In the UK, the Recommended Summary Plan for Emergency Care and Treatment (ReSPECT) is a widely used process, designed to facilitate shared decision-making between a clinician and a patient or, if the patient lacks capacity to participate in the conversation, a person close to the patient. A key outcome of the ReSPECT process is a set of recommendations, recorded on the patient-held ReSPECT form, that reflect the conversation. In an emergency, these recommendations are intended to inform clinical decision-making, and thereby (...) enable the attending clinician—usually a general practitioner (GP) or paramedic—to act in the patient’s best interests. This study is the first to explore the extent to which ReSPECT recommendations realise their goal of informing best interests decision-making in community contexts. Using a modified framework analysis approach, we triangulate interviews with patients and their relatives, GPs and nurses and care home staff. Our findings show that inconsistent practices around recording patient wishes, diverging interpretations of the meaning and authority of recommendations and different situational contexts may affect the interpretation and enactment of ReSPECT recommendations. Enacting ReSPECT recommendations in an emergency can be fraught with complexity, particularly when attending clinicians need to interpret recommendations that did not anticipate the current emergency. This may lead to decision-making that compromises the patient’s best interests. We suggest that recording patients’ values and preferences in greater detail on ReSPECT forms may help overcome this challenge, in providing attending clinicians with richer contextual information through which to interpret treatment recommendations. The present text aims to discuss a certain kind of internet meme, construed here as an image macro, in the context of Conceptual Blending Theory (Fauconnier and Turner 2002). Image-based memes (image macros) that contain a verbal layer appear to be the most popular form of such memes. Their concise form demonstrates a creative potential: memes are packed with meaning, reflected in various conceptual packets integrated into their verbal and visual semiosis. Blending theory thus seems an appropriate framework for (...) a discussion of the cognitive mechanisms of memes and the relation between the verbal and the visual, as well as for tracing how the analogy between given domains/input spaces contributes to the effectiveness of a message. Julia Annas explores how Plato's account of the relation of virtue to law developed, and how his ideas were taken up by Cicero and by Philo of Alexandria. She shows that, rather than rejecting the account given in his Republic, Plato develops in the Laws a more careful and sophisticated version of that account.

DOES A STANDARD OF DESERT BELONG IN CHRISTIAN CONCEPTIONS OF distributive justice? This essay places John Calvin and John Rawls, two of desert's most incisive critics, in conversation to examine the theological and philosophical issues raised by this question. Calvin and Rawls make similar arguments against deservingness as a moral principle, but Calvin emerges as the more adamant detractor, noting that God's grace and humanity's corrupt nature make the validity of positive human desert claims virtually unthinkable. Still, the moral force (...) of desert invites a reevaluation of both Calvin's and Rawls's objections and the fittingness of this principle to theological conceptions of distributive justice. The "hard problem" of today's consciousness studies is subjective experience: understanding why some brain processing is accompanied by an experienced inner life. Recent scientific advances offer insights for understanding the physiological and chemical phenomenology of consciousness. But by leaving aside the internal experiential nature of consciousness in favor of mapping neural activity, such science leaves many questions unanswered. In Ontology of Consciousness, scholars from a range of disciplines -- from neurophysiology to parapsychology, from mathematics to anthropology and indigenous non-Western modes (...) of thought -- go beyond these limits of current neuroscience research to explore insights offered by other intellectual approaches to consciousness. These scholars focus their attention on such philosophical approaches to consciousness as Tibetan Tantric Buddhism, North American Indian insights, pre-Columbian Mesoamerican civilization, and the Byzantine Empire. Some draw on artifacts and ethnographic data to make their point. Others translate cultural concepts of consciousness into modern scientific language using models and mathematical mappings. Many consider individual experiences of sentience and existence, as seen in African communalism, Hindi psychology, Zen Buddhism, Indian vibhuti phenomena, existentialism, philosophical realism, and modern psychiatry. Some reveal current views and conundrums in neurobiology to comprehend sentient intellection. As an approach to public engagement, deliberation has the potential to pursue a range of goals identified by public participation theorists including the opportunity to substantively inform policy processes, increase the public’s knowledge and understanding of public issues and create or restore loss of public trust and confidence in public institutions. Baum and colleagues (2009) offer several important take-home messages for policy makers and public health leaders about the value of engaging with the public about ethically challenging, value-laden and resource (...) intensive policy issues such as pandemic response planning. The contributors set out to meaningfully grapple with the bewildering ecological and social crises that humanity faces in this young century. He ultimately became regarded as the greatest Catholic historian of the twentieth century, and became a Harvard Professor and a cult figure for American and European Catholics. (...) This article applies Karl Polanyi’s concept of a double movement to the trajectory of rural state policies in China since 1949. It argues that Chinese socialism created a contradictory social contract that has fueled an ongoing struggle between state and peasantry over the surplus generated from rural land. This struggle has shaped a historical oscillation between state policies that facilitate extraction of agricultural surpluses and policies that introduce social protections in the form of household farming and revitalized collective ownership. Based (...) on secondary sources, this article compares the arc of rural policies during the Mao era and in the transition to and during the current state capitalist period. Then, based on original interview-based and ethnographic fieldwork undertaken in rural Sichuan Province, it analyzes the current introduction of urban and agrarian capital into the rural economy, revealing dynamics of a current countermovement from state-led extraction to compromise. Bioethicists recognize the conflicts of interest that can arise for clinicians and scientists. However, few scholars exploring the moral dimensions of medicine and the sciences publicly address potential conflicts of interest concerning their own research. Increasingly, however, bioethicists will be confronted with difficult choices in which opportunities to obtain funding will sometimes conflict with the pursuit of critical, rigorous scholarship conducted without regard for corporate interests.

