ABSTRACT Public health communication campaigns have been credited with helping raise awareness of risk from chronic illness and new infectious diseases and with helping promote the adoption of recommended treatment regimens. Yet many aspects of public health communication interventions have escaped the scrutiny of ethical discussions. With the transference of successful commercial marketing communication tactics to the realm of public health, consideration of ethical issues becomes an essential component in the development and application of public health strategies. Ethical issues (...) in public health communication are explored as they relate to eight topics: ‘targeting’ and ‘tailoring’ public health messages to particular population segments; obtaining the equivalence of informed consent; the use of persuasive communication tactics; messages on responsibility and culpability; messages that apply to harm reduction; and three types of unintended adverse effects associated with public health communication activities that may label and stigmatise, expand social gaps, and promote health as a value. We suggest that an ethical analysis should be applied to each phase of the public health communication process in order to identify ethical dilemmas that may appear subtle, yet reflect important concerns regarding potential effects of public health communication interventions on individuals and society as a whole. Historically, the transgender population has faced prejudice and discrimination within society. The purpose of diagnostic terms is to direct clinical care and facilitate insurance coverage. However, the existence of a medical diagnosis for gender nonconformity can exacerbate the stigmatization of transgender people with adverse consequences on their emotional health and social life. Whether transgenderism and gender dysphoria are indeed a psychopathological condition or even any kind of nosological entity is a contested issue. Many advocates of human rights, trans activists, social (...) scientists, and clinicians support either the removal of gender incongruence from the list of mental disorders or at least its transfer to a separate category. Reforming the classification is an intermediate step toward depathologization and permits access to transgender-related care. Nonetheless, it partly preserves the stigma associated with abnormality and puts the availability of psychiatric care at risk. A more radical approach dictates that the classification of diseases serves exclusively medical purposes and must be dissociated from the respect for the legitimacy of one’s autonomy and dignity. In the long term, only a swing in societal values can detach stigma from mental and physical illnesses. Enhancing collective respect for life, human rights, and diversity is the best way to achieve cohesion and well-being among members of society. The text provides an introduction to analyses of the social exclusion of mothers of children with autism spectrum disorders. The aim is to highlight and discuss several stories from the increasing number of printed accounts by mothers describing their daily struggles with disability and social judgment. The discourses of exclusion concerning this social group are a hihgly interesting research topic due to its multidimensionality and stereotypes ingrained in society.

Chapter three shows that MacIntyre's misunderstanding of what psychoanalysis means by the unconscious leads him to treat it as unobservable. In any intelligible sense, the unconscious is not absolutely unobservable, or else being unobservable is no stigma unique to it; conscious ideas, wishes, e.g., will have to be classed as unobservable, too. MacIntyre's central error is his failing to see that free-association makes the unconscious observable. The chapter concludes with an examination of the concepts of absolute unobservability and observability (...) in contemporary philosophy of science. The idea of an entity or property whose essence it is to be unobservable --that is, of an entity or property that is unobservable in all possible worlds--is absurd. ;Chapter two exposes the fallaciousness of James's claim that on a priori grounds alone the very idea of unconscious mental activity is incoherent. Then, against his claim that brain-processes and/or conscious mental processes that are "quickly gone, unattended to, or forgotten" can explain any phenomenon supposed to need unconsicous mental processes to explain, I argue that the case of resistance to one's own ideas and wishes cannot be accounted for in any of the ten ways James considers; indeed, there are reasons why no explanation of the sorts he considers can be adequate to explain resistance. ;This difference in logical form is missed by Wittgenstein when he confuses the logcial form of an interpretation with the logical form of that of which it is an interpretation--e.g., a dream or symptom. Freud does treat dreams, symptoms, as having meaning, saying things; the condensations and displacements in dreams say "This is really this," "This is all a repetition of that." For Freud, the form of the interpretation is "This dream says 'This is really this,'" Wittgenstein is able to make it plausible to treat interpretations as mythological only by assuming that interpretations are of the same logical form as the dreams they are about. ;They fail, I argue, because each distorts or fails to take account of the same crucial features of unconscious mental processes as psychoanalysis attributes them; these critics assume that the phenomenon the unconscious is