Background: As actors with the key responsibility for the protection of human research participants, Research Ethics Committees (RECs) need to be competent and well-resourced in order to fulfil their roles. Despite recent programs designed to strengthen RECs in Africa, much more needs to be accomplished before these committees can function optimally.Objective: To assess training needs for biomedical research ethics evaluation among targeted countries.Methods: Members of RECs operating in three targeted African countries were surveyed between August and November 2007. Before implementing (...) the survey, ethical approvals were obtained from RECs in Switzerland, Cameroon, Mali and Tanzania. Data were collected using a semi-structured questionnaire in English and in French.Results: A total of 74 respondents participated in the study. The participation rate was 68%. Seventy one percent of respondents reported having received some training in research ethics evaluation. This training was given by national institutions (31%) and international institutions (69%). Researchers and REC members were ranked as the top target audiences to be trained. Of 32 topics, the top five training priorities were: basic ethical principles, coverage of applicable laws and regulations, how to conduct ethics review, evaluating informed consent processes and the role of the REC.Conclusion: Although the majority of REC members in the targeted African countries had received training in ethics, they expressed a need for additional training. The results of this survey have been used to design a training program in research ethics evaluation that meets this need. (shrink)

John Dewey's analyses of the relationships among ethical theory, intellectual-growth, and the nature of democratic societies are of continuing interest to social and political philosophers, especially those who hold an evolutionary view of these inquiries. The ontological analysis of society and social facts, recently advanced by John Searle, provides us with an alternative way to approach Dewey's thought that is at variance with traditional Deweyan scholarship. John Dewey’s analysis of the role of emotion in moral reasoning, presented in the later Ethics, led him to conclude that our development of moral reasoning should be less focused on the secondary interest of attention to ourselves or others, and attend to the more complete interests of the welfare and integrity of the social groups in which we participate. In that analysis, Dewey identified the essential role of empathic understanding in moral decisions, referred to by neuroscientists as social (...) intelligence. Dewey’s discussion of the essential role of emotion in these decisions is further supported by research in neuroscience which has established that general intelligence is located in an area of the brain distinct from the area that supports social intelligence, our capacity for empathic experience. These findings suggest that the presence of individuals with developed social intelligence in the groups in which we participate provide increased opportunities for growth. (shrink)

John Dewey’s famous early twentieth-century account of the relationship between education as growth and democratic societies, presented in Democracy and Education, was later rejected by him, because it failed to properly identify the role of societal structures in growth and experience. In the later Ethics, Dewey attempts to correct that omission, and adumbrates the argument required to reconstruct his theory, which is an appeal to the role of institutions in individual growth and experience. It is the contention of this (...) paper that the inadequacy Dewey finds in his early account is more comprehensively explained by means of recent analyses in philosophy of society, especially John Searle’s ontological analysis. Following in Dewey’s direction, a more complete analysis of the role of society in the theory of growth is presented in terms of the deontological analysis of institutions. It is suggested that the current analyses in philosophy of society provide the means for the completion of Dewey’s views of both growth and experience, making Dewey’s account more useful today than it was a century ago. In the conclusion to this multi-part article I first review the discussions carried out around the six essential questions in psychiatric diagnosis – the position taken by Allen Frances on each question, the commentaries on the respective question along with Frances’ responses to the commentaries, and my own view of the multiple discussions. In this review I emphasize that the core question is the first – what is the nature of psychiatric illness – and that in some manner all further (...) questions follow from the first. Following this review I attempt to move the discussion forward, addressing the first question from the perspectives of natural kind analysis and complexity analysis. This reflection leads toward a view of psychiatric disorders – and future nosologies – as far more complex and uncertain than we have imagined. (shrink)

