Post-modernism offers a revolutionary approach to the study of society: in questioning the validity of modern science and the notion of objective knowledge, this movement discards history, rejects humanism, and resists any truth claims. In this comprehensive assessment of post-modernism, Pauline Rosenau traces its origins in the humanities and describes how its key concepts are today being applied to, and are restructuring, the social sciences. Serving as neither an opponent nor an apologist for the movement, she cuts through post-modernism's often (...) incomprehensible jargon in order to offer all readers a lucid exposition of its propositions. Rosenau shows how the post-modern challenge to reason and rational organization radiates across academic fields. Subsequent to the end of the Cold War, analysts groped for an understanding of the overall structures of world politics that marked the emergence of a new epoch. As a result, the concept of empire became a major preoccupation, with the economic and military power of the United States considered sufficient for regarding it as an empire. Due to the proliferation of new microelectronic technologies and for a variety of other specified reasons, however, the constraints inherent in the new epoch (...) make it seem highly unlikely that the U.S. or any other country can ever achieve the status of an empire. In effect, the substantial shrinkage of time and distance in the current period has led to the replacement of the age of the nation-state that originated with the Treaty of Westphalia in 1648 with the age of the networked individual. It is an age that has developed on a global scale and that has brought an end to the history of empires. (shrink)

BackgroundClinical ethics case consultations (CECCs) provide a structured approach in situations of ethical uncertainty or conflicts. There have been increasing calls in recent years to assess the quality of CECCs by means of empirical research. This study provides detailed data of a descriptive quantitative and qualitative evaluation of a CECC service in a department of cardiology and intensive care at a German university hospital.MethodsSemi-structured document analysis of CECCs was conducted in the period of November 1, 2018, to May 31, 2020. All documents were analysed by two researchers independently.ResultsTwenty-four CECCs were requested within the study period, of which most (n = 22; 92%) had been initiated by physicians of the department. The patients were an average of 79 years old (R: 43–96), and 14 (58%) patients were female. The median length of stay prior to request was 12.5 days (R: 1–65 days). The most frequent diagnoses (several diagnoses possible) were cardiology-related (n = 29), followed by sepsis (n = 11) and cancer (n = 6). Twenty patients lacked decisional capacity. The main reason for a CECC request was uncertainty about the balancing of potential benefit and harm related to the medically indicated treatment (n = 18). Further reasons included differing views regarding the best individual treatment option between health professionals and patients (n = 3) or between different team members (n = 3). Consensus between participants could be reached in 18 (75%) consultations. The implementation of a disease specific treatment intervention was recommended in five cases. Palliative care and limitation of further disease specific interventions was recommended in 12 cases.ConclusionsTo the best of our knowledge, this is the first in-depth evaluation of a CECC service set up for an academic department of cardiology and intensive medical care. Patient characteristics and the issues deliberated during CECC provide a starting point for the development and testing of more tailored clinical ethics support services and research on CECC outcomes. In the Netherlands, current policy opinion emphasizes demand-driven health care. Central to this model is the view, advocated by some Dutch health policy makers, that patients should be encouraged to be aware of and make use of health quality and health outcomes information in making personal health care provider choices. The success of the new health care system in the Netherlands is premised on this being the case. After a literature review and description of the new Dutch health care system, (...) the adequacy of this demand-driven health policy is tested. The data from a July 2005, self-administered questionnaire survey of 409 patients (response rate of 94%) as to how they choose a hospital are presented. Results indicate that most patients did not choose by actively employing available quality and outcome information. They were, rather, referred by their general practitioner. Hospital choice is highly related to the importance a patient attaches to his or her physician’s opinion about a hospital. Some patients indicated that their hospital choice was affected by the reputation of the hospital, by the distance they lived from the hospital, etc. but physician’s advice was, by far, the most important factor. Policy consequences are important; the assumptions underlying the demand-driven model of patient health provider choice are inadequate to explain the pattern of observed responses. An alternative, more adequate model is required, one that takes into account the patient’s confidence in physician referral and advice. (shrink)

The study is designed to provide an informal summary of what is known about consumer switching of health insurance plans and to contribute to knowledge about what motivates consumers who choose to switch health plans. Do consumers switch plans largely on the basis of critical reflection and assessment of information about the quality, and price? The literature suggests that switching is complicated, not always possible, and often overwhelming to consumers. Price does not always determine choice. Quality is very hard for (...) consumers to understand. Results from a random sample survey ( n = 2791) of the Alkmaar region of the Netherlands are reported here. They suggest that rather than embracing the opportunity to be active critical consumers, individuals are more likely to avoid this role by handing this activity off to a group purchasing organization. There is little evidence that consumers switch plans on the basis of critical reflection and assessment of information about quality and price. The new data reported here confirm the importance of a group purchasing organizations. In a free-market-health insurance system confidence in purchasing groups may be more important for health insurance choice than health informatics. This is not what policy makers expected and might result a less efficient health insurance market system. (shrink)