needed in order to explain is that of inadvertent, unwitting behavior. Even Freud sometimes slipped in this way--the argument from post-hypnotic suggestion examined in Chapter two also assumes that unwitting behavior of a certain sort requires unconscious mental activity for its explanation. In reality, it is resistance and transference phenomena which justify introduction of the concept of the unconscious. Besides, Freud's method of attributing unconscious mental activity is based on free-association, a wholly novel technique, which produces statements--interpretations--of a different logical form from non-psychoanalytic formulations attributing unconscious mental activity in order to explain inadvertent action. ;Chapter one deals with Wittgenstein's view of psychoanalytic interpretations as a mythology imposted upon subjects. Chapter two examines one of Freud's main proofs for the existence of unconsious mental activity, the argument from post-hypnotic suggestion, and then considers William James's arguments to show that the concept of unconscious mental activity is incoherent, i.e., self-contradictory, and that the concept is unnecessary in that any phenomenon supposed to require the concept to be explained can be explained instead by reference merely to conscious mental processes and/or brain-processes. Chapter three deals with A. MacIntyre's argument to show that the unconscious is unobservable in a way that distinguishes it from legitimate unobservables in science; unlike them, the unconsicous is dispensable in principle. Sexuality education is fundamental in higher and tertiary education institutions (HTEIs). Evidence suggests that its effective education is through translations into the first language of learners. However, in global and multilingual cultural communities such as HTEIs, the foundations for these translations are still a researchable area. Notably, in HTEIs adolescents, young adults and adults co-exist and therefore, any translations must be toned to balance across these groups. The aim of this study was to establish strategies that could enable sexuality educators (...) to effectively transfer information that is steeped in indigenous language, religious and cultural meaning without precipitating stigma and taboo issues. The study setting was the National University of Science and Technology where a cross section of indigenous cultures, languages and religions co-existed with other global cultures. A University Community-based Participatory Research (CBPR) methodology was administered by the researchers using a Delphi research approach to students and staff. Multilingual, multilayered, multireligious, and multicultural nature of the setting presented serious challenges that compromised the prevention of stigma and taboo issues when dealing with sexuality education. We recommend that the strategy to present sexuality education in indigenous languages to the generality of HTEIs population should, in the first instance, involve systematic identification of appropriate interventions that are religious, cultural, and language-specific. Contribution: This research reveals paramount ethical and religious violations that have a potential to precipitate opportunistic sexual relationships that may lead to impediments in capacity building for learners. Nichelle Cherry (bio), and Martina Darragh* (bio)In 1995, the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) expanded its patient rights standards to include requirements for assuring that hospital business practices would be ethical. Renamed “Patient Rights and Organization Ethics,” these standards are based on the realization that a hospital’s obligation to its (...) patients is derived from two relationships: the patient-provider relationship governed by “clinical ethics,” and the customer-supplier relationship governed by “business ethics.” “[This] boundary between ‘clinical’ ethics and ‘business’ ethics is not clear and in many cases is nonexistent. While marketing and admission practices are seen as issues related to ‘business’ ethics, they can lead to unneeded admissions or demand for unneeded services, both of which can unnecessarily expose the patient to the risk of side effects or complications. Likewise, underutilization of needed services is likely to lead to less than optimal health outcomes” (I. Schyve 1996, p. 17). “These new standards arose out of well-publicized concerns about abuses in which patients were admitted to hospitals unnecessarily and were discharged or transferred only after their insurance expired” (I. Emergency services personnel are a high stress occupation, being frequently confronted with highly consequential stressors and expected to perform: without fault; under high pressure; and in unpredictable circumstances. Research often invokes similarities between the experiences of emergency services personnel and elite athletes, opening up the possibility of transferring learnings between these contexts. Both roles involve genuine risks to emotional wellbeing because their occupations involve significant stress. Similarly, both roles face obstacles and injury, and their “success” is dependent on high-quality execution (...) of their skills under pressure. As such, both occupations are required to have resilience and effective coping abilities to ensure psychological well-being. Researchers suggest emotional awareness may be a key variable in the management and maintenance of resilience. This study: explored the experiences of emergency services personnel; characterised connections between emotional