Following the introduction of do-not-resuscitate orders in the 1970s, there was widespread misinterpretation of the term among healthcare professionals. In this brief report, we present findings from a survey of healthcare professionals. Our aim was to examine current understanding of the term do-not-attempt-resuscitate, decision-making surrounding DNAR and awareness of current guidelines. The survey was distributed to doctors and nurses in a university teaching hospital and affiliated primary care physicians in Dublin via email and by hard copy at educational meetings from (...) July to December 2014. A total of 519 completed the survey. The response rate in the hospital doctors group was 35.5%, 19.8% in the nurses group but 68.8% in the specialist nurses group and 40% in the primary care physician group.Alarmingly, our results demonstrate that 26.8% of staff nurses and 30% of primary care physicians surveyed believed that a patient with a DNAR order could not receive any/at least one of a list of simple treatments including antibiotics, physiotherapy, intravenous fluids, pain relief, oxygen, nasogastric feeding or airway suctioning, which were higher percentages compared to the other hospital doctors and experienced nurses groups with statistically significant differences. Furthermore, a higher percentage of staff nurses and primary care physicians believed that a patient with a DNAR order could not be referred to hospital from home/a nursing home, when compared with other healthcare groups. Our findings highlight continued misunderstanding and over-interpretation of DNAR orders. Further collaboration and information is required for meaningful Advance Care Plans. (shrink)

So my keen interest in this book and its topic is more than just theological [End Page 209] curiosity. It includes personal and emotional dimensions, as I'm sure it will for many readers.This book is an attempt to understand modern adoptive practices from a primarily Christian/Catholic perspective, with helpful allusions to the beliefs and practices of the other Abrahamic faiths. Overall, I believe the book succeeds in presenting a solid theological foundation for adoption in general and, more specifically, for its current mainstream manifestation in the developed world. The author doesn't shy away from the various controversies related to adoption—like assisted reproduction, LGBTQ adoption, embryo adoption—and he takes on other authors who make claims about adoption that he believes distract or obscure the central purpose of adoption, which is to provide a child a "needed place of belonging" within the network of loving relationships that is the ideal of the family (38).My reservations about the book are two. The first has to do with the inconsistent tone of the book, which tends to swing, abruptly at times, from a theological and analytical tone to one that feels more popular and homiletic—even to the point of being maudlin. As stated earlier, I have high regard for most of the analytical sections and find the overarching thesis convincing. Generally, in these sections, the method begins with the lived experience of adoptees and their parents and works through scripture, tradition, and critical reflection to arrive at a reasonable conclusion. However, occasionally the book drifts into uncritical and merely emotive territory, and this is especially the case in the sections called "interludes," where the author presents letters to adoptees from their parents reprinted from the Christian Century magazine. These seem out of place and even distracting. Of even greater concern is the concluding chapter, where the author should be summarizing and highlighting the strengths of his thesis. Instead he chose to draw homiletic lessons from Anne of Green Gables (110).My second reservation is not so much a critique of this book but of the state of contemporary Catholic sexual ethics, which forms a backdrop for the entire book. The book accurately reflects a mainstream position in Catholic sexual ethics, which abandoned a narrow focus on procreation decades ago in favor of one highlighting the unitive and relational aspects of human intimacy. This move away from a strictly deontological framework stressing the duty to procreate and toward a teleological ethic stressing the need to model human relationships after divine ones has been celebrated for good reasons, but it has not come without costs. Like all teleological ideals, this sexual ethic is attractive to purveyors of high philosophy but tends to be thoroughly abstract and almost entirely incapable of offering concrete, practical guidance. Because it is so abstract, its application to specific moral issues can seem arbitrary and at the whim of the author. Such is the case in this book, for instance, when this teleological ideal is wielded to shed doubt on LGBTQ adoption. It is particularly telling [End Page 210] that the author neglects to consult lived experience in this section, ignoring the mountains of evidence readily available that LGBTQ families can be just as loving and nurturing as those of traditional families. How is it that we think and refer in the first-person way? For most philosophers in the analytic tradition, the problem is essentially this: how two apparently conflicting kinds of properties can be reconciled and united as properties of the same entity. What is special about the first person has to be reconciled with what is ordinary about it. The range of responses reduces to four basic options. The orthodox view is optimistic: there really is a way of reconciling these (...) apparently contradictory properties as contained within the same thing. Although connectionism is advocated by its proponents as an alternative to the classical computational theory of mind, doubts persist about its _computational_ credentials. Our aim is to dispel these doubts by explaining how connectionist networks compute. We first develop a generic account of computation—no easy task, because computation, like almost every other foundational concept in cognitive science, has resisted canonical definition. We opt for a characterisation that does justice to the explanatory role of computation in cognitive science. Next we examine (...) what might be regarded as the “conventional” account of connectionist computation. We show why this account is inadequate and hence fosters the suspicion that connectionist networks aren’t genuinely computational. Lastly, we turn to the principal task of the paper: the development of a more robust portrait of connectionist computation. The basis of this portrait is an explanation of the representational capacities of connection weights, supported by an analysis of the weight configurations of a series of simulated neural networks. O'Brien provides a critical assessment of Ernest Mandel's 1975 monograph Late Capitalism. In so doing, he offers a historical narrative that puts into question Mandel's framing of 'waves' of capitalist development as a process of capital accumulation that was dependent upon uneven development in the Third World. O'Brien starts by problematising Mandel's argument that an initial concentration of money, capital and bullion in the hands of Europeans explains combined and uneven development. He goes on to demonstrate that Mandel's notion of (...) imperialism as a necessary conduit for the flow of surplus capital from industrialising Europe does not stand up to the historical evidence. In fact, O'Brien maintains an alternative thesis, namely that these marginalised regions were insuffciently penetrated by European capital. (shrink)

