Essential for us humans is the well-being of our interior cosmos, or Saint Teresa of Avila's interior castle, in all interactions with each other \& the greater environment. The year 2009 marked the bicentennial of Charles Darwin's birth and the 150th anniversary of the publication of The Origin of Species. From 1840 to his death in 1882, Darwin was constantly plagued by chronic illnesses that allowed him to work only a few hours at a time and by an obsession with his physical health. Was this the psychosomatic product of stress resulting from the development and public reception to his theory of evolution or the result of a (...) disease or parasite obtained during the world traveler's excursions? In 1977, the author of this volume argued persuasively for the former explanation in his book To Be an Invalid: The Illness of Charles Darwin, now out of print, but considered to be one of the century's most important works on Darwin's life. Expanding and reworking his earlier arguments to take into account new information, this book paints a more intimate portrait of the nature and possible causes of Darwin's lifelong illness, of the ways he and Victorian physicians tried treating it, and how it influenced his scientific work and relations with his family and friends. Not merely to bear what is necessary, still less conceal it—all idealism is mendaciousness in the face of what is necessary—but love it. —Friedrich Nietzsche In (...) her widely circulated book On Death and Dying, Elisabeth Kübler-Ross, M.D., addresses herself to a changed historical context for work in medicine. In societies with increasingly effective medical care, she argues, doctors and nurses find themselves working with patients whose health problems range from imperceptible yet terminal sicknesses to psychosomatic ailments. Today, especially, it is easy to see the reality of her description of this situation: forms of terminal cancer can be detected earlier and earlier in a patient's life, rendering the disconcerting reality of a terminal illness with few or no symptoms. Equally strange for doctors and patients today are forms of environmental illness like chronic fatigue syndrome, a condition suffered by many but still disputed by doctors and researchers. To this situation of medical risk, personal suffering, and therapeutic care, Kübler-Ross prescribes a communicative engagement with the dying. This engagement, she argues, not only teaches doctors about death and dying, but it also provides respite for those who must experience these events.The problem of communicating with the dying, the main focus of Kübler-Ross's book, is nevertheless eclipsed by the other, more notable aspect of On Death and Dying. For out of this book emerged the now very popular understanding of the stages of death, the normal sequence of an individual's emotional reconciliation to dying: the processes of denial and isolation, anger, bargaining, depression—the whole series of intrapsychic processes leading up to a final acceptance of death and dying. Now we live amongst a whole panoply of staged progresses towards the acceptance of trauma: trauma associated not only with death and dying but also with substance abuse, catastrophe, and forms of violence. The COVID-19 pandemic outbreak has led to a general reorganization of health services and an increase in outpatient telemedicine in mental healthcare for physically ill people. Current literature highlights facilitators and obstacles concerning the use of new technologies in psychotherapy, an underrated topic of research in the context of supportive expressive psychotherapy. More insight is needed to explore the characteristics of video in therapeutic alliance for treatment of specific mental disorders experienced in psychosomatics, particularly with people suffering from a disturbed (...) body- and self-image. Using two clinical vignettes, it is the authors’ intention to enrich the critical debate on current knowledge in psychosomatic remote psychotherapy, with special focus on mentalization deficits and their impact on therapeutic alliance in the consultation-liaison psychiatry setting. In particular, we will question the interpersonal processes at stake related to mirroring and the disruption caused by the use of videoconference applications. We will also reflect upon the relationship between the therapeutic alliance and the medical team, and that between patient and psychotherapist. The aim is to improve psychotherapeutic alliance maintained during the pandemic for specific mental disorders and to inform about possible clinical factors that could be the subject of future empirical studies or professional guidelines. Between Sickness and Health is about illness rather than disease, and recovery rather than cure. The book argues that illness is an experience, represented by the feeling that 'I am not myself'. From the book's phenomenological point of view, feelings of illness cannot be 'unreal' or 'fake', whatever their biological basis, nor need they be categorised as 'physical', 'psychosomatic' or 'psychiatric'. The book challenges the disease-centred ethos of medicine and medical education. It demonstrates that a clearer (...) conception of illness, as distinct from disease, is therapeutic. The feeling that 'I am once again myself' can return, in some degree, whatever state the body is in. Resilience becomes more available when it is seen as a set of personal skills that can be developed, rather than as an inborn trait. It is argued that a radical relocation of subjectivity began several thousand years ago. A subjectivity experienced in the centric region of the heart, and in the body as a whole, began to be avoided in favor of the eccentric head as a new location of subjectivity. In ancient literature, for example in Homer's epics, the heart and various other bodily organs were described as centers of subjectivity and organs of perception for