awareness and resilience; and reflected on the ways these findings can be extrapolated to elite athletes. We analysed 11 interviews with emergency services personnel. Participants identified resilience as crucial when coping with stress, however, many defined resilience as remaining unaffected by stress rather than, for example, managing and responding to it. Participants defined emotional awareness as understanding their emotions, and they recognised associated benefits for coping, resilience, and burnout. Nevertheless, most participants did not engage in practices to improve their emotional awareness. Barriers, such as maladaptive beliefs and help-seeking stigma, interfered with participants’ ability to cultivate emotional awareness, to promote resilience. In contrast, some participants described profound improvements in resilience and coping following the cultivation of emotional awareness. This finding illustrates that systemic change must target the individual, team, and organisation to correct misperceptions about resilience, emotional awareness, and psychological help-seeking. Developing emotional awareness may help emergency services personnel and other high stress occupations like elite athletes process difficult experiences and enhance their resilience, promoting well-being, and career longevity. BackgroundHuman biological materials are usually stored for possible future use in research because they preserve valuable biological information, save time and resources, which would have been spent on collection of fresh samples. However, use of these materials may pose ethical challenges such as unauthorized disclosure of genetic information, which can result in dire consequences for individuals or communities including discrimination, stigma, and psychological harm; has biosecurity implications; and loss of control or ownership of samples or data. To understand these (...) problems better, we evaluated the extent to which tuberculosis (TB) clinical research protocols that were used to collect and store biological materials for future use conform to the requirements stated in the Uganda national guidelines for research involving humans as participants.MethodsThis was a retrospective review of TB clinical research projects approved by the Uganda National Council for Science and Technology (UNCST) from 2011 to 2015, to examine whether they fulfilled the requirements for ethical collection and use of human materials. Data were abstracted through review of the project protocols using a template developed based on the informed consent and the Materials Transfer Agreement (MTA) requirements in the national guidelines.ResultsOut of 55 research protocols reviewed, most of the protocols 83.6% had been used to collect the stored samples (sputum, blood and sometimes urine), 28% had a section on specimen collection and 24% mentioned ownership of the biological materials. With respect to review of the consent forms used in the studies that stored materials for future use, only 9% of the protocols had a separate consent form for storage of materials, 4.5% of the consent forms explained the risks, 11.4% explained the purpose of the study while 6.8% mentioned the place of storage for the collected materials.ConclusionMany of the studies reviewed did not meet the requirements for collection and storage of biological materials contained in the national guidelines, which indicates a need to additional training on this topic. During recovery from psychosis (diagnosed as schizophrenia) things must often be done slower than normally expected. The tempo of the socially shared reality is often experienced as being too fast for the recovering person. We will describe how this impairment stems from the pre-reflective mental structure underlying psychosis and how it can be transferred into an active skill supporting recovery, often including social retreat. In this paper, co-written by a psychiatrist and a person experienced in psychosis (= participatory health research), (...) we will draw on personal experiences, (participatory) qualitative research and first-hand accounts for our arguments. We will sketch out the process and three crucial building blocks of recovery: laid-back situations, switch-off skills, places to talk. We will then describe the acquired skill of slowing-down as an amalgam of the building blocks ‘switch-off skill’ and ‘laid-back situation’, since slowing-down answers the situation in a laid-back manner and enforces a slow behavioral pace on it. While this might be okay in private and in some situations shared with others (i.e. during serene situations), it may be tantalizing for others in many everyday situations resulting in stigma or exclusion (i.e. during work). Slowing-down therefore challenges the surrounding others and demonstrates the necessity to synchronize private and public tempos. This is most challenging while living in two realities at once, being the second and often a long-lasting stage of recovery (stage of double realities). (shrink)

