It is commonly assumed that grounding relations are asymmetric. Here I develop and argue for a theory of metaphysical structure that takes grounding to be nonsymmetric rather than asymmetric. Even without infinite descending chains of dependence, it might be that every entity is grounded in some other entity. Having first addressed an immediate objection to the position under discussion, I introduce two examples of symmetric grounding. I give three arguments for the view that grounding is nonsymmetric (I call this view (...) ‘metaphysical interdependence’). These arguments are: (i) that metaphysical interdependence is the only theory able to reconcile competing intuitions about grounding; (ii) that it is the only theory consistent with both ‘gunk’ and ‘junk’; and (iii) that offers a satisfactory solution to the problem concerning whether or not grounding is itself grounded. Attempts to elucidate grounding are often made by connecting grounding to metaphysical explanation, but the notion of metaphysical explanation is itself opaque, and has received little attention in the literature. We can appeal to theories of explanation in the philosophy of science to give us a characterization of metaphysical explanation, but this reveals a tension between three theses: that grounding relations are objective and mind-independent; that there are pragmatic elements to metaphysical explanation; and that grounding and metaphysical explanation share a (...) close connection. Holding fixed the mind-independence of grounding, I show that neither horn of the resultant dilemma can be blunted. Consequently, we should reject the assumption that grounding relations are mind-independent. (shrink)

The perfectly natural properties and relations are special—they are all and only those that "carve nature at its joints." They act as reference magnets, form a minimal supervenience base, figure in fundamental physics and in the laws of nature, and never divide duplicates within or between worlds. If the perfectly natural properties are the (metaphysically) important ones, we should expect being a perfectly natural property to itself be one of the (perfectly) natural properties. It is now conventional wisdom that conscious experience — or in Nagel’s canonical characterization, “what it is like to be” for an organism — is what makes the mind-body problem so intractable. By the same token, our current conceptions of the mind-body relation are inadequate and some conceptual development is urgently needed. Our overall aim in this paper is to make some progress towards that conceptual development. We first examine a currently neglected, yet fundamental aspect of consciousness. This aspect is (...) the spontaneity of consciousness, by which we mean its inner plasticity and inner purposiveness. We then sketch a “neurophenomenological” framework for thinking about the relationship between the spontaneity of consciousness and dynamic patterns of brain activity as studied in cognitive neuroscience. We conclude by proposing that the conscious mentality of sentient organisms or animals is active and dynamic, and that this “enactive” conception of consciousness can help us to move beyond the classical dichotomy between materialism and dualism. Background There is no definition of what constitutes a lie when working with people with moderate to severe dementia. Lies are often defined as therapeutic with no evidence of how therapeutic value is gauged. There is no previous research that observes lies being told or the impact the lies have on people with dementia. Aim The aim was to develop a taxonomy of lies for use when supporting people with moderate to severe dementia and then use this to develop a (...) model, which could be used to explore the impact of lie telling. Research design Ethnography was used, with the researcher observing and recording lies told to people with moderate to severe dementia, by professional caregivers. Data analysis was based on Burnard’s thematic analysis. Ethical consideration Ethical approval for the study was obtained from Northumbria University, the Research Ethics Committee (REC) and the Health Research Authority (HRA) in the United Kingdom. Study number 227508. Findings Six categories of lie were identified; blatant, avoidance, familiarity, props, banter and going along with. Spontaneous and planned lies were observed. Motivation, validation and genuineness were key in terms of how the person with dementia responded. These affective domains were used to develop the Lie ARM (affective reflective model). Discussion Planned and spontaneous lies are an effective intervention for people with moderate to severe dementia. Healthcare professionals need to be aware of their motivation for using lies, as well as the impact they have on patients. Conclusion The taxonomy is an objective way to categorise lies. It supports increased reflection, particularly if used in conjunction with the Lie ARM. The Lie ARM is a useful tool to help staff reflect on practice or to predict whether a proposed lie is likely to be therapeutic. (shrink)

