In an effort to respond to the large surge in COVID-19 cases in Arizona that began between May and July 2020, the Arizona State University (ASU) Student Outbreak Response Team (SORT) formed a remote, volunteer-based case investigation team that worked in partnership with a local public health department through delegated public health authority.

Shortages of new therapeutics to treat coronavirus disease (COVID-19) have forced clinicians, public health officials, and health systems to grapple with difficult questions about how to fairly allocate potentially life-saving treatments when there are not enough for all patients in need (1). Shortages have occurred with remdesivir, tocilizumab, monoclonal antibodies, and the oral antiviral Paxlovid (2) -/- Ensuring equitable allocation is especially important in light of the disproportionate burden experienced during the COVID-19 pandemic by disadvantaged groups, including Black, Hispanic/Latino and (...) Indigenous communities, individuals with certain disabilities, and low-income persons. However, many health systems have resorted to first-come, first-served approaches to allocation, which tend to disadvantage individuals with barriers in access to care (3). There is mounting evidence of racial, ethnic, and socioeconomic disparities in access to medications for COVID-19 (4, 5). -/- One potential method to promote equitable allocation is to use a weighted lottery, which is an allocation strategy that gives all eligible patients a chance to receive the scarce treatment while also allowing the assignment of higher or lower chances according to other ethical considerations (6). We sought to assess the feasibility of implementing a weighted lottery to allocate scarce COVID-19 medications in a large U.S. health system and to determine whether the weighted lottery promotes equitable allocation. RationaleSurrogates of critically ill patients often have inaccurate expectations about prognosis. Yet there is little research on how intensive care unit clinicians should discuss prognosis, and existing expert opinion-based recommendations give only general guidance that has not been validated with surrogate decision makers.ObjectiveTo determine the perspectives of key stakeholders regarding how prognostic information should be conveyed in critical illness.MethodsThis was a multicenter study at three academic medical centers in California, Pennsylvania, and Washington. One hundred eighteen key stakeholders completed in-depth semistructured (...) interviews. Participants included 47 surrogates of adult patients with acute respiratory distress syndrome; 45 clinicians working in study ICUs, including physicians, nurses, social workers, and spiritual care providers; and 26 experts in health communication, decision science, ethics, family-centered care, geriatrics, healthcare disparities, palliative care, psychology, psychiatry, and critical care.Measurements and main resultsThere was broad support among surrogates for existing expert recommendations, including truthful prognostic disclosure, emotional support, tailoring the disclosure strategy to each family's needs, and checking for understanding. In addition, stakeholders offered suggestions that add specificity to existing recommendations, including: In addition to conveying prognostic estimates, clinicians should help families "see the prognosis for themselves" by showing families radiographic images and explaining the clinical significance of physical manifestations of severe disease at the bedside. Many physicians did not support using numeric estimates to convey prognosis to families, whereas many surrogates, clinicians from other disciplines, and experts believed numbers could be helpful. Clinicians should conceptualize prognostic communication as an iterative process that begins with a preliminary mention of the possibility of death early in the ICU stay and becomes more detailed as the clinical situation develops. Although prognostic information should be initially disclosed by physicians, other members of the multidisciplinary team-nurses, social workers, and spiritual care providers-should be given explicit role responsibilities to reinforce physicians' prognostications and help families process a poor prognosis emotionally.ConclusionsFamily members, clinicians, and experts identified specific communication behaviors that clinicians should use to discuss prognosis in the critical care setting. These documents, published here for the first time, present the Treasury's counter-arguments during the period when Keynes was developing the ideas that led to the Keynesian revolution in economic policy. Keynes spent much effort trying to persuade the Treasury to adopt policies designed to raise employment and stabilise prices, and to create an international monetary system that would favour these objectives. His arguments are set out fully in the Royal Economic Society's 30-volume set of The Collected Writings of John Maynard (...) Keynes. In contrast, the views of his Treasury critics have hitherto been much less accessible.Economists and historians have tended to assume that Keynes was right and the Treasury was wrong; this volume shows that the Treasury anticipated the political problems that would be encountered in putting Keynes's ideas into practice. Much of what Keynes published was deliberately polemical: he believed that words should be 'a little wild', for they were 'the assault of thought on the unthinking'. Treasury officials were by no means as unthinking as Keynes tended to portray them, and they had a coherent and intellectually respectable understanding of public finance. Ministers in the inter-war period and early in the Second World War