Medical aid in dying (MAiD), despite being legal in many jurisdictions, remains controversial ethically. Existing surveys of physicians’ perceptions of MAiD tend to focus on the legal or moral permissibility of MAiD in general. Using a novel sampling strategy, we surveyed physicians likely to have engaged in MAiD-related activities in Colorado to assess their attitudes toward contemporary ethical issues in MAiD.

Introduction: The coronavirus disease 2019 pandemic has placed intensive care units (ICU) triage at the center of bioethical discussions. National and international triage guidelines emerged from professional and governmental bodies and have led to controversial discussions about which criteria—e.g. medical prognosis, age, life-expectancy or quality of life—are ethically acceptable. The paper presents the main points of agreement and disagreement in triage protocols and reviews the ethical debate surrounding them. Sources of data: Published articles, news articles, book chapters, ICU triage guidelines (...) set out by professional societies and health authorities. Areas of agreement: Points of agreement in the guidelines that are widely supported by ethical arguments are (i) to avoid using a first come, first served policy or quality-adjusted life-years and (ii) to rely on medical prognosis, maximizing lives saved, justice as fairness and non-discrimination. Areas of controversy: Points of disagreement in existing guidelines and the ethics literature more broadly regard the use of exclusion criteria, the role of life expectancy, the prioritization of healthcare workers and the reassessment of triage decisions. Growing points: Improve outcome predictions, possibly aided by Artificial intelligence (AI); develop participatory approaches to drafting, assessing and revising triaging protocols; learn from experiences with implementation of guidelines with a view to continuously improve decision-making. Areas timely for developing research: Examine the universality vs. context-dependence of triaging principles and criteria; empirically test the appropriateness of triaging guidelines, including impact on vulnerable groups and risk of discrimination; study the potential and challenges of AI for outcome and preference prediction and decision-support. This book, published in 1986, addresses questions concerned with a central normative principle in contemporary assessments of economic policies and systems. What does 'consumer sovereignty' mean? Is consumer sovereignty an appropriate principle for the optimization and evaluation of the design and performance of economic policies, institutions and systems? If not, what is a more appropriate principle? The author argues that the conception of consumer sovereignty has to be broadened so that it is not limited to the market mechanism but includes (...) environmental, work and social preferences. However, even this version runs into serious difficulties as the principle of consumer sovereignty still relies on too subjectivist a conception of the interests of individuals to be suitable for the evaluation of economic institutions. An alternative basis for such evaluation is 'human interests' that are not contingent on particular economic systems, After considering various possibilities, a basic-needs approach is proposed and its use in economic evaluation illustrated. According to the ‘conceivability argument’ [1] it’s conceivable that a conscious human being H may have a perfect physical duplicate H* who isn’t conscious, [2] whatever is conceivable is possible, therefore [3] H* may possibly exist. This paper argues that the conceivability argument can’t help in discussion of the ‘mind–body problem’ even if [2] is allowed to be true. This is not because [1] is false, but because we don’t and can’t know enough about the nature of the physical to (...) know whether or not [1] is true. This follows from ‘the silence of physics’—the fact that physics neither does nor can tell us about the intrinsic non-structural nature of the physical, and the consequences of this fact for any adequate account of the meaning of the word ‘physical’. The nearest that we get to contact with it in this period is in the writings of Plutarch. He was himself a student there, and was well read in the books of Plato and the commentaries thereon.

Controversy surrounds several experiments that have addressed whether selective synaptic strengthening occurs during learning. To date, the evidence suggests that widespread alterations in synaptic strength, through either kindling or electroconvulsive shock, can disrupt this hypothetical process. The lack of evidence for selective modification of learning through LTP stimulation, however, provides difficulties for both the prevailing hypothesis and the hypothesis advanced by Shors & Matzel. Subsequent experiments may indicate a role for LTP in both learning and arousal.

The social aspects of causality in medicine and healthcare have been emphasized in recent debates in the philosophy of science as crucial factors that need to be considered to enable, among others, appropriate interventions in public health. Therefore, it seems central to recognize the bearing of social causes (broadly understood, e.g., social inequalities and socio-economic status) in bringing about certain concrete pathologies. Being aware of the relevance of social causes in medicine and healthcare is particularly important in the face of (...) the role that artificial intelligence-based systems (such as machine learning algorithms) are increasingly playing in these high-stakes fields. In fact, these systems bear the dangerous potential of concealing relevant social causes. This is highly problematic not only because it reinforces issues of distributive injustice but also because it can pave the way for issues of epistemic injustice. The central aim of this chapter is to make a first effort to point out possible connections between the importance of recognizing social causes in medicine and healthcare and forms of epistemic injustice.