This is a study in the philosophy of social science. It takes the form of a comparative critique of three contemporary approaches: ordinary language philosophy, hermeneutics and critical theory, represented here respectively by Ludwig Wittgenstein, Paul Ricoeur and Jürgen Habermas. Part I is devoted to an exposition of these authors' views and of the traditions to which they belong. Its unifying thread is their common concern with language, a concern which nonetheless reveals important differences of approach. For whereas ordinary language (...) philosophers tend to treat linguistic activity as the ultimate object of inquiry, both Ricoeur and Habermas regard it as a medium which betrays more fundamental dimensions of human experience and the social world. Part II complements the exposition with a critical analysis of its central themes: the conceptualisation of action, the methodology of interpretation, and the theory of reference and truth. The author defends many aspects of the work of Ricoeur and Habermas, such as the emphasis on power and ideology, the strategy of depth interpretation, and the link between consensus and truth; but he argues that there are serious deficiencies and obscurities in their work. He proposes solutions to these difficulties and concludes with a sketch of a critical and rationally justified theory for the interpretation of action - a critical hermeneutics. (shrink)

Background Ethical climate refers to the shared perception of ethical norms and sets the scope for what is ethical and acceptable behaviour within teams. Aim This paper sought to explore perceptions of ethical climate amongst healthcare workers as measured by the Ethical Climate Questionnaire (ECQ), the Hospital Ethical Climate Survey (HECS) and the Ethics Environment Questionnaire (EEQ). Methods A systematic review and meta-analysis was utilised. PSYCINFO, CINAHL, WEB OF SCIENCE, MEDLINE and EMBASE were searched, and papers were included if they (...) sampled healthcare workers and used the ECQ, HECS or EEQ. Ethical consideration Ethical approval was not required. Results The search returned 1020 results. After screening, 61 papers were included ( n = 43 HECS, n = 15 ECQ, n = 3 EEQ). The overall sample size was over 17,000. The pooled mean score for the HECS was 3.60. Mean scores of individual studies ranged from 2.97 to 4.5. For the HECS studies, meta-regression was carried out. No relationship was found between the country of the studies, the study setting (ICU v non-ICU settings) or the mean years of experience that the sample had. For the ECQ, sub-scales had mean scores ranging from 3.41 (instrumental) to 4.34 (law) and were all observed to have significant and substantial heterogeneity. Three studies utilised the EEQ so further analysis was not carried out. Conclusions The above results provide insight into the variability of scores as measured by the HECS, ECQ and EEQ. To some extent, this variability is not surprising with studies carried out across 21 countries and in a range of healthcare systems. Results also suggest that it may be that more local and context specific factors are more important when it comes to predicting ethical climate. (shrink)

Social contract theory has been criticized as a “theory in search of application.” We argue that incorporating the nano, or individual, level of analysis into social contract inquiry will yield more descriptive theory. We draw upon the psychological contract perspective to address two critiques of social contract theory: its rigid macro-orientation and inattention to the process of contract formation. We demonstrate how a psychological contract approach offers practical insight into the impact of social contracting on day-to-day human interaction. This article critically assesses the moral arguments that speak in favour of three consumer options: buying local food, buying global (non‐local) food, and buying global food while also purchasing carbon offsets to mitigate the environmental impact of food transportation. We argue that because the offsetting option allows one to provide economic benefits to the poorest food workers while also mitigating the environmental impact of food transportation it is morally superior to the alternatives.

The difficulty of handling complex problems has spawned challenges to the traditional paradigm of technical rationality in design, planning, and policy making. One of the most frequently proposed solutions is an interdisciplinary approach, though few writers have described the operational dynamics of such an approach. A global model of interdisciplinary problem-solving is presented based on the premise that the unity of the interdisciplinary approach derives from the creation of an intermediary process that relies on common language, shared information, a mutual (...) sense of stakeholding, and the resolution of disciplinary differences. The theoretical underpinning of this approach is the conceptualization of interdisciplinary problem-solving as a communicative process that requires attention to the rhetorical and political dynamics of working with competing interests, practices, and disciplines. The practice portion is a composite picture of effective models, skills, activities, strategies, and techniques employed by actuals interdisciplinary teams. This global model offers a way of both theoretically and practically visualizing Th. K. van Lohuizen's ideal of achieving unity of town planning, an ideal that has profound implications for the organization of both professional practice and training. (shrink)

