BackgroundIn gender-affirming medical care (GAMC), ethical challenges in decision-making are ubiquitous. These challenges are becoming more pressing due to exponentially increasing referrals, politico-legal contestation, and divergent normative views regarding decisional roles and models. Little is known, however, about what ethical challenges related to decision-making healthcare professionals (HCPs) themselves face in their daily work in GAMC and how these relate to, for example, the subjective nature of Gender Incongruence (GI), the multidisciplinary character of GAMC and the role HCPs play in assessing (...) GI and eligibility for interventions. Given the relevance and urgency of these questions, we conducted a qualitative study among HCPs providing GAMC to transgender adults in the Netherlands.MethodsIn this qualitative research, we conducted 11 semi-structured interviews between May 2020 and February 2021 with HCPs (six mental health professionals, two HCPs in endocrinology, two in plastic surgery, and one in nursing) working in two distinct GAMC settings. We purposively sampled for professional background and years of experience in GAMC. We analyzed our interview data using thematic analysis. As some respondents were more inclined to speak about what should or ought to be done to arrive at good or right decision-making, we identified both ethical challenges and norms. Furthermore, in our analysis, we differentiated between respondents’ explicit and implicit ethical challenges and norms and ascertained the specific context in which these challenges emerged.ResultsRespondents’ ethical challenges and norms centered on (1) dividing and defining decisional roles and bounds, (2) negotiating decision-making in a (multidisciplinary) team, and (3) navigating various decision-making temporalities. These themes arose in the context of uncertainties regarding (1) GAMC’s guidelines, evidence, and outcomes, as well as (2) the boundaries and assessment of GI.ConclusionsThis interview study provides detailed empirical insight into both the explicit and implicit ethical challenges that HCPs experience and their ethical norms regarding decision-making. It also describes how uncertainties and (implicit) normativities concerning GAMC and GI pre-structure the moral environment in which these challenges and norms manifest. We provide normative reflections and recommendations on handling these ethical challenges in a way that is sensitive to the context in which they arise. (shrink)

Zusammenfassung Am Beispiel der häufig zum Symptom eines narzisstischen Zeitalters erklärten Selfie-Praxis wird in diesem Text das Verhältnis von Selbstdarstellung und Verkündigung bedacht. Das in den Sozialen Medien verbreitete Phänomen, sich selbst zum Bild zu machen und anderen Bilder von sich selbst zu präsentieren, tritt dabei als ein religiös grundiertes Unterfangen vor Augen. Eine Sehnsucht nach unvergänglicher Gegenwart lässt sich in der Selfie-Praxis ebenso artikuliert finden wie ein Bewusstsein für die undarstellbare Dimension des individuellen Selbst. Beide Aspekten werden zudem als (...) Versuch eines produktiven Umgangs mit herausfordernden Entwicklungen der Moderne verstanden: mit der durch den sozialen Beschleunigungsdruck hervorgerufenen Gegenwartsschrumpfung auf der einen und dem Zwang zur digitalen Selbstpräsentation auf der anderen Seite. Mithin lässt sich das im Selfie zum Ausdruck kommende Bewusstsein auf die Rede von Gott beziehen: auf Gott als Korrelat jener Sehnsucht nach unvergänglicher Gegenwart und als Inbegriff einer dem menschlichen Selbst unsichtbar eingeschriebenen Lebendigkeit. In dieser ihm innewohnenden Verweisstruktur lässt sich das Selfie selbst als verkündigungsförmig ansehen, was den auch in der Theologie verbreiteten Narzissmusvorwurf gegenüber religiösen Selbstdarstellungspraktiken nicht nur in der Predigtarbeit entkräften sollte. When people are asked to estimate the probabilities of uncertain events, they often neglect the additivity principle, which requires that the probabilities assigned to an exhaustive set of outcomes should add up to 100%. Previous studies indicate that additivity neglect is dependent on response format, self-generated probability estimates being more coherent than estimates on rating scales. The present study made use of eye-tracking methodology, recording the movement, frequency and duration of fixations during the solution of ten additivity problems and two (...) control tasks. Participants produced more non-additive estimates in the Scale format than in the Self-generated format. Self-generated estimates also led to longer decision time and a higher number of repeated inspections, suggesting a deliberate comparison process. In contrast, the Scale format seemed to encourage a case-based approach where each outcome is evaluated in isolation. (shrink)

A teoria da implicatura constitui o principal legado de Paul Grice aos modernos estudos da pragmática linguística. Embora associado à filosofia da linguagem ordinária, seu pensamento não se apresenta marcado pela aversão à sistematização teórica que, segundo se diz, caracterizou essa corrente filosófica. Pelo contrário, a obra do filósofo inglês, apesar de dispersa em artigos só postumamente reunidos em livro, constitui um esforço coerente e rigoroso de analisar os princípios que explicam o comportamento racional do homem. Este ensaio oferece uma (...) tentativa de interpretação da análise que ele propôs para o fenômeno da implicatura, buscando integrar tal análise, progressivamente, ao quadro mais geral de sua filosofia da linguagem e de sua psicologia filosófica. De passagem, é sugerida uma aproximação, quanto aos propósitos, entre o programa filosófico de Grice e certa ensaística crítica do poeta argentino Jorge Luis Borges, sugestão talvez surpreendente, mas que pode se mostrar instrutiva. There is renewed interest in what can be called an experimentist approach to education research. The claim is that if researchers would focus on experiments and evidence-based policies and practices, irreversible progress in education can be achieved. This experimentist approach cannot provide the understanding of knowledge and human beings needed for meaningful progress in education. Lacking is adequate appreciation for the role of theory, particularly ethical and other philosophical theory. We especially need a theory of our human condition and a (...) keen awareness for how some conceptions of scientific work can influence, and distort, our conceptions of human beings and worthwhile lives. As paradoxical as it might seem at first, to move ahead in education and our service to persons? welfare, we need to look backward to revisit old truths. (shrink)

Vielmehr ist sie eine der wenigen Beispiele für den Sachverhalt, dass mit dem zeitlichen Abstand der Entstehung eines Werks die systematische Bedeutung immer noch zunehmen kann. Heute werden die großen ethischen Themen der Gegenwart, von den Menschenrechten bis zur Bioethik, nicht ohne (...) ausdrücklichen Bezug auf Kants Ethik behandelt. In diesem Band werden Beiträge vorgelegt, die durchgehend ein systematisches Interesse verfolgen. Sie rekonstruieren ausgehend von einem exegetisch gesicherten Ort im Werk Kants die Erträge für die gegenwärtige Philosophie. Mit Kant wird gleichsam über den ursprünglichen Horizont seines Denkens hinausgegangen. Dabei zeigt sich, dass die kantische Ethik nicht nur auf den ihr vertrauten Gebieten von Gerechtigkeit, Autonomie, Verbindlichkeit und Recht Bedeutendes leistet, sondern auch zum Problem der Motivation Lösungsvorschläge bereithält - einem Bereich, der lange Zeit als ihr Schwachpunkt angesehen worden ist.