ABSTRACT Objectives There is little research on moral uncertainties and distress of palliative and hospice care providers (PHCPs) working in jurisdictions anticipating legalising voluntary assisted dying (VAD). This study examines the perception and anticipated concerns of PHCPs in providing VAD in the State of Queensland, Australia prior to legalisation of the practice in 2021. The findings help inform strategies to facilitate training and support the health and well-being of healthcare workers involved in VAD. Design The study used a qualitative approach (...) to examine and analyse the perception and anticipated concerns of PHCPs regarding challenges of providing assisted dying in Queensland. Fourteen PHCPs were recruited using a purposive sampling strategy to obtain a broad representation of perspectives including work roles, geographical locations and workplace characteristics. Data were collected via one in-depth interview per participant. The transcripts were coded for patterns and themes using an inductive analysis approach following the tradition of Grounded Theory. Setting The study was conducted in hospital, hospice, community and residential aged care settings in Queensland, Australia. These included public and private facilities, secular and faith-based facilities, and regional/rural and urban facilities. Participants Interviews were conducted with fourteen PHCPs: 10 nurses and 4 physicians; 11 female and 3 male. The median number of years of palliative care practice was 17, ranging from 2 to 36 years. For inclusion, participants had to be practising palliative and hospice care providers. Results PHCPs are divided on whether VAD should be considered part of palliative care. Expectations of moral distress and uncertainty about practising VAD were identified in five areas: handling requests, assessing patient capacity, arranging patient transfers and logistical issues, managing unsuccessful attempts, and dealing with team conflicts and stigma. Conclusions The possibility of having to practise VAD causes moral distress and uncertainty for some PHCPs. Procedural clarity can address some uncertainties; moral and psychological distress, however, remains a source of tension that needs support to ensure ongoing care of both patients and PHCPs. The introduction of VAD post-legalisation may present an occasion for further moral education and development of PHCPs. (shrink)

Exaptations are adaptations that have undergone a major change in function. By recruiting genes from sources originally unrelated to vision, exaptation has allowed for sudden and critical photosensory innovations, such as lenses, photopigments, and photoreceptors. Here we review new or neglected findings, with an emphasis on unicellular eukaryotes (protists), to illustrate how exaptation has shaped photoreception across the tree of life. Protist phylogeny attests to multiple origins of photoreception, as well as the extreme creativity of evolution. By appropriating genes and (...) even entire organelles from foreign organisms via lateral gene transfer and endosymbiosis, protists have cobbled photoreceptors and eyespots from a diverse set of ingredients. While refinement through natural selection is paramount, exaptation helps illustrate how novelties arise in the first place, and is now shedding light on the origins of photoreception itself. -/- . This article discusses our experience of other people from both phenomenological and psychoanalytic perspectives. Drawing on Husserl and Freud, I will distinguish between different temporal modes of the other: while Husserl carefully examines the ways in which others are constituted as synchronous or as asynchronous, Freud underlines that others may also appear in a temporally displaced, anachronous manner, whereby one’s experience of some past other dominates in the experience of the present other. Freud discusses this third kind of relationship to (...) the other under the rubric “transference”. The main objective of this article is to argue that the Freudian concept of transference is a structural element in our experience of others, and that, in this respect, the psychoanalytic concept of transference should be seen as complementing the phenomenological account of empathy and interpersonal understanding. This paper examines the Transference Theory of causation, developed originally by Aronson (1971) and Fair (1979). Three difficulties for that theory are presented: firstly, problems associated with the direction of transference and causal asymmetry; secondly, the case of persistence as causation, for example where a body's own inertia is the cause of its motion; and thirdly the problematic notion of identity through time of physical quantities such as energy or momentum. Finally, the theory is compared with the Conserved (...) Quantity Theory (Dowe 1992c), and it is shown that that account embodies the modifications that the transference theory needs to adopt. The classic example of this transference phenomenon is the concept of evolution; originating in the social sciences, it was taken over into biology and thence to the whole range of current thought. This paper aims at tracing the use of one idea--the (...) struggle for existence--by Malthus in 1798 in an attempt to solve a problem in economics and politics, its elaboration by Darwin sixty years later into the theory of natural selection for the purpose of solving a problem in biology, and the immediate application of the combined doctrines by Walter Bagehot in an effort to solve the problem of social change. A collection of thoughtul presentations on transference and countertransference highlights _The Realities of Transference_, Volume 6 in the Progress in Self Psychology series. The selfobject transferences receive special attention. Elsewhere in this volme, selfobject phenomena are examined in relation to the process of working through, the origins of ambition, the psychology of addiction, the psychodynamic consequences of AIDS, and creativity. An exploration of the selfobjects of the second half of life offers new insight into later development.

