Definition of the problem: Doctor-patient-conversation is still a great challenge for doctors and patients despite intense discussion, legal normation, and multiple efforts. It seems to be particularly difficult in cases of telling the truth about diagnosis or prognosis which can be threatening to the patient.Method: It is shown by two case studies that the patient directs a specific need to the doctor which has been neglected in both the ethics discourse and in practical medicine: the need to evaluate the patient's (...) options for action at an argumentative-critical level. Derived from a general theory of language functions a model of doctor-patient-conversation is developed which allows for both ideal case typology as well as for problem analysis. Referring to a Three-Stages-Model of Doctor-Patient-Dialogue, an integrative model “Language and Relationship Between Doctor and Patient” is formulated which permits a better understanding of the processes and difficulties between doctor and patient.Conclusion: The integrative model presented may help to realize a more comprehensive doctor-patient-conversation. It may help as well to decide which of the components of this conversation may be selectively sufficient. Consequences for the conceptualization of the physician's role as well as the scope of the model in practice are discussed. (shrink)

In this second issue of volume one, a welcome feature are those articles that bring to our readers, new historical information about women philosophers, new analyses of important positions supported by and questions addressed by select women philosophers, as well as articles that compare and contrast the views of several women philosophers on particular topics. This issue reflects on the context of women’s theoretical contributions, with articles that address the question of women’s agency and the historical account through which women (...) were acknowledged (or not) as originators of or contributors to important theoretical developments within the mentioned disciplines. In recent decades an overwhelming amount of philosophical works by women has been made available by historians of philosophy. This issue reflects those strides towards recovering philosophical writings by women. Issue 2 of this volume is organized chronologically and starts with a paper from Jonathan Head: “Anne Conway on Heaven and Hell”. Conway’s The Principles of the Most Ancient and Modern Philosophy (1690) offers representations of heaven and hell, revising our conventional image of hell. Conway presents hell in a way that she believes is more consistent with the postulate of a loving and just God. In part, Conway denies the possibility of eternal torment and emphasizes the benefits that suffering brings to a living being. Conway describes heaven as a realm of “perfect rest” where creatures enjoy unity and harmony with Christ and other heavenly spirits. This state involves an increasing understanding of the world, a continual process of perfection and harmony with other heavenly spirits. The article concludes with a description of Conway’s eschatology in the broader intellectual context of the revival of Origenist theology in its intellectual setting and in the changing framework of eschatological thought in the early Quaker community. Monica M. Mastrantonio discusses in her paper, “Lady Mary Wortley Montagu – a philosopher? The epistolary woman, because women could not be called Philosophers, Scientists, Entrepreneurs” aspects of a different and new transcultural worldview that reflects diversity and knowledge combined with philosophy in a new way. In the past, there were two roles with which Lady Mary Wortley Montagu was associated. One was her experience at the Turkish Embassy, where she highlighted cultural diversity. A second role she was associated with was international travel. Mastrantonio argues that Montagu was more than a literary aristocrat who could describe customs and traditions from abroad. Using Montagu’s complete letters, the author brings into discussion the fact that Montagu was also a vaccine entrepreneur and a journalist on political and urban issues, paving the way for discussions on philosophical questions. Many letter writers of the 16th and 17th centuries were already feminists who were politically and artistically active in their time and wrote letters with precision and expertise. The letters of Lady Mary Wortley Montagu are an important aspect in building a transcultural worldview in which diversity and knowledge combine with philosophy differently than it was before. In this short but engaging article, “American Women in the Transcendental and Pragmatist Movements”, Carol Bensick provides an overview of two important philosophical movements of the late 19th and early 20th centuries, Transcendentalism and the Pragmatism that emerged in part from that movement. In the course of identifying numerous women who contributed to both movements, Bensick examines how some of them were included in the early history of American philosophy, only to be omitted from later histories. Bensick argues that the recent designation of women philosophers as “feminist pragmatists” is historically an inaccurate identification and one that pragmatists did not use to describe themselves. The French philosopher Simone Weil, the Swiss Jeanne Hersch and the Spanish Maria Zambrano each examined how bodily elements function in the rational search for truth. In his paper Oblique Paths to Truth: Myth and Bodily Elements in Simone Weil, Jeanne Hersch and Maria Zambrano, Piergiacomo Severini examines popular “body” myths referred to by these philosophers and analyses the language they use to express that bodily elements precede consciousness and reason. Through these myths/stories we learn that truth cannot be known directly through objects. Instead, the “knowing subject” necessarily elaborates indirect paths to transcendent truth. Severini argues that the effort to reintegrate the human attachment to the marginal world is peculiar to women philosophers in the history of philosophy and thus points to an alternative path to the human search for meaning that differs from the mainstream of Western male philosophy. As a philosopher, Hedwig Conrad-Martius has been intensively engaged throughout her life in researching the question of what reality is and has contrasted her reflections with those of Husserl and others. In her contribution “Conrad-Martius: the ´Real-Reality of the World – in Dialogue with Aristotle, Aquinas and Husserl”, author Irene Breuer deals with Conrad-Martius’ concepts of reality as “real reality”, with being, existence and essence and relates them to the concepts in Aristotle, Aquinas and Husserl. She discusses once again the questions of the hypostasis of being – by which the author claims that this is about the noema itself, and about the emergence of being in particular. The author’s thesis is that although Conrad-Martius rejects transcendental reduction, Conrad-Martius does accept eidetic reduction and the positing of a sphere of primordial and given facts. Driving this sphere back into a transphysical realm, it can provide a ground for Husserl’s sphere of original facts, which itself remains beyond the reach of phenomenological enquiries. Consequently, the author concludes, Conrad-Martius and Husserl meet here in their investigations, and yes, they complement each other, at a point where “the real bursts into reality”. Join us in unveiling this universe of women’s ideas. We are happy to welcome our contributors and readers, as we improve our understanding of the history of philosophical ideas by adding new perspectives and new insights. We enter a new universe of praiseworthy ideas by women theorists, one that will doubtlessly contribute to a world description that is more accurate than a narrative written without these voices. We include also the description of another systems of logic programming with uncertain information that are based on other logics of uncertainty which are different from fuzzy logic. The paper discusses an ethical analysis of three common cases involving the most popular requests in rhinoplasty. As the nose plays a fundamental role in the aesthetics and physiognomy of the human face, the request for rhinoplasty may be an expression of both subjective discomfort and objective dysfunction. The paper aims, therefore, to fill the gap between qualitative-subjective impressions related to bodily self-perception and its quantitative-objective assessment. Ethical evaluation should start with consideration of the formal object and circumstances of the (...) act, by posing the following question: Is this a real clinical case? Only after an undoubtedly positive answer to this question, should we consider specific case-related aspects; i.e., the best scientific evidence, available nosography and informed consent, before conducting an ethical evaluation of the case. Background: Information sharing is one of the most important means of public health nurses collaborating with other healthcare professionals and community members. There are complicated ethical issues in the process. Research objectives: To describe the ethical dilemmas associated with client information sharing that Japanese public health nurses experience in daily practice and to clarify their decision-making process to resolve these dilemmas. Research design: Data were collected using a three-phase consensus method consisting of semi-structured interviews, self-administered questionnaires and a group interview. (...) Participants and research context: We surveyed administrative public health nurses in Shizuoka Prefecture, Japan. The semi-structured interviews were carried out with 12 administrative public health nurses, and the self-administered questionnaires were sent to all 899 administrative public health nurses. The group interview was carried out with eight administrative public health nurses. Ethical considerations: Ethical approval was granted by the ethics committee of the School of Health Sciences, Nagoya University, Japan (8-158, 9-130). Findings: Information-sharing ethical dilemmas occurred most often when clients’ decisions did not coincide with the nurses’ own professional assessments, particularly when they faced clinical issues that were inherently ambiguous. In their decision-making processes, nurses prioritised ‘protection of health and life’. Discussion: These findings suggest that, above all, they sought to address urgent risks to clients’ lives while upholding the principle of client autonomy as much as possible. In such cases, the nurses made decisions regarding whether to share information about the client depending on the individual situation. Conclusion: Public health nurses should protect the client’s health while taking into consideration their relationship with the client. (shrink)