were sensitive to the use that political opponents might make of Keynes's arguments; officials had to provide counter-arguments, and in doing so they revealed much about their views on economics and public finance. Once Keynes became an adviser to the Chancellor of the Exchequer in 1940, the debate became internal to the Treasury, but officials continued to subject Keynes's ideas to critical analysis.The documents in this volume show Treasury responses to Keynes on a range of issues crucial to understanding the period and the context of the Keynesian revolution in public policy. With increasing complexity in the evolving structure of work in organizations, employees’ preferences for working from home (WFH) relative to working on-site can lead to systematic differences in perceived career implications. An emerging tension associated with WFH versus work-at-work is whether this locational divide is associated with concerns over career progression, especially among racial minorities. Here, we seek to determine whether Black employees, relative to their White counterparts, have more concerns over career progression relating to WFH compared with their (...) on-site colleagues. From a sample of employees in the May 2021 cross-section of the Survey of Working Arrangements and Attitudes (SWAA), we find that relative to higher (lower) income White employees, higher (lower) income Black WFH employees have significantly higher (lower) concerns over career progression than their on-site counterparts. These findings speak both to the nontrivial choices employees may be forced to make as the structure of work continues to evolve and consequences for racial inequality. (shrink)

Interdisciplinary collaboration figures centrally in frontier research in many fields. Participants in inter-disciplinary projects face problems they would not encounter within their own disciplines. Among those are problems of mutual understanding, of finding a language to communicate both within projects and with the scientific community and society at large, and of needing to master concepts and methods of different disciplines. We think that a concentrated research and development effort is necessary to analyze, on the one hand, cognitive conditions of successful (...) understanding, communication, and interaction and, on the other, to develop specific tools and methods that support and facilitate inter-disciplinarity both in practice and in educational projects that prepare future generations of professionals within and outside of academia. Introduction: Early recognition of prediabetes may prevent progression to diabetes, yet not all adults are aware of their prediabetes risk. To reach all adults unaware of their risk, additional risk assessment strategies are warranted. Objectives: The objective of this study was to evaluate the potential scope of benefit from prediabetes risk assessment in the dental care setting and to identify characteristics of dental patients likely to unknowingly have prediabetes or diabetes. Methods: Data from 10,472 adults in the National Health and (...) Nutrition Examination Survey from 2013 to 2014 and 2015 to 2016 were analyzed for associations among prediabetes/diabetes risk factors, health care use, and hemoglobin A1C levels according to chi-square tests and multivariate logistic regression. Results: A total of 7.73% of US adults had seen a dentist but not a medical provider in the past 12 mo. The composition of this subpopulation was significantly different from that who saw a medical provider, in ways that might affect their diabetes risk. In addition, 31.27% of this subpopulation would be identified as being at high risk for prediabetes according to the CDC Prediabetes Screening Test, and 15.83% had hemoglobin A1C levels indicative of undiagnosed prediabetes or diabetes. Screening in a dental setting would have the highest odds of identifying someone unaware of his or her diabetes risk among those who were non-White, obese, or ≥45 y old. Conclusion: Extrapolation from this analysis indicates that screening for prediabetes at dental visits has the potential to alert an estimated 22.36 million adults of their risk for prediabetes or diabetes. Incorporating prediabetes or diabetes risk assessment into routine dental visits may enable 1) those with prediabetes to take action to decrease their risk of developing diabetes and 2) those with diabetes to engage in treatment to decrease their risk of diabetes-related complications. Knowledge Transfer Statement: Screening for prediabetes and diabetes during dental visits has the potential to raise patients’ awareness of diabetes risk and prevent prediabetes from progressing to diabetes. For some patients, the dental visit may be the only point of contact with the health care system, which heightens the importance of including diabetes risk assessment for patient well-being. This article examines mixed ℋ -infinity and passivity synchronization of Markovian jumping neutral-type complex dynamical network models with randomly occurring coupling delays and actuator faults. The randomly occurring coupling delays are considered to design the complex dynamical networks in practice. These delays complied with certain Bernoulli distributed white noise sequences. The relevant data including limits of actuator faults, bounds of the nonlinear terms, and external disturbances are available for designing the controller structure. Novel Lyapunov–Krasovskii functional is constructed to verify (...) the stability of the error model and performance level. Jensen’s inequality and a new integral inequality are applied to derive the outcomes. Sufficient conditions for the synchronization error system are given in