As medical technology becomes more sophisticate the ability to manipulate nature and manage disease forces the dilemma of when can becomes ought. Indeed, most bioethical discourse is framed in terms of balancing the values and interests and the benefits and burdens that inform principled decisions about how, when, and whether interventions should occur. Yet, despite advances in science and technology, one caregiver mandate remains as constant and compelling as it was for the earliest shaman—the relief of pain. Even when (...) cure is impossible, the physician's duty of care includes palliation. Moreover, the centrality of this obligation is both unquestioned and universal, transcending time and cultural boundaries.Although universally acknowledged, pain is a complex phenomenon for both the patient and the caregiver, influenced as much by personal values and cultural traditions as by physiological injury and disease. David Lewis (1969) introduced sender-receiver games as a way of investigating how meaningful language might evolve from initially random signals. In this report I investigate the conditions under which Lewis signaling games evolve to perfect signaling systems under various learning dynamics. While the 2-state/2- term Lewis signaling game with basic urn learning always approaches a signaling system, I will show that with more than two states suboptimal pooling equilibria can evolve. Inhomogeneous state distributions increase the likelihood of pooling equilibria, but (...) learning strategies with negative reinforcement or certain sorts of mutation can decrease the likelihood of, and even eliminate, pooling equilibria. Both Moran and APR learning strategies (Bereby-Meyer and Erev 1998) are shown to promote successful convergence to signaling systems. A model is presented that illustrates how a language that codes state-act pairs in an order-based grammar might evolve in the context of a Lewis signaling game. The terms, grammar, and the corresponding partitions of the state space co-evolve to generate a language whose structure appears to reflect canonical natural kinds. The evolution of these apparent natural kinds, however, is entirely in service of the rewards that accompany successful distinctions between the sender and receiver. Any metaphysics grounded on the structure of a natural language that evolved in this way would track arbitrary, but pragmatically useful distinctions. (shrink)

The use of deception in medical care is highly suspect in this country. Yet there is one condition for which deception is often used as a diagnostic tool. Nonepileptic seizures, a psychiatric condition in which emotional or psychological conflicts manifest themselves unconsciously through bodily symptoms, are currently diagnosed by a procedure called “provocative saline infusion.” The test is fundamentally deceptive, requiring the physician to intentionally and directly lie to the patient, causing the patient to believe that the administered solution caused (...) his seizures. This chapter considers two types of fine‐tuning, those dealing with the initial conditions of the universe and those based on fixed parameters. Three approaches have been taken to argue that fine‐tuning does not need any special explanation. The first is an appeal to coincidence. The second is that the data are biased by our own observations. The third has to do with the nature of probability itself. The chapter assesses each of these objections in detail. Many naturalistic explanations have been (...) offered for fine‐tuning. The chapter considers the three most important ones. The first is that future discoveries might show that fine‐tuning was not improbable after all. The second is that other life‐permitting ranges for the fine‐tuned constants (FTCs) might exist, allowing for exotic forms of life unlike ours. The third is that we might live in a multiverse of many different universes all with different values of FTCs. (shrink)

