BackgroundFear of cancer recurrence is common amongst cancer survivors. There is rapidly growing research interest in FCR but a need to prioritize research to address the most pressing clinical issues and reduce duplication and fragmentation of effort. This study aimed to establish international consensus among clinical and academic FCR experts regarding priorities for FCR research.MethodsMembers of the International Psycho-oncology Society Fear of Cancer Recurrence Special Interest Group were invited to participate in an online Delphi study. Research domains identified in Round (...) 1 were presented and discussed at a focus group to consolidate the domains and items prior to presentation in further survey rounds aimed at gaining consensus on research priorities of international significance.ResultsThirty four research items were identified in Round 1 and 33 of the items were consolidated into six overarching themes through a focus group discussion with FCR experts. The 33 research items were presented in subsequent rounds of the delphi technique. Twenty one participants contributed to delphi round 1, 16 in round 2, and 25 and 29 participants for subsequent delphi rounds. Consensus was reached for 27 items in round 3.1. A further four research items were identified by panelists and included in round 3.2. After round 3.2, 35 individual research items were ratified by the panelists. Given the high levels of consensus and stability between rounds, no further rounds were conducted. Overall intervention research was considered the most important focus for FCR research. Panelists identified models of care that facilitate greater access to FCR treatment and evaluation of the effectiveness of FCR interventions in real world settings as the two research items of highest priority. Defining the mechanisms of action and active components across FCR/P interventions was the third highest priority identified.ConclusionThe findings of this study outline a research agenda for international FCR research. Intervention research to identify models of care that increase access to treatment are based on a flexible approach based on symptom severity and can be delivered within routine clinical care were identified as research areas to prioritize. Greater understanding of the active components and mechanisms of action of existing FCR interventions will facilitate increased tailoring of interventions to meet patient need. (shrink)

While cancer survivors commonly experience fear and anxiety, a substantial minority experience an enduring and debilitating fear that their cancer will return; a condition commonly referred to as fear of cancer recurrence. Despite recent advances in this area, little is known about FCR among people from Indigenous or other ethnic and racial minority populations. Given the high prevalence and poor outcomes of cancer among people from these populations, a robust understanding of FCR among people from these groups is critical. The (...) current review identified and aggregated existing literature on FCR amongst adult cancer survivors from Indigenous and minority populations. The protocol of this review was registered with PROSPERO in July 2020. A systematic search of bibliographic databases was conducted for relevant articles published from 1997 to November 2019. Data from eligible articles were extracted and appraised for quality by two independent reviewers. Nineteen articles from four countries met the inclusion criteria, including 14 quantitative, 4 qualitative and 1 mixed-methods study. Only one article reported on an Indigenous population. Few studies reported on FCR prevalence or severity. While the variation in tools used to measure FCR hindered a robust estimate of severity, results suggested some differences in FCR severity between minority and dominant populations, although these may have been due to study metholodological differences. Few factors were reported as being associated with FCR in minorities across multiple studies. The qualitative synthesis found five themes associated with the lived experience of FCR: variations in the lived experience of FCR; spirituality and worldview impacting on FCR; the importance of staying positive; complexities around support; and increasing cancer knowledge. The findings of this review highlight differences in FCR across cultures and contexts, which reinforces the need for culturally-specific approaches to this condition. The dearth of research in this area is of concern given the significant burden of cancer in these populations. A deeper understanding of this condition among Indigenous and minority populations is critical to developing and delivering appropriate and effective psychosocial care for cancer survivors from these groups. Desire satisfaction has not received detailed philosophical examination. Yet intuitive judgments about the satisfaction of desires have been used as data points guiding theories of desire, desire content, and the semantics of ‘desire’. This paper examines desire satisfaction and the standard propositional view of desire. Firstly, I argue that there are several distinct concepts of satisfaction. Secondly, I argue that separating them defuses a difficulty for the standard view in accommodating desires that Derek Parfit described as ‘implicitly conditional on their (...) own persistence’, a problem posed by Shieva Kleinschmidt, Kris McDaniel, and Ben Bradley. The solution undercuts a key motivation for rejecting the standard view in favour of more radical accounts proposed in the literature. (shrink)

I provide an argument from consciousness for God’s existence. I first consider a version of the argument which is ultimately difficult to evaluate. I then consider a stronger argument, on which consciousness, given our worldly laws of nature, is rather substantial evidence for God’s existence. It is this latter argument the paper largely focuses on, both in setting it out and defending it from various objections.