This paper presents a formalization of informal logic using the Carneades Argumentation System, a formal, computational model of argument that consists of a formal model of argument graphs and audiences. Conflicts between pro and con arguments are resolved using proof standards, such as preponderance of the evidence. CAS also formalizes argumentation schemes. Schemes can be used to check whether a given argument instantiates the types of argument deemed normatively appropriate for the type of dialogue.

Integrated Information Theory (IIT) stands out as one of the most promising approaches to scientifically understand the emergence of consciousness. Even if it borrows from the phenomenology of consciousness to derive its axiomatic formulation, IIT does not initially adhere to any particular ontological position. However, its founder leans towards Panpsychism. More recently, Owen has studied the pros and cons of different ontologies as a metaphysical basis for IIT, defending a hylomorphic stance where en-grounding, en-forming relations gain the upper hand in (...) the causal narrative. This paper aims at (1) complementing Owen’s work by invoking additional arguments against physicalist IIT, both reductive and non-reductive; and (2) improving Owen’s characterization of non-physicalist causality through a “nested” version of hylomorphism that avoids the combination problem of Panpsychism. : Fodor was passionately unwilling to compromise. Of his several commitments, I focus here on informational atomism. Fodor staunchly rejected semantic holism for two conspiring reasons. He took it to threaten his commitment to the nomic character of psychological explanation. He also took it to pave the way towards relativism, which he found deeply offensive. In this paper, I reconstruct the strands of Fodor’s commitment to the computational version of the representational theory of mind that led him to informational atomism. (...) I take issue with three features of informational atomism. First, I argue that it deprives content from its expected causal role in psychological explanation. Secondly, I take issue with Fodor’s claim that only informational atomism can meet the requirements of the principle of compositionality. Finally, I argue that informational atomism yields a bloated or unwieldy category of nomic properties. Lo vedeva come minaccia per il suo impegno verso il carattere nomico della spiegazione psicologica e come porta aperta verso il relativismo, cosa che considerava profondamente minacciosa. In questo lavoro, intendo riprendere le fila dell’impegno di Fodor verso la versione computazionale della teoria rappresentazionale della mente che lo ha portato all’atomismo informazionale, chiarendo tuttavia che non sono d’accordo con tre aspetti dell’atomismo informazionale. In primo luogo, mostrerò come questo sottragga al contenuto il suo ruolo causale nella spiegazione psicologica. In secondo luogo, non sono d’accordo con l’affermazione di Fodor per cui solo l’atomismo informazionale possa soddisfare i requisiti del principio di composizionalità. Illustrerò infine come l’atomismo informazionale ceda il passo a un insieme di proprietà nomiche ampio o difficile da gestire. The present study examines the representation of Arabic and Hebrew in Covarrubias’s Tesoro de la lengua castellana o española from a comparative perspective. Analyzing the lexicographer’s ideological and meta-linguistic discourse, I reconstruct his postulates regarding the respective histories of Arabic and Hebrew, their interrelationship, and place in the history of Spanish. In light of these postulates, and taking into account the lexicographer’s access to knowledge, sources, and informants, I examine some of his etymological practices, focusing on his attempts to use (...) grammatical and lexical knowledge of Hebrew to illuminate Arabic etymologies of Spanish words. Combining the ideological and technical levels of the text, I reflect on the interplay of knowledge and ignorance in Covarrubias’s treatment of Semitic languages. (shrink)

