In this article I am concerned with whether it could be morally significant to distinguish between doing something 'in order to bring about an effect' as opposed to 'doing something because we will bring about an effect'. For example, the Doctrine of Double Effect tells us that we should not act in order to bring about evil, but even if this is true is it perhaps permissible to act only because an evil will thus occur? I discuss these questions in (...) connection with a version of the so-called Trolley Problem known as the Loop Case. I also consider how these questions may bear on whether a rational agent must aim at an event which he believes is causally necessary to achieve an end he pursues. (shrink)

In The Gay Science, Friedrich Nietzsche argued that only a form of philosophizing that sprung from a deep commitment to the subject could ever hope for success. ‘All great problems,’ he wrote, ‘demand great love.’ He continued: It makes the most telling difference whether a thinker has a personal relationship to his problems and finds in them his destiny, his distress, and his greatest happiness, or an ‘impersonal’ one, meaning he is only able to touch them with the antennae of (...) cold, curious thought. In the latter case nothing will come of it, that much can be promised; for even if great problems should let themselves be grasped by them, they would not allow frogs and weaklings to hold on to them. Philosophers often find themselves in disagreement with contemporary philosophers they know full well to be their epistemic superiors on the topics relevant to the disagreement. This looks epistemically irresponsible. I offer a detailed investigation of this problem of the reflective epistemic renegade. I argue that although in some cases the renegade is not epistemically blameworthy, and the renegade situation is significantly less common than most would think, in a troublesome number of cases in which the situation arises the renegade is (...) blameworthy in her disagreement with recognized epistemic superiors. I also offer some thoughts on what it would mean for philosophical practice for us to refrain from being renegades. Finally, I show how a new kind of radical skepticism emerges from modest theses regarding the renegade. (shrink)

Introduction: representationalismMost theorists of cognition endorse some version of representationalism, which I will understand as the view that the human mind is an information-using system, and that human cognitive capacities are representational capacities. Of course, notions such as ‘representation’ and ‘information-using’ are terms of art that require explication. As a first pass, representations are “mediating states of an intelligent system that carry information” (Markman and Dietrich 2001, p. 471). This volume continues the examination of issues of life and death which F.M. Kamm began in Morality, Mortality, Volume I. Kamm continues her development of a non-consequentialist ethical theory and its application to practical ethical problems. She looks at the distinction between killing and letting die, and between intending and foreseeing, and also at the concepts of rights, prerogatives, and supererogation. She shows that a sophisticated non-consequentialist theory can be modelled which copes convincingly with practical ethical issues, and throws considerable (...) light on some of the key distinctions and concepts of ethical discourse. The monk Rodulfus Glaber is best known for his Five Books of Histories, a major source for events in the first half of the eleventh century, and valuable above all for revealing the mental furniture of an eleventh-century monk - for his account of the millennium, of relics genuine and false, of church-building, and visions of saints and demons. This edition, the first since 1866, presents the only critical text of the Histories, accompanied by a complete translation and a full (...) historical commentary. Glaber also wrote a Life of his mentor, St William of Dijon, the renowned monastic reformer. The Life is reprinted after the Histories, again with translation and notes. Regardless of who you are or how you live your life, you disagree with millions of people on an enormous number of topics from politics, religion and morality to sport, culture and art. Researchers in the health sciences regularly discover information of potential health importance unrelated to their object of study in the course of their research. However, there appears to be little guidance available on what researchers should do with this information, known in the scientific literature as incidental findings. The study described here was designed to determine the extent of guidance available to researchers from public sources. This empirical study was part of a larger two-year project funded by the National Human (...) Genome Research Institute to generate guidance on how incidental findings should be managed in human subjects research, especially genetics, genomics, and imaging research. We generated empirical analysis of publicly available guidance and consent forms to help guide a multidisciplinary Working Group of experts in their formulation of normative recommendations reported in this symposium. A new kind of skepticism about philosophy is articulated and argued for. The key premise is the claim that many of us are well aware that in the past we failed to have good responses to substantive objections to our philosophical beliefs. The conclusion is disjunctive: either we are irrational in sticking with our philosophical beliefs, or we commit some other epistemic sin in having those beliefs.