This essay assesses Robert Merrihew Adams' contribution to the religion-morality debate in light of questions in philosophical semantics and metaphilosophy, questions Adams raises without addressing directly. It sketches a holistic theory of the use of language in thought in the hope of providing a context for determining the value and philosophical relevance of Adams' semantic claims. It concludes by suggesting that descriptive metaethics should give way to explicitly historical studies, and by maintaining that historians of ethics need not postulate "meanings" (...) in order to make sense of what they do. Abstract: C. S. Peirce's psychological analysis of belief, doubt, and inquiry provides insights into the nature of scientific knowledge. These in turn can be used to construct an account of scientific knowledge where the notions of belief, truth, rational justification, and inquiry are determined by the relationships that must hold between these notions. I will describe this account of scientific knowledge and some of the problems it faces. The change in shape of the C. elegans embryo from an ovoid ball of cells into a worm-shaped larva is driven by three events within the cells of the hypodermis (epidermis): (1) intercalation of two rows of dorsal cells, (2) enclosure of the ventral surface by hypodermis, and (3) elongation of the embryo. While the behavior of the hypodermal cells involved in each of these processes differs dramatically, it is clear that F-actin and microtubules have essential functions in each of (...) these processes, whereas contraction of actomyosin structures appears to be involved specifically in elongation. Molecular analysis of these processes is revealing components specific to C. elegans as well as components found in other systems. Since C. elegans hypodermal cells demonstrate dramatically different behaviors during intercalation, enclosure and elongation, the study of cytoskeletal dynamics in these processes may reveal both unique and conserved activities during distinct epithelial morphogenetic movements. The ACTWith model is a synthesis across these disciplines, integrating ancient and contemporary insights into the human condition, while distilling this synthesis into a practicable dynamic simplified via architectural paradigms imported from theories of computational models of human learning. It was developed in response to the need in these fields for a clear articulation of (...) conscience. In the world at large, conscience is often referenced, yet hardly understood. Each consists in a base game that evolves a language descriptive of nature and a metagame that coevolves a language descriptive of the base game and its evolving language. The first composite game shows how a pragmatic notion of truth might coevolve with a simple descriptive language. The second shows how a pragmatic notion of probability might similarly coevolve. Accuracy in the philosophical theory of rationality demands that we recognize particular beliefs as arising within the context of larger units, the cultural or conceptual schemes, patterns, or practices, involvement in which itself provides standards and grounds for their rational evaluation. At the same time, though, a satisfactory account of rationality cannot hold the standards, values, or commitments of one particular culture, practice, or conceptual scheme, even one’s own, immune from rational criticism. In order to accurately and responsibly picture the (...) shape of our commitments and the dynamics of their revision over time, in other words, the theory of rationality must reconcile the immanence of reason to particular cultural and conceptual units with its transcendence of them. This raises a deep and far-ranging problem of perspective. In his perspicuous presentation of it, Hilary Putnam put the problem this way: There are two points that must be balanced, both points that have been made by philosophers of many different kinds: (1) talk of what is ‘right’ and ‘wrong’ in any area only makes sense against the background of an inherited tradition; but (2) traditions themselves can be criticized … On the one hand, there is no notion of reasonableness at all without cultures, practices, procedures; on the other hand, the cultures, practices, procedures we inherit are not an algorithm to be slavishly followed. As Mill said, commenting on his own inductive logic, there is no rule book which will not lead to terrible results ‘if supposed to be conjoined with universal idiocy.’ Reason is, in this sense, both immanent (not to be found outside of concrete language games and institutions) and transcendent (a regulative idea that we use to criticize the conduct of all activities and institutions). This essay explores the significance of Agamben’s sparse references to medieval Jewish thinkers (that is, the Rishonim) and raises the question as to whether the modern interpretive horizon of “history” is adequate for providing an understanding of these thinkers.

Although there is a long-standing international debate concerning the privatization and corporatization of health services, there has been relatively little systematic analysis of the ways these types of reform manifest. We examine the impact of privatization and corporatization on public hospitals, and in particular on hospitals’ autonomy and accountability, with two aims: to uncover the key themes in the literature, and to consider implementation issues. The review of 2,319 articles was conducted using content analysis and a discussion of selected key (...) issues. Several major themes appear in the privatization and corporatization literature, including their use as tools in health systems reform, and the role of governments in sponsoring the processes. We show that much of the underlying argument is ideological rather than evidence based. Those who promote versions of privatization or corporatization claim that decreased government involvement in the management of hospitals leads inter alia to benefits such as greater efficiency, better quality services, and increased choice for patients. Those who argue against say that increased privatization leads to deleterious outcomes such as decreased equity, compromised efficiency and poorer quality of care. The evidence is often weak and at times conflicting. Privatization and corporatization are difficult to implement, and at best produce mixed results, and their impact seems to depend more on the motivation of the evaluator than the standard of the results. These debates are of a type that is to a large extent only resolvable ideologically. (shrink)