In 1972,Ernst Ulrich and Christine von Weizs ¨acker introduced the concept of pragmatic information with three desiderata:(i) Pragmatic information should assess the impact of a message upon its receiver;(ii)Pragmatic information should vanish in the limits of complete (non-interpretable)'novelty 'and complete 'confirmation';(iii)Pragmatic information should exhibit non-classical properties since novelty and confirmation behave similarly to Fourier pairs of complementary operators in quantum mechanics. It will be shown how these three desiderata can be naturally fulfilled within the framework of Gardenfors' dynamic semantics of (...) Bayesian belief models.(i)The meaning of a message is its impact upon the epistemic states of a cognitive agent. A pragmatic information measure can then be quanti .ed by the average information gain for the transition from a prior to a posterior state.(ii)Total novelty can be represented by the identical proposition, total con- .rmation by the logical consequence of propositions. In both cases, pragmatic information vanishes.(iii)For operators that are neither idempotent nor commuting, novelty and confirmation relative to a message sequence can be defined within Gardenfors' theory of belief revisions.The proposed approach is consistent with measures of relevance derived from statistical decision theory and it contains Bar-Hillel 's and Carnap's theory of semantic information as a special case. Se realiza la investigación con el objetivo de perfeccionar la obtención del consentimiento informado en las amputaciones mayores de causa vascular en el Servicio de Angiología del Hospital Universitario Manuel Ascunce Domenech. Se constató debilidades en la institucionalización del consentimiento informado y específicamente en el caso de tales amputaciones. Se utilizaron métodos y técnicas del nivel empírico: encuestas a 30 pacientes con riesgo inminente de amputación. El 64% eran mayores de 65 años, 73 % femeninos, todos escolarizados y 73% residentes (...) urbanos. Los resultados más relevantes fueron: que el 54% de los pacientes manifestaron debe existir consentimiento en la aceptación de la amputación, aunque un grupo no despreciable el 37% declinó este derecho a favor de sus familiares. A los médicos se les realizó una entrevista y aunque no hubo unanimidad de criterios, todos coincidieron en que debía legislarse este consentimiento, como parte del perfeccionamiento de los servicios de salud. Research is conducted to improve the process of obtaining informed consent for major amputations due to vascular causes in the Angiology Service of the Manuel Ascunce Domenech Teaching Hospital. Deficiencies were identified in the institutionalization of informed consent protocols, most specifically as they relate to major lower limb amputations. Empirical methods and techniques were used. Thirty patients at imminent risk of amputation were surveyed by questionnaire methods; 64% of those surveyed were of 65 years of age, 73% were female, all of them had an average educational level, and 73% were urban residents. The results indicated that 54% of patients recognized the importance of informed consent in cases of amputation, although a rather considerable group declined this right for their families. Physicians were also interviewed and, although there was no unanimity of criteria, all of them considered that this consent should be legislated as part of the improvement of health services. Salomon Maimon's Versuch über die Transzendentalphilosophie [Essay in Transcendental Philosophy] (1790) challenges and reworks Kant's arguments in the Kritik der reinen Vernunft [Critique of Pure Reason] (1785, 2nd ed. 1787) about the foundations of natural science and of Newtonian physics in particular. Kant himself was impressed both with Maimon's grasp of his critical project and also with the force of his challenge to it. While Maimon's significance on the later development of German Idealism is now widely acknowledged, another aspect of (...) Maimon's Versuch has not been fully appreciated, namely, its engagement with the central questions of the Spinozastreit [Spinoza Quarrel] that erupted in 1785 with the publication of Friedrich Heinrich Jacobi's Über die Lehre des Spinoza in Briefen an den Herrn Moses Mendelssohn [Concerning the Doctrine of Spinoza in Letters to Moses Mendelssohn]. The Spinoza Quarrel centered on whether and to what extent philosophy's rational understanding of God needs to be grounded in an unmediated and suprarational revelatory experience. This paper is the first extended effort at placing Maimon's Versuch into the context of the Spinoza Quarrel. I argue that the Spinoza Quarrel and Maimon's self-proclaimed philosophical mission in response to it—the replacement of revealed faith by reason—deeply inform the goals he pursues in his Versuch. I show how Maimon's Versuch can be read as not only a response to Kant, but also to Jacobi's defense of the revelatory nature of sense experience in David Hume über den Glauben (1787), the book in which Jacobi offers his own skeptical challenge to Kant's Kritik. Situating Maimon's Versuch as a response to Friedrich Jacobi's David Hume allows us to understand how one of Maimon's objectives in his Versuch is to keep Jacobian Glaube [faith] at bay by demonstrating, using a revised Kantian framework, the conditions of the impossibility of experiencing miracles. Los juegos de mesa son un producto cultural emergente estrechamente vinculado a actividades de aprendizaje informal sobre disciplinas variadas como la biología, las matemáticas o los idiomas. De entre estas áreas de conocimiento destaca la Historia, ya que desde el inicio del juego de mesa moderno hace 50 años se han publicado miles de juegos inspirados en las más diversas eras y hechos históricos, desde la Prehistoria hasta la Guerra Fría. Existe un interés académico creciente para desarrollar métodos de Aprendizaje (...) Basado en Juegos para aprovechar el potencial del juego como herramienta didáctica, pero la mayor parte se centra en el uso de videojuegos en el aula, y son escasos los trabajos que analizen el juego de mesa histórico en tanto que actividad de aprendizaje informal. Este trabajo explora el contenido histórico recreado en miles de juegos de mesa entendidos como motores de aprendizaje informal, con el objetivo de conocer qué temas de la Historia aparecen en juegos de mesa comerciales y, por tanto, son susceptibles de aprendizaje por parte de los jugadores. Para ello se ha desarrollado un método computacional basado en el modelado de tópicos y la visualización de datos para la identificación automática de temas en juegos de mesa a partir de sus descripciones textuales. El análisis presentado aquí se centra en un estudio comparativo de tres períodos con una escala diferente: a) la época Medieval, b) el siglo XIX, y c) Segunda Guerra Mundial. Los resultados del trabajo muestran como las temáticas tratadas en los juegos dependen directamente de la época en la que éstos se enmarcan, así como la existencia de marcados sesgos hacia la sobrerepresentación de guerras y la historia de Europa. Estas contribuciones sugieren que los juegos de mesa pueden promover el aprendizaje informal crítico sobre hechos históricos y al mismo tiempo reforzar una percepción pública desigual de nuestro pasado. La lógica de la conciencia es una extensión de la lógica epistémica que solventa el problema de la omnisciencia lógica incorporando un operador de conciencia para separar el conocimiento explícito del implícito. Este informe recopila los principales textos tanto de los orígenes de esta lógica, así como de sus desarrollos en las últimas tres décadas. En concreto analiza los enfoques desde la lógica epistémica dinámica, desde su combinación con otras lógicas y los enfoques de teoría de juegos.