If you retain your belief upon learning that a large number and percentage of your recognized epistemic superiors disagree with you, then what happens to the epistemic status of your belief? I investigate that theoretical question as well has the applied case of philosophical disagreement—especially disagreement regarding purely philosophical error theories, theories that do not have much empirical support and that reject large swaths of our most commonsensical beliefs. I argue that even if all those error theories are false, either (...) (a) the average philosopher’s true commonsensical beliefs are epistemically impoverished, or (b) a good portion of philosophy is bunk and philosophers should give up most of their error theories despite the fact that their supporting arguments are generally as good as or even better than other philosophical arguments. Those of us who take skepticism seriously typically have two relevant beliefs: (a) it’s plausible (even if false) that in order to know that I have hands I have to be able to epistemically neutralize, to some significant degree, some skeptical hypotheses, such as the brain-in-a-vat (BIV) one; and (b) it’s also plausible (even if false) that I can’t so neutralize those hypotheses. There is no reason for us to also think (c) that the BIV hypothesis, for instance, is plausible (...) or probably true. In order to take skepticism seriously it’s sufficient to hold (a) and (b); one need not hold (c). Indeed, philosophers who accept (a) and (b) never endorse (c). Show me a philosopher who suspects that he is a brain in a vat and I’ll show you someone who is deranged! That’s one thing that bothers undergraduates in philosophy. They object: why on earth do some philosophers take the BIV hypothesis to pose any threat at all to our beliefs given that those very same philosophers think that there’s no real chance that the BIV hypothesis is true? Sure, the BIV hypothesis is formally inconsistent with my belief that I have hands, so if the former is true then my belief is false. But so what? Why should that bare inconsistency matter so much? Is this strange attitude amongst philosophers the result of some logic fetish infecting the philosophical community? It is sometimes said that the skeptical hypotheses are not only inconsistent with our beliefs but are explanatory of our experiences, which is supposed to make them more of a threat. But students aren’t fooled: although the skeptical hypotheses may attempt to explain why our experience is as it is, it’s the kind of attempt appropriate for science fiction movies that are all special effects and virtually no plot. No one with any sense of reality will take the evil demon hypothesis to be even tenuously explanatory. (shrink)

What happens when something happens? In Thinking the Event, senior continental philosophy scholar François Raffoul undertakes a philosophical inquiry into what constitutes an event as event, its very eventfulness: not what happens or why it happens, but that it happens, and what "happening" means. If, as Leibniz posited, it is true that nothing happens without a reason, does this principle of reason have a reason? For Raffoul, the event always breaks the demands of rational thought. Bringing together philosophical insights from (...) Heidegger, Derrida, Nancy, and Marion, Raffoul shows how the event, in its disruptive unpredictability, always exceeds causality, subjectivity, and reason. It is that "pure event," each time happening outside or without reason, which remains to be thought, and which is the focus of this work. In the final movement of the book, Raffoul takes on questions about the inappropriability of the event and the implications this carries for ethical and political considerations when thinking the event. In the wake of the exhaustion of traditional metaphysics, the notion of the event comes to the fore in an unprecedented way, with key implications for philosophy, ontology, ethics, and theories of selfhood. This is a short essay that presents what I take to be the main questions regarding the epistemology of disagreement.

For years philosophers argued for the existence of distinct yet materially coincident things by appealing to modal and temporal properties. For instance, the statue was made on Monday and could not survive being flattened; the lump of clay was made months before and can survive flattening. Such arguments have been thoroughly examined. Kit Fine has proposed a new set of arguments using the same template. I offer a critical evaluation of what I take to be his central lines of reasoning.

How do technically-skilled women negotiate the male-dominated environments of technology firms? This article draws upon interviews with female programmers, technical writers, and engineers of diverse racial backgrounds and sexual orientations employed in the San Francisco tech industry. Using intersectional analysis, this study finds that racially dominant women, who identified as LGBTQ and presented as gender-fluid, reported a greater sense of belonging in their workplace. They are perceived as more competent by male colleagues and avoided microaggressions that were routine among conventionally (...) feminine, heterosexual women. We argue that a spectrum of belonging operates in these occupational spaces dominated by men. Although white and Asian women successfully navigated workplace hostilities by distancing themselves from conventional heterosexual femininity, this strategy reinforces inequality regimes that privilege male workers. These findings provide significant theoretical insights about how race, sexuality, and gender interact to reproduce structural inequalities in the new economy. (shrink)