When scientists consider the interaction of science and value judgments, debates often occur. When public policy grows out of science, disagreements between scientists can become even more spirited. This paper examines the case of nutrition policy in the United States, which has been both at the interface between agriculture and medicine and the object of serious discord concerned with the strength and validity of the scientific evidence and the responsibility for action. The development of indirect intervention policies, designed to educate (...) and inform the public on diet and health, is traced as a practical demonstration of the effects of involvements of nutritional scientists of different disciplines and philosophic bents. Controversies centered mainly on the issues of diet and coronary heart disease and of diet and cancer in nutritional guidelines for Americans and the recommended dietary allowances. But the arguments turned on a complex web of values and interests as well as scientific questions. A remarkable turnaround by animal agriculture and its scientific support occurred, changing from a defensive, rightist stance to one that appears to recognize a moral responsibility to the public health. The convincing point was likely the changing market. Nutritional scientists, however, remained divided on the issue of whether a public health strategy keyed to public education should replace a strategy to identify persons of high risk and modify the risk by treatment. Our analysis suggests that the tension between libertarian and utilitarian social values of scientists is at least as important as disagreements relative to validity and strengths of the scientific evidence. According to the relational approach we have obligations to members of future generations not because of their interests or properties but because, and only because, they are our descendants or successors. Common accounts of relational duties do not explain how we can have obligations to people who do not yet exist. In this defence of the relational approach I examine three sources of intergenerational obligations: the concern of parents for their children, including their future children; the desire of community members (...) to pass on a heritage to their descendants; and the relationship of citizens in an intergenerational polity. BackgroundFunding agencies have long used panel discussion in the peer review of research grant proposals as a way to utilize a set of expertise and perspectives in making funding decisions. Little research has examined the quality of panel discussions and how effectively they are facilitated.MethodsHere, we present a mixed-method analysis of data from a survey of reviewers focused on their perceptions of the quality, effectiveness, and influence of panel discussion from their last peer review experience.ResultsReviewers indicated that panel discussions were (...) viewed favorably in terms of participation, clarifying differing opinions, informing unassigned reviewers, and chair facilitation. However, some reviewers mentioned issues with panel discussions, including an uneven focus, limited participation from unassigned reviewers, and short discussion times. Most reviewers felt the discussions affected the review outcome, helped in choosing the best science, and were generally fair and balanced. However, those who felt the discussion did not affect the outcome were also more likely to evaluate panel communication negatively, and several reviewers mentioned potential sources of bias related to the discussion. While respondents strongly acknowledged the importance of the chair in ensuring appropriate facilitation of the discussion to influence scoring and to limit the influence of potential sources of bias from the discussion on scoring, nearly a third of respondents did not find the chair of their most recent panel to have performed these roles effectively.ConclusionsIt is likely that improving chair training in the management of discussion as well as creating review procedures that are informed by the science of leadership and team communication would improve review processes and proposal review reliability. Loyalty, whether moral duty or dangerous attachment, is a cognitive phenomenon — an attitude that resides in the mind of the individual. In this article, weconsider loyalty from a psychological contract perspective – that is, as an individual-level construction of perceived reciprocal obligations. Viewing loyalty in this way helps clarify definitional inconsistencies, provides a finer-grained analysis of the concept, and sheds additional light on the ethical implications of loyalty in organizations. Training residents to become humanistic physicians capable of empathy, compassionate communication, and holistic patient care is among our most important tasks as physician educators. Narrative medicine aims to foster those highly desirable characteristics, and previous studies have shown it to be successful in fostering self-reflection, emotional processing, and preventing burnout. We aimed to evaluate pediatric residents’ perceptions of a novel narrative medicine curriculum. After the initiation of a longitudinal narrative medicine curriculum, focus groups were conducted with residents who participated in (...) at least one narrative medicine session. The curriculum was viewed positively, and residents found the sessions to be helpful in developing empathy, offering a space for reflection, and introducing new perspectives. Challenges noted were perception of relevance, timing of sessions, and interpretation by non-native English-speaking residents. With attention to linguistics and thematic undertones, narrative medicine is a feasible, replicable, and accepted teaching modality for pediatric residents to foster empathy, process emotions, and participate in self-reflection.