In the half-century history of clinical practice of diagnosing brain death, informed consent has seldom been considered until very recently. Like many other medical diagnoses and ordinary death pronouncements, it has been taken for granted for decades that brain death is diagnosed and death is declared without consideration of the patient’s advance directives or family’s wishes. This essay examines the pros and cons of using informed consent before the diagnosis of brain death from an ethical point of view. (...) As shared decision-making in clinical practice became increasingly indispensable, respect for the patients’ autonomous wishes regarding how to end their lives has a significant role in deciding how death is diagnosed. Brain death, as a fully technologically controlled death, may require a different ethical framework from the old one for traditional cardiac death. With emerging and proliferating options in end-of-life care for those who suffer from catastrophic brain injury, the traditional reasoning that ‘death gives no choice, hence no consent’ requires another examination. Patients facing imminent brain death now have options other than undergoing the diagnostic workup for brain death, such as donation after circulatory death and withdrawal of life-sustaining treatment with maximum comfort measures for death with dignity. Nevertheless, just as in the debate over opt-in versus opt-out organ donation policies, informed consent before the diagnosis of brain death faces fierce opposition from consequentialists urging the expansion of the donor pool. This essay examines these objections and provides constructive replies along with a proposal to accommodate this morally required consent. (shrink)

Más de la mitad de la fuerza laboral mexicana se encuentra en la informalidad. Sin embargo, existe un elevado número de transiciones del empleo informal al empleo formal en el corto plazo, lo que se ha interpretado como evidencia de que la informalidad es una decisión óptima para incrementar el bienestar. El objetivo de este trabajo es analizar las condiciones de ingreso y estabilidad laboral de los empleados que se insertan a la formalidad, después de iniciar su trayectoria laboral en (...) la informalidad. Se utiliza el Módulo de Trayectorias Laborales (MOTRAL) 2015, encuesta con representatividad nacional de las zonas urbanas mexicanas. Para ello se comparan las condiciones de ingreso y la estabilidad laboral en 2010 entre los trabajadores formales utilizando dos grupos: quienes tuvieron su primer empleo en la informalidad y quienes tuvieron su primer empleo en la formalidad. Se construyen matrices de transición entre el primer empleo, el empleo en 2010, y en 2015 para corroborar la alta movilidad encontrada por la literatura previa; posteriormente se analizan con modelos multivariados las condiciones de ingreso y la estabilidad laboral. Los resultados indican que una proporción significativa de quienes se iniciaron en la informalidad acceden posteriormente a empleos formales. Sin embargo, se encuentra que el acceso al empleo formal desde la informalidad ocurre en condiciones de desventajas en ingresos y contratación de largo plazo, sin que las desventajas desaparezcan posteriormente, por lo que el inicio de la vida laboral en la informalidad está asociado a desventajas permanentes en el mercado de trabajo. Las evaluaciones internacionales en el ámbito de la educación se han convertido en nuestros días en una cuestión de actualidad. A raíz de la elaboración de distintos estudios, sobre todo el conocido "Informe PISA", la información sobre el funcionamiento de los sistemas educativos en diferentes países ha dejado de ser asunto de pocos especialistas. Por el contrario, desde hace algún tiempo, los datos sobre los resultados de los estudiantes a escala internacional aparecen publicados, muchas veces con tintes sensacionalistas, en los (...) distintos medios de comunicación y se han convertido en objeto de comentarios y debates, tanto en la esfera política como e la sociedad en general. Se presenta los referentes teóricos a partir del empleo de un enfoque de sistema que supone el análisis y la síntesis, la inducción y la deducción como métodos de investigación, con el propósito de dar conocer los hitos en las universidades y organizaciones internacionales relacionadas. La modelación fue empleada para la construcción de un nuevo proyecto de desarrollo de competencias informacionales desde la perspectiva de la (...) formación profesional universitaria ajustada a las necesidades de la educación superior en Cuba. El nuevo proyecto se configura en tres unidades básicas, con sus respectivos elementos de competencia, saberes esenciales e indicadores para la evaluación del desempeño. El artículo fundamenta la tesis de que el desarrollo de competencias informacionales debe ser sistemáticamente estructurado, planificado y ejecutado como parte integral de la formación curricular en interacción con los contenidos de las disciplinas docentes, bajo la dirección del profesor y la participación protagónica de los profesionales en formación. The article aims at describing the competence to be developed during professional training. It presents the theoretical referents by means of an approach system that includes synthesis and analysis, induction and deduction as research methods in order to reveal the milestones of information skills development in universities and related international organizations. Modeling was used for the construction of a new skills development project from the perspective of university professional training according to the needs of Higher Education in Cuba. The new project is composed of three basic units, with their respective competence elements, essential knowledge and indicators for the evaluation of the performance. The article supports the theory that information development skills must be systematically structured, planned and implemented as an integral part of the curricular training in interaction with the contents of subjects, under the teacher´s guidance and the active participation of profesionals in training. Background: Biological products are widely used in the treatment of burns, chronic wounds, and other forms of acute injury. However, the religious and ethical issues, including consent, arising from their use have never been addressed in the medical literature.Aims: This study was aimed to ascertain the views of religious leaders about the acceptability of biological products and to evaluate awareness among healthcare professionals about their constituents.Methods: The religious groups that make up about 75% of the United Kingdom population were identified (...) and a questionnaire on 11 biological products was sent to its leaders. Another questionnaire concerning 17 products was sent to 100 healthcare professionals working in seven specialist units in the UK.Results: All religious leaders replied, some after consultation with international bodies. Among them, 77% said that patients should be informed of the constituents of the biological products and consent obtained. Some leaders expressed concerns about particular products including the transmission of viral and prion diseases, cruelty to animals, and material derived from neonates. None of the healthcare professionals surveyed knew the constituents of all the products correctly.Conclusion: Ignoring religious sensitivities and neglecting consent in the usage of biological products could have very serious implications, including litigation. Hospitals and manufacturers should take immediate measures to enlighten healthcare professionals of the constituents of these products so that they can obtain informed consent from patients. El estudio se centra en el nuevo escrito que el autor inserta en esta edición: "El Chacho, último caudillo de la montonera de los llanos". Se aborda, desde el campo de la Historia Intelectual, la historicidad del concepto "caudillismo", analizando los usos que hace Sarmiento del término en sus diversos contextos de enunciación. A su vez se propone (...) un análisis de la historicidad del texto mismo, observando las estrategias discursivas con que opera su autor para intervenir en la escena política. The aim is a reading of the 3rd edition of Facundo as a strategy by Domingo Faustino Sarmiento to achieve political purposes. The study focuses on the addition by the author in this edition of: "El Chacho, último caudillo de la montonera de los llanos". It discusses, from the field of Intellectual History, the historicity of the concept of "warlordism", analyzing the uses that Sarmiento makes of the term in its various contexts of enunciation. Background While it is generally acknowledged that self-prescribing among physicians poses some risk, research finds such behaviour to be common and in certain cases accepted by the medical community. Largely absent from the literature is knowledge about other activities doctors perform for their own medical care or for the informal treatment of family and friends. This study examined the variety, frequency and association of behaviours doctors report providing informally. Informal care included prescriptions, as well as any other type of personal (...) medical treatment (eg, monitoring chronic or serious conditions). Method A survey was sent to 2500 randomly-selected physicians in Colorado, 600 individuals returned questionnaires with usable data. The authors hypothesised: (1) physicians would prescribe the same types of treatment at home as they prescribed professionally; and (2) physicians who informally prescribed addictive medications would be more likely to engage in other types of informal medical care. Results Physicians who wrote prescriptions for antibiotics, psychotropics and opioids at work were more likely to prescribe these medications at home. Those prescribing addictive drugs outside of the office treated more serious illnesses in emergency situations, more chronic conditions and more major medical/surgical conditions informally than did those not routinely prescribing addictive medications. Physicians reported a variety of informal care behaviour and high frequency of informal care to family and friends. Discussion The frequency and variety of informal care reported in this study strongly argues for profession-wide discussion about ethical and guideline considerations for such behaviour. Ordinary common sense suggests that we have just one set of shape concepts that we apply indifferently on the bases of sight and touch. Yet we understand the shape concepts, we know what shape properties are, only because we have experience of shapes. And phenomenal experience of shape in vision and phenomenal experience of shape in touch seem to be quite different. So how can the shape concepts we grasp and use on the basis of vision be the same as (...) the shape concepts we grasp and use on the basis of touch? I think this is the intuitive puzzle that underlies the question sent by the Dublin lawyer Molyneux to John Locke. This concerns a man born blind, who learns by the use of his touch to discriminate cubes from spheres. Suppose him now to gain the use of his sight. And suppose him to be presented with a cube and a sphere, of nighly the same bigness. Quaere, will he be able to tell, by the use of his vision alone, which is the sphere, and which the cube? BackgroundIn specific situations it may be necessary to make an exception to the general rule of informed consent for scientific research with an intervention. Earlier reviews only described subsets of arguments for exceptions to waive consent.MethodsHere, we provide a more extensive literature review of possible exceptions to the rule of informed consent and the accompanying arguments based on literature from 1997 onwards, using both Pubmed and PsycINFO in our search strategy.ResultsWe identified three main categories of arguments for the (...) acceptability of a consent waiver: data validity and quality, major practical problems, and distress or confusion of participants. Approval by a medical ethical review board always needs to be obtained. Further, we provide examples of specific conditions under which consent waiving might be allowed, such as additional privacy protection measures.ConclusionsThe reasons legitimized by the authors of the papers in this overview can be used by researchers to form their own opinion about requesting an exception to the rule of informed consent for their own study. Importantly, rules and guidelines applicable in their country, institute and research field should be followed. Moreover, researchers should also take the conditions under which they feel an exception is legitimized under consideration. After discussions with relevant stakeholders, a formal request should be sent to an IRB. (shrink)