terms of linear matrix inequalities, which can be analyzed easily by utilizing general numerical programming. In this study, we investigate the effectiveness of a robust sampled-data H ∞ load frequency control scheme for power systems with randomly occurring time-varying delays. By using the input-delay technique, the sampled-data LFC model is reformulated as a continuous time-delay representation. Then, Bernoulli-distributed white noise sequences are used to describe randomly occurring time-varying delays in the sampled-data LFC model. Some less conservative conditions are achieved by utilizing the Lyapunov–Krasovskii functional and employing Jensen inequality and reciprocal convex combination lemma to (...) ensure the considered power system has mean-square asymptotic stability under the designed control scheme. The derived results are based on linear matrix inequalities that can readily be solved using the MATLAB LMI toolbox. The criteria obtained are used to analyze the upper bounds for time delays, and a comparison study to validate the efficacy of the presented method is presented. (shrink)

By the time George Wilton Field concluded his work at the marine laboratory his initial scientific concerns had forced him directly into local politics. He pleaded with little success with the community of South Kingstown, and with no success with the town of Narragansett, to create and maintain a permanent breach:Is it not possible for the acute business sense and the broad philanthropy of the community to sweep aside petty, local, and personal jealousies which are now blocking practical progress for (...) the establishment of a permanent breach at Point Judith Pond? It is truly criminal neglect which permits fifteen hundred acres of valuable water-farming area to lie practically idle and rapidly deteriorate with each passing year.... In the opinion of the writer the Point Judith Pond and those of similar type could be made the seat of oyster, clam, crab, herring, white perch, and striped bass fisheries.30In the summer of 1899 Field was invited to teach a summer course on echinoderms at the MBL in Woods Hole, and to conduct summer research in a laboratory of the U.S. Fish Commission, also located at Woods Hole. When the summer was over, he remained there. Whether he had intentions of returning to resume his position in Rhode Island is unclear. At this point all correspondence with the Agricultural Experiment Station ceases, and Field's last report is a brief statement in the annual report of the experiment station for 1900 wherein he laments the variety of experiments he has not been able to carry to conclusion, such as a study of the artificial fertilization of water analogous to the method of chemically fertilizing the land for crops.The correspondence reveals that the enthusiasm Field brought to Point Judith Pond in 1896 was gradually sapped by his own fragile health, by three years' exposure to the local politics surrounding the southern Rhode Island fishing industry, and by a college administration determined to remove the stench of his invertebrates. He sought a refuge in the sheltered world of pure research at the U.S. Fish Commission Laboratory, where he set out to investigate the “Origin of Sex” using, as his animal models, squid and toadfish.On November 14, 1899, the Board of Managers of the college ordered the director of the experiment station to dispose of the marine laboratory at Point Judith Pond.31 How long the laboratory at Buttonwood Point survived in the institutional memory of the University of Rhode Island is open to question. The current Graduate School of Oceanography, in the event, traces its history back to 1937, not 1896.Nevertheless, Field and his one-room marine station established a precedent of land-grant marine research that other state colleges would follow, including Rhode Island itself, which reestablished its marine station, this time permanently, at South Ferry in 1937. In his brief research career in Rhode Island, George Wilton Field had discovered the same coastal attributes that would lead later to the creation of one of the world's major marine research centers at the University of Rhode Island's Graduate School of Oceanography.And, in a measure of triumph for his work, little more than a year after Field left for Woods Hole and his laboratory was dismantled, the town of South Kingstown voted the funds necessary to begin the construction of a permanent breachway.32 Whether Field's scientific reasoning and the conclusions of his marine research played any part in finally deciding the thirty-year-old debate in the affirmative will probably never be known. "Mr. Lincoln came in through the side passage which had lately been constructed," Carpenter writes, "holding in his hand a roll of manuscripts." Lincoln turned to his guests and quipped, "Lots of wisdom in that document, I suspect; it is what will be called my 'second inaugural' " (quoted in Barondess, 62).The content and modesty of Carpenter's description of the "preliminaries of the Second Inaugural" give the rhetorical critic a telling preview of what might be discovered in a reading of Lincoln's composition of that manuscript. The understatement in the length and simplicity of the address subtly and poignantly charms the reader. And there is "lots of wisdom" in the Second Inaugural: along with the Gettysburg Address, it contains the words of Lincoln that most abide. In a letter to an admiring Thurow Weed, who had penned Lincoln that his inaugural was "the neatest but the most pregnant and effective use to which the English Language was ever put," Lincoln wrote that he "expected" the Second Inaugural to "wear as well as—perhaps better than—anything I