When it comes to applying computational theory to the problem of phenomenal consciousness, cognitive scientists appear to face a dilemma. The only strategy that seems to be available is one that explains consciousness in terms of special kinds of computational processes. But such theories, while they dominate the field, have counter-intuitive consequences; in particular, they force one to accept that phenomenal experience is composed of information processing effects. For cognitive scientists, therefore, it seems to come down to a choice between (...) a counter-intuitive theory or no theory at all. We offer a way out of this dilemma. We argue that the computational theory of mind doesn't force cognitive scientists to explain consciousness in terms of computational processes, as there is an alternative strategy available: one that focuses on the representational vehicles that encode information in the brain. This alternative approach to consciousness allows us to do justice to the standard intuitions about phenomenal experience, yet remain within the confines of cognitive science. (shrink)

Heidegger, History and the Holocaust is an important contribution to the longstanding debate concerning Martin Heidegger's association with National Socialism. Although a difficult topic, this ambitious new work moves the entire debate on the Heidegger controversy forward. -/- Following Being and Time Heidegger expands on his notion of authenticity and related notions such as historicity and discusses the possibility of an authentic Dasein of a people along structurally consistent lines to his account of authenticity in Being and Time. O'Brien argues (...) that the same difficulties which appear to hamstring the early account of authenticity further affect the notion of an authentic Dasein of a people; Heidegger's political myopia in the thirties can thus be attributed to an underlying failure to come to terms with some of the difficulties discussed in this study. O'Brien concedes that Heidegger's philosophy is influenced by its historical period and context but argues that, however inflammatory, Heidegger's rhetoric cannot be simply reduced to crude Nazi jingoism. -/- This book is a genuinely philosophical approach to the Heidegger controversy and a much-needed re-examination of his ideas and influences. The connectionist vehicle theory of phenomenal experience in the target article identifies consciousness with the brain’s explicit representation of information in the form of stable patterns of neural activity. Commentators raise concerns about both the conceptual and empirical adequacy of this proposal. On the former front they worry about our reliance on vehicles, on representation, on stable patterns of activity, and on our identity claim. On the latter front their concerns range from the general plausibility of a vehicle theory to (...) our specific attempts to deal with the dissociation studies. We address these concerns, and then finish by considering whether the vehicle theory we have defended has a coherent story to tell about the active, unified subject to whom conscious experiences belong. Oaksford & Chater (O&C) have rejected logic in favor of probability theory for reasons that are irrelevant to mental-logic theory, because mental-logic theory differs from standard logic in significant ways. Similar to O&C, mental-logic theory rejects the use of the material conditional and deals with the completeness problem by limiting the scope of its procedures to local sets of propositions.