According to the dominant position among philosophers of language today, we can legitimately ascribe determinate contents to natural language sentences, independently of what the speaker actually means. This view contrasts with that held by ordinary language philosophers fifty years ago: according to them, speech acts, not sentences, are the primary bearers of content. François Recanati argues for the relevance of this controversy to the current debate about semantics and pragmatics. Is 'what is said' determined by linguistic conventions, or is it (...) an aspect of 'speaker's meaning'? Do we need pragmatics to fix truth-conditions? What is 'literal meaning'? To what extent is semantic composition a creative process? How pervasive is context-sensitivity? Recanati provides an original and insightful defence of 'contextualism', and offers an informed survey of the spectrum of positions held by linguists and philosophers working at the semantics/pragmatics interface. La télévision, en tant que « lieu de reconnaissance », constitue un enjeu important pour ces populations. Cependant, le petit écran semble marginaliser depuis toujours ces minorités visibles non seulement en leur laissant une place limitée à l'antenne mais aussi en leur renvoyant des images majoritairement caricaturales et dévalorisantes d'elles-mêmes. Une enquête qualitative auprès de 43 individus d'origine africaine (...) et antillaise montre que les minorités sont capables de mettre en place des réactions défensives ou stratégies identitaires afin de réduire le décalage certain entre leur identité sociale - souvent dépréciatrice - et leur identité personnelle , prenant les médias comme ressources pour l'action.In France, persons belonging to minorities, mainly non-European origin , are likely to experience discrimination, stigmatization but also exclusion. Television as a "place of recognition," is an important issue for these people. However, the small screen seems to marginalize always visible minorities by giving them not only a limited role in the antenna but also by returning images mostly caricatures and demeaning to themselves. A qualitative survey of 43 individuals of African and Caribbean origin shows that minorities are able to implement defensive reactions or identity strategies to reduce the gap between some social identity - often dépréciatrice - and their personal identity , taking the media as resources for action. Suppose you know that someone is your epistemic peer regarding some topic. You admit that you cannot think of any relevant epistemic advantage you have over her when it comes to that topic; you admit that she is just as likely as you to get P's truth-value right. Alternatively, you might know that she is your epistemic superior regarding the topic. And then after learning this about her you find out that she disagrees with you about P. In those situations (...) it appears that the confidence with which one holds one's belief should be significantly reduced. My primary goal in this essay is to present and reflect upon a set of cases of disagreement that have not been discussed in the literature but are vital to consider. I argue that in the new cases one is reasonable in not lowering one?s confidence in the belief. Then I articulate and defend an ambitious principle, the Disagreement Principle, meant to answer the question 'Under what conditions am I epistemically blameworthy in retaining my belief with the same level of confidence after I have discovered recognized peers or superiors who disagree with me?'. In this article I offer a three-pronged defense of Millian theories, all of which share the rough idea that all there is to a proper name is its referent, so it has no additional sense. I first give what I believe to be the first correct analysis of Kripke’s puzzle and its anti-Fregean lessons. The main lesson is that the Fregean’s arguments against Millianism and for the existence of semantically relevant senses (that is, individuative elements of propositions or belief contents (...) that are sensitive to our varying personal conceptions of the referents of those elements) are viciously circular. Thus, the Fregean must give new arguments for her central claims. Second, I offer an original, positive argument for the Millian idea that the thoughts that Cicero was bald and that Tully was bald are identical. Incredibly, the argument appeals to nothing but highly intuitive, pre-theoretical principles regarding folk psychological usage—traditionally the source of Fregean intuitions. Third, I examine one of the most important recent papers on Kripke’s puzzle, that by David Sosa (1996). Sosa claims to have found a way to turn the tables on Kripke’s puzzle by using it to argue against Millian theories. I argue that Sosa’s argument on behalf of the Fregean is question-begging. I conclude that Millian theories can be seriously defended without any use of theoretical constructs such as guises or Russellian propositions, and Fregeans need to start over arguing for their theory’s central claims. Did Augustine anticipate postmodern criticism, particularly in his theory of signs? While there are superficial similarities, Augustine's position is fundamentally invested in issues of truth and reference that postmodernism brackets out.

