BackgroundHIV prevention research in resource-limited countries is associated with a variety of ethical dilemmas. Key amongst these is the question of what constitutes an appropriate standard of health care (SoC) for participants in HIV prevention trials. This paper describes a community-focused approach to develop a locally-appropriate SoC in the context of a phase III vaginal microbicide trial in Mwanza City, northwest Tanzania.MethodsA mobile community-based sexual and reproductive health service for women working as informal food vendors or in traditional and modern (...) bars, restaurants, hotels and guesthouses has been established in 10 city wards. Wards were divided into geographical clusters and community representatives elected at cluster and ward level. A city-level Community Advisory Committee (CAC) with representatives from each ward has been established. Workshops and community meetings at ward and city-level have explored project-related concerns using tools adapted from participatory learning and action techniques e.g. chapati diagrams, pair-wise ranking. Secondary stakeholders representing local public-sector and non-governmental health and social care providers have formed a trial Stakeholders' Advisory Group (SAG), which includes two CAC representatives.ResultsKey recommendations from participatory community workshops, CAC and SAG meetings conducted in the first year of the trial relate to the quality and range of clinic services provided at study clinics as well as broader standard of care issues. Recommendations have included streamlining clinic services to reduce waiting times, expanding services to include the children and spouses of participants and providing care for common local conditions such as malaria. Participants, community representatives and stakeholders felt there was an ethical obligation to ensure effective access to antiretroviral drugs and to provide supportive community-based care for women identified as HIV positive during the trial. This obligation includes ensuring sustainable, post-trial access to these services. Post-trial access to an effective vaginal microbicide was also felt to be a moral imperative.ConclusionParticipatory methodologies enabled effective partnerships between researchers, participant representatives and community stakeholders to be developed and facilitated local dialogue and consensus on what constitutes a locally-appropriate standard of care in the context of a vaginal microbicide trial in this setting.Trial registrationCurrent Controlled Trials ISRCTN64716212. Common monogenic genetic diseases, ones that have unexpectedly high frequencies in certain populations, have attracted a great number of conflicting evolutionary explanations. This paper will attempt to explain the mystery of why two particularly extensively studied common genetic diseases, Tay Sachs disease and cystic fibrosis, remain evolutionary mysteries despite decades of research. I review the most commonly cited evolutionary processes used to explain common genetic diseases: reproductive compensation, random genetic drift (in the context of founder effect), and especially heterozygote advantage. (...) The latter process has drawn a particularly large amount of attention, having so successfully explained the elevated frequency of sickle cell anemia in malaria-endemic areas. However, the empirical evidence for heterozygote advantage in other common genetic diseases is quite weak. I introduce and illustrate the significance of a hierarchy of genetic disease phenomena found within the genetic disease explanations, which include the phenomena: single mutation variants of a common genetic disease, single genetic diseases, and classes of diseases with related phenotypic effects. I demonstrate that some of the confusion over the explanations of common genetic diseases can be traced back to confusions over which phenomena are being explained. I proceed to briefly evaluate the existing evidence for two common human genetic diseases: Tay Sachs disease and cystic fibrosis. The above considerations will ultimately shed light on why these diseases’ evolutionary explanations remain so deeply unresolved after so such a great volume of research. (shrink)

Luc Faucher and Edouard Machery’s recent article in this journal uses evidence from psychological studies to criticize Jorge Garcia’s view of racism. This brief response argues that their critique fails because they misinterpret Garcia’s view and engage in some conceptual equivocation. It also argues that their focus on affect and human psychology is in fact compatible with Garcia’s view of racism as rooted in the human heart. Hence the evidence that they cite should be seen as empirical enrichment of Garcia’s (...) basic view, rather than as evidence against that view. (shrink)

