BackgroundAdvance directives imply the promise of determining future medical treatment in case of decisional incapacity. However, clinical practice increasingly indicates that standardized ADs often fail to support patients’ autonomy. To date, little data are available about the quality and impact of ADs on end-of-life decisions for incapacitated acute stroke patients.MethodsWe analyzed the ADs of patients with fatal stroke, focusing on: their availability and type, stated circumstances to which the AD should apply, and stated wishes regarding specific treatment options.ResultsBetween 2011 and (...) 2014, 143 patients died during their hospitalization on our stroke unit. Forty-two of them had a completed and signed, written AD, as reported by their family, but only 35 ADs were available. The circumstances in which the AD should apply were stated by 21/35 as a “terminal condition that will cause death within a relatively short time” or an ongoing “dying process.” A retrospective review found only 16 of 35 ADs described circumstances that, according to the medical file, could have been considered applicable by the treating physicians. A majority of patients objected to cardiopulmonary resuscitation, mechanical ventilation, and artificial nutrition, while almost all directed that treatment for alleviation of pain or discomfort should be provided at all times even if it could hasten death.ConclusionsThe prevalence of ADs among patients who die from acute stroke is still low. A major flaw of the ADs in our cohort was their attempt to determine single medical procedures without focusing on a precise description of applicable scenarios. Therefore, less than half of the ADs were considered applicable for severe acute stroke. These findings stress the need to foster educational programs for the general public about advance care planning to facilitate the processing of timely, comprehensive, and individualized end-of-life decision-making. A collection of articles in English, German and French by fourteen Hungarian authors. Of particular interest and value is "Über die erkenntnistheoretischen Ansichten des jungen Marx" by György Markus. Although the footnotes are full of criticisms of the "revisionists", the early Marx is taken seriously as a philosophical thinker in his own right, and an effort is made to lay out what might be a theory of knowledge for this early Marx. Other articles of interest are by Edit Rózsahegyi, "The (...) Place and Role of Purpose in the Process of Cognition" and a criticism of Heisenberg by A. Szabó. On the whole, the articles are genuinely philosophical and deserve attention, although long, boring and dogmatic pieces such as "Objective Contradiction" by G. Tamás, and "Communism and Conditions for the Development of Personality" by A. Wirth are also present.--K. A. M. (shrink)

In the past couple of decades, interest in the philosophy of language has, according to the introduction written by Zoltan Szabó for this volume, cooled somewhat. This is because pursuit of many of the philosophical issues that the study of language had set out to solve in the first place has developed into not so much the study of language, but, rather, of mind, and adjacent areas of philosophy. As Tyler Burge has put it, “Many philosophers felt that the philosophy (...) of language had done its job.” This volume attempts to illuminate the positive side of these developments for the philosophical study of language. Szabó says, “The scientific results are interesting on their own, their relevance to the philosophical questions remains clear”. This, then, puts philosophy of language on a new footing, where, “Freed from the responsibility of ultimately answering all philosophical questions, philosophers of language were free to confess an interest in language per se”. The stated purpose of the volume is to present the reader with a variety of representative papers which “exemplify the current reorientation of philosophy of language,” which Szabó characterizes as a joint linguistic and philosophical investigation into “the syntax and semantics of various natural language constructions.”. In the tradition of Stalnaker there is a number of well-known problems that need to be addressed, because revision of iterated belief modalities is required in this case. These problems have already been investigated in detail in recent works on DDL Leitgeb/Segerberg 2007)and DEL see e.g. Ditmarsch et. Another strategy would be to maintain and revise assumptions independently of the beliefs of an agent.I will briefly discuss the advantages and disadvantages of each of these views. In both views, assumptions constitute (...) the subjective context in which an agent interprets an utterance and encounters the world. The result of an interpretation is in turn checked against the agent’s original beliefs, and if the checking operation succeeds the agent revises his beliefs by the result in the normal way described by the AGM paradigm. The second of the above questions needs to be addressed on the basis of concrete examples. Considering utterance like David is ready’ or ‘John is tall’that from a contextualist viewpoint express semantically incomplete content in the sense of Bach is needed in order to obtain a useful model of the checking step, since fortunately not everybody believes everything that other people say. These requirements put the theory of interpretation based on assumptions in the frontline of ongoing research on the implementation of belief revision and update in dynamic logics. Such a theory might also be useful for contextualist accounts of strong knowledge, as it can be argued convincingly that when a knowledge ascription appears to be context-sensitive, this is so because the embedded proposition is context-sensitive and not because knowledge itself is context-sensitive. Hence,the context-sensitivity of embedded propositions in knowledge claims and how different agents in the same situation arrive at different assessments about them may be explained by an inferential theory of interpretation similar to the one outlined here but with another underlying concept of assumptions. A population policy, that assures the negative freedom of the individuals who are not forced to proceed on abortion, their well-being and their positive freedom of escaping from preventable mortality, of avoiding mortality during infancy and childhood, of being free from hunger and undernutrition, is possible in developing countries, if the developed states should realize that they overconsume and that they use nature as their own resource, at the expense of the rest of the world.By avoiding rich nations' overconsumption and (...) injustice towards the poor ones we are moving towards a restraint on resource depletion, environmental degradation, poverty and population explosion, on the destruction of a sustainable life in general. Though many agree that we need to account for the role that social factors play in inquiry, developing a viable social epistemology has proved to be a difficult task. According to Longino, it is the processes that make inquiry possible that are aptly described as social, for they require a number of people to sustain them. These processes not only facilitate inquiry, but also ensure that the results of inquiry are more than mere subjective opinions, and thus deserve to be (...) called knowledge. In this paper, I explain Longino’s epistemology and defend it against criticisms recently raised by Kitcher, Schmitt and Solomon. Longino rightly recognizes that not all social factors have the same affect on inquiry. She also recommends that we reconceptualize ‘knowledge,’ distinguishing knowledge from opinion by reference to a social standard. (shrink)

Nature and Narrative is the launch volume in a new series of books entitled International Perspectives in Philosophy and Psychiatry. Nature(representing interest in the causes of a problem) and Narrative (for understanding its meanings) will introduce the field and the series, by touching on a range of issue relevant to this interdisciplinary 'border country'.

Cowan's experimental techniques cannot constrain subject's recall of presented information to distinct independent chunks in short-term memory (STM). The encoding of associations in long-term memory contaminates recall of pure STM capacity. Even in task environments where the functional independence of chunks is convincingly demonstrated, individuals can increase the storage of independent chunks with deliberate practice – well above the magical number four.

The "gang of four" — Wang Hung-wen, Chang Ch'un-ch'iao, Chiang Ch'ing and Yao Wen-yuan — have created great chaos by confusing the relations between ourselves and the enemy, obliterating the differences between the two kinds of contradiction, wrecking Chairman Mao's policy of uniting with, educating and reforming intellectuals, calling intellectuals the "stinking number nine," smothering the revolutionary initiative of the broad masses of intellectuals, and destroying the ranks of revolutionary intellectuals.
