There is little information regarding arterial elasticity in heart transplant recipients. This study aimed to characterize small and large artery elasticity in (...) heart transplant recipients in comparison with an asymptomatic population free of overt cardiovascular disease. A second goal was to identify demographic and clinical factors associated with arterial elasticity in this unique population.Methods: Arterial pulse waveform was registered noninvasively at the radial artery in 71 heart transplant recipients between 2008 and 2010. SAEs and LAEs were derived from diastolic pulse contour analysis. Comparisons were made to a healthy cohort of 1,808 participants selected from our prevention clinic database. Multiple regression analyses were performed to evaluate associations between risk factors and SAE and LAE within the heart transplant recipients.Results: LAE and SAE were significantly lower in heart transplant recipients than in the normal cohort. Female sex and history of ischemic cardiomyopathy were significantly associated with reduced LAE and SAE. Older age and the presence of moderate cardiac allograft vasculopathy were also significantly associated with reduced SAE. Transplant duration was associated with increased SAE.Conclusion: Heart transplants are associated with peripheral endothelial dysfunction and arterial stiffness, as demonstrated by a significant reduction in SAE and LAE when compared with a healthy population. Furthermore, cardiac allograft vasculopathy is associated with a significant reduction in SAE, suggesting a potential use in screening heart transplant recipients at risk for developing this complication.Keywords: heart transplant, endothelial function, arterial elasticity, cardiac allograft vasculopathy. Caesar Augustus, that all the world should be taxed.... And all went to be taxed, every one into his own city.” Thus the familiar words of Luke 2:1, 3, telling of the return of Joseph and Mary from Nazareth to be registered in their own home town of Bethlehem.We have no census returns from ancient Judaea. What we do have, however, are 300 or so returns from Roman Egypt, written on papyrus and submitted to the local authorities in connection with a similar house-to-house census which was held every 14 years from early in the reign of Augustus until the mid-third century A.D. The returns detail the composition of individual households, including in many cases tenants and slaves as well as family members, and constitute perhaps the best source available for the study of any ancient population. It is these texts which form the basis of Bagnall and Frier’s study.This is not, of course, the first time that the demographic importance of this body of evidence has been recognised. The information contained in the census returns was brought together and comprehensively discussed as long ago as the 1950s by M. Hombert and C. Préaux, Recherches sur le recensement dans l’Égypte romaine, Leiden 1952. But many more documents have been published since then and the science of demography itself has developed considerably in the last 40 years. As a result The Demography of Roman Egypt represents a quantum leap forward in the study of Roman Egypt’s population.The book is divided into two parts. The first, written mainly by Frier, analyses and interprets the data and makes use of the techniques of modern demography [End Page 341] to construct as full a picture as possible of the population of Roman Egypt throughout the 250 years covered by the census returns. The second consists of a Catalogue, in which Bagnall sets out all the known returns in their chronological and topographical order. Bagnall has examined all available texts, many in the original, and corrected and improved the reading of many of them in a series of articles in BASP 1990–93. His results are summarised here in a form comprehensible to the nonspecialist reader and there can be no doubt that the study’s database is as accurate, complete and up-to-date as is humanly possible to make it.This does not mean, however, that what finally emerges from the evidence is anywhere near a complete portrait of an ancient population. The majority of the census returns come from only three nomes (the Arsinoite with its three merides, the Oxyrhynchite, and Prosopite, representing the Fayum, Middle Egypt and the Delta respectively) out of a total of some 50. They are concentrated almost entirely in the second century A.D., with the examples from the Prosopite deriving almost entirely from the census of 173. Nevertheless throughout their work the authors remain alert to the chronological and geographical limitations of their data and sensible of the dangers of pushing their conclusions too far. The result is the construction of a hypothetical population, which probably corresponds quite closely to the historical ancient population of the Arsinoite and the Oxyrhynchite, which remain the two best documented regions of Greco-Roman Egypt.The first part of the book begins with a chapter on the history of the census insofar as it is known, the census process and the uses which the government made of the information extracted. The usual way for new cells to come into being is by division of old cells. So the zygote, which is a—new—single cell formed from two, the sperm and ovum, is an exception. Textbooks of human genetics usually say that this new cell is beginning of a new human individual. What this indicates is that they suddenly forget about identical twins.