Philosophers of mind tend to hold one of two broad views about mental representation: they are either robustly realist about mental representations, taking them to have determinate, objective content independent of attributors’ explanatory interests and goals, or they embrace some form of anti-realism, holding that mental representations are at best useful fictions. It is becoming increasingly clear that neither view is satisfactory. Realists disagree about the basis for objective content, and computational neuroscientists and psychologists, while making widespread use of representational (...) talk in articulating their theories, seem indifferent to the efforts of these philosophers to articulate a secure foundation for such talk, suggesting a disconnect between the preoccupations of philosophers of cognitive science and actual practice in these sciences. Fictionalists, for their part, must deny that representational mental states can be causes of cognition and behavior, and that appeal to mental representation can be genuinely explanatory, views that flatly contradict assumptions central to both science and everyday life. I develop and defend a distinctive ‘third way’ – a view I call a deflationary account of mental representation – that both resolves philosophical worries about content and best fits actual practice in science and everyday life. According to the deflationary account, appeal to mental representation does indeed purport to pick out causes of behavior, but the attribution of content to these causes is best understood as a pragmatically motivated gloss, justified in part by attributors’ explanatory interests and goals. Content plays an explanatory role in the deflationary account, but one quite different than that assumed by robust representational realists. (shrink)

This article examines Swedish migrant women to the United States. It asks how racially privileged European migrants adapt to US racial and gender hierarchies that require them to relinquish their economic security and gender autonomy in a neoliberal state? Drawing upon interviews and focus group discussions with 33 Swedish women and three of their spouses, and participant observation between 2006 and 2008 in a network for Swedish speaking women living in the US, the article discusses how a group of ‘white’ (...) migrant women who arrive in the US with an ideology of gender egalitarianism negotiate a more socially conservative and economically vulnerable lifestyle, as the wives of upper-middle-class men. The article argues that while Swedish women benefit from their racial and social privileges in the US they lose their sense of economic security, acquiring new anxieties that make them reluctant to renounce their Swedish citizenship which they conceive of as a ‘flexible’ resource. When is industry self-regulation (ISR) a legitimate form of governance? In principle, ISR can serve the interests of participating companies, regulators and other stakeholders. However, in practice, empirical evidence shows that ISR schemes often under-perform, leading to criticism that such schemes are tantamount to firms marking their own homework. In response, this paper explains how current management theory on ISR has failed to separate the pragmatic legitimacy of ISR based on self-interested calculations, from moral legitimacy based on normative approval. The (...) paper traces three families of management theory on ISR and uses these to map the pragmatic and moral legitimacy of ISR schemes. It identifies tensions between the pragmatic and moral legitimacy of ISR schemes, which the current ISR literature does not address, and draws implications for the future theory and practice of ISR. Connections are drawn here between the writings of Mexico's most important seventeenth-century poet, Sor Juana Inés de la Cruz and the development of the cult of the Virgin of Guadalupe, still today a dominant feature of Catholic worship in Latin America. Techniques of conversion used by the Spanish missions are examined as background to the apparition of the Virgin, in 1531, and to clarify Sor Juana's response in her public verse. Later uses of the synecdoche are set out to illustrate (...) the Guadalupe's shape-shifting powers besides those of other legendary women in Mexican myth and history. A final section looks at the possibilities for new, creative aspects of feminist spirituality and liberation theology challenging Latin American Catholicism, and suggests ways in which women worshipping, and the worship of woman, are evolving in modem Mexico. François Jullien, the leading philosopher and specialist in Chinese thought, has always aimed at building on inter-cultural relations between China and the West. In this new book he focuses on the following questions: Do universal values exist? Is dialogue between cultures possible? To answer these questions, he retraces the history of the concept of the universal from its invention as an aspect of Roman citizenship, through its neutralization in the Christian idea of salvation, to its present day manifestations. This raises (...) the question of whether the search for the universal is a uniquely Western preoccupation: do other cultures, like China, even have a notion of the universal, and if so, how does it differ from ours? Having considered the meaning of the concept in the East and West, Jullien argues that, if communication between cultures is to be meaningful, facile assumptions of universal values and complacent relativism need to be examined. It follows, therefore, that dialogue between cultures should not begin with issues of identity and difference, but rather by considering divergence and profusion. By no longer simply assuming universality, we allow for greater self-reflection. This wide-ranging and engaging study will be of particular interest to students and scholars of philosophy and of Chinese culture and society. Only four brief texts remain which scholars agree may represent authentic examples of cultic prayer: the twoword prayer of Eleusinian initiates, the Athenian [End Page 632] prayer to Zeus for rain, a prayer to Demeter for the barley (...) sheaf, and the dithyramb of Elis, for a grand total of forty words. In addition, there are three fifth–century hymns from Epidaurus, a form not universally agreed to be prayer. In the area of epigraphic evidence, the largest body of prayer–like addresses derives from curse tablets which mostly postdate the classical period and which arguably belong not to the study of religion but to that of magic. Votive inscriptions, while plentiful, are rarely in prayer form. Thus, of necessity, the scholar is largely dependent on literary evidence, most of which is poetic.Thus it is a challenging task which Simon Pulleyn took on in this revised version of his doctoral thesis: to describe and interpret the mentality of all types of prayer in archaic and classical Greece in a relatively slight volume (216 pages plus appendices, etc.). The resulting monograph, "the first book–length study of Greek prayer to appear in English" (jacket), will prove a useful introduction to the body of evidence and major lines of research on Greek prayer. His work will be especially welcome to those not competent in the ancient languages, for Pulleyn translates all of the prayers proffered as examples, as well as modern foreign–language quotations. He also includes a brief glossary of fifteen Greek words especially important in the study of prayers. While he quotes an abundance of illustrative prayers, both literary and epigraphic, and devotes considerable space to lexicographic discussions, nevertheless he produces a quite readable text.Rejecting the broader definition of prayer as any address or appeal to supernatural powers, used by Aubriot–Sevin (Prière et conceptions religieuses en Grèce ancienne, Lyon, 