have ever produced" (quoted in Barondess 1954, 78).Underscoring Lincoln's perspicacity when he wrote to Weed that he "expected" the speech to "wear well," Carl Schulz's designation of the address as a "sacred poem" and the frequency with which that epithet is quoted by other critics confirm Lincoln's assessment of the oratorical endurance [End Page 223] of the piece. The Second Inaugural seemingly speaks beyond its content and situation.Contained within the aura of adoration that surrounds the Second Inaugural is a significant charge to the rhetorical critic: what is it particularly about the address that sustains its agency and permits it to escape the evanescence accorded to most rhetorical efforts? When the agency of discourse helps to eliminate or extenuate the immediate exigency that calls forth that speech, it usually becomes remanded to the annals of history and is examined by the public or by scholars only when that historical nidus forces itself back upon the public's or the scholar's attention. The speech is exhausted by its situation and swallowed by its surrounding and future events. There are, however, those speeches like the Gettysburg Address and the Second Inaugural that practice their persuasion on audiences beyond the immediate exigency. They persist, they talk to us still, and their agency endures. Why is this true of Lincoln's Second Inaugural? This is the question that motivates my close reading of Lincoln's speech; and, as I hope to demonstrate, it is only through a close reading that such a question even makes sense. The aim of this paper is to defend a famous quotation from Martin Luther King, stating that “The arc of the moral universe is long, but it bends toward justice.” The quotation is inscribed on the King Memorial in Washington, D.C. and President Obama had it woven into a rug for the Oval Office in the White House. The quotation has become something of a contemporary proverb, and is certainly worthy of our close attention. In order to evaluate the (...) dictum, questions concerning its meaning will first be addressed and clarified, and various possible misinterpretations will be set aside. It will be argued that the appeal, and an effective defense of this moral claim, depend upon the pre-existing values of the people to whom the claim is addressed. The dictum is clearly intended to support hopes of social change and to encourage support for ideals of racial equality, but we want to know whether it is true or false and exactly what it means. King’s dictum can easily be taken as involving a doctrine of “divine Providence” or “historical inevitability.” But many are skeptical of these ideas and hold that we cannot be sure that the future will eventuate in desired moral outcomes. But, if so, what would it possibly mean to claim that the “moral universe” or the human world “bends toward justice”? On the other hand, holding that the moral universe “bends toward justice” claims more than saying that we can now act or organize to support justice; instead, it tells us that there is some pre-existing support for our related activities. Her comment was swiftly seized upon by journalists and members of rival political campaigns, who used it to portray Mrs. Obama as "angry" and unpatriotic. In the weeks that followed, (...) Mrs. Obama's undergraduate sociology thesis on black students at Princeton was cited as further evidence of her supposed racial resentment and lack of patriotism, and there were widespread rumors that an audiotape existed of her using the word whitey.1 Around the same time, Fox News ran a headline referring to Mrs. Obama as "Obama's Baby Mama" (Koppleman 2008). Subsequently, there was a concerted effort on the part of Mr. Obama's campaign to "soften" her image through appearances on the shows The View and Paula's Party, and a speech at the Democratic National Convention that highlighted her motherhood and adoration of her husband, and avoided mention of her own career or political work. Despite such efforts, and perhaps in tension with the Obama family's general popularity following Mr. Obama's election, Mrs. Obama was later characterized by a cable news pundit as being reminiscent of "Stokely Carmichael in a designer dress" (The O'Reilly Factor 2009) and thus as a potential political liability for the Obama administration.While some feminists decried the "momification" (Traister 2008) of Michelle Obama as an act of appeasement by an administration that would [End Page 150] capitulate to the oppressive expectations of a public unwilling to find expressions of independent thought or professional ambitions appropriately First Lady-like, other critics noted that Mrs. Obama was being reviled more for daring to suggest that the United States' racial history was far from being strictly historical, opening her to the charge of being an angry Negro—while, ironically, being accused of "playing the race card." Others have pointed out that specific formulations such as "baby mama" are illustrative of the fact that such representations of Mrs. Obama are not easily classifiable as either sexist or racist, but instead draw on both sorts of tropes, as have such familiar representations of black women as promiscuous, or as militant, emasculating mammies.In this article, I consider methodologies for theorizing the function of such oppressions in the public discourses surrounding Michelle Obama, and suggest that a consideration of their manifestations in this context illustrates the need for more complex theoretical tools that can problematize the constitutive effects of such discourse for a multiplicity of subjects. Through parallel analyses of Beauvoir's The Second Sex and Fanon's Black Skin White Masks, I