IntroductionRheumatologists are the primary healthcare professionals responsible for patients with rheumatic diseases and should acquire medical ethical competencies, such as the informed consent process (ICP). The objective clinical structured examination is a valuable tool for assessing clinical competencies. We report the performance of 90 rheumatologist trainees participating in a station designed to evaluate the ICP during the 2018 and 2019 national accreditations.MethodsThe station was validated and represented a medical encounter in which the rheumatologist informed a patient with systemic lupus erythematosus (...) with clinically active nephritis about renal biopsy. A trained patient–actor and an evaluator were instructed to assess ICP skills (with a focus on kidney biopsy benefits, how the biopsy is done and potential complications) in obtaining formal informed consent, delivering bad news and overall communication with patients. The evaluator used a tailored checklist and form.ResultsCandidate performance varied with ICP content and was superior for potential benefit information (achieved by 98.9% of the candidates) but significantly reduced for potential complications (37.8%) and biopsy description (42.2%). Only 17.8% of the candidates mentioned the legal perspective of ICP. Death (as a potential complication) was omitted by the majority of the candidates (93.3%); after the patient–actor challenged candidates, only 57.1% of them gave a clear and positive answer. Evaluators frequently rated candidate communications skills as superior (≥80%), but ≥1 negative aspect was identified in 69% of the candidates.ConclusionsEthical competencies are mandatory for professional rheumatologists. It seems necessary to include an ethics competency framework in the curriculum throughout the rheumatology residency. (shrink)

In agreement with Behrendt & Young (B&Y), we considered the role of perception disturbances in schizophrenia in our first clinical approaches, using the Bender test with schizophrenic patients. Following this, we reproduced nuclear symptoms of schizophrenia in animal models, showing that perceptual disturbances, acquisition disturbances, and decrease in affective levels can be induced by glutamatergic blockade within the nucleus accumbens septi. Our results link the proposed corticostriatal dysfunction with the thalamocortical disturbances underlying perceptual problems reviewed by B&Y.

El objetivo fue examinar el valor del ocio lúdico y cultural compartido entre abuelos y nietos para el bienestar de ambas generaciones. Se utilizaron técnicas cuantitativas -cuestionario ad hoc administrado a 1080 estudiantes de primaria- y cualitativas-2 grupos de discusión-. Las actividades lúdicas y culturales fueron preferentesen el ocio compartido, practicándose más con las abuelas. Como aportacionesdestacaron el incremento de la comunicación, la diversión, el aprendizaje mutuo y elbienestar afectivo. The doctrine of abstraction is fundamental to the Aristotelian-Thomistic theory of knowledge. The article seeks to present the foundation for this doctrine, from both the epistemological and the ontological points of view, examining the conditions in which abstraction is legitimate, according to its various modes, giving rise to different types of knowledge and grounding the division of the speculative sciences. The analysis is based above all upon the profound analysis of the Angelic Doctor in his commentary on the De Trinitate (...) of Boethius, but also comparing this with other parallel texts in his Opera Omnia. In this essay, I argue that pronatalism—a social bias in favor of gestational motherhood—and geneticism—a social bias in favor of genetic motherhood—are conceptually and operationally distinct social forces that influence some women's reproductive decision making. Each of these social forces shapes the reproductive landscape, relates differently to women's identities, and causes different social stigmatization and harm. Pronatalism and geneticism warrant feminist concern because they can compromise some women's reproductive autonomy and well-being. I suggest that combating pronatalism and geneticism will require (...) different sets of media, policy, education, and health care practice strategies. (shrink)

In this paper, I propose and defend a pair of necessary conditions on evidence-based knowledge which bear resemblance to the troubled sensitivity principles defended in the philosophical literature. We can think of the traditional principles as simple but inaccurate approximations of the new proposals. Insofar as the old principles are intuitive and used in scientific and philosophical contexts, but are plausibly false, there’s a real need to develop precise and correct formulations. These new renditions turned out to be more cautious, (...) so they won’t be able to do everything the old principled promised they could. For example, they respect closure for knowledge. But these sober formulations, or something like them, might be the best that we can do with respect to sensitivity. And there’s value in understanding the limits to these types of principles. Individuals and businesses make numerous payments every day. They sometimes have choices about what forms of payment to make or accept, and at other times are effectively forced to use a particular form. Often there is an asymmetric power relationship between payer and payee that raises the issue of whether one side unfairly exploits the other. Is it unethical exploitation for an employer to pay employees with a fee-laden payroll card over other more convenient forms of payment? Does the fee (...) structure of payment networks such as Visa and MasterCard unfairly exploit merchants? The bitcoin payment system is an ethical as well as technological evolution as it was designed to be an electronic payment system that does not rely upon trust. Can an entire payment system like bitcoin be “evil,” as charged by Krugman? Payment tools as such are ethically neutral, but can be used in an ethical or unethical manner. (shrink)

