Background Few studies have been conducted to assess the quality of orientation practices for ethics advisory committees that conduct ethics consultation. This survey study focused on several Harvard teaching hospitals, exploring orientation quality and committee members’ self-evaluation in the American Society of Bioethics and Humanities ethics consultation competencies. Methods We conducted a survey study that involved 116 members and 16 chairs of ethics advisory committees, respectively. Predictor variables included professional demographics, duration on committees and level of training. Outcome variables included (...) familiarity with and preparedness in the ASBH competencies and satisfaction with orientations. We hypothesised that responses would be associated with both the aforementioned predictors and whether or not participants had encountered the ASBH competencies in training. Results A majority of respondents found their orientation curricula to be helpful, although a significant portion of respondents did not receive any orientation or were unsatisfied with their orientation. Familiarity with ASBH competencies was a statistically significant predictor of respondents’ self-evaluation in particular categories. Standard educational materials were reported as offered during orientation, such as readings and case studies ; different medium resources were less evidenced such as videos on ethics consultation. Conclusions Institutions should re-evaluate orientation practices for ethics committee members that perform ethics consultation. Integrating ASBH competencies and useful methods into a resourceful pedagogy will help improve both member satisfaction with orientation and preparation in consultation. (shrink)

Three ethical conflicts in particular are paradigmatic of what we define as “clerkship ethics.” First, a distinction that differentiates the clerkship student from the practicing physician involves the student’s principal role as a learner. The clerkship student must skillfully balance her commitment to her own education against her commitment to patient care in a fashion that may compromise patient care. While the practicing physician can often resolve the tension between these two goods when they come into conflict, the clerkship student (...) is left with a more ambiguous set of choices. Second, evaluative scrutiny during clinical clerkships often forces medical students to balance doing what is morally fitting against the perceived expectations of the medical teams in which they work. Third and finally, a deeply entrenched culture of medical hierarchy presents a particular challenge to innovation and improvement in ethics education during the clerkship years. Students regard faculty as exemplars, but are not provided with the tools to assess when technical medical competence is not matched by moral competence; moreover, these faculty are unlikely to have experienced the ethics education in which students are asked to demonstrate mastery. The present piece focuses on the influence of Plotinus' understanding of time and eternity as articulated in Plotinus' third and fifth Enneads upon Mīr Dāmād's (d. 1631–2) conception of eternity, perpetuity, and time found in his Book of Blazing Brands (Kitab al‐Qabasāt). Although Mīr Dāmād's conception of eternity, perpetuity, and time resembles that of Plotinus' cosmology and ontology, he departs from Plotinus' hypostases in establishing strict parameters for each domain. Unlike Plotinus, Mīr Dāmād argues that the realm of eternity is (...) reserved for God alone, while the realm of Perpetuity contains the Platonic Forms. For Mīr Dāmād, the realm of time is an effect of the realm of Perpetuity and a tool for human beings to understand how to measure events in the temporal world. Unlike many other Shī'ite philosophers, Mīr Dāmād's articulation of these three cosmological realms incorporates thought found in the works of both prominent Sunni and Shī'ite scholars such as Ibn Sīnā, Abū Ḥāmid al‐Ghāzālī, Suhrawardī, and Naṣīr al‐Dīn Ṭūsī. Although his most successful student, Mullā Ṣadrā Shirazī, had ultimately disagreed with his teacher's cosmological doctrine, he remained influenced by the multitude of sources that his teacher had used. This collection of essays looks at the distinctively English intellectual, social and political phenomenon of Latitudinarianism, which emerged during the Civil War and Interregnum and came into its own after the Restoration, becoming a virtual orthodoxy after 1688. Dividing into two parts, it first examines the importance of the Cambridge Platonists, who sought to embrace the newest philosophical and scientific movements within Church of England orthodoxy, and then moves into the later seventeenth century, from the Restoration onwards, culminating in essays (...) on the philosopher John Locke. These new contributions establish a firmly interdisciplinary basis for the subject, while collectively gravitating towards the importance of discourse and language as the medium for cultural exchange. The variety of approaches serves to illuminate the cultural indeterminacy of the period, in which inherited models and vocabularies were forced to undergo revisions, coinciding with the formation of many cultural institutions still governing English society. This book discloses a largely unnoticed dialogue between Muslim and Western social thought on the search for meaning and transcendence in the human sciences. The disclosure is accomplished by a comparative reading of contemporary Muslim debates on secular knowledge on the one hand, and of a foundational Western debate on the demise of metaphysics in the human sciences on the other hand. This article presents a critique of an article previously featured in Nursing Philosophy by Ursula Naue