spiritual experience and communion with others and the (...) world. Mind and body were integrated. Bur also in the early historical record, as in the Old Testament, the heart and body were increasingly described as rebellious and rejected as impure. Head and heart, mind and body, became estranged. The body was judged an unsuitable, impure vessel for spiritual experience. This change in the location of subjectivity presaged the later development of Platonic, Gnostic, Christian, and Cartesian distinctions favoring mind over and against the body. It may also have contributed to some of the characteristic psychological and pathological processes (e.g., psychosomatic illnesses, repression, narcissism) currently attributed to the psychology of the modern Western, and specifically, North American self. This paper intends to reflect, on one hand, the need for an integral education of healthcare agents, that addresses the spiritual dimension; and, on the other hand, the need for a better preparation of religious agents that work in hopitals and health centers, so that, in the interest and concern for sick people, dichotomies in the name of faith or in the name of reason are prevented. The failure to eliminate dualisms has been chiefly the failure to discern distinct levels of existence and the complex relations between them: to assert a difference between the organic level and the conscious level need not lead us into an impass of dualism. Mucchielli shows that not all psychosomatic disorders are psycogenetic, but that there are (...) organic illnesses with definite organic etiology. He uses Descartes as a model for style and organization, and revives the relatively ignored Descartes of "the third idea of medicine" and "the union of soul and body," showing how and why Descartes himself did not develop an adequate medical theory of psychosomatic illness. Illness and trauma challenge self-narratives. Traumatized individuals, unable to speak about their experiences, suffer in isolation. In this paper, I explore Kristeva’s theories of the speaking subject and signification, with its symbolic and semiotic modalities, to understand how a person comes to speak the unspeakable. In discussing the origin of the speaking subject, Kristeva employs Plato’s chora (related to choreo , “to make room for”). The chora reflects the mother’s preparation of the child’s entry into language and forms an (...) interior darkroom, the reservoir of lived experience, from which self-narratives issue. Unable to speak of their suffering, traumatized individuals need someone to help them make room for a time of remembrance, someone who is a willing and capable listener. I call such a person a healing witness . Through the mediating presence of the healing witness, fragmented memories of trauma are recreated and incorporated into self-narratives that are sharable with others. Unfortunately, opportunities for witnessing are vanishing. In the last section, I examine the failure of modern media and communication technologies to bear (“hold,” “carry,” “transport”) acts of witnessing. I argue that they perturb the semiotic. According to Kristeva, meaning arises from the dialectical tension between the semiotic (drives and affects) and the symbolic (logic and rules) and is threatened by arid discourse, psychosomatic illnesses, and outbreaks of violence when the semiotic is not represented . Unless we open technology to the imaginary, we risk losing the capacity to bear witness to one another and to create narratives and connections that are meaningful. (shrink)

Throughout the Western world people turn towards the health care system seeking help for a variety of psychosomatic/psychosocial health problems. They become “patients” and find themselves within a system of practises that conceptualizes their bodies as “objective” bodies, treats their ill health in terms of the malfunctioning machine, and compartmentalizes their lived experiences into medically interpreted symptoms and signs of underlying biological dysfunction. The aim of this article is to present an alternative way of describing ill health and rehabilitation (...) using the philosophy of Maurice Merleau-Ponty in order to deepen our understanding of the rehabilitation process. I will explore how the experience of chronic pain ruptures the natural connection between body and world and how the rehabilitation process can be understood as the re-insertion of the body into the flow of experience, where the body “disappears” into its natural silence in order to allow the world to once again unfold. The experience of chronic pain places the painful body in focus, resulting in a diminished articulation of both self and world. Persons with illness suffer not only from the physical aspects of pain and discomfort but also from a loss of identity where one feels alienated and detached from things that used to give meaning to ones life. Rehabilitation must not only address the material (medical) body but also the diminished sense of self as well as the retreat from the world outside of the painful body. (shrink)