Countries throughout Asia promote themselves as leading destinations for international travelers seeking inexpensive healthcare. India, Indonesia, Malaysia, Singapore, the Philippines, and Thailand are all trying to attract greater numbers of what their promotional campaigns call “medical tourists.” Government tourism initiatives, hospital associations, medical tourism companies, and individual hospitals advertise hip and knee replacements, spinal surgery, cosmetic surgery, and other medical procedures. In contrast to most nations marketing treatments to international patients, the Philippines differentiates itself by selling “all inclusive” kidney transplant (...) packages. The squatting movement is a social movement that seeks to use unoccupied land or temporarily or permanently abandoned buildings as farmland, housing, meeting places, or centers for social and cultural purposes. Its main motivation is to denounce and at the same time respond to the economic difficulties that activists believe exist to realize the right to housing. Much of what we know about this movement comes from the informational and journalistic literature generated by actors that are close or even belong (...) to the movement. However, there is also a significant diversity of knowledge and scientific evidence on the squatters’ movement that is being produced by academia and that is worth knowing and grouping together. With the aim of defining and understanding how the squatters’ movement is constituted and organized, and how it acts, this research analyzes what the scientific literature affirms about it. Through qualitative research based on the systematic literature review method, information was sought in the Web of Science and Scopus databases. The initial universe of 262 articles was finally reduced to a sample of 32 articles. These have been analyzed by means of a categorized classification content analysis. The results obtained allow us to establish the state of the art on the squatting movement, placing special emphasis on its dynamics of resistance, its process of political subjectivation and its mechanisms of action and self-management. The study suggests that the movement is understood based on collective actions with a political role of resistance to neoliberalism and the inequalities it generates, and of response to the basic and social needs of the communities through self-management.Systematic review registration[doi: 10.5281/zenodo.7179670], identifier [7179670]. The problem of the role of experts in society may seem to be a topic marginal to the main concerns of sociology, but it is in fact deeply rooted in the sociological project itself. Sociologists and social thinkers have long been concerned with the problem of the role of knowledge in society. Certain Enlightenment thinkers, notably Turgot and Condorcet, believed that social progress depended on the advance of knowledge and the wider dispersion of knowledge in society. But Condorcet especially recognized (...) that this idea had complex political implications. On the one hand, it required science, which for him included social science, to be supported by the state, yet retain independence or self‐governance in order to advance without political interference. On the other hand, he recognized that social advance required that the most enlightened be the rulers, and that this conflicted with the ideas of democracy and equality. (shrink)