Growth, the central concept in Dewey's ethical naturalism, is typically ignored in commentary on his philosophic analyses. When growth is overlooked, as it is by some of Dewey's most competent reviewers, his treatment of other concepts such as democracy and equality cannot be fully appreciated or understood. Underestimating the pivotal role of growth in Dewey's thinking weakens his account of philosophic naturalism, in which there is current interest in the philosophic literature. The ecology of fear has become a common rhetoric in efforts to support climate mitigation. The thesis of the collapse is an extreme version, asserting the inevitable collapse of the world. Fear, then, becomes the ultimate emotion for spurring action. In this article, drawing on the work of the pragmatist John Dewey, we show that fear is an ambiguous emotion. Dewey stressed the quality of an emotion. Following his reasoning, this article draws a distinction between intense and moderate fear. (...) Intense fear annihilates action, while moderate fear fulfils the conditions for an emotion of quality (in the Deweyan sense), which enables action. For this reason, the thesis of the collapse must be rejected, while an ecology of fear, drawing on moderate fear, may be maintained. (shrink)

Teachers and critics have much to learn from [Harold] Bloom's work, and in this paper I want to try to show what it is we can learn from him and how we might go about it. In doing so, I also mean to analyze his attack upon formal criticism and to consider the merits of that attack. In the end, I propose an assessment of what in my view is the crucial weakness of both formal and dialectical criticism alike. This (...) will involve an explication of the meaning of critical care and an enlargement of our customary understanding of critical method and procedure. . . . In The Anxiety of Influence Bloom presents theory based frankly upon Freudian models, or what Bloom calls Family Romance. Every new poet is caught up in a struggle with his forebears, or precursors. Being Freudian forebears, they naturally both teach the poet and threaten him as teachers. The problem for the poet is to learn from his forebearing family without losing his integral self. If he succeeds he becomes what Bloom calls a "strong poet," and, hence, again quite naturally, he lives to present much trouble to coming generations, who have their own paths to go. We are amused to think that anyone ever felt Byron might have been mad, bad, and dangerous to know. We are not amused by the Cantos. Like Pound’s letters and so much of his prose, the Cantos is difficult to like or enjoy. It is a paradigm of poetic obscurity because its often cryptic style is married to materials which are (...) abstruse, learned, even pedantic. The poem also makes a mockery of poetic form; and then there are those vulgar and bathetic sinking which it repeatedly indulges through its macaronic turns of voice.All that is scandalous, but the worst has not been said. For the Cantos is a fascist epic in a precise historical sense.1 Its racism and anti-Semitism are conceived and pursued in social and political terms at a particular point in time and with reference to certain state policies. Those policies led to a holocaust for which the murder of six million Jews would be the ultimate exponent. That is truly scandalousFor anyone convinced that works of imagination are important to human life, however, the scandal takes a last, cruel twist. Pound’s magnum opus is one of the greatest achievements of modern poetry in any language. That is more a shocking than a controversial idea. It shocks because it is outrageous to think so; but it is in fact a commonplace judgment passed on the poem by nearly every major writer and poet of this century. The greatness of the Cantos was an apparent to Pound’s contemporaries as it has been to his inheritors, to his enemies as to his friends, to those who have sympathized with Pound’s ideas and to those who have fought against them. 1. What is the significance of that loose collective enterprise, sprung up in the aftermath of the sixties, known as L=A=N=G=U=A=G=E Writing? To answer this question I will be taking, initially, a somewhat oblique route. And I shall assume an agreement on several important social and political matters: first, that the United States, following the Second World War, assumed definitive leadership of a capitalist empire; second, that its position of leadership generated a network of internal social contradictions which persist to this (...) day ; third, that this postwar period has been characterized, at the international level, by an extended cold war shadowed by the threat of a global catastrophe, whether deliberate or accidental. Whatever one’s political allegiances, these truths, surely, we hold as self-evident.Postwar American poetry is deployed within that general arena, and to the degree that it is “political” at all, it reflects and responds to that set of overriding circumstances.1 In my view the period ought to be seen as falling into two phases. The first phase stretches from about 1946 to 1973 . This period is dominated by a conflict between various