Maternal health is considered a key global priority by the World Health Organization, and several strategies are used to promote it. Especially in lower- and lower-middle-income countries, one widely used strategy employs financial incentives to motivate pregnant women to access available healthcare. While such interventions have been extensively empirically evaluated, their normative aspects appear to have received less attention. To address this gap, we systematically searched and reviewed normative and qualitative literature to map and analyze the ethical considerations of using (...) financial incentives to promote maternal health. Twenty-three articles (four normative and 19 qualitative) were included in the review. We grouped ethical considerations under six categories, namely the program structure, promoting institutional births, of the ethical significance of cash, justifying conditionalities for maternal health, effects on individuals’ decision-making and the experiences of vulnerable groups. While incentives could be used for beneficent goals, we show how they may emphasize global rather than local solutions, impose unfamiliar sociocultural values, unfairly expect behavior change and disempower individuals and groups. We argue that future maternal health policy, especially those targeting vulnerable groups and minorities, would benefit from taking these normative considerations into account. (shrink)

Are we humans destined to become ‘posthuman’? In this paper, we question the claims of posthumanism, accepting some of its broader insights whilst proposing a more empirically and ethically appropriate ‘vitalist’ response. We argue that despite recent changes in styles of thought that question the uniqueness of ‘the human’, and despite novel technological developments for augmenting human bodies, we remain – fundamentally – persons. Humans, as persons, are constitutively embedded in and scaffolded by the material, social, semantic and cultural niches (...) they have constructed for themselves. An ‘ethopolitical’ response to our present that recognizes this fact not only requires us to engage with the consequences of making lives in the unequal niches that restrict and stunt the personhood of so many human beings at present, but also requires us to broaden the bandwidth of those who are recognized fully as persons. In our conclusion we briefly develop the theme that this emphasis on personhood can be seen as a vitalist – rather than a humanist – stance. (shrink)

Individuals often face administrative hurdles in attempting to access health care, public programmes, and other legal statuses and entitlements. These ordeals are the products, directly or indirectly, of institutional and policy design choices. I argue that evaluating whether such ordeals are justifiable or desirable instruments of social policy depends on assessing, beyond their targeting effects, the process-related burdens they impose on those attempting to navigate them and these burdens’ distributive effects. I here examine specifically how ordeals that levy time costs (...) reduce and constrain individuals’ free time, and how such time-cost ordeals may thereby create, deepen and compound disadvantages. _The Social Work Psychoanalyst's Casebook_ begins with an interview with Dr. Sanville, who reflects on her evolution as a social work analyst, theoretician, writer, teacher, and leader. These reminiscences are followed by accounts of nine analytic treatments, each of which offers an unusual window into what actually transpired between analyst and analysand during the treatment hours. These case studies concern particularly troubled, often traumatized patients-the very "hard to reach" or "difficult to treat" clients with whom social workers have long been (...) familiar. They include a reanalysis by the same analyst of a patient whose first therapy ended in a stalemate; an account of transference and countertransference phenomena during termination; a report on the analysis of a young woman who experienced both chronic and stress-related trauma; and an account of the special issues involved in the treatment of an aging woman. Most of the case studies reflect the influence of Dr. Sanville, whose work has long evinced the therapeutic imagination and disciplined creativity to which all the contributors aspire. Tthe contributors to this volume offer the salutary reminder that analytic work is built on a relationship of respect and empathy and that treatment success follows from the therapist's willingness to accommodate the unique needs of individual patients. In honoring Jean Sanville, _The Social Work Psychoanalyst's Casebook_ speaks to the robustness of a multidisciplinary approach to psychopathology that transcends the bounds of any single profession-an approach in which contemporary psychoanalysis is enlarged by the insights and emphases of social work just as social work is enriched by the clinical wisdom of psychoanalysis. This paper critically evaluates an objection to introspective arguments for human freedom found within Spinoza's Ethics. The objection-- which we call Spinoza's dream argument -- challenges the evidentiary value of a person's experience of her own freedom by pointing out that some choices made within dreams are experienced as no less free than choices made while awake despite the fact that choices made within dreams are not free. After reconstructing Spinoza's dream argument, we critically evaluate it, concluding ultimately that it (...) fails in two respects. First, we draw upon the results of an experiment we conducted that suggests that dreamt choices are not experienced as free to the same extent as choices made while awake. Second, we argue that there is no compelling justification for the claim that choices made within dreams are not free. (shrink)