and Thilo Kroll, who suggested that people living with dementia are assigned a negative status upon receipt of a diagnosis, holding the identity of the ‘demented other’. Specifically, in this critique, we suggest that unwitting use of the adjective ‘demented’ to define a person living with the condition is ill‐informed and runs a risk of defining people through negative attributes, which has a deleterious (...) impact upon that person's social and relational personae. Moreover, use of the locution ‘demented’ reinforces a divide between the ‘demented’ and the ‘healthy others’ . Social constructionist theory, malignant positioning and viewing people with dementia as semiotic subjects are the philosophical pillars through which we construct the main arguments of the critique. The article concludes with the voice of one of the authors, a younger person with dementia, asking for language in dementia care to be carefully reconsidered and reframed and for the recognition of the diagnosed person's agency in the conduct of their day‐to‐day lives. This research aspires to represent the subversion of pre-defined gender roles in the novels; The Shadow of the Crescent Moon by Fatima Bhutto, Butterfly Season by Natasha Ahmed, and Stained by Abda Khan. The researchers aim to depict the destabilization of gender-based stereotyped identity from the Pakistani perspective. The selected method of study is Feminist Analysis by Tyson, which examines literature as a medium to represent feminist issues, whereas; the theoretical angle of “Matrix of Domination” from Collins’ Feminist/Gender theory is (...) used as a principle to analyze and depict the subversion of pre-defined gender roles in the selected novels. This study aims to establish the notion of gender as a social construct that could be subverted through literary discourses that have the potential to challenge the power-based gender roles within a patriarchal society. In this regard, different critical works of prominent theorists and writers have been discussed briefly in the literature review to project the significance of the works by contemporary Pakistani women writers as a medium to subvert identities formed by their society. Gender is also a means of power through which the dominant seeks to control the subordinate. The objective of this research is to suggest that gender is socially constructed, therefore, it can be deconstructed through literature. The selected novels exemplify the current gender inclinations of today’s Pakistan with the pen of female writers. Stereotyped gender identity is a socially constructed vice that divides humans into segments, hence it is required of the contemporary discourses to decenter such power discourses that perpetuate hegemonic boundaries and restrict the women into social shackles of imposed identities. Although sinology cannot exist without translation, the goals of the two are not always the same. One is usually put in service subordinate to the other, whether a conflict is explicitly recognized or not. The perennial dispute surrounding “free” vs. “literal” translation often ensues as an inevitable consequence. Brief remarks are offered here in hope of clarifying certain issues and methods, with special attention to the translation of medieval poetry.

This study is based on Post-cyberpunk in order to highlight the prominence of Postcyberpunk as an emerging representative genre of Postmodern Literature. Apart from the deconstruction of metanarratives this study signifies the relevance of Post-cyberpunk as Postmodernist Literature that represents society through multidimensional discursivity such as capitalism, hypercasaulization, imperialism, religion, economy and technology. This study aims to deconstruct the metanarratives of identity that were considered to be permanent and claimed to be power narratives. It equally represents the deconstruction of center/margin dichotomy by bringing mininarratives into the center. In 2012, Loyola University Chicago's Stritch School of Medicine became the first medical school in the United States to actively recruit and accept undocumented immigrants who received protections granted under the Deferred Action for Childhood Arrivals program that was established by presidential memorandum. By 2016, sixty-one medical schools were considering applications from DACA recipients for admission, and more than 110 students applied. According to the Association of American Medical Colleges, sixty-five DACA recipients matriculated in U.S. medical schools in the 2016–2017 (...) school year. These students contribute both diversity and talent to our medical community; they could also have a significant impact on the care of underserved and immigrant populations. The recent decision by the Trump administration to rescind DACA therefore comes with serious pragmatic and ethical ramifications, impeding our medical community's efforts to develop a diverse and representative workforce committed to improving access to quality care for all patients. : In the poetry of Daud Kamal, water figures as an image of mercy, as in the Quran, and as a mirror that reflects divine hidden presence. The rock pool evokes the memory of Gandhara and other foundational civilizations born in love and creative ferment. Conversely, the images of drought, heat, and dust symbolize a parched spiritual order. The river, a recurring archetypal image in Kamal’s poetry, represents the fluid self that is subsumed into collective identity to become a poetic (...) distillate of history. I argue for a teleological account of events in progress. Details aside, the proposal is that events in progress are teleological processes. It follows from this proposal that final causes are ubiquitous: anything happening at any time is an event with a telos.

