Les observatoires régionaux fédèrent au long du XIXe siècle ce mouvement qui rassemble des professionnels intéressés par les applications de ces observations, puis, sous l'impulsion de personnalités comme Camille Flammarion, un public élargi d'amateurs. During public health crises including the COVID-19 pandemic, resource scarcity and contagion risks may require health systems to shift—to some degree—from a usual clinical ethic, focused on the well-being of individual patients, to a public health ethic, focused on population health. Many triage policies exist that fall under the legal protections afforded by “crisis standards of care,” but they have key differences. We critically appraise one of the most fundamental differences among policies, namely the use of criteria to categorically exclude (...) certain patients from eligibility for otherwise standard medical services. We examine these categorical exclusion criteria from ethical, legal, disability, and implementation perspectives. Focusing our analysis on the most common type of exclusion criteria, which are disease-specific, we conclude that optimal policies for critical care resource allocation and the use of cardiopulmonary resuscitation (CPR) should not use categorical exclusions. We argue that the avoidance of categorical exclusions is often practically feasible, consistent with public health norms, and mitigates discrimination against persons with disabilities. (shrink)

Teaching is not testimony. Although both convey information, they have different uptake requirements. Testimony aims to impart information and typically succeeds if the recipient believes that informationon account of having been told by a reliable informant. Teaching aims to equip learners to go beyond the information given—to leverage that information to broaden, deepen, and critique their current understanding of a topic. Teaching fails if the recipients believe the information only because it is what they have been told.

Issues of balancing data accessibility with ethical considerations and governance of a genomics research biobank, Generation Scotland, are explored within the evolving policy landscape of the past ten years. During this time data sharing and open data access have become increasingly important topics in biomedical research. Decisions around data access are influenced by local arrangements for governance and practices such as linkage to health records, and the global through policies for biobanking and the sharing of data with large-scale biomedical research (...) data resources and consortia. We use a literature review of policy relevant documents which apply to the conduct of biobanks in two areas: support for open access and the protection of data subjects and researchers managing a bioresource. We present examples of decision making within a biobank based upon observations of the Generation Scotland Access Committee. We reflect upon how the drive towards open access raises ethical dilemmas for established biorepositories containing data and samples from human subjects. Despite much discussion in science policy literature about standardisation, the contextual aspects of biobanking are often overlooked. Using our engagement with GS we demonstrate the importance of local arrangements in the creation of a responsive ethical approach to biorepository governance. We argue that governance decisions regarding access to the biobank are intertwined with considerations about maintenance and viability at the local level. We show that in addition to the focus upon ever more universal and standardised practices, the local expertise gained in the management of such repositories must be supported. A commitment to open access in genomics research has found almost universal backing in science and health policy circles, but repositories of data and samples from human subjects may have to operate under managed access, to protect privacy, align with participant consent and ensure that the resource can be managed in a sustainable way. Data access committees need to be reflexive and flexible, to cope with changing technology and opportunities and threats from the wider data sharing environment. To understand these interactions also involves nurturing what is particular about the biobank in its local context. (shrink)

The development of genomics has dramatically expanded the scope of genetic research, and collections of genetic biosamples have proliferated in countries with active genomics research programs. In this essay, we consider a particular kind of collection, national biobanks. National biobanks are often presented by advocates as an economic ‘‘resource’’ that will be used by both basic researchers and academic biologists, as well as by pharmaceutical diagnostic and clinical genomics companies. Although national biobanks have been the subject of intense interest in (...) recent social science literature, most prior work on this topic focuses either on bioethical issues related to biobanks, such as the question of informed consent, or on the possibilities for scientific citizenship that they make possible. We emphasize, by contrast, the economic aspect of biobanks, focusing specifically on the way in which national biobanks create biovalue. Our emphasis on the economic aspect of biobanks allows us to recognize the importance of what we call clinical labor—that is, the regularized, embodied work that members of the national population are expected to perform in their role as biobank participants—in the creation of biovalue through biobanks. Moreover, it allows us to understand how the technical way in which national biobanks link clinical labor to databases alters both medical and popular understandings of risk for common diseases and conditions. Clinical research is a necessity if effective and safe treatments are to be developed. However, this may well include the need for research that is best described as ‘invasive’ in that it may be associated with some discomfort or inconvenience. Limitations in the undertaking of invasive research involving people with intellectual disabilities (ID) are perhaps related to anxieties within the academic community and among ethics committees; however, the consequence of this neglect is that innovative treatments specific to people with ID (...) may not be developed. Such concerns are likely to continue while there is limited published knowledge regarding the actual experiences of people with ID who have participated in invasive clinical research. As part of a pilot study trialling the novel use of a surgically inserted device to curb overeating in people with Prader–Willi syndrome (PWS) we have investigated the experience of research through semistructured qualitative interviews involving three participants and their carers. Thematic analysis revealed that the adults with PWS and their family carers rated their participation positively, seeing it as a rewarding and enriching experience. This brief report discusses findings from our interview data in order to highlight strategies which may ensure that research is acceptable to participants, meets the necessary ethical standards and is able to achieve the aims set out by the researchers. To our knowledge, this is the first study to record experiences directly from people with PWS and their carers regarding their involvement in invasive clinical research. (shrink)