This paper claims that we have direct and complete perceptual access to other people’s emotions in their bodily and behavioural expression. The claim is understood, not by analogy with the perception of three-dimensional objects or physical processes, but as a form of Gestalt perception. In addition, talk of direct perceptual access to others’ emotions is shown not to entail a behaviourist view of mind; and talk of complete perceptual access is shown to include both the phenomenological character and the dispositional (...) nature of emotions. The lectures we examine in this essay, and which took place in 1978, are at the genesis of the trilogy Sentient Intelligence by X. Zubiri. In the first place, they demonstrate that Zubiri`s Philosophical project does not fit in with today´s fashionable concept of new realism, or with idealism. In the second place, they make it clear that what he considers Actuality cannot be understood as the passivity of intelligence. And lastly, maybe more than in any of his other writings, (...) these lectures concisely clarify how reality is meant to be understood in his work, as what he refers to as total concept. In all, these lectures needed an examination to reveal these key concepts and is the objective of the article at hand. (shrink)

La cultura occidental, con la que vivimos y entendemos, nació bajo los auspicios de un triple ente: la conjugación de lacultura arábigo-musulmana, la civilización germano-eslava y el mundo clásico-bíblico; todos éstos vendrían a encajar, en principio, los miembros articulados del hombre moderno occidental. En concreto, las lenguas modernas han resultado un vehículo habitual y pertinente de la cultura clásica, en mayor o menor proporción y la someten a su propio arbitrio, creando una nueva dinámica literaria, determinada por razones estilísticas o (...) de prestigio. Por ello, no dejan de ser preocupantes los constantes vaivenes de una sociedad ávida de informaciones y conocimientos, sin ningún rumbo a seguir ni alguna meta humanamente satisfactoria. (shrink)

Los objetivos de la investigación fueron describir el desempeño de los estudiantes de educación secundaria en una evaluación directa y una evaluación indirecta de la escritura, y determinar el grado de relación entre el desempeño en ambas evaluaciones. La metodología fue cuantitativa e incluyó la participación de una muestra de estudiantes de curso final de educación secundaria en Chile. Los participantes realizaron una evaluación directa de la escritura, específicamente un ensayo argumentativo que fue calificado en 8 dimensiones; y una evaluación (...) indirecta, que correspondió a ejercicios de uso de conectores y ordenación de enunciados. En el estudio se comprobó que los estudiantes exhiben un desempeño medio-bajo en ambas evaluaciones. En la evaluación directa las dimensiones Ortografía, Coherencia, Cohesión y Uso de argumentos fueron las más descendidas. Por último, se observó que el desempeño en la evaluación directa y la evaluación indirecta de la escritura son independientes entre sí, es decir, no están correlacionados. (shrink)

The IKt-algebras that we investigate in this paper were introduced in the paper An algebraic axiomatization of the Ewald’s intuitionistic tense logic by the first and third author. Now we characterize by topological methods the subdirectly irreducible IKt-algebras and particularly the simple IKt-algebras. Finally, we consider the particular cases of finite IKt-algebras and complete IKt-algebras.

I argue that truth is relative (in the sense recently defended by some prominent analytical philosophers) by focusing on some semantic issues raised by Einstein's theory of relativity together with our ordinary attributions of truth.