Proposals for increasing organ donation are often rejected as incompatible with altruistic motivation on the part of donors. This paper questions, on conceptual grounds, whether most organ donors really are altruistic. If we distinguish between altruism and solidarity – a more restricted form of other-concern, limited to members of a particular group – then most organ donors exhibit solidarity, rather than altruism. If organ donation really must be altruistic, then we have reasons to worry about the motives of existing donors. (...) However, I argue that altruism is not necessary, because organ donation supplies important goods, whatever the motivation, and we can reject certain dubious motivations, such as financial profit, without insisting on altruism.Once solidaristic donation is accepted, certain reforms for increasing donation rates seem permissible. This paper considers two proposals. Firstly, it has been suggested that registered donors should receive priority for transplants. While this proposal appears based on a solidaristic norm of reciprocity, it is argued that such a scheme would be undesirable, since non-donors may contribute to society in other ways. The second proposal is that donors should be able to direct their organs towards recipients that they feel solidarity with. This is often held to be inconsistent with altruistic motivation, but most donation is not entirely undirected in the first place (for instance, donor organs usually go to co-nationals). While allowing directed donation would create a number of practical problems, such as preventing discrimination, there appears to be no reason in principle to reject it. (shrink)

John Rawls raises three challenges – to which one can add a fourth challenge – to an impartial spectator account: (a) the impartial spectator is a utility-maximizing device that does not take seriously the distinction between persons; (b) the account does not guarantee that the principles of justice will be derived from it; (c) the notion of impartiality in the account is the wrong one, since it does not define impartiality from the standpoint of the litigants themselves; (d) the account (...) would offer a comprehensive, rather than a political, form of liberalism. The narrow aim of the article is to demonstrate that Adam Smith's impartial spectator account can rise to Rawls's challenges. The broader aim is to demonstrate that the impartial spectator account offers the basis for a novel and alternative framework for developing principles of justice, and does so in the context of a political form of liberalism. Mill's most famous departure from Bentham is his distinction between higher and lower pleasures. This article argues that quality and quantity are independent and irreducible properties of pleasures that may be traded off against each other higher pleasures’ lexically dominate lower ones, and that the distinction is compatible with hedonism. I show how this interpretation not only makes sense of Mill but allows him to respond to famous problems, such as Crisp's Haydn and the oyster and Nozick's experience machine.

Utilitarianism directs us to act in ways that impartially maximize welfare or utility or at least aim to do that. Some find this view highly compelling. Others object that it has intuitively repugnant results; that it condones evildoing and injustice; that it is excessively imposing and controlling; that it is alienating; and that it fails to offer meaningful practical guidance. In this 'Little Debates' volume, James Lenman argues that utilitarianism's directive to improve the whole universe on a cosmic time scale (...) is apt to lead it down a path of imperious moral overreach. Lenman further maintains that the project ultimately shipwrecks upon an extreme lack of epistemic humility in framing the determinants of what is morally right and wrong beyond the limits of what we can ever hope to know. Utilitarianism thus leaves us morally clueless. In contrast, Ben Bramble seeks to develop and defend an original form of utilitarianism, less vulnerable than other, more familiar versions to a number of important objections (including those raised by Lenman). Bramble's form of utilitarianism aims to avoid other unappealing results by presenting it as a claim about what we have the most reason to do and not as a theory of right action (which Bramble urges we should understand quite differently by referencing what would motivate virtuous people). The two authors debate various forms of this longstanding ethical theory, arguing for very different conclusions, in a way that is sure to leave readers with new views of their own moral thoughts and lives. This article has the aim of bringing some fresh observations to the interpretation of a Martial epigram. Beyond the individual poem, it seeks to read Martial's poetics more broadly, particularly with regard to the presence of Greek avatars, of various kinds, in his poetic production. The strategy will be an exact reading of the literary avatars in 10.4, with an attempt to specify the tone with which individual writers are associated. Once this strategy is developed in the case of well-recognized (...) intertextual models, it will be used to explore an underrated intertext in 10.4. (shrink)

Gillian Piggott's study of the resonances between the work of Charles Dickens and Walter Benjamin arrives in the wake of an increasing critical interest in Benjamin's life and thought, as an array of books from Graham Gilloch's Myth and Metropolis: Walter Benjamin and the City (1996) to Esther Leslie's biographical Walter Benjamin (2007), and beyond, makes clear. While many critics have addressed Benjamin's importance to discussions of nineteenth-century modernity, Piggott's is the first booklength attempt to compare Benjamin and Dickens. Fler (...) goal is to produce a "creative comparison of the authors' responses to modernity" (4) rather than establish direct lines of influence, though she points out that Benjamin was aware of at least some of Dickens's work, particularly The Old Curiosity Shop and Great Expectations (11), and included fifteen entries about him in the Arcades Project (1927-40). In many circumstances compromises seem to be of great value to our well-being; compromises can help us avoid disputes and fights and enable us to live peacefully with each other. However, compromises can also require us to surrender some of our values. These two opposing aspects implicit in compromise express the need to be sensitive to external circumstances and in particular to the wishes of other people, and at the same time to be willing to relinquish something of value. So (...) are compromises, and in particular romantic compromises, good or bad for our well-being? McIntyre’s view in general and Aristotle’s distinction between extrinsically and intrinsically valuable activities in particular, are very useful in this regard. Compromises can be characterized as involving dissatisfied acceptance of a gap between a perceived feasible desire and our actual situation. The acceptance of the gap is merely behavioral stemming from unfavorable external circumstances. It is not psychologically accepted, as deep down we are not happy with the compromise, and still yearning for a better solution. Romantic compromises are particularly hard to accept in light of the prominent place of the Romantic Ideology, in which ideal love can overcome all obstacles and hence there is no place for compromises.