A significant proportion of conservationists' work is directed towards efforts to save disappearing species. This relies upon the belief that species extinction is undesirable. When justifications are offered for this belief, they very often rest upon the assumption that extinction brought about by humans is different in kind from other forms of extinction. This paper examines this assumption and reveals that there is indeed good reason to suppose current anthropogenic extinctions to be different in kind from extinctions brought about at (...) other times or by other factors. Having considered – and rejected – quantity and rate of extinction as useful distinguishing factors, four alternative arguments are offered, each identifying a way in which anthropogenic extinction is significantly different from other forms of extinction, even mass extinction: (1) Humans are a different kind of natural cause from other causes of extinction; (2) Extinctions brought about by humans are uniquely persistent; (3) Anthropogenic extinctions are effectively random whereas past mass extinctions are rule-bound; (4) The impact of the current anthropogenic extinction event differs from the impact of other extinction events of the past, such that future recovery may not follow past patterns. Together, these four arguments suggest that the present-day extinction event brought about by humans may be unprecedented and that we cannot clearly extrapolate from past to present recovery from extinctions. Although insufficient as justification for the claim that present-day extinctions are undesirable, the arguments provide some ammunition for conservationists' conviction that species extinction – in which humans play an accelerating role – ought to be prevented. One of the ways of dividing all philosophers into two kinds is by saying of each whether he is an ordinary man's philosopher or a philosophers' philosopher. Thus Plato is a philosophers' philosopher and Aristotle an ordinary man's philosopher. This does not depend on being easy to understand: a lot of Aristotle's Metaphysics is immensely difficult. Nor does being a philosophers' philosopher imply that an ordinary man cannot enjoy the writings, or many of them. Plato invented and exhausted a form: (...) no one else has written such dialogues. So someone with no philosophical bent, or who has left his philosophical curiosity far behind may still enjoy reading some of them. (shrink)

Knowledge of the ethical and legal basis of medicine is as essential to clinical practice as an understanding of basic medical sciences. In the UK, the General Medical Council requires that medical graduates behave according to ethical and legal principles and must know about and comply with the GMC’s ethical guidance and standards. We suggest that these standards can only be achieved when the teaching and learning of medical ethics, law and professionalism are fundamental to, and thoroughly integrated both vertically (...) and horizontally throughout, the curricula of all medical schools as a shared obligation of all teachers. The GMC also requires that each medical school provides adequate teaching time and resources to achieve the above. We reiterate that the adequate provision and coordination of teaching and learning of ethics and law requires at least one full-time senior academic in ethics and law with relevant professional and academic expertise. In this paper we set out an updated indicative core content of learning for medical ethics and law in UK medical schools and describe its origins and the consultative process by which it was achieved. Advances in the sharing of best practices in the donation community (...) have contributed greatly to this aim over the past 5 years. Studies published during the past five years were analyzed to gain insight on the evolving organizational areas and tools that the procurement and transplantation pathways have been focused on. The hypothesis assessed is that networking and efficacious handling of this complex path may be ameliorated by an adaptive organizational toolbox. Methods: A thorough search has been conducted using various databases, ie, Cochrane library, PubMed, EMBASE, Federico II University Open Archive. The evidence was considered following the Effective Practice and Organization of Care Group checklist. Prevalent organizational attitudes and areas were assessed, and various scenarios were analyzed. Initially, all titles and abstracts were screened. In the next phase, the full text of all abstracts considered potentially relevant by at least one of the reviewers was evaluated. Inconsistencies in decision-making within this second phase were solved based on consensus between both reviewers. In this phase, for every study we defined whether the organization was considered relevant and what the scenario was. The information was extracted from each study based on bibliographic details. As many as 1,071 studies were analyzed, and 81 were selected as potentially relevant. Results: We found three prevalent areas of interest focused on organizational elements: global organizational strategies scenario, clinical hospital organization, and citizenship and social scenario. Conclusion: We reached the conclusion that organization has a central role in different scenarios of procurement and transplantation in a continuum from government to hospital and finally among citizens. A standardized hospital pathway definitely remains the essential step in order to ameliorate either procurement or transplantation. Based on the exactly solvable case of a harmonic oscillator, we show that the direct correspondence between the Bohr-Sommerfeld phase of semiclassical quantum mechanics and the topological phase of Aharonov and Anandan is restricted to the case of a coherent state. For other Gaussian wave packets the geometric quantum phase strongly depends on the amount of squeezing.