Low‐back pain can be invalidating physically as well as mentally. Despite professional help to treat and prevent low‐back pain, the pain often persists, and so do the problems related to low‐back pain. An intervention that made it possible for a significant part of patients with low‐back pain to improve health and well‐being raised the question: Why was it possible to help some and not others? The aim of the present paper was to achieve a deeper understanding of factors patients experienced (...) as helpful in professional support related to low‐back pain. This was explored using a hermeneutic‐phenomenological approach while analysing 20 interviews with patients with low‐back pain purposively chosen interviews conducted in relation to the intervention. An analysis was made using Ricoeur's interpretation theory. Data on both positive and negative experiences were read and reflected upon. We found that healthcare professionals’ adoption of a narrative approach facilitating the patient's perspective was perceived as helpful. Patients experienced this as being taken seriously; an experience that could be explained at a deeper, more nuanced level using Heidegger's philosophy. Facilitating the patient's perspective was conditional not only on the professional obtaining access to the patient's perspective but also on understanding and acknowledging the patient's existence. The challenge for healthcare professionals in this respect is to bridge the gap between the consultation's fact‐focused concern with the medical implications of low‐back pain and the patient's concern with the implications low‐back pain has for his or her personal identity and life. Listening to the patient's perspective in itself supports the subjective recovery process, while also supporting the quality of patient‐centred support and strengthening the patient's trust in its helpfulness. A model of Zeno's dichotomy paradox is presented in Newtonian collision mechanics. One of several resolutions of the paradox illustrates the point that even in Newtonian ontology there is a spacetime weave. In a Newtonian system in which the base rules permit only spatial contact interactions, we find the mechanical emergence of action-at-a-distance effects.

For a standard Finsler F on a manifold M, the domain is the whole tangent bundle T M and the fundamental tensor g is positive-definite. However, in many cases, these two conditions hold in a relaxed form only, namely one has either a psuedo-Finsler metric or a conic Finsler metric. Our aim is twofold. First, we want to give an account of quite a few subtleties that appear under such generalizations, say, for conic pseudo-finsler metrics. Second, we aim to provide (...) some criteria that determine when a pseudo-Finsler metric F obtained as a general homogeneous combination of Finsler metrics and one-forms is again a Finsler-metric -- or, more precisely, that the conic domain on which g remains positive-definite. Such a combination generalizes the known -metrics in different directions. Remarkably, classical examples of Finsler metrics are reobtained and extended, with explicit computations of their fundamental tensors. (shrink)

Se presenta un estudio sobre un aspecto de la fundamentación lógica de la Bibliotecología y Estudios de la Información. Por fundamentación lógica se entiende el análisis de las relaciones entre los componentes del aparato teórico de una disciplina. El aspecto que se pretende realizar es el análisis de las relaciones dialécticas entre dichos componentes. Una relación dialéctica es aquella por la cual se infieren unas categorías a partir del análisis de elementos internos y su interrelación presentes en una categoría inicial. (...) Se parte de una categoría primaria, la cual contiene todos los demás elementos de la estructura teórica de la ciencia. A semejanza de Marx quien en su obra “El Capital” comienza con el concepto “mercancía” para después inferir la teoría de la economía capitalista, nosotros partimos del concepto “documento” para obtener la teoría de la Bibliotecología. Finalmente siguiendo el desarrollo dialéctico del conocimiento se transita de la categoría inicial como relación más simple, a la esencia, después a la apariencia y finalmente a la realidad. Se propone como esencia de la Bibliotecología el satisfacer necesidades de información documental, como apariencia el servicio que se presta al público y como realidad a un sistema de información concreto. A LÓGICA DO SISTEMA CATEGORIAL DA BIBLIOTECONOMIA E ESTUDOS DA INFORMAÇÃO DOCUMENTAL: UMA ANÁLISE DIALÉTICA Resumo Apresenta-se estudo de um aspecto da fundamentação lógica da Biblioteconomia e Estudos da Informação. Por fundamentação lógica entende-se a análise das relações entre os componentes do aparato teórico de uma disciplina. O aspecto que se pretende observar compreende a análise das relações dialéticas entre esses componentes. A relação dialética é aquela pela qual algumas categorias são inferidas a partir da análise de elementos internos e suas interrelações presentes em uma categorização inicial. Parte-se de uma categoria principal que contém todos os outros elementos da estrutura teórica da ciência. Como Marx, que na sua obra “O Capital” começa com o conceito de "mercadoria" para daí inferir sua teoria da economia capitalista, partimos do conceito de "documento" para chegar a uma teoria da Biblioteconomia. Finalmente, após o desenvolvimento dialético do conhecimento, transita-se da categoria incial como a relação mais simples, a essência, e daí para a aparência e, finalmente, para a realidade. Propõe-se como essência da Biblioteconomia a satisfação da informação documental, como aparência o serviço que se presta ao público e como realidade os sistemas de informação concretos. (shrink)