The field, as Steven Cooper describes it, is comprised of the inextricably related worlds of internalized object relations and interpersonal interaction. Furthermore, the analytic dyad is neither static nor smooth sailing. Eventually, the rigorous work of psychoanalysis will offer a fraught opportunity to work through the most disturbing elements of a patient's inner life as expressed and experienced by the analyst - indeed, a disturbance in the field. How best to proceed when such tricky yet altogether common therapeutic situations arise, (...) and what aspects of transference/countertransference should be explored in the service of continued, productive analysis? These are two of the questions that Steven Cooper explores in this far-ranging collection of essays on potentially thorny areas of the craft. His essays try to locate some of the most ineffable types of situations for the analyst to take up with patients, such as the underlying grandiosity of self-criticism; the problems of too much congruence between what patients fantasize about and analysts wish to provide; and the importance of analyzing hostile and aggressive aspects of erotic transference. He also tries to turn inside-out the complexity of hostile transference and countertransference phenomena to find out more about what our patients are looking for and repudiating. Finally, Cooper raises questions about some of our conventional definitions of what constitutes the psychoanalytic process. Provocatively, he takes up the analyst's countertransference to the psychoanalytic method itself, including his responsibility and sources of gratification in the work. It is at once a deeply clinical book and one that takes a post-tribal approach to psychoanalytic theory - relational, contemporary Kleinian, and contemporary Freudian analysts alike will find much to think about and debate here. This paper aims to contribute to epistemology by studying the psychoanalytic notion of transference. Contrary to previous philosophical research on Freud, I claim that traditional models of interpretation or translation are insufficient to explain psychoanalytic understanding. After exploring the conceptual roots of this mistake, which I define as generic, I focus on the notion of transference and claim that it should be viewed as a unique form of understanding that enables the emergence of unconscious meaning or truth. I (...) elaborate this claim with a detailed study of the development of this notion in Freud’s texts. Transference is thus delineated as a form of understanding which is closely related to the matrix of love and defined in relation to the following parameters: intersubjectivity; visuality; and performativity. The article offers a panoramic view of the tropes of “space” and “place” in the poetry of Avot Yeshurun, and explores the radical transformation they underwent throughout the years – from the early poems of the 1930s, to the last volume of poems published before the poet’s death in 1992. I contend that the shift in the nature of the Yeshurunian space, caused by the catastrophe of the Shoah, the foundation of the State of Israel, and the Palestinian Nakba that (...) proceeded it, can be viewed through the prism of what I call “the problem of transference”. Struggling to re-connect the now insurmountably severed worlds – the pre-catastrophe world of the golah, and the new, post-catastrophe, Israeli world – Yeshurun’s poetry exhibits an ever-growing tendency to transfer language-materials from one side of the abyss to the other, desperately attempting to both salvage the remains of the ruined home, and establish the new world as a proper dwelling place. This back-and-forth movement of traumatic transference, I argue, is a powerful interpretive tool by which to understand the poet’s entire poetic project and to account for its peculiarities, and above all - its famous “clash of languages” which consists of Hebrew, Yiddish, and Arabic. Sensory substitution devices (SSDs) are most familiar from their use with subjects who are deficient in a target modality (e.g. congenitally blind subjects), but there is no doubt that the use and potential value of SSDs extend to persons without such deficits. Recent work by Amedi and his team (in particular Levy-Tzedek et al. 2012) has begun to explore this. Their idea is that SSDs may facilitate behavioural transference (BT) across sense modalities. In