In February 2016, the Cleveland Clinic initiated the first attempt at cadaveric uterine transplantation (UTx) in the United States. The transplantation was ultimately unsuccessful, but it opened doors for further research on both live and cadaveric UTx. While initial strides toward successful transplantation have been made, questions persist on the ethics of UTx: whether the uterus is a vital organ, whether we should prioritize live or cadaveric options, and how the procedure should be covered by health insurance. If we agree (...) that the goal of the medical profession is both to treat and improve quality of life, then the question of whether or not infertility is considered a disease becomes inconsequential in the discussion. As such, the medical enterprise should move forward with research in UTx. In doing so, considering the ethical implications of UTx remains essential—and we must remember to pair innovation with regulation. (shrink)

Corporations significantly influence the public and political spheres. In light of this corporate power in society, academics have criticized the lack of legitimization (i.e., the legitimacy gap) and highlighted a potential divergence between corporate resource allocation and the needs and preferences of the public (i.e., the social issues gap). To address these problems, democratizing organizations has been proposed as a potential solution. In line with this, the authors argue that an increase in corporate power outside the economic realm should be (...) counterbalanced by more democratic corporate governance (i.e., an internalization of democracy). While important groundwork exists, academic attention to these topics remains limited. The authors provide an overview of the literature linking corporate power and the claim for democratizing organizations, advocate for legitimization, identify research gaps, and develop a research agenda to inspire and guide future research efforts. The paper's findings suggest the importance of conceptual and empirical research on the micro-, meso-, and macro-levels and the cross-level effects, which is needed to advance this incipient research area. Future research must (1) clearly define corporate power and discuss how democratizing organizations can contribute to more legitimate organizational decisions, (2) unveil novel approaches for more democratic corporate governance, and (3) understand the organizational- and individual-level factors necessary for the successful realization of democratic organizations. The purpose of their article is to demonstrate Hildegard’s holistic, or biopsychosocial, approach to healing as a model that we in the twenty-first century have lost but (...) would do well to recover. Several questions come to mind:What is Hildegard’s model?Have we lost it?Is it something we wish to emulate?The basics of the case history is that a noblewoman, Sigewiza, is ill or possessed of an evil spirit. Attempts by her local abbot and community to send her on pilgrimages or otherwise to cure or exorcise her, even with Hildegard’s recommendation, have only short-lived success, after which Sigewiza’s illness or demon returns. Finally, the abbot asks Hildegard to take the noblewoman into her Benedictine monastery, and after a suitable time and various interventions, Sigewiza is permanently (presumably) cured. The arduous process of effecting this cure has left Hildegard weak and ill herself, a pattern of poor physical health from which Hildegard suffered all her life.Phillips and Boivin claim that this is an early example of conceptualizing and utilizing a bio-psychosocial model to treat a complex illness. Hildegard, well known for her medical and pharmaceutical knowledge, applies the proper dietary changes to correct Sigewiza’s humoral (biological) imbalance. She treats the psyche and its partner, the soul, by long hours of discussion with Sigewiza and verbal duels with her demon, as well as by exorcising the demon, and she includes the social aspects of treatment by bringing Sigewiza into the Benedictine community of sisters as a participating member. Which of these three modalities may be sufficient for a cure depends upon the medieval literary genre in which the case appears. If in a medical text, then the proper dietary treatment and application of compounds to neutralize and correct the offending humoral imbalances would be the therapeutic agents. In medieval hagiography, [End Page 369] the biological modality is never sufficient and often ridiculed, because the entire thrust of the text is to demonstrate the futility of medical (secular) ministrations and the effectiveness of saintly intervention in producing a cure, whether or not exorcism is technically part of the process. In Sigewiza’s case, perhaps because of the stubbornness of the possessing demon or because of other factors attached to Sigewiza’s condition, the three (biopsychosocial) modalities