I start by asking what Aristotle knew (or thought) about Heraclitus: what were the key features of Heraclitus's philosophy as far as Aristotle was concerned? In this section of the paper I suggest that there are some patterns to Aristotle's references to Heraclitus: besides the classic doctrines (flux, ekpyrosis and the unity of opposites) on the one hand, and the opening of Heraclitus's book on the other, Aristotle knows and reports a few slightly less obvious sayings, one of which is (...) in my title. Secondly, I ask some further questions. Is there any systematic connection between the bits that Aristotle reports? Do they hang together? Ought we to see here some relic of an Aristotelian understanding of what made Heraclitus tick? If we juxtapose these themes and Aristotle's comments on them, I suggest, we can generate some suggestive motifs, in particular a rather curious fascination with smoking, and the pleasures of inhaling. Finally, I suggest that smells (and the way the world would be if smells were the only differences to be discerned) are important in understanding what Heraclitus was doing. (shrink)

As a response to Moran's (1994) recommendation that Heidegger's Destruktion be extensively elaborated and critiqued, this paper suggests a way in which Heidegger's thinking can be more clearly understood as a search for how better to ‘say' the destruction. By briefly tracing how Heidegger's thinking on the Destruktion repeatedly turns against itself throughout his writings, it is demonstrated that Heidegger does indeed revise the notion by abandoning the term in his later writing; to replace it first with the concept of (...) ‘overcoming', and subsequently with the notion of Verwindung . This self-critical reworking of the Destruktion is evident in his turning towards these concepts; which is taken up by Derrida's deconstruction in its simultaneous turning towards and away from Heidegger's Destruktion . The popularization of the term “clinical death” for the absence of vital signs suggests the possibility of a radical change in our understanding of death. While death used to be considered something that we do not have the power to reverse, contemporary optimism suggests that we may be able to restore life to a dead organism. In this article, I examine how the term “death” is used today to clarify what kind of irreversibility we ought to assign to it. I (...) conclude that the kind of irreversibility closest to our concept of death is one that depends on implicit metaphysical commitments: we generally treat death as though it were metaphysically irreversible, and therefore not reversible by us. The article explores the problem of nursing as a practical discipline and suggests that there are several kinds of nursing science. Following the lead of Jacques Maritain and Yves R. Simon, the authoress begins with an account of the distinguishing characteristics of theoretical knowledge, to which the term “science” has historically been applied, and distinguishes it from practical knowledge or prudence. Next she reviews Maritain and Simon’s discussion of two intermediate levels of inquiry that share some characteristics of both science (...) and practical knowledge. Finally, using the writings of several nurse theorists whose seminal ideas in this area have established a basis for nurse theorist’s discussion of these issues, she distinguishes four kinds of nursing inquiry which range from the very theoretical to the very practical. Research on children interacting with each other encompasses a wide variety of specific research interest, including but not limited to a focus on language. In this introduction to an issue of Discourse Studies devoted to the contribution of peer talk to pragmatic development, we define ‘peer talk’ as an interdisciplinary field of inquiry and we critically review literature on the role of peer talk in children’s pragmatic development. We suggest that ‘peer talk’ as a field of inquiry properly encompasses studies (...) of the development of language through peer interaction as well as the ways in which peer talk functions in children’s co-construction of their social and cultural worlds. Research reviewed here suggests that peer talk may be a crucial site for pragmatic development, offering children a wide range of opportunities for mutual learning of interactive as well as linguistic skills. The evidence we review, which comes from the fields of language socialization, second language learning, and child-discourse, highlights two major contextual features of peer talk: its collaborative, multi-party, symmetrical participation structure; and its role in the co-constructed worlds of childhood culture. Finally, we call for research and theorizing that would bridge the gap between developmental and ethnographically-informed approaches to the study of child discourse, in order to ensure that children’s peer talk receives the attention due it as a major site for both the development of discourse skills and the creation of childhood culture. Why are young women less likely than young men to persist in academic science and engineering? Drawing on 57 in-depth interviews with PhD students and postdoctoral scholars in the United States, we describe how, in academic science and engineering, motherhood is constructed in opposition to professional legitimacy, and as a subject of fear, repudiation, and public controversy. We call this the “specter of motherhood.” This specter disadvantages young women and amplifies anticipatory concerns about combining an academic career with motherhood. By (...) specifying the content of cultural discourses about motherhood in academic workplaces and the processes by which these ideas circulate, produce disadvantage, and inform young, childless scientists and engineers’ career plans, our findings offer novel insight into mechanisms contributing to inequality in academic careers. L’analyse textométrique s’appuie sur le balisage TEI des séquences et le langage de requête CQL de TXM. Pour ce faire, nous analysons l’interaction entre les segments introducteurs, les incises et la ponctuation gauche du DD. Conducted among parents of young ice hockey players, this field experiment tested if making salient increasingly popular social norms that promote sportspersonship, learning, and having fun in sports, increases parents’ own self-determined endorsement of these behaviors and values, improves their psychological well-being, and impacts on their children’s on-ice behaviors. Hockey parents were randomly assigned to the experimental condition vs. control condition. Parents’ motivations for encouraging their child to learn and to have fun in hockey were then assessed. Score sheets for (...) the games that followed the study provided access to their children’s on-ice behaviors, as indicators of sportspersonship. Parents in the experimental condition reported higher self-determination for encouraging their child to learn and have fun in hockey compared to parents in the control condition. Furthermore, children of parents in the experimental condition had more assists. A mediation model revealed that the dynamic norms manipulation increased parents’ self-determined motivation for encouraging their child to learn and to have fun in hockey, which in turn, predicted higher psychological well-being. Together, these results provide support for the contention that highlighting increasingly popular social norms that promote sportspersonship, learning, and fun in sports, represents a promising strategy for creating positive social change in this life context. La disproportion est grande et ne cesse d'augmenter ces dix dernières années, entre la mémoire de la Shoah et celle du Goulag. La première, ponctuée par les commémorations internationales des 50 et 60 ans de la Libération d'Auschwitz ne cesse de cristalliser l'attention tandis que le regard porté sur le Goulag se tient, lui, nettement aux marges de la vie culturelle, médiatique et scolaire, du moins en Europe occidentale. Cette mémoire bancale a créé un litige qui semble se normaliser, voire (...) s'effacer. Cependant l'Europe demeure un paysage mémoriel fractionné, les différences entre les mémoires des camps en témoignent.There is an imbalance between memories of the Shoah and memories of the Gulag, and the gap has been steadily growing in the last ten years. The Shoah with international 50 and 60-year commemorations of the liberation of Auschwitz continues to focus attention, while reflections on the Gulag have remained very much on the sidelines in the spheres of culture, education and the media, at least in Western Europe. This lopsidedness has created an area of contention that seems to be straightening out, and even fading away. But European memory is still a fractured landscape, as evidenced by the differences between memories of the camps. (shrink)