In a recent paper del Valle-Inclan and Schlöder argue that bilateral calculi call for their own notion of proof-theoretic harmony, distinct from the usual (or ‘unilateral’) ones. They then put forward a specifically bilateral criterion of harmony, and present a harmonious bilateral calculus for classical logic. In this paper, I show how del Valle-Inclan and Schlöder’s criterion of harmony suggests a notion of normal form for bilateral systems, and prove normalisation for two (harmonious) bilateral calculi for classical logic, (...) HB1 and HB2. The resulting normal derivations have the usual desirable features, like the separation and subformula properties. HB1-normal form turns out to be strictly stronger that the notion of normal form proposed by Nils Kürbis, and HB2-normal form is neither stronger nor weaker than a similar proposal by Marcello D’Agostino, Dov Gabbay, and Sanjay Modgyl. In the scenario of growing polarization of promises and dangers that surround artificial intelligence (AI), how to introduce responsible AI and robotics in healthcare? In this paper, we develop an ethical–political approach to introduce democratic mechanisms to technological development, what we call “Caring in the In-Between”. Focusing on the multiple possibilities for action that emerge in the realm of uncertainty, we propose an ethical and responsible framework focused on care actions in between fears and hopes. Using the theoretical perspective of (...) Science and Technology Studies and empirical research, “Caring in the In-Between” is based on three movements: the first is a change of focus from the world of promises and dangers to the world of uncertainties; the second is a conceptual shift from assuming a relationship with robotics based on a Human–Robot Interaction to another focused on the network in which the robot is embedded (the “Robot Embedded in a Network”); and the last is an ethical shift from a general normative framework to a discussion on the context of use. Based on these suggestions, “Caring in the In-Between” implies institutional challenges, as well as new practices in healthcare systems. It is articulated around three simultaneous processes, each of them related to practical actions in the “in-between” dimensions considered: monitoring relations and caring processes, through public engagement and institutional changes; including concerns and priorities of stakeholders, with the organization of participatory processes and alternative forms of representation; and making fears and hopes commensurable, through the choice of progressive and reversible actions. Recommender Systems (RecSys) have been around since the early days of the Internet, helping users navigate the vast ocean of information and the increasingly available options that have been available for us ever since. The range of tasks for which one could use a RecSys is expanding as the technical capabilities grow, with the disruption of Machine Learning representing a tipping point in this domain, as in many others. However, the increase of the technical capabilities of AI-powered RecSys did not (...) come with a thorough consideration of their ethical implications and, despite being a well-established technical domain, the potential impacts of RecSys on their users are still under-assessed. This paper aims at filling this gap in regards to one of the main impacts of RecSys: personal autonomy. We first describe how technology can affect human values and a suitable methodology to identify these effects and mitigate potential harms: Value Sensitive Design (VSD). We use VSD to carry out a conceptual investigation of personal autonomy in the context of a generic RecSys and draw on a nuanced account of procedural autonomy to focus on two components: competence and authenticity. We provide the results of our inquiry as a value hierarchy and apply it to the design of a speculative RecSys as an example. (shrink)

A growing body of literature has engaged with mass incarceration as a public health problem. This article reviews some of that literature, illustrating why and how bioethicists can and should engage with the problem of mass incarceration as a remediable cause of health inequities. “Mass incarceration” refers to a phenomenon that emerged in the United States fifty years ago: imprisoning a vastly larger proportion of the population than peer countries do, with a greatly disproportionate number of incarcerated people being members (...) of marginalized racial and ethnic groups. Bioethicists have long engaged with questions of health justice for incarcerated people, including consent issues for those participating in research and access to health care. This article provides an overview of the individual and public health impacts of mass incarceration. The article argues that mass incarceration is a bioethics issue that should be addressed in medical education, identifies opportunities for bioethicists to guide hospitals’ interactions with law enforcement officials, and calls on bioethicists to be in conversation with medical and nursing students and health care professionals about these groups’ advocacy efforts concerning structural racism, police violence, and mass incarceration. Ian Rumfitt (2000) developed a bilateralist account of logic in which the meaning of the connectives is given by conditions on asserted and rejected sentences. An additional set of inference rules, the coordination principles, determines the interaction of assertion and rejection. Fernando Ferreira (2008) found this account defective, as Rumfitt must state the coordination principles for arbitrary complex sentences. Rumfitt (2008) has a reply, but we argue that the problem runs deeper than he acknowledges and is in fact related to (...) the challenge of establishing proof-theoretic harmony. We motivate a distinctively bilateral criterion for harmony and show how the bilateralist can meet it. This also resolves Ferreira's complaint. (shrink)