foreground the theoretical pitfall the consideration of sex- or race-based oppressions tends to occasion: the tacit presumption of some ostensibly "neutral" subject whose constitution is inadequately addressed. I suggest, with theorists of intersectionality, that this problem will not be solved merely by the addition of alternative explanatory frameworks (e.g., adding racism to sexism as a discrete phenomenon), since doing so is neither adequately representative of the experience of oppressions nor sufficient to the task of undermining the normative/perverse dichotomy or presumption of generic subjectivity that sustains them. However, I also argue that the work of intersectionality theorists should be taken farther than it has frequently been in the past (which, though given lip service by much of the feminist community, has not always been taken to imply the need for a major methodological shift), extending its insights to offer a critique of foundational subjectivity. Concomitantly, I argue that a failure by feminist philosophers to offer this critique could unintentionally result in the reinscription of the normativity of particular sorts of subjects—which is precisely what theorists of intersectionality have been concerned to call into question. I suggest that an analysis of the discourse surrounding Michelle Obama demonstrates that insofar as the intersectionality paradigm is understood merely to require the interrogation of overlapping oppressions on a given subject or subjects, and thus as leaving uninterrogated the constitutively privileging and subjectivizing effects of oppressive discourse, uses of intersectionality will reiterate... Consecutive waves of paleolithic migrants crossing the Bering land bridge from Siberia to North America between 80,000 and 7,000 b.c. brought with them the shamanic way of harnessing supernatural powers. This way prevailed until the White intrusion 400 years ago, into the living space of the aboriginal peoples of North America. Wherever European political, religious, and economic dominance was established, shamanic institutions became the focus of negative attention. The shamanic practitioner was variously depicted by governmental and ecclesiastic authorities as (...) a charlatan and imposter or a purveyor of evil influence. Some well-known ethnological and medico-psychological experts have until very recently portrayed the shaman as a mentally deranged person whose “primitive” culture permits the acting-out of psychopathology in a prestigious role, a eurocentric and positivistic fallacy rooted in Western misinterpretations of learned behavior manifested during shamanic rituals involving altered states of consciousness. Legal measures to suppress shamanic ceremonials were taken in the United States, especially in the aftermath of the Ghost Dance. This shaman-inspired movement, originating in the Prophet Dance of the Pacific Northwest (Spier 1935), sent waves of hectic sacro-nativistic ceremonial activity through many Amerindian tribes, and finally culminated in the Sioux uprising of 1890 which ended in the tragedy of Wounded Knee (cf. The first part develops six different theories of happiness and the second presents different controversies about happiness. In the first chapter, the author proposes Aristotle's eudemonism [[sic]] as the most complete and most influential of all theories of happiness, and he uses it as a matrix for most of the discussions in the (...) second part. Chapters following this initial exposition of Aristotle treat Plato's "mixed" eudemonism [[sic]], Stoic suppression of desire, concepts of transcendent happiness in Plotinus, Augustine and Aquinas, Kant's valorization of duty and Hegel's critique thereof, and Bentham's and Mill's utilitarianism. Moral issues discussed in the second part include: self-realization, happiness vs. performance of duty, fulfillment vs. prudence, and eudemonism [[sic]] vs. hedonism. The last thirty pages focus on contemporary pursuits of the good life, centered around the issue of self-actualization. This section is predominantly psychological, and treats very briefly some theories of Kurt Goldstein, G. W. Allport, Robert W. White, A. H. Maslow, Marie Jahoda, Carl Rogers, and Fromm. Although the author take sides, he does stimulate reflection about the good life through expositions which avoid more difficult philosophical problems but which definitely evoke practical individual and social overtones.--C. According to the prevailing sentiment, changing the past is logically impossible. The prevailing sentiment is wrong. In this paper, I argue that the claim that changing the past entails a contradiction ultimately rests upon an empirical assumption, and so the conclusion that changing the past is logically impossible is to be resisted. I then present and discuss a model of time which drops the empirical assumption and coherently models changing the past. Finally, I defend the model, and changing the past, (...) against objections. Alchemy is central to Jung's hypothesis of the collective unconscious. In this volume he begins with an outline of the process and aims of psychotherapy, and then moves on to work out the analogies between alchemy, Christian dogma and symbolism and his own understanding of the analytic process. Introducing the basic concepts of alchemy, Jung reminds us of the dual nature of alchemy, comprising both the chemical process and a parallel mystical component. He also discusses the seemingly deliberate mystification of (...) the alchemists. Finally, in using the alchemical process as providing insights into individuation, Jung emphasises the importance of alchemy in relating to us the transcendent nature of the psyche. (shrink)