are utilized. It seems that Sigewiza has a treatment-resistant illness.The narrative follows a familiar topos in saint’s lives (Kroll and Bachrach 2005). Attempts to cure a demoniac by prayers, pilgrimages, intercessions of other local saints or reputedly holy persons, and sometimes by physicians (when available) are to no avail. The sufferer is brought, amidst great fanfare and public attention (a media event), to the holy woman who is the central figure of the particular Vita, and the sufferer is promptly, or protractedly, cured. In such an account, the holiness of the holy woman is further demonstrated and her reputation, shrine, and influence are duly extended and enhanced. The holy person, with humility, attributes the intercession and cure to the power of God, or Jesus or Mary or a particular patron saint, but the crowd of onlookers and the audience of the Vita understand that God (or Jesus or Mary) has in fact granted the miracle cure as a way of demonstrating the holiness of this holy person in contrast to those who previously tried to cure the ill person. The abbot who ‘referred’ Sigewiza to Hildegard, and the previous saints’ shrine to whom Sigewiza was brought, although generally acknowledged as being of fine character or respectability, had to suffer the ignominy of their petitions being rejected to dramatize the special regard in which Hildegard is held.Although we can say that the “purposes” for which Hildegard’s hagiographer includes such anecdotes is to demonstrate her holiness and her importance in God’s eyes, this does not stop us, nine centuries later, from using such... Szabó (Philos Rev 120(2):247–283, 2011) and Santorio (Philos Stud 164(1):41–59, 2013) have revived the case for Fodor’s (The linguistic description of opaque contexts, Ph.D. thesis, Massachusetts Institute of Technology, 1970) _specific yet opaque_ readings of determiner phrases in intensional constructions. Szabó claims that the existence of such readings gives reason to abandon standard theories of movement. Santorio claims that such readings imply that a quantification analysis of indefinites is false. I’m not so sure. This paper examines the contest between Canadian American industrialist Cyrus Eaton and the Pugwash scientists’ leadership for influence over the early Pugwash scientists’ conferences. Eaton's activism has generally been dismissed in the historical literature as ineffective, naive and too uncritical of the Soviet Union. This paper argues that he was genuinely committed to international peace and security, that Eaton shared with Pugwash scientists a belief in the importance of intellectuals to global unity, and that he worked to bring about greater (...) international peace and understanding through both his personal activism and his own conferences held in the town of Pugwash. Eaton, however, favoured a broader push for peace, which included participation by a wider range of intellectuals and a call for rapprochement with the Soviet Union. These differences between Eaton and the Pugwash scientists, I argue, were more than simply about approach: they represented different internationalist visions and manifested in different conceptualizations for the Pugwash scientists’ conferences. Eaton hoped to incorporate non-scientists and humanism into the conferences, and integrate them into his own broader conference programme at his Thinkers’ Lodge at Pugwash. The scientists, however, wanted to keep their conferences as distinctly scientists’ events, tied to science and its authority. (shrink)

In lieu of an abstract, here is a brief excerpt of the content:Philosophy, Psychiatry, & Psychology 9.2 (2002) 159-160 [Access article in PDF] The Nine Lives of the Dynamic Unconscious Jerome Kroll IN THEIR PROVOCATIVE ARTICLE "Dispensing with the Dynamic Unconscious," O'Brien and Jureidini offer two basic arguments against the existence or, more accurately, because we are dealing here with constructs, the plausibility, of the dynamic unconscious. First, they assert, in contradistinction to the psychoanalytic claim that evidence of a (...) cognitive unconscious supports the claim for a dynamic unconscious, that the notion of a cognitive unconscious is incompatible with that of a dynamic unconscious. Second, they argue on the grounds of parsimony that the phenomena explained by a dynamic unconscious can be better explained (accounted for) by other mechanisms.I would argue that neither of their contentions is correct. O'Brien and Jureidini have essentially taken data and evidence from cognitive science, issued a new vocabulary, and then asserted that their vocabulary is superior and constitutes better explanations for the phenomena and observations under question. They have failed to show any incompatibility between the two explanatory constructs. O'Brien and Jureidini agree with the basic Freudian tenet that most mental activity is unconscious, that is, out