Despite the flood of philosophical texts on postmodernism, relatively few attempts have been made to gauge the importance of postmodern ideas for the philosophy of science. However, Lyotard's enormously influential text The postmodern condition (1979) focussed on science and knowledge. He put the term metanarrative (grand narrative) into circulation. Lyotard defines the term modern to refer to the way in which science tries to legitimate its own status by means of philosophical discourse which appeal to some kind of grand narrative (...) (Lyotard 1984:xxiii). Science needs to legitimate itself as being true knowledge by making use of another kind of knowledge, which Lyotard calls narrative knowledge (Lyotard 1984:29,30). Without this legitimation science would presuppose its own validity and proceed on prejudice (Lyotard 1984:29). He examines two such grand narratives that previously legitimated science in the modern world, but now have lost their credibility. One is science as the liberator of humanity and the other science as a good influence on the character of its participants. He describes his now famous definition of the concept postmodern, namely an incredulity toward metanarratives, as being extremely simplified (Lyotard 1984:xxiv). Lyotard (1984:18-27) also strongly focuses attention on the important role of narratives in human life in that narratives provide a certain kind of knowledge that cannot be had in any other way. He regards narrative and scientific knowledge as two distinct species of discourse (Lyotard 1984:26-27), which both fulfill legitimate functions and no one's existence is more or less necessary than the other's. Lyotard himself sees the function of legitimation as the primary role for narrative knowledge, and discusses it mostly in that context (1984:27-37). In this article I want to look at philosophers who give different appraisals of the merits of postmodern ideas - with Lyotardian ones featuring prominently - for philosophy of science and our understanding of science. I will examine the attempts by Nancey Murphy (1990), Pauline-Marie Rosenau (1992), Zuzana Parusnikova (1992) and Joseph Rouse (1990, 1991a, 1991b) to develop (or reject) a postmodern philosophy of science. In a final section I will determine the worth of their views for our understanding of science and philosophy of science. (shrink)

Ces thèmes sont au coeur des différents textes qui composent ce livre dont le point commun est que, comme peut-être aucune autre science, la philosophie vit de la réflexion des penseurs sur ce qui donne à penser et de la discussion avec d'autres (...) penseurs. C'est ici que se situe l'étude de sa propre tradition, comme l'a montré Dominique Kahang'a Rukonkish. Ceci est particulièrement nécessaire dans des situations de crise comme celle que traverse l'Afrique contemporaine. Ce qui importe dans ce cas est de questionner le passé à partir du contexte actuel en vue de ses potentialités à venir, tout en l'ouvrant à la possibilité d'une rencontre avec les autres traditions. Les auteurs des contributions réunies dans ce livre remercient Dominique Kahang'a Rukonkish pour ce qu'ils sont devenus grâce à lui. C'est pour eux un devoir de mémoire auquel ils se plient. What recourse do democrats have if a present generation uses democratic legal procedures to abrogate constitutional essentials, dissolving past commitments and denying future generations the fundamental freedoms and equalities it currently enjoys? In Sovereignty across Generations, Alessandro Ferrara responds to this issue by re-examining key concepts related to democracy, including sovereignty, constituent power, liberalism and constitutionalism. This review analyses Ferrara’s theory of ‘sequential sovereignty’, which reconceives ‘the people’ as a body spanning generations and grounds it in the ideal of vertical (...) reciprocity. Ferrara provides a sound normative framework for understanding transgenerational obligations and the unamendability of constitutional essentials. However, this review also identifies some limits to Ferrara’s approach: his focus on implied unamendability risks creating a dissonance between legality and legitimacy, and his reluctance to fully engage with ‘containment’ creates a blind spot in democracy’s self-defence. The review concludes that explicit unamendability and containment are consistent with Ferrara’s theory of sequential sovereignty and provide a better safeguard for democracy across generations. Serving as a healthcare worker in a conflict zone is an experience that is characterized by peculiar and unimaginable challenges. This commentary is an exposition on twelve collated stories of healthcare providers currently serving or who have previously served in war. The stories bring to bear the heaviness of emotions such as fear and guilt that the authors grappled with, while concurrently showing that they embody virtues such as altruism, self-sacrifice, courage, and solidarity. In these stories, we see highlighted recurrent (...) ethical themes including the tension between the duty to others and the duty to self, prioritization and allocation of scarce health resources, ethics of research in war, ethics of virtue and the lasting effects of war. This commentary thus explores and heightens our awareness of the interplay between personal morals and the translation of these into ethics of healthcare provision in war. In so doing, the commentary urges us to reflect on ways by which we can engage the discourse pertaining to war and healthcare ethics. (shrink)