lines of traditional poetry, on one hand, and the countering urgencies of the “New American Poetry” on the other. In the diversity of this last group Donald Allen argued for a unifying “characteristic”: “a total rejection of all those qualities typical of academic verse.”2Of course, this representation of the conflict between “tradition” and “innovation” obscures nearly as much as it clarifies. The New American poets were, in general, must moe inclined to experimentalism than were writers like Richard Wilbur, Anthony Hecht, Louis Simpson, or Donald Justice. But Allen’s declaration can easily conceal the academic and literary characteristics of the innovators. Robert Duncan and Charles Olson, for example, key figures in the New American Poetry, can hardly not be called “literary” or even “academic” poets. If they opened certain new areas in the field of poetic style, no less could and has been said of Lowell, even in his early work. And if Frank O’Hara seems the antithesis of academic work, John Ashbery is, in his own way, its epitome. Yet both appear in Allen’s New American Poetry anthology. Moreover, who can say, between O’Hara and Ashbery, which is the more innovative of the two—so different are their styles of experimentation? 1. In face of the multiple controversies surrounding the DSM process in general and the development of DSM-5 in particular, we have organized a discussion around what we consider six essential questions in further work on the DSM. The six questions involve: 1) the nature of a mental disorder; 2) the definition of mental disorder; 3) the issue of whether, in the current state of psychiatric science, DSM-5 should assume a cautious, conservative posture or an assertive, transformative posture; 4) the role (...) of pragmatic considerations in the construction of DSM-5; 5) the issue of utility of the DSM - whether DSM-III and IV have been designed more for clinicians or researchers, and how this conflict should be dealt with in the new manual; and 6) the possibility and advisability, given all the problems with DSM-III and IV, of designing a different diagnostic system. Part I of this article will take up the first two questions. With the first question, invited commentators express a range of opinion regarding the nature of psychiatric disorders, loosely divided into a realist position that the diagnostic categories represent real diseases that we can accurately name and know with our perceptual abilities, a middle, nominalist position that psychiatric disorders do exist in the real world but that our diagnostic categories are constructs that may or may not accurately represent the disorders out there, and finally a purely constructivist position that the diagnostic categories are simply constructs with no evidence of psychiatric disorders in the real world. The second question again offers a range of opinion as to how we should define a mental or psychiatric disorder, including the possibility that we should not try to formulate a definition. The general introduction, as well as the introductions and conclusions for the specific questions, are written by James Phillips, and the responses to commentaries are written by Allen Frances. These papers span the clinical topics of obsessiveness, sublimation, dreams and self-analysis, and analyzability, and also delve into applied psychoanalysis and art history, with two studies of Vincent van Gogh and another of Alberto Giacometti. These papers not only convey (...) the impressive range of Gedo's own interests, but embody the high scholarly and clinical standards that Gedo has long held, both for himself and for the field in general. Section III offers original contributions to clinical analysis in the form of the consideration of the role of affective engagement in the analyst's "usability"; thoughtful assessment of the perils of parental projection in child analytic work; and comparison of a failed and successful supervision in the same psychoanalytic case. Section IV examines psychoanalysis and the arts, with two further studies of van Gogh, an analytic reading of Nabokov's _Lolita_, and more general examinations of psychoanalysis in relation to dramatic art and film analysis. The volume closes with two provocative scholarly essays bearing on the roots of psychoanalysis: the correspondence between Mabel Dodge and her analysts Smith Ely Jelliffe and A. A. Brill as a vehicle for reviewing the issue of extra- and postanalytic contact between analyst and patient; and an examination of Freud, Lacan, and the uneasy relationships among literature, psychoanalysis, and the female subject. In addition to publishing two clinical papers by the Chicago analyst John Gedo, _The Annual_ introduces readers to two prominent Japanese neuroscientists whose work is relevant to psychoanalysis. Hiroshi Utena links brain development to the individual's freedom to make optimal adaptive choices, whereas Makoto Iwata outlines the modular organization of vision in the brain and then illustrates each modular potential by examining the paintings of four artists: Mondrian, Duchamp, Seurat, and Rothko. Kenneth Newman's