A model of information technology (IT) ethical work climates is presented. Using these ethical work climates and data collected from a national mail survey of Association for Computing Machinery (ACM) members, empirical measures were developed and evaluated. A mailing of 2446 questionnaires was sent to ACM members and 136 usable responses were returned (5.6%). Using these data, an exploratory factor analysis was performed using principle components analysis to identify the IT ethical work climates from the data. Six of these (...) work climates were identified as predicted by the model. Two ethical work climates that were combinations of the proposed climates were also identified.From the results of the exploratory factor analysis, a confirmatory factor analysis was performed using Calis in PC SAS version 8. The purpose of this analysis was to evaluate the overall fit of these measures to the data and evaluate the psychometric properties of the measures. The fit of the IT ethical work climates model was acceptable. The psychometric properties of these measures were good. Based on these results, conclusions, limitations of the study, and directions for future research are proposed. (shrink)

Objectives: To gather information about the practices and attitudes of providers of maternity care with respect to informed consent for newborn screening .Methods: A questionnaire concerning information provision and parental consent for NBS was sent to all 1036 registered lead maternity carers in New Zealand.Results: 93% of LMC in New Zealand report giving parents information concerning NBS, most frequently after delivery and in the third trimester . The majority of LMC currently obtain some form of consent for NBS (...) from parents and consider this to be the ideal approach . Despite this a significant minority of LMC reported considering that NBS should be mandatory. Of those in our survey who believed that NBS should be mandatory, paradoxically most still believed that some form of parental consent should be obtained; of those who believed testing should not be mandatory, only a small proportion would accept parental refusal without question.Conclusions: When the results of this survey are considered in conjunction with existing evidence there appears to be a consensus that good quality information in the prenatal period should be an integral part of any NBS programme. The issue of consent is more complex and there is less agreement on the preferred degree of parental involvement in decisions to allow babies to undergo NBS. A policy that both strongly recommends NBS but also allows parental choice appears to be most consistent with the views of LMC in this survey. (shrink)

Tan bucólico enclave enseguida le generará preguntas de índole histórica. ¿Cómo llegó hasta aquí esta Inglaterra en miniatura? Si les consulta a los guías turísticos, estos le compartirán un relato convenientemente amable: todo comenzó con la _Rio Tinto Company Limited_, una empresa que adquirió las minas de cobre de Rio Tinto en 1873 y que construyó el lugar con miras a que le sirviese de morada a sus técnicos y trabajadores. El visitante abandonará el paraje con la impresión de que deja tras de sí la huella patrimonial de una historia de progreso compartido, de crecimiento económico y de modernización sociocultural. Lo que no sabe es que el Barrio Inglés de Bella Vista es el testimonio de un pasado mucho más complejo y mucho menos amable de lo que es perceptible en la inmediatez de su paisaje. Tal pasado oculta una relación desigual de subordinación comercial, financiera y geoestratégica. Una relación que puso a la Monarquía española parcialmente a merced de los intereses británicos. ABSTRACT The American envoy in Paris, Jefferson, sent on 17 June 1789 to Jay, the Secretary of Foreign Affairs, an assessment of Necker, then at the apex of his political fortune, provided by an informant whose name was not disclosed by Jefferson. This paper intends to show that the author of this report was the philosopher, mathematician and economist Condorcet – certainly the French friend who had with Jefferson the most intense intellectual intercourse – and sketches a history of (...) the relationship between Condorcet, Necker and Jefferson from which the report arose. This paper presents a formalization of informal logic using the Carneades Argumentation System, a formal, computational model of argument that consists of a formal model of argument graphs and audiences. Conflicts between pro and con arguments are resolved using proof standards, such as preponderance of the evidence. Carneades also formalizes argumentation schemes. Schemes can be used to check whether a given argument instantiates the types of argument deemed normatively appropriate for the type of dialogue.