While there has been renewed attention to racial justice in the United States and around the world recently, there is a long tradition among philosophers and other theorists of reflecting on the nature racial injustice and the remedies that it demands. This article discusses two prominent approaches to racial justice, liberal egalitarian theory and critical race theory, and focuses on four issue areas: reparations, affirmative action and race‐conscious policy, integration, and criminal justice. Although liberal and critical approaches to racial justice (...) are sometimes seen as in opposition to each other, it is argued here that there is often convergence as well. Despite their differing pedigrees, a shared commitment to racial equality and a shared recognition of the profound, structural nature of racial inequality often leads scholars from different starting points to similar, radical conclusions. Assim, se é possível falar aqui de uma cartografia, ela sem dúvida designará o intenso movimento filosófico de ocultamento do corpo, que paradoxalmente (...) dá a ver o que tanto se buscou evitar: é que o corpo sempre resiste, insistindo em se fazer disponível aos «mundos múltiplos e divergentes» que a cada vez a arte aproxima. Thus, if it is possible to speak about a cartography, it will undoubtedly designate the intense philosophical movement of body concealment, which paradoxically hints at what it has sought so much to avoid: is that the body always resists, insisting on making itself available to « multiple and divergent worlds» which art approaches each time. La Allgemeine Psychopatologiede Karl Jaspers es una obra clave del pensamiento del siglo XX. Une de forma interdisciplinar la medicina, la psicología y la filosofía existencial. De entre los conceptos que trata, este ensayo verá la función del concepto de símbolo y a través de él la función de la Filosofía como guía de la Psiquiatría y de la Psicopatología en general. En la Psiquiatría contemporánea cada vez se da más importancia a la “simbolización” como concepto clave bajo el que (...) el sujeto humano interpreta su existencia. El símbolo es un esquema de realidad que se construye y reconstruye continuamente a través de las experiencias del ser humano. Puede decirse entonces que, a través del símbolo, el ser humano construye su mundo. En esa construcción, la Filosofía puede suponer una ayuda si se plantea como “ansia de saber” o, en su lugar, como una rémora si se presenta como una totalidad omnicomprehensiva inflexible bajo la que subsumen todas las experiencias de la vida. La presencia de migrantes provenientes de contextos diversos constituye un elemento definitorio de las plurales sociedades del norte global, por lo que se perciben como necesarias acciones que favorezcan la convivencia entre personas migrantes y autóctonas. En esta línea, una de las propuestas planteadas desde la Educación para el Desarrollo pasa por fomentar la integración mediante acciones que sensibilicen sobre el fenómeno migratorio y las experiencias de los y las migrantes, destacando proyectos en que estos asumen el rol de agentes (...) de desarrollo, vinculando migraciones y desarrollo, del modo positivo planteado desde el codesarrollo. A partir del análisis de un proyecto llevado a cabo en Granada, pretendemos reflexionar sobre cómo toma forma el codesarrollo en las sociedades receptoras, cuál es el papel de los y las migrantes en estos procesos y cómo puede la Antropología aportar una visión compleja y crítica sobre el codesarrollo y la Educación para el Desarrollo. El propósito de este estudio consiste en mostrar la crítica de Kierkegaard a la metafísica de la lectura a través de su problematización de la noción de autor y su revisión de la noción de público en su obraDos épocas. Una recensión literaria. Para ello, se rechazará la interpretación habitual de esta obra, basada en la distinción público/privado, y se valorará la propuesta de Kierkegaard mediante las nociones de cuerpo e intimidad. Ello implicará una redefinición del lenguaje desde el secreto (...) y el silencio, y apuntará hacia una relación compleja entre la literatura, el cuerpo, la subjetividad y la religión. (shrink)

Among the many applications of artificial intelligence (AI), virtual assistants are one of the tools most likely to grow in the future. The development of these systems may play an increasingly important role in many facets of our lives. Therefore, given their potential importance and present and future weight, it is worthwhile to analyze what kind of challenges they entail. In this chapter, we will provide an overview of the ethical aspects of artificial virtual assistants. First, we provide a conceptual (...) clarification of the term ‘virtual assistant’, including different types of interfaces and recommender systems. Second, we address three ethical issues related to the deployment of these systems: their effects on human agency and autonomy and the subsequent cognitive dependence they would generate; the human obsolescence that may cause a generalized extension of the dependence problem; and the invasions of privacy that virtual assistants may cause. Finally, we outline the debates about the use of virtual assistants to improve human moral decisions and some areas in which these systems can be applied. (shrink)

Targeting high-risk populations for public health interventions is a classic tool of public health promotion programs. This practice becomes thornier when racial groups are identified as the at-risk populations. I present the particular ethical and epistemic challenges that arise when there are low-risk subpopulations within racial groups that have been identified as high-risk for a particular health concern. I focus on two examples. The black immigrant population does not have the same hypertension risk as US-born African Americans. Similarly, Finnish descendants (...) have a far lower rate of cystic fibrosis than other Caucasians. In both cases the exceptional nature of these subpopulations has been largely ignored by the designers of important public health efforts, including the recent US government dietary recommendations. I argue that amending the publicly-disseminated risk information to acknowledge these exceptions would be desirable for several reasons. First, recognizing low-risk subpopulations would allow more efficient use of limited resources. Communicating this valuable information to the subpopulations would also promote truth-telling. Finally, presenting a more nuanced empirically-supported representation of which groups are at known risk of diseases (not focusing on mere racial categories) would combat harmful biological race essentialist views held by the public. In order to develop a collaborative experience of reflexive writing, this article explores the ethnographic process through two communication devices used by the authors in their respective fieldwork: tarot readings and evolutionary astrology. Reflecting on their distinct (if not opposing) backgrounds, the authors explore and interpret how their different backgrounds and conversational devices shape their ethnographic experience as a process of revealing the unknown, following the not-knowing approach (Anderson, 1997). The dialogic exchange also reveals how the not-knowing approach affects the (...) collaborative aspect of the reflexive writing experience. (shrink)
