Często przyjmuje się, że psychoterapia dotyczy zaburzeń psychicznych, poradnictwo zaś kryzysów rozwojowych i innych problemów u osób zdrowych. Przyjęcie takiej perspektywy oraz etyki psychoterapeutycznej jest równoważne z przyjęciem dyskursu interwencji medycznej i wiąże się z traktowaniem pacjenta w sposób paternalizujący i dyrektywny. Jest to problematyczne, gdyż sama kategoria „zaburzeń psychicznych”, jak i kryteria diagnostyczne poszczególnych diagnoz, krytykowane są z wielu różnych pozycji. Przykład Soterii, podejścia terapeutycznego (...) dla osób z diagnozą psychozy lub schizofrenii, pokazuje, że nawet w przypadku takiej „ciężkiej choroby psychicznej” model pomocy bliższy poradnictwu, pozwalający na zachowanie większej autonomii i podmiotowości pacjenta, może być równie – a nawet bardziej – skuteczny niż konwencjonalne podejście medyczne i psychoterapeutyczne operujące w aksjologii medycyny. Artykuł stanowi punkt wyjścia do rozważań dotyczących sformułowania właściwego obszaru zainteresowań poradnictwa i psychoterapii, celów tych interwencji oraz konsekwencji etycznych takich decyzji. This book takes Edmund Husserl's phenomenology and applies it to help psychotherapy practitioners formulate complex psychological problems. A qualitative study was conducted with 48 Chinese counselors and psychotherapists who were interviewed in 2006 and an independent sample of 50 participants who responded to a survey in 2014. This study aims to explore how the new emerging expansion of mental health practice is related to issues and challenges of dual role relationship and how the well-engrained values and social characteristics of Chinese culture influences perceptions and resolution of ethical dilemmas. The participants identified similar dual relationships in 2006 and (...) 2014: Incidental boundary crossings, a sexual or romantic relationship, a dual social relationship, and a professional dual relationship were the most frequently reported types of dual relationships encountered by Chinese practitioners, and the socioeconomic and cultural context and the development trajectory of counseling and psychotherapy in China play a significant role in shaping practitioners’ behaviors. The comparison of the data in 2006 and 2014 indicated that practitioners in psychotherapy and counseling have an improved awareness and understanding of ethical issues, evidenced by the decreasing cases of romantic and sexual relationships and increasing cases of social dual relationships influenced by the interrelatedness of traditional Chinese culture. Over the past decade, psychotherapy and counselling have become more and more popular, with many people turning to therapists in the hope of finding a better, happier, more fulfilling life. In this analysis of Yalom’s account of philosophical counseling I show that his perception of it is largely informed by his own ideas about existential psychotherapy and group therapy. Additionally I find that When Nietzsche Wept, and The Schopenhauer Cure comply with Yalom’s personal development and struggles in psychotherapy with philosophy, religion, and boundary violations. Conflicting ideas and attitudes concerning the formerly mentioned are traced also in other works by Yalom.

The dominant framework for understanding selfhood in contemporary psychology has been one that privileges a highly individualistic conception of self. This is reflected in both the language and approaches of psychotherapy where the influence of contextual factors are given marginal consideration in order to maintain some type of 'objectivity' or 'neutrality' in counseling. We argue that an understanding of selfhood which does not take into account the 'relational' nature of selfhood as well as the cultural or historical context of (...) the client, will likely alienate clients who do not view their self through the individualized lenses of psychology. In order to deal with this problem, we adopt an approach to cultural psychology that views the self as a relational narrative. Such a narrative does not imply an unrestricted freedom to construct our self, but understands the limits to selfhood implied in the web of meanings constitutive of our culture and the web of relations from which our self emerges. 2012 APA, all rights reserved). Currently there are several psychotherapy modalities utilising theory and research from neuroscience in treatment frameworks for mental health and recovery from trauma. In Australia this includes: the Conversational Model of Psychodynamic Psychotherapy, a contemporary psychodynamic approach used for treating Borderline Personality Disorder and other trauma-related disorders; Electroencephalogram Neurofeedback, a brain training therapy which has been used as an adjunct to counselling/psychotherapy in traumatic stress and developmental trauma; and Somatic Experiencing, an integrative mind-body approach based on body responses (...) to threat and fear, especially thwarted attempts to enact fight or flight in the face of threat. These modalities have a promising but still-limited evidence base. This paper explores the role of different types of evidence to frame a descriptive review of the current evidence base available for clinicians practicing in these three modalities. While large clinical trials can provide evidence of comparative effectiveness, case studies, observational, qualitative and practice-based enquiry can assist in discovering other aspects of therapy important for individual clients and practitioners. Existing frameworks of evidence- and ethics-informed health and clinical decision-making suggest an ethical approach can incorporate new, developing and evidence-building therapies. Recommendations include careful consideration of informed consent for therapy including disclosure of efficacy and safety, and specific to psychotherapies incorporating neuroscience, a need for practitioners to reflectively assess their own knowledge, competence, heuristic approaches and biases. Psychotherapy and counselling services are now available on-line, and expanding rapidly. Yet there appears almost no ethical analysis of this on-line mode of delivery of such professional services. In this paper I present such an analysis by considering the limitations on-line contact imposes on the nature of the professional–client relationship. The analysis proceeds via the contrast between the face-to-face case and the on-line case. At the core of the problem must be the recognition that on-line interaction imposes a physical (...) barrier largely permitting only those disclosures of self we choose to make available, and greatly restricting the range of involuntary features and behaviours. I show why this is problematic, first, for the development of a close professional–client relationship, with particular emphasis on such failures as diagnosis and monitoring of the patient. Second I describe the importance of the development of professional character, and of how the on-line environment fails to provide a context for such character traits to emerge and develop. (shrink)

Background Female genital mutilation and circumcision (FGM/C) practices present physicians in Germany with numerous challenges. One possible intervention is elective clitoral reconstruction for esthetic and physiological recovery after FGM/C. Even if the study situation regarding the results achieved by clitoral reconstruction is controversial, the range of reconstruction options is increasing. Arguments The aim of this study is to critically examine the epistemic and ethical dimensions of the interdisciplinary debate on clitoral reconstruction that has arisen over the last 20 years. It (...) is intended to provide orientation in the complex field in which cultural, gender-specific, sexual, and psychosocial factors interact with anatomical–physiological knowledge and the lack thereof. The analysis shows the ambivalent role of evidence claims and promises in relation to clitoral reconstruction. Conclusions Difficulties in testing and comparing existing surgical procedures and in the use of standardized questioning tools demonstrate how essential cultural competence and diversity awareness are in dealing with those affected. The prima facie correct argument of insufficient evidence on the efficacy, comparability, and long-term consequences of reconstruction procedures proves to be problematic for ethical and epistemic reasons. In order to generate valid and useful data on sexuality, body perception, pain, and pleasure before and after clitoral reconstruction, survey modalities that address the culturally heterogeneous target group of women with FGM/C are needed. Cultural competence and sensitivity in medicine are also urgently needed for target group-oriented education and counseling as a prerequisite for self-determined decisions for/against clitoral reconstruction. Philosophical counseling is an educational activity in which philosophers engage in dialogue with clients who wish to address questions or manage problems that arise during the course of everyday life. This chapter offers three contrasting perspectives on the question of what philosophical counselors do: first, an institutional scope of practice for philosophical counseling; second, an anecdotal list of common issues for which clients seek philosophical counseling; and third, a heterogeneous set of views by several pioneers of philosophical counseling.