: Creativity, mostly ignored in Western philosophy due to its supposed mysteriousness, has recently become a respected research topic in psychology, neuroscience, and artificial intelligence. We discuss how in science the approach has mainly been to describe creativity as an information-based process, coherently with a computational view of the human mind started with the cognitive revolution. This view has produced progressively convincing models of creativity, up to current artificial neural network systems, vaguely inspired by biological neural processing, but already competing (...) with human creativity in several fields. These successes suggest that creativity might not be an exclusively human function, but actually a way of functioning of any natural or artificial system implementing the creative process. We conclude by acknowledging that the information-based view of creativity has tremendous explanatory and generative power, but we propose a thought experiment to start discussing how it actually leaves out the experiential side of being creative. Vogliamo illustrare il modo in cui lo sguardo scientifico sia rivolto prevalentemente a considerare la creatività come processo information-based, coerentemente con la prospettiva computazionale sulla mente umana aperta dalla rivoluzione cognitiva. Questa prospettiva ha prodotto modelli della creatività sempre più convincenti, fino agli attuali sistemi di reti neurali artificiali, vagamente inspirati al processamento biologico neurale, ma già competitivi rispetto alla creatività umana in molti ambiti. Questi successi suggeriscono che la creatività possa non essere una funzione esclusivamente umana ma in effetti un modo di funzionare di un sistema naturale o artificiale capace di implementare il processo creativo. In conclusione, pur riconoscendo come il considerare la creatività come processo information-based possieda grande potere esplicativo e generativo, proporremo un esperimento mentale per aprire una discussione sul come questa prospettiva non copra in effetti il lato esperienziale dell’essere creativo. This paper proposes an agent community based information retrieval method, which uses agent communities to manage and look up information related to users. An agent works as a delegate of its user and searches for information that the user wants by communicating with other agents. The communication between agents is carried out in a peer-to-peer computing architecture. In order to retrieve information related to a user query, an agent uses two histories : a query/retrieved document history(Q/RDH) and a query/sender agent (...) history(Q/SAH). The former is a list of pairs of a query and retrieved documents, where the queries were sent by the agent itself. The latter is a list of pairs of a query and sender agents and shows ``who sent what query to the agent''. This is useful to find a new information source. Making use of the Q/SAH is expected to cause a collaborative filtering effect, which gradually creates virtual agent communities, where agents with the same interests stay together. Our hypothesis is that a virtual agent community reduces communication loads to perform a search. As an agent receives more queries, then more links to new knowledge are achieved. From this behavior, a ``give and take''(or positive feedback) effect for agents seems to emerge. We implemented this method with Multi-Agents Kodama which has been developed in our laboratory, and conducted preliminary experiments to test the hypothesis. The empirical results showed that the method was much more efficient than a naive method employing 'broadcast' techniques only to look up a target agent. (shrink)

De acuerdo con el Instituto Nacional de Medicina Legal y Ciencias Forenses, en su informe Forensis 2017. Datos para la vida, los hechos de discriminación y violencia de pareja contra las mujeres en Colombia se han incrementado en los últimos años; esta situación podría catalogarse como producto de una sociedad que ha sido moldeada a partir del esquema machista y patriarcal que caracteriza a las sociedades que han estado influenciadas por tradiciones como la religión católica, que parecen justificar, en algunos (...) de los relatos de sus textos sagrados, la condición de dominación que ejerce el hombre sobre la mujer. El Estado colombiano, que en sus inicios normativos se vio influenciado por esta tradición, ha buscado, a partir de la Constitución de 1991, y a través de su posterior normatividad, no solo la reivindicación de la mujer, sino la sanción a los agresores; sin embargo, estas medidas no han sido eficientes ya que no se ha podido garantizar un verdadero ambiente de seguridad y respeto por los derechos fundamentales de las mujeres. Por lo tanto, se hace necesario intervenir la configuración de la cultura colombiana, en aras de modificar las bases culturales sobre las cuales se construyó el imaginario social que perpetúa la estructura de dominación y discriminación contra la mujer. Throughout the history of molecular biology, the primary meaning of biological information has been taken from the image of a word-based linguistic code. I want to argue that the metaphor of such a code does not begin to capture either the variety or the richness of the processes by which nucleotide sequences inform biological processes. Current research demonstrates that nucleotide sequences inform not only development but also heredity and evolution, and they do so in all sorts of ways. Even though (...) they do not exhaust the varieties of biological information employed in these processes, I claim that the power of DNA sequences to inform these processes is richer and perhaps far greater than the conventional understanding of genetic information permits, indeed richer than what any of our images of simple linguistic codes or of senders and receivers permits. Rather than a tape in a Turing machine or a message or signal sent through the generations, DNA is first and foremost a physicochemical structure with a range of potential uses by the physicochemical arsenal of biological cells that is so large as to expose the poverty of our most familiar metaphors. Recognition of this fact leads us to conclude that DNA is both more and less than we thought—more because it carries both symbolic and non-symbolic information and less because accepting that fact undermines its radical distinction from other biological molecules. El objetivo central de este trabajo fue identificar los patrones funcionales-valorativos presentes en un corpus de informes de arbitraje, género clave en la producción de conocimiento científico. Para el análisis, se utilizó un procedimiento que implicó, primero, la identificación de los elementos funcionales o propósitos comunicativos más frecuentes y, luego, su descripción con algunas categorías del modelo de la valoración. El corpus, de carácter intencionado, estuvo compuesto por 42 informes de arbitraje de la revista de lingüística y traducción de la (...) Pontificia Universidad Católica de Chile, Onomázein. Algunos de los resultados muestran que los propósitos comunicativos más frecuentes son “Describir un problema”, “Destacar un aspecto positivo”, “Solicitar una acción” y “Sugerir una acción”. La alta frecuencia de los propósitos “Describir un problema” y “Destacar un aspecto positivo” eran esperables en un género evaluativo. Ambos propósitos se focalizan más en elementos de contenido y presentan un fuerte posicionamiento monoglósico intensificado con significados de Gradación. El patrón monoglósico puede explicarse por la situación de poder en que se encuentra el árbitro al ser el evaluador del artículo. La alta frecuencia de los propósitos comunicativos “Solicitar acción” y “Sugerir acción” da cuenta de la función negociadora del género. Por último, tanto en la solicitud como en la sugerencia de acciones, los significados de actitud tienen como foco los elementos de forma y no de contenido. (shrink)