Modern socio?cultural studies of medicine demonstrate the symbolic character of much of medical reality. This symbolic reality can be appreciated as mediating the traditional division of medicine into biophysical and human sciences. Comparative studies of medical systems offer a general model for medicine as a human science. These studies document that medicine, from an historical and cross?cultural perspective, is constituted as a cultural system in which symbolic meanings take an active part in disease formation, the classification and cognitive management of (...) illness, and in therapy. Medicine's symbolic reality also forms a bridge between cultural and psychophysiological phenomena; the basis for psychosomatic and socioso?matic pathology and therapy. It can be remedied without revolutionizing either the political or the economic-legal systems of pluralist democratic societies. He certainly belongs in the select, if occasionally scary, company of writers whose name, or that of one of their fictional characters, has lent itself to the designation of a psycho-physical condition (...) often with medical characteristics. A short list includes: Stendhal ('Stendhal syndrome,' the condition of obsessional attachment to works of art); Musset (the 'Musset Sign,' manifested as an uncontrollable tremor of the head); Gogol's Plyushkin in Dead Souls ('compulsive hoarding syndrome'); Dickens's Miss Havisham in Great Expectations ('senile squalor syndrome'); Wilde's Lady Windermere ('Lady Windermere syndrome,' associated with pulmonary illness and breathing difficulties).There is no evidence that Proust was directly acquainted with Wilde's play, but there can be little doubt that what came to be called 'Lady Windermere Syndrome' would have spoken to him loud and clear. This paper aims to interpret the role of the soul as ontological, intellectual or cognitive and as the moral principle within the frame of the holistic conception of human psychosomatic health that emerges from the context of Zalmoxian medicine in the proemium of Plato’s Charmides. It examines what the ontological status of the soul is in relation to the body and the body–soul complex of man considered as a psychosomatic whole. By comparing the presentation of the soul as (...) principle in the Charmides and the Phaedrus, the paper defends the thesis that in the former dialogue, Plato develops his own anthropological ontology, which paves the way for the salvation of human existence and health. The soul is bestowed with an ontological primacy that determines the philosophical and medical presuppositions for treating human illness under a holistic view. The interpretation of the ontological relation of the soul to the body and the entire human being in the context of Zalmoxian holistic medicine is based on Hermias Alexandrinus’ exegesis of the conception of the soul as principle in the Phaedrus. This paper demonstrates that, from both the medical holistic viewpoint and the anthropological philosophical perspective, the soul is the principle and πρῶτον with regard to the body and the body–soul complex without being the whole that the corresponding medical epistemology must apprehend. Definition of the problem: Discussions of “holistic” medical practice confine themselves to the treatment of so-called “psychosomatic disorders”. This paper traces ways in which the patient's personal unity may become apparent to medical practitioners during three critical steps of everyday practice: physical examination, “medical history” (anamnesis) and treatment. Physical examination touches on the patient's “Leiblichkeit” (his “organ” of bodily self-awareness of being a person). Objectifying of the “Leiblichkeit” may constitute an infringement upon the patient's personal unity and is only (...) justified by strict medical indication. The patient not only is ill but he “acts” his illness (subject of and to his disease). Medical history concentrates on the development of a disease but also has to keep in mind the potential biographical meaning of being ill and the patient's biographical identity. The aim of medical treatment, finally, may touch on the patient's existential goals.Conclusion: These three steps of medical practice are not discipline-specific, but span all areas of medical practice. A conceptual model of a partnership in ‘like-minded thinking’ consists of the following components: a relationship, a shared goal with mutual agreement to work toward that goal, and reciprocal encouragement between two people. A like-minded alliance is a relationship that offers support while at the same time encourages hope and establishes a reciprocating emotional attitude of hopefulness.The discussion focuses on the principles of such a model that is designed primarily as a lay intervention for anyone who has a close friend (...) with cancer and who wants to assist the friend in maintaining a hopeful attitude in the face of illness. While this model is not directed at healthcare professionals it may be transferable into psychosocial interventions to assist persons toward sustaining hopefulness in the context of the cancer trajectory. The reason is that doing so would harm black people most of all. As Baldwin (1963, 113) assures his readers,I am very much concerned that American Negroes achieve their freedom here in the United States. But I am also concerned for their dignity, for the health of their souls, and must oppose any attempt that Negroes may make to do to others what has been done to them. I think I know—we see it around us every day—the spiritual wasteland to which that road leads.... I would not like to see Negroes ever arrive at so wretched a condition.Likewise, when Martin Luther King Jr., urged his black church members to love their white enemies rather than return their racial hate, he did so because he believed that “hate distorts the personality of the hater. While Baldwin was an avowed atheist and King a Christian minister, both warned of the devastating effects of white racism on “the souls of white folk,” and they pleaded and preached that black people should not stoop to white people’s debased level.1 Out of all the damage that white racism has done to African Americans and other people of color, the most damaging of all would be for it to turn them into the debilitated, deplorable people that white folk generally are.We might say that on Baldwin’s (and perhaps also King’s) view, white people are the truly “wretched of the earth” (Fanon 1965). It has been argued that white racism has cost white people their capability for intimacy, their affective lives, their authenticity, their sense of connection to other people, and their spiritual selves (Segrest 2001, 65). These claims are not an attempt to trump or