In this paper I want to propose that we see solipsism as arising from certain problems we have about identifying ourselves as subjects in an objective world. The discussion will centre on Wittgenstein’s treatment of solipsism in his Tractatus Logico- Philosophicus. In that work Wittgenstein can be seen to express an unusually profound understanding of the problems faced in trying to give an account of how we, who are subjects, identify ourselves as objects in the world. We have in his (...) compressed remarks, the kernels of a number of arguments which all come together to form what can be called the problem of self-identification. I want to argue that the solipsism of the Tractatus arises at least in part as a solution to, or – to put it less optimistically – as a symptom or articulation of this problem. In approaching Wittgenstein’s early discussion of solipsism in this way I will obviously be in disagreement with some other interpretations of the work. For example, there are those who think that there is no ‘solipsism of the Tractatus’.1 In fact, the Tractarian arguments presented below as motivating solipsism have been seen as fulfilling the quite opposite function of refuting it. I do not intend in this piece to engage with alternative interpretations. Let me say a little bit about why I have granted myself the licence not to do so. First, the focus of my concern with solipsism is on how it connects with what I have called the problem of self-identification. Christopher Lasch called the book, first published over twenty years ago, "an astute analysis of our spiritual malaise." God and the New Haven Railway, with a new preface by the author, speaks to us still with humor and hope because neither God nor the railroad seems to be running much better today. Heidegger's thinking in the decades following the publication of Being and Time is often deemed irreconcilable with that work. Critics contrast the notion of "resoluteness" in Being and Time with Heidegger's post-war account of "releasement" in an attempt to establish a discrepancy between the allegedly voluntarist humanism of his early work and the supposedly 'anti-humanist' thinking of his later work. By contrast, Mahon O'Brien argues for the structural and thematic coherence of Heidegger's movement from authenticity to the search for an (...) authentic free relation to the world - as captured by the term "releasement". By demonstrating the structural and thematic unity of Heidegger's thought in its entirety, O'Brien paves the way for a more measured and philosophically grounded understanding of the issues at stake in the Heidegger controversy. This issue brings together papers by Australasian philosophers on language, thought, and their relationship. Contributors were given complete freedom to treat these topics in any way they saw fit. The results reflect the diverse interests of Australasian philosophers, and, perhaps even more strikingly, the diversity of philosophical methods they employ to pursue these interests.

Cognitive science is founded on the conjecture that natural intelligence can be explained in terms of computation. Yet, notoriously, there is no consensus among philosophers of cognitive science as to how computation should be characterised. While there are subtle differences between the various accounts of computation found in the literature, the largest fracture exists between those that unpack computation in semantic terms (and hence view computation as the processing of representations) and those, such as that defended by Chalmers (2011), that (...) cleave towards a purely syntactic formulation (and hence view computation in terms of abstract functional organisation). It will be the main contention of this paper that this dispute arises because contemporary computer science is an amalgam of two different historical traditions, each of which has developed its own proprietary conception of computation. Once these historical trajectories have been properly delineated, and the motivations behind the associated conceptions of computation revealed, it becomes a little clearer which should form the foundation for cognitive science. The interface between mental health services and the criminal justice system presents challenges both for professionals and patients. Both systems are stressed and inherently complex. Section 136 of the Mental Health Act is unusual being both an aspect of the Mental Health Act and a power of arrest. It has a long and controversial history related to concerns about who has been detained and how the section was applied. More recently, Section 136 has had a public profile stemming from the (...) use of police cells as places of safety for young, mentally disturbed individuals. This paper explores the current state of health of this piece of legislation. Specifically, we consider whether alternative approaches are more suitable for those individuals in crisis and/or distress who come into contact with the police. This requires careful thought as to the proper role of both health and criminal justice professionals who are daily grappling with an ethically contentious domain of multiagency work. (shrink)