Resumen El objetivo de este trabajo es determinar la relevancia de la teoría informacional del conocimiento para el problema de la suerte epistémica. Argumento que el clásico enfoque de Dretske es equivalente a la condición de seguridad de Pritchard. Sin embargo, considero que esta manera de eludir la suerte epistémica exige lidiar con el llamado "problema de la generalidad". Argumento que una respuesta a este problema requiere una noción de seguridad diferente y propongo un enfoque informacional equivalente a esta versión (...) del requisito. Concluyo que este enfoque es preferible a la condición de seguridad propuesta porque desvela relaciones conceptuales entre los elementos que constituyen el conocimiento.The aim of this paper is to determine the relevance of an informational theory of knowledge to address recently discussed epistemic problems. I argue that Dretske's classical approach is equivalent to the safety condition proposed by Pritchard to avoid epistemic luck. However, I claim that Pritchard's condition has to be modified to avoid the so-called "generality problem" for reliabilism. I propose an alternative version of the requirement to deal with these problems and provide an equivalent informational condition. I conclude that this informational version is preferable since it reveals conceptual relationships between the constituents of knowledge. (shrink)

Introducción: El consentimiento informado es un procedimiento que brinda oportunidad a los pacientes de disponer de información y participar en la toma de decisiones médicas. Sin embargo, existe el criterio de que el nivel de entendimiento de la información aportada por los facultativos al paciente, sus familiares o ambos, es bajo y además, muchos médicos lo consideran como una herramienta de trabajo secundaria o que no es necesario realizarla con inmediatez. Objetivo: Evaluar la percepción y experiencia del personal sanitario de (...) la unidad de terapia polivalente del Hospital Docente Clínico Quirúrgico "Joaquín Albarrán" acerca del consentimiento informado. Método: Estudio descriptivo realizado en la unidad de terapia polivalente del Hospital "Joaquín Albarrán" de enero a marzo del 2013. Se aplicó una encuesta a 67 trabajadores que accedieron voluntariamente a responderla. Resultados: El 60 % de los encuestados tenía menos de dos años de trabajo en la unidad y solo el 45 %, conocimientos básicos de ética médica. Únicamente el 50 % de los interrogados consideró que este procedimiento puede mejorar la relación médico-paciente. Conclusiones: Es necesario incrementar el conocimiento sobre la utilidad del consentimiento informado entre los trabajadores de la unidad para aprovechar al máximo sus beneficios. Introduction: Informed consent is a procedure that offers full opportunity for patients to have access to information and participate in medical decision-makings. However, it is believed that the information given by the doctors are not fully understood by the patients, their families or both, and besides that many doctors consider it as a second tool or something that doesn't have to be done right away. Objective: Evaluate the perception and experience of the health staff of the polyvalent unit of the surgical clinical teaching hospital Joaquin Albarrán about informed consent. Method: Descriptive study carried out in the polyvalent unit of Joaquin Albarrán Hospital from January to march 2013. A survey was given to 67 workers who voluntarily agreed to answer it. Results: The 60% of the participants had been working in the unit for less than 2 years and only 45% of them had basic knowledge of medical ethics. Only 50% of the participants considered that this procedure could help to improve doctor-patient relationship. Conclusions: It is necessary to increase the knowledge of the usefulness of the informed consent among the workers of the unit to have full profit from it. (shrink)

Estamos ante el primer volumen de una serie sobre ciencia cognitiva publicada en Vancouver por la Universidad de British Columbia, con el objeto de tratar interdisciplinariamente, mediante contribuciones originales, los temas de preocupación central actualmente en ciencia cognitiva, como son los relativos a la percepción, aprendizaje, memoria, razonamiento ordinario y, en general, procesamiento de la información y del lenguaje, que corresponden a congresos y reuniones científicas dedicadas a tales campos en Vancouver.