1992), Pulleyn defines Greek prayers as "articulate requests directed toward the gods" and proposes a four–part classification of (1) prayers based on ξενία (2) curses (), (3) entreaties (λιταί), and (4) supplications ().While there is considerable ancient evidence to support the association of prayer with petition, such a narrow description leaves in limbo other types of address to the gods, such as thanksgivings and greetings, which are too important to be left out of this or any comprehensive study of prayer. And so Pulleyn is forced to devote much space to the discussion of material which he does not technically consider prayer. In fact, he himself varies between the technical use of the term "prayer" and the more common modern definition, a practice which naturally leads to some confusion. On occasion, he resorts to the term "prayer–like" to describe nonpetitionary addresses to gods.Perhaps the reason for this emphasis on petition is the author's main thesis that is the primary characteristic of the relationship between the ancient Greeks and their gods, and thus of prayer as well. Pulleyn explains thus: "it refers to a whole nexus of related ideas that we would call reciprocity. When one gives something to a god, one is giving in the sense that the offering is pleasing; but equally one is storing up for oneself a feeling of gratitude on the part of the god, which is also The whole two–way relationship can be called one of " (4). He is especially concerned with the da quia dedi(t) model, which presents an argument for fulfillment of the accompanying request based on sacrifices previously offered. Numerically, in Homer and elsewhere, this is not a common prayer type. Although it would seem to violate the typical pattern of sacrifice offered in conjunction with prayer requests, Pulleyn argues convincingly for interpretation of this prayer type within the context of reciprocity. Philosophers may play the role of insider, e.g., serving as advisor to government commissions, or of outsider, commenting on the work of such commissions. Each role may raise dilemmas. It is argued that as insider the philosopher's primary duties should be to clarify and inform, as well as philosophize with the commissioners, and help them stay on a course in which moral considerations are given their proper weight. Fulfilling these duties means that the philosopher will sometimes have to help produce (...) a weaker intellectual document than he would prefer, or lose a chance to directly promote the public good. The insider philosopher will also have to consider whether it is appropriate for a policy proposal from a commission to differ from much current government policy, and how morally appropriate compromise can be reached among commissioners. Given his understanding of how an insider philosopher should function and how commission reports are constructed, the outsider philosopher can comment both on how close a report comes to being perfect of its type, and how far short of an ideal philosophical analysis even a perfect government report is. It may be appropriate for him to give greater weight to the public good, if his comments are very likely to affect it, than the insider philosopher. Examples for discussion are drawn from government reports on organ transplantation, embryo research, terminating care, and compensation for research subjects. Adeney Louisville Presbyterian Theological Seminary Current understandings of how religions may reflect divine truth often use a model developed in England by Alan Race that designates attitudes toward other religions as exclusive, inclusive, or pluralist. John Hick's use of this seemingly simple paradigm, in conversation with scholars in the United States, presupposes (...) the reality of the divine, along with the unknowability of that reality through any human effort or religious tradition. Hick mediates that tension by calling on practitioners of all religions to recognize the relativity of their beliefs and, on that basis, tolerate the religions of others. Both use of and critiques of that model have generated lively discussion among scholars and practitioners of religions.Consider, for example, two possible ways to move beyond this model. The first is theological. Attempts to describe the divine by speaking of transcendence, immanence, or emptiness stretch human ability to conceptualize and communicate. Enlarging the understanding of God from within a tradition itself to a view that understands God's presence to be active in all traditions, moves us beyond the pluralist category. A second way of going beyond the current model does not attempt to elucidate pan-religious conceptions of the divine but strives to analyze particular traditions themselves. The history of particular traditions and their encounters with one another influence the theologies of the religions themselves and shape theories of religious otherness purported by those traditions. Finding a particular historical set of conditions and beliefs conducive to a positive pluralism that maintains the integrity of each tradition without relativizing all positions would move us beyond Hick's current relativizing pluralist position. These two approaches each struggle with a different side of the tension resulting from presuppositions in the theory of religions put forward in the exclusivist, inclusivist, pluralist model. The first grapples with the ontological issues and the second addresses the epistemological issues of that tension.Harry Wells chooses to work with the first—the ontological assumption of divine reality. He outlines a Christian Trinitarian relational view of God that not only encompasses but goes beyond the pluralist category as outlined by John Hick. At the same time, he finds the source for that view in a particular historical tradition of Christianity—the early church fathers. [End Page 133]Wells begins by wondering how he can move beyond Raimon Panikkar's pluralism, which, he claims, already moves beyond Hick's understanding of the pluralist category. Wells then outlines his own pluralist understanding by describing a dynamic and relational understanding of the divine. In so doing, he entertains the notion of knowing something of noumenal reality, no matter how indirectly, through the eyes of a particular tradition. By drawing the circle of divine presence very widely, Wells attempts to express from within the Christian tradition a view of God that allows for a distinctive divine work through other traditions. The practitioner, through experience, encounters a reality that changes perceptions both of the divine and of the religious other.Here Wells moves beyond the necessity of deconstructing all absolute claims to truth by different religions in order to maintain a pluralist position. Rather than relativize all particular religious traditions, as Hick suggests, Wells utilizes a perspectival approach and projects from it a view of the divine that allows for truth in other traditions. In this way he removes the debilitating necessity of embracing agnosticism by all religious traditions and shows, rather, how pluralist claims may be formed from within a single deeply held tradition.This broader understanding of the divine, rooted in early Christian mysticism according to Wells, does not rule out tolerance of other religions but embraces tolerance for reasons of faith, rather than through agnosticism. Nicholas Shea offers Varitel Semantics as a naturalistic account of mental content. I argue that the account secures determinate content only by appeal to pragmatic considerations, and so it fails to respect naturalism. But that is fine, because representational content is not, strictly speaking, necessary for explanation in cognitive science. Even in Shea’s own account, content serves only a variety of heuristic functions.