The author analyses the concept of practices which has only recently come to prominence in social theory. The ‘rules’ or ‘norms’ model of society is a misleading abstraction and ‘practices’ better captures the fact that living in society is not simply a matter of rules but of the practical mastery of the cues and expectations of others. The locus of explanation shifts from culture as a determinant in the social system to a more pragmatic understanding of the ongoing effects of (...) practices. Drawing on his past work as an interpreter of Adam Smith, Hanley offers an account of Fénelon’s social and political thought that emphasizes the role (and the dangers) of pride and self-love in human affairs. Fénelon does not join those, like Mandeville and La Rochefoucauld, who seek to understand and possibly to control self-love. Instead, he attempts to wed a far more rigorous and classical approach to the problem of pride—namely, the refinement of self-love through virtuous practices—to a modern view (...) of the state and political economy. Contemporary scholarship examining clinical outcomes in medical travel for cosmetic surgery identifies cases in which patients traveled abroad for medical procedures and subsequently returned home with infections and other surgical complications. Though there are peer‐reviewed articles identifying patient deaths in cases where patients traveled abroad for commercial kidney transplantation or stem cell injections, no scholarly publications document deaths of patients who traveled abroad for cosmetic surgery or bariatric surgery. Drawing upon news media reports extending from 1993 to 2011, this article (...) identifies and describes twenty‐six reported cases of deaths of individuals who traveled abroad for cosmetic surgery or bariatric surgery. Over half of the reported deaths occurred in two countries. Analysis of these news reports cannot be used to make causal claims about why the patients died. In addition, cases identified in news media accounts do not provide a basis for establishing the relative risk of traveling abroad for care instead of seeking elective cosmetic surgery at domestic health care facilities. Acknowledging these limitations, the case reports suggest the possibility that contemporary peer‐reviewed scholarship is underreporting patient mortality in medical travel. The paper makes a strong case for promoting normative analyses and empirical studies of medical travel. In particular, the paper argues that empirically informed ethical analysis of ‘medical tourism’ will benefit from rigorous studies tracking global flows of medical travelers and the clinical outcomes they experience. The paper contains practical recommendations intended to promote debate concerning how to promote patient safety and quality of care in medical travel. (shrink)

The Occupy Wall Street movement of 2011 and its corollaries, Occupy Sandy and Occupy Debt, have been largely understood as secular movements. In spite of this, religious actors not only participated, but in some cases played an integral role within the movement, lending material support, organizing expertise, and public statements of support. We rely on interviews with faith leaders in New York and Oakland, and engage in an analysis of print and online media to explore the role of religious actors (...) and groups in Occupy Wall Street. Religious participants were often long-time veterans of progressive political struggles and drew inspiration from their faith traditions. Nonetheless, religious commitments were secondary to political objectives shared by themselves and their secular counterparts. Religious leaders believed they offered symbolic authority to the movement and highlighted this in their engagement in the hope of giving it greater moral weight. Current discussions on postsecularism and public religions are considered. (shrink)

One of Ian C. Jarvie’s most interesting contributions is his discussion of the thinking of Karl Popper and Michael Polanyi on the nature and workings of the scientific community and their relation to politics : 545–564, 2001). The self-image these thinkers contributed to still lingers, but their accounts capture a historical moment that has passed and was idealized even when they were written. In this chapter, I examine this tradition and identify the central themes which dominated this literature and ask (...) the question of how this history is relevant today. The answer will perhaps be surprising: despite the fact that the Left of the past held positions that are the opposites of the positions of the present participatory Left, the considerations that motivated the older Left remain. This special volume of Oxford Studies in Ancient Philosophy presents sixteen specially written essays on virtue and happiness, and the treatment of these topics by thinkers from the fifth century BC to the third century AD. It is published in honour of Julia Annas--one of the leading scholars in the field.

Explanations in the natural historical sciences often take the form of stories. This paper examines two accounts of the sources of narrative’s explanatory power: Beatty’s suggestion that narrative explanation is closely connected to historical contingency, and that narratives explain by contrasting what happened with what might have happened; and Ereshefsky and Turner’s view that narratives explain by organizing events around a central subject with a distinctive direction of historical development. In both accounts, it turns out that non-epistemic values typically (...) play a role in the construction of narrative, and hence contribute to narrative’s explanatory force. Two case studies from historical science – the plate tectonic story of the Avalonian terrane, and the story of the evolution of trichromatic vision in (some) mammals – help to motivate and illustrate this argument.