This article seeks to explore the spiritual guidance of Saint Ignatius of Loyola as a worthy spiritual guide for our time as we consider some ways that we might seek to more clearly understand and authentically live out the call of Christ in our life journey. In this context of considering the work of Ignatius’ Spiritual Exercises as a viable tool for such interior conversion, it is necessary for the work of this article to take a closer look at some (...) highlights of Ignatius’ own life journey in order to appropriately consider the significance of key sections of his Spiritual Exercises and the dynamics for Christian formation and transformation inherent in these selected texts. I seek to argue that any assessment of our spiritual maturity in Christ finds clear context in our everyday life experiences as Saint Ignatius so aptly demonstrates. (shrink)

Leibniz’s metaphysics has been cited as a source of the dynamic and organic worldview of romantic Naturphilosophie. This chapter evaluates that claim by examining two distinct lineages of Leibniz’s metaphysical conception of dynamic appetition. On one hand, by demonstrating the existence of a “vis viva” in inanimate objects and by ascribing two distinct powers—perception and appetition—to all plants and animals as well as to his incorporeal “monads,” Leibniz seemed to restore force to physics and experience and intentionality to animals. On (...) the other hand, on account of the publication history of his writings, Leibniz’s pre-established harmony and preformation theory were seen by most philosophers before Herder and Schelling as reducing humans as well as animals to machines without free-will and their development to a mechanically predetermined succession of states. While appreciation of Leibniz’s views on activity and creativity in nature did increase towards the end of the century, other philosophical antecedents for the “drive” motif in German philosophy should be considered. What did nineteenth-century chemists know? This essay uses Emil Fischer’s classic study of the sugars in 1880s and 90s Germany to argue that chemists’ knowledge was not primarily vested in the theories of valence, structure, and stereochemistry that have been the subject of so much historical and philosophical analysis of chemistry in this period. Nor can chemistry be reduced to a merely manipulative exercise requiring little or no intellectual input. Examining what chemists themselves termed the “art of chemical experimentation” reveals (...) chemical practice as inseparable from its cognitive component, and it explains how chemists integrated theory with experiment through reason.
