Scientists, engineers, and healthcare professionals are currently developing a variety of new devices under the category of brain-computer interfaces (BCIs). Current and future applications are both medical/assistive (e.g., for communication) and non-medical (e.g., for gaming). This array of possibilities comes with ethical challenges for all stakeholders. As a result, BCIs have been an object of both hope and concern in various media. We argue that these conflicting sentiments can be productively understood in terms of personhood, specifically the impact of BCIs (...) on what it means to be a person and to be recognized as such by others. To understand the dynamics of personhood in the context of BCI use and investigate whether ethical guidance is required, a meeting entitled "BCIs and Personhood: A Deliberative Workshop" was held in May 2018. In this article, we describe how BCIs raise important questions about personhood and propose recommendations for BCI development and governance. (shrink)

This essay is intended to deepen our understanding of the fundamental causes of the characteristic features of contemporary adolescence, and of their significance. Since the reader will not be unfamiliar with the phenomena, his personal experience may be relied on to supplement a description which would otherwise be too concise. The description is based on data, research and summary accounts drawn from the literature of various countries, which is suggestive because of its uniformity. It suggests five conclusions which bring into (...) focus the general condition and position of contemporary adolescence:1. While the development of the adolescent used to take place during the few years traditionally called “adolescence” (ages 15 to 18), it has steadily expanded during the last century, and recently with an ever-increasing speed. Nowadays, it reaches up to age 25, and in many cases and in many areas beyond this, and includes in the opposite direction the majority of the 13-to-14-year-olds without, however, having ceased to exert its attraction on what was traditionally called “childhood.” An enormous extension of adolescence has thus taken place, and this is already reflected to a large extent in the development of legal notions. G. W. F. Hegel (1770–1831), the influential German philosopher, believed that human history was advancing spiritually and morally according to God’s purpose. At the beginning of this masterwork, Hegel writes: “What the history of Philosophy shows us is a succession of noble minds, a gallery of heroes of thought, who, by the power of Reason, have penetrated into the being of things, of nature and of spirit, into the Being of God, and have won for us by their labours the (...) highest treasure, the treasure of reasoned knowledge.” In his introduction to this Bison Book edition, Frederick C. Beiser notes the complex and controversial history of Hegel’s text. He makes a case that this English-language translation by E. S. Haldane and Frances H. Simson is still the most reliable one. In the paper translated here, Carnap and Bachmann shows that the apparently metalinguistic ?extremal' axioms that are added to some axiom systems to the effect that the foregoing axioms are to apply as broadly, or as narrowly, as possible may be formulated directly as proper axioms. They analyze such axioms into four fundamental types, with the help of a concept of ?complete? isomorphism.

Was aber kommt zum Ausdruck, wenn 'nur' die Stimme zu hören ist, wenn Klang, Rhythmus, Schrei, Atem und Stocken der Stimme jenseits aller Worte, aller Bedeutungen und Signikate vernehmbar sind? Gerne wird diese Stimme jenseits der Sprache an die Musik gebunden, an die Vorstellung eines reinen Klangs, der sich den symbolischen Gesetzen entzieht. In der 'göttlichen Stimme der Diva' lebt noch etwas fort vom Gründungsmythos der Oper, die aus dem leidenschaftlichen Gesang Orpheus', aus der Beschwörung von Liebe und Tod entstanden sei. Auch andere Urszenen und Gründungsmythen kreisen um die Stimme; so die Stiftung des Bundes zwischen Gott und seinem Volke Israel oder auch die Homerische Szene von Odysseus' Kampf mit der verführerischen vielstimmigen Gesang der Sirenen. Die langjährige monomanische Verehrung der Schriftreligion und Bildersucht durchbrechend, soll mit den hier versammelten Beiträge eine Kultur- und Mediengeschichte der Stimme skizziert werden. BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. Objectives To characterize the prevalence and content of pediatric triage policies.Methods We surveyed and solicited policies from U.S. hospitals with pediatric intensive care units. Policies were analyzed using qualitative methods and coded by 2 investigators.Results Thirty-four of 120 institutions (28%) responded. Twenty-five (74%) were freestanding children’s hospitals and 9 (26%) were hospitals within a hospital. Nine (26%) had approved policies, 9 (26%) had draft policies, 5 (14%) were developing policies, and 7 (20%) did not have policies. Nineteen (68%) institutions shared (...) their approved or draft policy. Eight (42%) of those policies included neonates. The polices identified 0 to 5 (median 2) factors to prioritize patients. The most common factors were short- (17, 90%) and long- (14, 74%) term predicted mortality. Pediatric scoring systems included Pediatric Logistic Organ Dysfunction-2 (12, 63%) and Score for Neonatal Acute Physiology and Perinatal Extensions-II (4, 21%). Thirteen (68%) policies described a formal algorithm. The most common tiebreakers were random/lottery (10, 71%) and life cycles (9, 64%). The majority (15, 79%) of policies specified the roles of triage team members and 13 (68%) precluded those participating in patient care from making triage decisions.Conclusions While many institutions still do not have pediatric triage policies, there appears to be a trend among those with policies to utilize a formal algorithm that focuses on short- and long-term predicted mortality and that incorporates age-appropriate scoring systems. Additional work is needed to expand access to pediatric-specific policies, to validate scoring systems, and to address health disparities. (shrink)

Each contributor to this book has used personal experience as the basis from which to frame his individual sociological perspectives. Because they have personalized their work, their accounts are real, and recognizable as having come from 'real' persons, about 'real' experiences. There are no objectively-distanced disembodied third person entities in these accounts. These writers are actual people whose stories will make you laugh, cry, think, and want to know more.

We distinguish between the paradigm of game theory in which individuals act directly and that of social choice in which an impartial observer acts on the basis of social preferences, which in turn are derived from individual preferences. Much of the critique of Sen brought forward by Buchanan and de Jasay and Kliemt rests on a confusion of these two paradigms.

In order to reflect on the morality of the health care market this paper critiques some of H. T. Engelhardt's presuppositions. Engelhardt has created the vivid term ‘moral stranger’ and suggested that there can be a ‘morality of moral strangers’. However his position relies either on certain necessary presuppositions which he leaves unmentioned or on presuppositions that are—in a strict sense—not moral ones. Engelhardt advocates the market economy as the guiding principle of health care, and claims that the market needs (...) no moral presuppositions. But when the preconditions of a functioning market are examined it turns out that a functioning market requires property and ownership, and that property and ownership are moral institutions. Therefore the application of the idea of the market to health care undoubtedly has morally serious consequences: most important, the difference between commodities and human beings is obscured.