Teilhard’s cosmological speculation is a valuable basis for an environmental ethics that perceives individual natural objects as good in themselves and the world as good in itself. Teilhard perceives man as fundamentally part of a cosmic environmental whole that is greater than mankind taken individually or collectively. His holistic views on human biological and psychological and social evolution are, I argue,compatible with a biocentric environmental ethics. I discuss some similarities and differences with the views of the deep ecology movement. I (...) show that Teilhard’s hierarchical system is not humanistically oriented in a way that need be interpreted by Teilhardians as contrary to environmental well-being. I argue that Teilhard’s sympathies toward transportation technology, including the automobile, can be interpreted in his holistic manner. I conclude that Teilhard’s theocentric views are also a basis for supporting an environmental ethics which is both optimistic and not anthropocentric. (shrink)

Cohen has argued that there is a surprising truth in conservatism—namely, that there is a reason for some valuable things to be preserved, even if they could be replaced with other, more valuable things. This conservative thesis is motivated, Cohen suggests, by our judgments about a range of hypothetical cases. After reconstructing Cohen's conservative thesis, I argue that the relevant judgments about these cases do not favor the conservative thesis over standard, nonconservative axiological views. But I then argue (...) that there is a Mirrored Histories case that is such that, if one shares Cohen's conservative attitude, judgments about this case favor Cohen's conservative thesis over a wide range of non-conservative axiological views. Reflection on this case also suggests a different explanation of apparently conservative judgments that merits consideration in its own right. (shrink)

The application of a `Foucaultism' to contemporary critical analysis is by no means an easy task and Poster's book contains many valuable insights into the functioning of Foucault's work — particularly as it relates to modern Marxisms — within critical theory. Yet there remain a number of important themes which Poster appears merely to gloss over (eg, the precise nature of `historicity', the relation between the dialectic and reason, etc) providing no solution to the dilemmas thrown up by grounded theory (...) attempting to work within the field of a discursive analysis. The aim of this paper is to shed some light on the particular nature of `history' as it appears in Marx and in Foucault and to indicate some of the main characteristics of Foucault's analytical tools. The logic of the paper follows the logic of Poster's book, dealing with Marx first, Foucault second, and closes with an examination of some analytical implications of adopting a discursive/genealogical approach to the study of contemporary social phenomena. Carruthers presents evidence concerning the cross-modular integration of information in human subjects which appears to support the “cognitive conception of language.” According to this conception, language is not just a means of communication, but also a representational medium of thought. However, Carruthers overlooks the possibility that language, in both its communicative and cognitive roles, is a nonrepresentational system of conventional signals – that words are not a medium we think in, but a tool we think with. The evidence he cites (...) is equivocal when it comes to choosing between the cognitive conception and this radical communicative conception of language. (shrink)

Hosea-Zephaniah so closely conform to Zechariah's description of the “former prophets” that these books may have been written or edited as a prelude to Zechariah. Recognizing that Zechariah constructed a picture of earlier prophets that would support his own message in the Persian period invites modern readers to reflect on the promise and the dangers of engaging in a similar process of reshaping the past in the interests of the present.