of consciousness, and that much of what is out of consciousness can be brought into consciousness with proper cueing. No problem so far.The problem with the dynamic unconscious, as O'Brien and Jureidini see it, is that the cognitive unconscious is modular, best described as a series of subpersonal systems akin to a set of narrowly focused computational specialists, whereas the dynamic unconscious contains fully formed repressed memories. What O'Brien and Jureidini have now given us are multiple homunculi rather than just one CEO or executive homunculus. How literally do we want to take these subpersons? Are subpersonal mental states more satisfactory scientifically than personal-level mental states? Second, O'Brien and Jureidini unfairly attribute to the psychoanalytic position the claim that the dynamic unconscious "is presumed to contain fully formed repressed memories." With the compelling evidence from psychology and neuroscience that all memory is a reconstruction, I do not think that any contemporary psychodynamicist makes a case for fully formed repressed memories, nor is this notion essential to a theory of a dynamic unconscious. O'Brien and Jureidini invoke Occam's razor by invoking a straw horse argument. Their description of the action of subpersonal parts doing sufficient specific cuing sounds suspiciously like the work done in the dynamic unconscious. In the next section, an emotional response is described as pushing images out of mind before they can be reported on (assumedly, before they become conscious). What is doing the pushing here? Upon what principles or priorities are images pushed out of mind?The authors explain that mental "representations are inaccessible to consciousness by virtue [End Page 159] of architectural constraints rather than the operation of a repressive force." But architectural constraints sounds like another way of describing neural facilitation and inhibition. Certainly most neural activity, perhaps all, involves facilitation and inhibition of signals at multiple synapses. Repression is just a psychological construct to describe what the processes of facilitation and inhibition bring about in terms of meaningful mental activity. While we are on the topic of choice of words, the repeated description of the dynamic unconscious as subterranean is an inflammatory locution designed to rally opposition to the notion. The cognitive unconscious is equally subterranean.To look further into the supposed superiority of explanation via the cognitive unconscious, we see that O'Brien and Jureidini attribute the following processes to the cognitive unconscious: evaluation, decision making, editing out, filling in, reconstruction, and reconfiguration. But what are the bases of these processes? What guides or influences the way ideas, images, and emotions are reconstructed and reconfigured? It does not appear to be done randomly. When the reconstructive narrative becomes "significantly distorted" because the memory to be retrieved is fraught with distress, what accounts for the shape of the distortion and what (dare we say who?) recognizes that a memory is about to be distressing? We may all be unhappy with the notion of a dynamic unconscious... (shrink)

What is difficult to imagine is also surprising to perceive. This indicates that active visual imagery is an integral part of active visual perception. Erroneous mental transformations provide clues to prior assumptions in visual imagery, just as visual illusions provide clues to perceptual assumptions. Visual imagery and perception share generic assumptions about invariants in images of rigid objects.

I argue that Islam provides very efficient ethical principles for dealing with the present ecological crisis, a crisis rooted in moral deprivation. I reject the maximization of benefits from natural resources without giving due consideration to the adverse environmental impact of such actions, and argue that this practice is based on injustices generated by factors like greed, extravagance, and ignorance, among others. So far, Western solutions of such problems have generally been based purely on materialistic approaches which place emphasis on (...) secular technological models without any linkage with metaphysical doctrines. Islam recognizes that man by virtue of his creation is a superior being, one for the service of whom the Earth was created; but at the same time man has been made responsible for any departure inhis behavior from the ways laid down by Almighty Allah. Man’s activities, according to Islam, must be based on the idea that this world is a transitory abode, and that man has to gain God’s favor in order to be able to find a better place in the other world. Hence, man's actions, as manifestations of his faith, must be properly and effectively administered, requiring justice, Taqwa, and appropriate knowledge and understanding of environmental problems.