erase the enormous physical, psychological, economic, spiritual, and other types of pain that white people have caused people of color around the world. They instead are a recognition that white people’s attempts to gain material riches and political power through the domination of people of color have impoverished and depleted their own souls. White people qua white are ill in that their racial habits largely have been built out of greed, hatred, jealousy, fear, and cruelty. Their psychosomatic health has suffered and continues to suffer because of their toxic racial identities.So much the worse for white people, we might think. But although understandable, this response would be misguided since white people’s psychosomatic depletion has implications for the well-being of others. Put succinctly, white people’s unhealthiness helps fuel their abuse and domination of people of color. It thus matters to struggles for racial justice whether white people are psychosomatically healthy and strong. It matters which affects, emotions, and passions fund white people’s actions in general, and their work for racial justice in particular. In this paper, I will draw from Friedrich Nietzsche, Teresa Brennan, and other scholars to explain the ontological and power-full aspects of affect, touching briefly on the toxicity of white guilt and shame and focusing especially on the healthiness of what Nietzsche calls a bestowing self-love. This article proposes a multidisciplinary work perspective on couples who undergo assisted reproductive technology treatments, with particular attention paid to the treatment of women. The epistemological references underlying a vision of infertility and sterility that respect the psyche–soma unity of the person are illustrated: the biopsychosocial model and the psychoneuroimmunology and psychosomatic contemporary models of health and illness. Based on clinical experience in a private and institutional setting, different steps in the course of treatment are described with the (...) related areas of psychological work: from the search for pregnancy to the choice of adhering to an ART treatment, to conception, up to delivery and beyond. In Phillip Kerr’s 1994 spellbinding novel A Philosophical Investigation, the medical test to which the protagonist refers is a functional brain scan based on positron emission tomography. It is used to run large studies of male and female brains and, following a lead suggested by animal studies, has been used to identify rare cases of human male subjects who lack the ventral medial nucleus. This nucleus, in the experiment, is hypothesized to inhibit the activity of the sexually dimorphic nucleus, a (...) preoptic area of the male brain believed to be a repository of male aggressive response. Prior research, we learn from the plot, shows that 3 in 100,000 human males are VMN-negative. Thirty percent of those affected are believed to be in prison or to have a criminal record; 70% are believed to stabilize levels of aggression by producing increased levels of estrogen. Identities of subjects participating in the brain scan studies are protected through the conversion of real names to those of famous figures of the past — Bertrand Russell, Charles Dickens. Mit den Fortschritten im Bereich der psychiatrisch-genetischen Forschung gehen auch intensive Diskussionen bezüglich der daraus erwachsenden Implikationen einher. Hoffnungen wie Befürchtungen gegenüber der neuen Technologie und deren Folgen werden gleichermaßen zum Ausdruck gebracht. Über die Einstellung der Bevölkerung und insbesondere der Patienten mit affektiven oder schizophrenen Erkrankungen und ihrer Angehörigen in Deutschland war bisher allerdings wenig bekannt. Daher führten wir im Rahmen des Deutschen Humangenomprojektes erstmals eine Studie durch, um die Einstellungen und ihre beeinflussenden Faktoren zu erfassen. Wir entwickelten einen (...) Fragebogen zur Erfassung der Erwartungen, Hoffnungen und Befürchtungen sowie der die Einstellung beeinflussenden Faktoren. Hiermit wurde eine repräsentative Stichprobe von 3077 Personen aus der Allgemeinbevölkerung am Telefon befragt. Weiterhin wurden mit 316 Patienten und 163 Angehörigen persönliche standardisierte Interviews anhand des Fragebogens durchgeführt. Die Mehrzahl der Befragten hatte eine positive Einstellung zu psychiatrisch-genetischer Forschung, äußerte jedoch gleichzeitig ethische Bedenken. Übereinstimmend waren die Befragten der Ansicht, dass Informationen aus genetischen Untersuchungen vertraulich behandelt werden sollten. Kleine, aber signifikante Unterschiede zwischen den Gruppen zeigten sich bezüglich der Einstellung zu präsymptomatischer genetischer Diagnostik. Unsere Ergebnisse weisen auf eine Ambivalenz der Befragten gegenüber der psychiatrisch-genetischen Forschung und Untersuchung hin: Neben einer überwiegend positiven Einstellung wurden auch moralische Bedenken geäußert. The paper begins with the account of a focus group discussion of Hungarian female managers who demonstrated high level of trust. Drawing on the discussion the author explores the nature of trust and looks at works and research findings in different disciplines. In psychology Erikson’s findings on human growth and development are discussed. Representatives of Eastern and Western philosophy are quoted to highlight the underlying differences of thinking in relation to trust. As one of the most compelling technologies for imaging the brain, functional MRI (fMRI) produces measurements and persuasive pictures of research subjects making cognitive judgments and even reasoning through difficult moral decisions. Even after centuries of studying the link between brain and behavior, this capability presents a number of novel significant questions. For example, what are the implications of biologizing human experience? How might neuroimaging disrupt the mysteries of human nature, spirituality, and personal identity?