sensitive consideration of analyst self-discourse as the outcome of successful management of the countertransference and Frank Summers' astute assessment of the place of self psychology in the history of psychoanalytic ideas are followed by three engaging and instructive studies in applied analysis: Elaine Caruth and Milton Eber's examination of Woody Allen's _The Purple Rose of Cairo _as a metaphoric depiction of the blurring of boundaries in psychotherapy; Frank and Annette Lachmann's study of the creative process of Henrik Ibsen as a self-transformational response to narcissistic injury; and W. W. Meissner's exploration of the role of shame in Vincent van Gogh's life and art. The volume concludes with a provocative contribution to psychoanalytic history: J. Bos's social-constructivist rereading of the _Minutes of the Vienna Psycho-Analytic Society_ with an eye to illuminating why and how psychoanalysis changed during its early years. True to its distinguished lineage, volume 24 of _The Annual_ continues to broaden the conceptual, clinical, and historical vistas of its readers. [W. J. T.] Mitchell focuses on the exemplary status of the Wall of Fame in Sal’s Pizzeria, “an array of signed publicity photos of Italian-American stars in sports, movies, and popular music” . He argues that the Wall “exemplifies the central contradictions of public art” . “The Wall,” he writes, “is important to Sal not just because it displays famous Italians but because they are famous Americans … who have made it possible for Italians to think of themselves as Americans, (...) full-fledged members of the general public sphere” . For Buggin’ Out, the young black customer who angrily objects to the absence of photos of black people, the Wall “signifies exclusion from the public sphere” . Although the streets are saturated with images of “African-American heroes,” those “tokens of self-respect” are not enough for Buggin’ Out, who wants “the respect of whites, the acknowledgment that African-Americans are hyphenated Americans, too, just like Italians” . Mitchell astutely interprets the desired integration of the Wall as merely a symptom of a larger struggle for “full economic participation. As long as blacks do not own private property in this society,” he states, “they remain in something like the status of public art, mere ornaments to the public place, entertaining statues and abstract caricatures rather than full human beings” . By foregrounding the economic implications of the film, Mitchell has surely engaged one of the dominant goals of the man who formed Forty Acres and a Mule Productions and who recently opened the store called Spike’s Joint in New York City. Yet Mitchell’s sympathetic account belies the countercurrents that trouble the ostensible progressiveness of Spike Lee’s ambitious art. Volume 22 of _The Annual of Psychoanalysis_ begins with the provocative reflections of Jane Flax and Robert Michels on the current status and future prospects of psychoanalysis a century after Freud. Flax believes that analysis will not survive in the postmodern West if analysts cling to the medical model and the notion of analysis as a clinical science; Michels believes analysis will be revivified in the next century by reorganizing its training institutes within universities. A section on "Psychoanalysis and the (...) Visual Arts" includes John Gedo's probing examination of the inner world of Paul Gauguin and William Meissner's reflections on Vincent van Gogh as artist. Johann Michael Rotmann's examination of the transferential meanings of third-party payment within Germany's health insurance system is a timely consideration of issues that are increasingly salient for American analysts and therapists. A rich harvest of theoretical and clinical papers rounds out this volume of _The Annual_. In "A Time of Questioning," Leon Wurmser responds to the inner and outer challenges before analysis in a closely reasoned defense of the applicability of classical analytic technique to severely disturbed patients. Michael Hoit examines the noninterpretive, interactive aspects of the analytic relationship, arguing that the analyst's noninterpretive activities are intrinsic to treatment and must be incorporated into the theory of therapeutic action. In "Fables as Psychoanalytic Metaphors," Elaine Caruth describes some psychoanalytic metaphors contained within the lessons of the Aesopian fables, proposing that the fables address interpersonal and group conflicts that often involve moral issues. Wilma Bucci offers her "multiple code theory" as a new model of emotion and mind, based on current theory and research in cognitive science, that can account for clinical concepts and provide coherent framework for empirical research. And Ralph Roughton uses the case history of Laura to underscore the central role of repetition and interaction in the analytic process. Volume 22 will not disappoint readers of this distinguished continuing series. Like its predecessors, it is a thoroughly assembled collection responsive to the conceptual, clinical, and institutional challenges now before the field. (shrink)
