We use the format of a hypothetical case study to review issues related to pharmaceutical product approval and physician prescribing practices. In this case, a new FDA-approved drug is recommended for a patient who subsequently experiences an adverse event that may or may not be related to the prescription. This case raises a number of ethical and legal considerations physicians routinely face when deciding whether to recommend such drugs for their patients. Despite the need for ongoing observation by the regulatory (...) apparatus, physicians should be cognizant of the limitations of the drug approval system and the post-approval prescription drug surveillance system. We discuss physicians’ ethical obligations when faced with a newly approved drug, including seeking out independent sources of learning, reporting adverse effects, and notifying patients about limitations in available knowledge about therapeutic recommendations. Providing in-utero treatments to target specific conditions in the fetus is a relatively new approach in perinatal care, with the vast majority of these treatments being used off-label. The high degree of off-label medication use in the perinatal and neonatal settings raises concern for the safety of both the fetuses and expectant mothers. This report presents two examples of intra-amniotic drug administration based on reported clinical cases. From the ethical framework of medical principlism, we examine the competing ethical duties of (...) autonomy, beneficence, and non-maleficence in this setting. Among the most important questions critical educators can ask today are the following: Can schools play a role in making a more just society possible? If not, why not? If so, what can they do? These questions provide the basis for this article by Michael Apple, as well as for the books under discussion here. The books by David Blacker, John Marsh, Mike Cole, and Pauline Lipman discussed in this essay are either Marxist, have been influenced by Marxist and socialist (...) ideas, or are published by presses that have a long history of publishing material with a Marxist and/or socialist orientation. In order to adequately deal with them, Apple devotes much of this essay to a set of arguments about the possibilities and limits of these ideas. After specifying those arguments, he discusses how they are developed in the books themselves. He grounds this discussion in a call for creating a broader “we” that is based on a more historically grounded understanding of the ways in which struggles over schooling actually can make a difference. This study examines two recent cases of ethical responses to crisis management; the 1995 fire at Malden Mills and Aaron Feuerstein''s response, and a 1998 fire at Cole Hardwoods, followed by the response of CEO Milt Cole. The authors describe these crises, the responses of Feuerstein and Cole, their motivations and the impact on crisis stakeholders using the principles of virtue ethics and effective crisis management. A.W. Moore presents eighteen of his philosophical essays, written since 1986, on representing how things are. He sketches out the nature, scope, and limits of representation through language, and pays particular attention to linguistic representation, states of knowledge, the character of what is represented, and objective facts or truths.

The paper begins with a defence of a new definition of privacy as the absence of undocumented personal knowledge. In the middle section, I criticise alternative accounts of privacy. Finally, I show how my definition can be worked into contemporary American Law.

This article examines the implicit promises of fairness in evidence based medicine , namely to avoid discrimination through objective processes, and to distribute effective treatments fairly. The relationship between EBM and vulnerable groups is examined. Several aspects of EBM are explored: the way evidence is created , and the way evidence is applied in clinical care and health policy. This analysis suggests that EBM turns our attention away from social and cultural factors that influence health and focuses on a narrow (...) biomedical and individualistic model of health. Those with the greatest burden of ill health are left disenfranchised, as there is little research that is relevant to them, there is poor access to treatments, and attention is diverted away from activities that might have a much greater impact on their health. This paper sketches an account of public health ethics drawing upon established scholarship in feminist ethics. Health inequities are one of the central problems in public health ethics; a feminist approach leads us to examine not only the connections between gender, disadvantage, and health, but also the distribution of power in the processes of public health, from policy making through to programme delivery. The complexity of public health demands investigation using multiple perspectives and an attention to detail that is capable (...) of identifying the health issues that are important to women, and investigating ways to address these issues. Theorists of affect and radical democracy have largely overlooked the importance of intentionally cultivating affective dynamics in the process of changing institutions. We address that lack by introducing the concept of musical groove as an intercorporeal feel for improvisational co-creation. Groove in a political context involves specific practices of modulating dynamics, receptivity, and affects in relationship to specific contexts, people, and practices to powerful effect. We explore how early democratic movements during the American Revolution sought to craft institutional forms capacious (...) enough for such democratic groove despite the fact that key intellectual leaders of the time, such as the Federalists and Benjamin Rush, feared the power of intercorporeal affective flows of democratic enthusiasm and popular improvisation. We read Rush’s critique and his efforts to diminish and control these emergences by harmonizing them in the order as a “negative image” of a radical democratic alternative that would foster improvisation and build enthusiasm for catalysing change today. We discuss how improvisation can, as Pierre Bourdieu perceived, simply function to reproduce the limits of a given socio-political game, which is deeply embedded within us as a “feel for the game” or set of affective dispositions. However, we turn Bourdieu’s own critique against itself to find openings for a habitus of political groove and democratic improvisation that he considered foreclosed. We conclude by fleshing out these insights in relation to affective dynamics among political organizing practices and modest institutional changes during a period of rich local transformation in which we participated in Durham, North Carolina. Given that most of the adults’ life is spent in the workplace, and because the quality of working life has a significant effect on family life and community health, it is crucial to study the components involved in the improvement of the workplace and people’s health in the work environment. Therefore, by examining the common literature of Islamic values, spirituality and spiritual health, an attempt has been made in this research to explain organisational values and spiritual health in the management (...) of organisations and to analyse the relationship of these variables with each other in Iraq using an Islamic approach. Similar to any other community, an Islamic community requires management proportional to its specific circumstances. In fact, an Islamic community needs a type of management that intertwines with divine and Islamic values and ethical principles and prioritises Islamic ideals. Therefore, this study aims to evaluate the role of organisational values in the improvement of Islamic values of Iraqi Muslims. To this end, 2800 employees selected from 30 oil and chemical compound manufacturing organisations are examined. According to the results, the Islamic values governing the Iraqi organisations play an effective role in the improvement of employees’ spiritual health.Contribution: Divine and religious values and Islamic ethics have always been considered in Islamic communities, where formal and informal procedures are taken into consideration. An Islamic approach to management is combined with Islamic values, behaviours and ethics, and these values affect Islamic administration. Therefore, a proper understanding of Islamic values will help develop the right plan for human resource management, as well as the maintenance of organisational and community health. (shrink)

Summary The model underlying the hippocratic humoral theory, as well as the corresponding part of hippocratic aetiology is reconstructed in precise, structuralist terms. Stress is laid on the presentation of the model, historical and philological derivations are suppressed. The global net structure of humoral theory in which the different diseases are described as specializations of the basic model is worked out, and the particular metatheoretical features of ‘therapeutical’ theories, as contrasted to ‘descriptive’ theories, are exemplified and stated in general.

In this paper we consider the use of cases in medical ethics research and teaching. To date, there has been little discussion about the consent or confidentiality requirements that ought to govern the use of cases in these areas. This is in marked contrast to the requirements for consent to publish cases in clinical journals, or to use personal information in research. There are a number of reasons why it might be difficult to obtain consent to use cases in ethics. (...) Many cases concern people who are incompetent, and thus unable to give consent. Often the material is of a sensitive nature, it is not clear who should give consent, or the ethicist has no access to those involved.