The object of this essay is to demonstrate a logical connection between beliefs and values. It is argued that such a connection can be established only if one keeps in mind the question: What is minimally required in order that it makes sense to speak of beliefs and values at all? Thus, the concept of minimal rationality is indispensable to the task at hand. A particular example of a logical connection between a belief and a value is examined, which leads (...) to a formal rule (Rule of Minimal Rationality) which expresses in general the logical connection between beliefs and values. This rule is elaborated to include attempted activities as well as activities simpliciter, activities having been discovered as essential to any logical connection between beliefs and values. Finally, some interesting implications and promising applications of the Rule of Minimal Rationality are outlined. (shrink)

In a century dominated by the exacting methods and dramatic successes of science, it is difficult to imagine an informed contemporary religious believer never shaken by the doubt that his or her most sacred ideas are atavisms to a benighted age, vain and empty fantasies. To such a believer, Freud's late monograph, The Future of an Illusion , with its warm, solicitous tone, but relentless scepticism, must seem the patient knell of his or her worst fears. For here Freud uses (...) his dialectical skill and his psychoanalysis to augment motives for self-doubt at least as old as Abraham: Perhaps the most cherished of my beliefs are a kind of madness, a condition I do not recognize because it is so widely shared. Perhaps like the madman bewitched by the tapestry of his delusions, I hold fast to my faith only because I could not endure the threadbare fabric of a life without it. But it is not as though the religionist is utterly without defence against this source of doubt, nor that the familiar defence is without considerable prima facie force against Freud. For when sceptics such as Freud turn their account of the origin of religious faith into a case against the truth of religious claims, the believer can attack this line with an unassailable axiom: No examination of the motives for a belief is relevant to an assessment of its truth. To assume otherwise is to commit the genetic fallacy. The question I want to deal with here is whether the religious believer should rest content with this rejoinder to Freud. (shrink)

Carr investigates the historical background to the criticisms of the "Metaphysics of the Subject" that are found in French post-structuralist thought (...) and in critical theories descended from the Frankfurt School. In explaining this background, he targets the widely held assumption that the history of modern philosophy can best be understood in terms of a fundamentally unified conception of the subject and subjectivity that unfolds inevitably from Descartes and culminates in twentieth-century phenomenology and existentialism. Those who share this assumption fail to recognize the significance of what Husserl (in the Crisis of European Sciences) called the "paradox of subjectivity," which derives from the necessity of understanding the knowing subject both as a subject for the world and as an object in the world.The origins of contemporary interpretations of the metaphysics of the subject are found primarily in Heidegger's thought. Carr thus devotes the first chapter of this study to the critical assessment of Heidegger's view of modern metaphysics starting with Descartes. For Heidegger, Descartes took over the dominant concept of substance from traditional ontology and transferred it to the human subject. The Cartesian subject, then, was revealed as an underlying hypokeimenon that absorbs or overpowers the world by reducing it to the representational contents of the thinking thing. Moreover, Heidegger maintained that modern thinkers did not go beyond the reductive conception of subjectivity intrinsic to the Cartesian metaphysics of substance. This Heideggerian judgment applies to the major modern thinkers. Even Kant and Husserl figure as representatives of a tradition in which "the knowing subject takes the role of substance; everything is reduced to first properties, whether as immanent in consciousness, object for the subject, or, as in transcendental philosophy, construct and product of subjective activity" (Carr, 31).In chapters 2-4, Carr argues that the Heideggerian interpretation is inadequate with respect to Kant and Husserl. In supporting this position, Carr emphasizes the close affinities between Kant's and Husserl's conceptions of objectivity for consciousness and between their portrayals of subject and self. Specifically, Carr argues (1) that both thinkers were committed to the representationalist view of cognition that is typical of other major modern philosophers; and (2) that both presented parallel, and indeed unifiable, accounts of the relationship between the transcendental and the empirical self.The key to explaining the fundamental agreement between Kant's and Husserl's transcendental theories lies in the concept of intentionality. There are, of course, significant differences between Husserl's phenomenological method and the procedure of inquiry into the possibility conditions of objective experience that Kant follows in the Critique of Pure Reason. Whereas Heyman provides scientific clarification and support for the first, the second remains problematic. I outline how this deficiency might be remedied via evolutionary considerations.
