuss how getting the right (...) group of benchmarks in human–robot interaction will, in future years, help inform on the foundational question of what constitutes essential features of being human.