this case, a motor skill learned (...) through visual perception might be subsequently employed in response to auditory perception, using an SSD as a mediator. They infer from the existence of such BT that the learned skill is amodally represented. After a brief overview I identify ways to more fully test for BT within this experimental paradigm and argue that their conclusion about amodal representation is premature. Additionally, I argue that their preferred SSD (Eyemusic) is of limited value for the project. While my remarks are critical, my intention is to be constructive, particularly in light of the fact that Levy-Tzedek et al. (2012), is, I believe, the first output from Amedi's lab concerning this line of research. (shrink)

This paper aims to deal with the disputes on transferring demonstration between the various sciences in the context of the medicine-geometry relationship. According to Aristotle’s metabasis-prohibition, these two sciences should be located in separate compartments due to the characteristics of their subject-matter. However, a thorough analysis of the critical passage in Aristotle’s Posterior Analytics on circular wounds forces a revision of the boundaries of the interactions between sciences, since subsequently Avicenna, on the grounds of this passage, would widen the area (...) of the transference of demonstration. Furthermore, the fact that Avicenna and Ibn al-Nafīs continued to use geometrical demonstrations in their anatomical investigations shows the need to understand kind-crossing prohibition as a reminder to take into account the present scientific infrastructure and logical rules before proceeding onto a scientific investigation instead of accepting it as a mere nominal doctrine. Therefore, whether kind-crossing was possible or not depended on the extent to which the conclusion derived at the end of the scientific investigation, using a different method after taking into account all these reminders, had contributed to the solution of a particular proposition or the achievement of an approximate truth. The difficulty in historical research is to develop an exchange with the "other" that is both sensitive to transferential displacement and open to the challenge of the "other's" voice. Contemporary sociocultural history has often tended simply to reverse the assumptions of an abstracted history of ideas and replicate its documentary treatment of artifacts as symptoms of society and economy rather than of mind. Its populism replicates the scapegoating propensities of populism in society. Even the best historians, Carl Schorske and Robert (...) Darnton, have tended to deny the contestatory dimensions of high culture and the challenge of forging new links between it and popular culture. Everyone is a mentalito~ case, but certain artifacts are exceptional products of cultural activity with critical power and an uncanny ability to play uncommon variations on commonplace things. Would a just society or government absolutely refrain from shaming or humiliating any of its members? "No," says this essay. It describes morally acceptable uses of shame, stigma and disgust as tools of social control in a decent (just) society. These uses involve criminal law, tort law, and informal social norms. The standard of moral acceptability proposed for determining the line is a version of perfectionistic prioritarian consequenstialism. From this standpoint, criticism is developed against Martha Nussbaum's view that to (...) respect the dignity of each person, society absolutely must refrain from certain ways of shaming and humiliating its members and rendering them objects of communal disgust. According to a dominant view of stigma, a person is stigmatized within a community if sufficiently many people within that community hold a bad view of her. I call this the 'Bad View Model'. In this paper, I argue against the Bad View Model on the grounds that such beliefs are neither necessary nor sufficient for stigma, and that the account cannot explain the distinctive phenomenology of stigma, including certain vulnerabilities to shame. I then develop an alternative (...) that explains these features of stigma, which I call the 'Shaming Model'. On this view, a person is stigmatized within a community if she is shamed by members of that community, and this is explained by their belief that she has deviated from some social norm and/or standard.