Morality, Mortality as a whole deals with certain aspects of ethical theory and with moral problems that arise primarily in contexts involving life‐and‐death decisions. The importance of the theoretical issues is not limited to their relevance to these decisions; however, they are, rather, issues at the heart of basic moral and political theory. This first volume comprises three parts. Part I, Death: From Bad to Worse, has with four chapters, and an appendix, discussing death and why it is bad for (...) the person who dies. Part II, Saving Lives: General Issues, has six chapters, and explores a cluster of moral problems that arise in saving lives. The general question raised is whether we should always, in aiding life, act so as to save the greater number of lives or to produce the greatest amount of good. Part III, Scarce Resources: Theoretical Issues, Specific Recommendations, and Organ Transplants, has five chapters, and deals with the problem of the acquisition and distribution of organs for transplantation. It allows us to apply the theoretical discussion of saving lives and relevant/irrelevant utilities presented in the previous parts. However, the discussion can be understood independently of the first two parts, and the conceptual issues and procedures on which it focuses are relevant to dealing with any scarce resource, including money and time, which are needed to use other plentiful resources. Although the book contains much theoretical and methodological argument, it is firmly grounded in practical ethical issues, and is illustrated throughout by examples. (shrink)

The purpose of this article is to investigate how cultural meanings associated with the left ventricular assist device inform acceptance and experience of this innovative technology when it is used as a destination therapy. We conducted open-ended, semistructured interviews with family caregivers and patients who had undergone LVAD-DT procedures at six U.S. hospitals. A grounded theory approach was used for the analysis. Thirty-nine patients and 42 caregivers participated. Participants described a sense of obligation to undergo the procedure because of its (...) promise for salvation. However, once the device was implanted, patients described being placed into a liminal state of being neither sick nor healthy, with no culturally scripted role. Consideration of end-of-life decisions was complicated by the uncertainties about how patients with LVADs die. Pre-implantation communications among patient, family, and clinicians should take into account the impact of the technology on meaning, identity, and patient experience. (shrink)