National Action Plans on business and human rights are a growing phenomenon. Since 2011, 42 such plans have been adopted or are in-development worldwide. By comparison, only 39 general human rights action plans were published between 1993 and 2021. In parallel, NAPs have attracted growing scholarly interest. While some studies highlight their potential to advance national compliance with international norms, others criticise NAPs as cosmetic devices that states use to deflect attention from persisting abuses and needed regulation. In response to (...) wider critiques of international human rights norms, and their failure to exact universal state compliance, experimentalist governance theory highlights the dynamic, dialogic and iterative character of human rights implementation as well as the role of stakeholders. In this article, we apply experimentalist governance theory to evaluate the role and character of business and human rights NAPs. Rather than attempting to evaluate NAPs’ ultimate consequences for rights-holders, which appears premature, we focus on NAPs processes. Specifically, we analyse NAPs processes in twenty-five states against five experimentalist governance criteria relating to stakeholder participation; agreement on a broad problem definition; local contextualisation; monitoring and peer review and periodic revision and learning. According to our findings, NAPs on business and human rights in most states demonstrate resemblance to the traits of experimentalist governance. In particular, our analysis points to the emergence of relatively sophisticated and demanding institutional governance mechanisms within NAPs — including the institutionalisation of complex deliberative processes. Nevertheless, our paper also identifies some significant shortcomings in NAPs, related to the lack of inclusion of vulnerable groups and the lack of explicit indicators and targets. (shrink)

In order to promote social distancing during the recent COVID-19 pandemic, physicians and healthcare systems have made efforts to replace in-person with virtual clinic visits when feasible. While these efforts have been well received and seem compatible with sound clinical practice, they do not perfectly replicate the experience of a face-to-face exchange between doctor and patient. This essay attempts to describe features of the virtual visit that distinguish it from its face-to-face analog and considers the phenomenological work of Emmanuel Levinas (...) in arguing that these differences may limit the force of the ethical summons a provider would otherwise experience before the face of a patient. The diminishment of this signal therapeutic experience may engender vocational as well as clinical consequences, which should be weighed against the practical benefits of the virtual visit as we consider whether our enthusiasm for this mode of practice should continue. Reformers urge that representation no longer earns its explanatory keep in cognitive science, and that it is time to discard this troublesome concept. In contrast, we hold that without representation cognitive science is utterly bereft of tools for explaining natural intelligence. In order to defend the latter position, we focus on the explanatory role of representation in computation. We examine how the methods of digital and analog computation are used to model a relatively simple target system, and show that representation (...) plays an in-eliminable explanatory role in both cases. Any creature that must move around in its environment to find nutrients and mates, in order to survive and reproduce, faces the problem of sensorimotor control. A solution to this problem requires an on-board control mechanism that can shape the creature’s behaviour so as to render it “appropriate” to the conditions that obtain. There are at least three ways in which such a control mechanism can work, and Nature has exploited them all. The first and most basic way is for (...) a creature to bump into the things in its environment, and then, depending on what has been encountered, seek to modify its behaviour accordingly. Such an approach is risky, however, since some things in the environment are distinctly unfriendly. A second and better way, therefore, is for a creature to exploit ambient forms of energy that carry information about the distal structure of the environment. This is an improvement on the first method since it enables the creature to respond to the surroundings without actually bumping into anything. Nonetheless, this second method also has its limitations, one of which is that the information conveyed by such ambient energy is often impoverished, ambiguous and intermittent. (shrink)

My concern is this paper is to consider the nature of obsessive thoughts with the aim of getting a clearer idea about the extent to which they are rightly identified as passive or as active. The nature of obsessive thoughts is of independent interest, but my concern with the question is also rooted in a general concern to map the extent of mental activity, and to defend the importance and centrality of a view of self-knowledge that appeals to agency. I (...) hold that much of our mental lives is active, and that the distinctive knowledge we have of our own minds is in many cases best explained by appealing to agency. Along with many others, I take knowledge of our actions and activities to be distinctive. We know our actions and activities in a way that we do not know anything else, and in a way that is distinctively and essentially self-conscious. (shrink)