This study assessed the attitudes and needs of physicians and health professional staff at a tertiary care hospital in Canada regarding the introduction of physician assisted dying (PAD) during 2015–16. This research aimed to develop an understanding of the wishes, concerns and hopes of stakeholders related to handling requests for PAD; to determine what supports/structures/resources health care professionals (HCP) require in order to ensure high quality and compassionate care for patients requesting PAD, and a supportive environment for all healthcare providers (...) across the moral spectrum. This study constituted a mixed methods design with a qualitative descriptive approach for the study’s qualitative component. A total of 303 HCPs working in a tertiary care hospital completed an online survey and 64 HCPs working in hospital units with high mortality rates participated in 8 focus group discussions. Both focus group and survey data coalesced around several themes to support the implementation of PAD following the decriminalization of this practice: the importance of high quality care; honoring moral diversity; supporting values (such as autonomy, privacy, beneficence); and developing resources, including collaboration with palliative care, education, policies and a specialized team. This study provided the foundational evidence to support the development of the PAD program described in other papers in this collection, and can be a model for gathering evidence from stakeholders to inform the implementation of PAD in any healthcare organization. In lieu of an abstract, here is a brief excerpt of the content:Feminist Studies 45, no. 1. © 2019 by Feminist Studies, Inc. 129 Miriam Kienle Dear Data: Feminist Information Design’s Resistance to Self-Quantification Every Sunday for one year, information designers Giorgia Lupi and Stefanie Posavec sent each other a hand-drawn postcard that featured a data visualization of their week as it pertained to a single aspect of their daily lives: doors opened, clocks checks, sounds heard, smells perceived, and (...) so on (figs. 1–4). With this series of postcards exchanged between Brooklyn and London, Lupi and Posavec gained intimate knowledge of one another through their small/slow data, and at the same time, they produced a critical examination of the capture, interpretation, and visualization of their daily data from a uniquely feminist perspective. Although the field of information design prizes clear, efficient, and seamless presentations of quantified activity, Lupi and Posavec’s Dear Data (2015) is comprised of visualizations that underscore complexity over clarity, present questions rather simply display information, and expose the instability of data itself.1 Against the quantification of embodied experience into seemingly objective and all-encompassing datasets, Dear Data instead looks at data from a feminist perspective, as tied to concrete bodies, localities, and temporalities and visualized in a manner 1. The approach to infographics that prioritizes efficiency, accuracy, and elegance, and thus suppresses the constructed and interpretative nature of both data and its visualization, is perhaps most attributed to Edward Tufte. Tufte, The Visual Display of Quantitative Information (Cheshire, CT: Graphics Press, 2001). 130 Miriam Kienle that is incomplete, contingent, and constructed.2 This paper will analyze Lupi and Posavec’s designs as providing a model for a feminist approach to data visualization—a model imperative in an age in which the quantitative information (much like a binary understanding of gender) is too often treated as a self-evident fact rather than what feminist philosopher Donna Haraway famously called “situated knowledges.”3 Recently acquired by MoMA’s architecture and design collection, the Dear Data project not only stands out in the collection as the product of two female designers working in the male-dominated fields of technology and design, but also as a project that stresses feminist knowledge production by underscoring how data voids shape datasets and debunking the myth that the world is data that can be captured and visualized with neutral, distanced, and all-encompassing technologies of vision. As Catherine D’Ignazio and Lauren Klein describe in Data Feminism, data visualization from a feminist perspective understands that “bar charts might seem neutral and objective, but are in fact the result of very human and necessarily imperfect design processes.”4 Such a perspective is evident across Dear Data. Take, for example, “Week 42: A week of laughter” (fig. 5). On the key in Posavec’s visualization, she notes: “I tried to capture my laughs which was really hard + got in the way of the enjoying life, hence the data voids. / For a card about laughter I am sad about how this card turned out.”5 This willingness to show the gaps in her data, the complexities of her process, and the imperfections of her visualizations contrasts with the seamless and straightforward displays of information 2. La actividad científico-informativa constituye un componente esencial de la actividad científica. Las etapas y tareas que la integran persiguen como propósito fundamental garantizar a los investigadores la información pertinente para el desarrollo exitoso de su trabajo. De ahí la necesidad de evaluar con periodicidad su comportamiento en las bibliotecas médicas. A partir de ello, el objetivo del estudio fue sistematizar los aspectos teórico-metodológicos correspondientes a la evaluación de la actividad científico- informativa en bibliotecas médicas. Para su realización se realizó revisión (...) bibliográfica en la que se analizaron tanto documentos normativos que rigen su accionar en el sector de la salud cubano, como materiales emitidos por otras instancias nacionales e internacionales. The informative work constitutes an essential component of the scientific work. The stages and tasks that comprise it pursue as fundamental purpose to ensure relevant information in order that researchers achieve the successful development of their work. Hence the need to assess regularly their behavior in the medical libraries. Based on that, the objective of the study was to systematize the methodological aspects relating to the evaluation of the scientific-informative work at the medical libraries. In order to accomplish this a broad literature review was made which analyzed both normative documents governing its actions in the Cuban health sector, as materials issued by other national and international bodies. Henry Johnstone's philosophical development was guided by a persistent need to reform the concept of validity -either by reinterpreting it or by finding a substitute for it. This project lead Johnstone into interesting confrontations with the concept of rhetoric and especiaUy with the work of Chaim Perelman and Olbrechts-Tyteca. The project culminated in a failed attempt to develop a formal ethics of rhetoric and argumentation, but this attempt was itself not consistent with some of Johnstone's other characterizations ofan ethics of (...) argument ation. A virtue ethics would be truer to the Johnstonian philosophical project than a formal ethics of argument. Resume. (shrink)

Background: Discussing treatment risks has become increasingly important in medical communication. Still, despite regulations, physicians must decide how much and what kind of information to present. Objective: To investigate patients’ preference for information about a small risk of a complication of colonoscopy, and whether medical and personal factors contribute to such preference. To propose a disclosure policy related to our results. Design: Vignettes study. Setting: Department of Gastroenterology, Academic Medical Centre, the Netherlands. Patients: 810 consecutive colonoscopy patients. Intervention: A home- (...) class='Hi'>sent questionnaire containing three vignettes. Vignettes varied in the indication for colonoscopy, complication severity and level of risk. Patients were invited to indicate their wish to be informed and the importance of such information. In addition, sociodemograhic, illness-related and psychological characteristics were assessed. Main outcome measurements: Wish to be informed and importance of information. Results: Of 810 questionnaires, 68% were returned. Patients generally wished to be informed about low-risk complications, regardless of the indication for colonoscopy or the severity of the complication. The level of risk did matter, though (OR = 2.48, SE = 0.28, p = 0.001). The information was considered less important if done for population screening purposes or diagnosis of colon cancer, if the complication was less severe (bleeding) and if the risk was smaller (0.01% and 0.1%). Patients’ information preference was also related to age, mood and coping style. Limitations: Difficulty of vignettes. Conclusions: Patients generally wish to be informed about all possible risks. However, this might become uninformative. A stepwise approach is suggested. (shrink)

In their recent article, Porsdam Mannet alpropose to share biomedical research data more widely, securely and efficiently using blockchain technologies. 1 They present compelling arguments for how the blockchain presents both a technological innovation, and a deontologically grounded policy innovation to traditional research consent. Their proposal can be read in conversation with a rich body of evidence to suggest current consent processes are problematic on at least one of tripartite bases in biomedical research: that it be fully informed. This (...) response attempts to further the author’s discussion of social justice discourse in, and of their proposed prosent model to enhance engagement among under-represented and vulnerable populations in research, specifically. Motivating this response is the view that advancing technological capabilities is no doubt necessary, but on its own insufficient to reinvigorate distributive, procedural and social justice as guiding principles for con/prosent processes. I offer three pros and cons to consider in effort to deepen the model’s commitments to social justice to historically marginalised groups in the biomedical research enterprise. (shrink)