This paper defends a Wittgenstein-inspired conception of the nature of mind and self-knowledge. Thus, it is claimed that the mind is to be conceived as expressive behaviour; and that knowledge of one’s own mind is not to be thought of as a matter of first-person access, i.e. a special sort of access available to oneself alone, but rather as a matter of ordinary access, similar to other people’s. It is also argued that this conception does not undermine the distinctness of (...) the first-person perspective, including the asymmetry with the third-person perspective, which should be construed semantically, rather than epistemically. As a result, an alternative to contemporary conceptions of mind and self-knowledge is provided. (shrink)

I respond to comments from Branden Fitelson, Chad Gonnerman and John Waterman, and Mark Walker. My response to Fitelson concerns how we should understand the notion of “sensitivity” which is central to my account of why we find skeptical premises intuitive. I argue against his recommendation except for a specific type of “loose” skeptical hypothesis. Gonnerman and Waterman push me to say more about the conditions under which we feel or not feel the skeptical pull. I clarify and expand on (...) the theory presented in the book. Finally, Walker presents a new type of skeptical challenge. To deal with this challenge, I distinguish between two types of skeptical hypotheses, a mild and more substantial one. Walker’s skeptical argument concerns mild skepticism. My account is aimed at more substantial accounts. However, I speculate about how we may respond to this challenge. (shrink)

Peacocke's explanation of the first-person concept provides non-circu/ar possession conditions for such a concept, accommodating two different constraints: the noncircularity requirement and Evans's Thesis. In this paper, it is argued that Peacocke's explanation faces some difficulties: on the one hand, it appears unable to meet a serious objection facing the non-circularity requirement; on the other hand, it misunderstands the constitutive constraints imposed by Evans's Thesis on a correct account of the first person.

El artículo se propone mostrar el puntode cruce entre el lenguaje ético y elmetafórico. Para ello emprende primero un análisis de la noción de sentido que parte de la elucidación heideggeriana del fenómeno del mundo, distingue entre la dimensión nominal y verbal del sentido y reinterpreta la concepción de Ricoeur de metáfora como innovación semántica abierta. En segundo lugar, y de acuerdo con Levinas, se caracteriza al lenguajeético como Decir. Finalmente se explicitan las diferentes analogías entreambos lenguajes, su unidad esencial:la (...) noción del lenguaje como puracuestión y su testimonio: la relación de remisión originaria al otro comofuente del sentido. En este artículo el autor realiza una interpretación del libro Sobre el gobierno tiránico del papa del filósofo medieval Guillermo de Ockham. En la exposición se ofrecen argumentos para justificar varias tesis: la primera, desmontar el prejuicio de que la ilustración moderna abandonó o canceló los contenidos y el procedimiento hermenéutico del pensamiento medieval. La segunda consiste en postular que en el libro citado se encuentran las raíces filosóficas de algunos conceptos centrales del pensamiento político y moral del liberalismo moderno (...) y la tercera tesis apunta a mostrar el modo como el pensamiento político de Guillermo de Ockham anticipa o diseña los rasgos esenciales del moderno concepto de opinión pública o razón pública, tan central hoy en los debates de la teoría y la filosofía política. En la a exposición de estas tres cuestiones se articula, finalmente, una reflexión sobre el papel del filósofo como mediadorintérprete entre el poder formal y la potencia informal de los intereses comunes de los individuos, elementos sobre los que bascula el sentido político poder. I selectively summarize some of the main ideas in my book Why We Doubt, focusing on the elements that are discussed in the replies from commentators. In the book, I investigated our skeptical intuitions which form the basis of the skeptic’s arguments. I argue that these intuitive judgments or inclinations to judge are produced by the deployment of a subconscious heuristic, pbs, which serves us well most of the time but misfires in esoteric cases like those associated with global skepticism. (...) I do this by identifying the rational function of pbs and arguing that when we apply this rational function to global skepticism, it doesn’t yield a skeptical result. (shrink)
